A guide to creating

your personal support package East Living offers more than care and support - we focus on helping you build your independence so you can do more of the things you enjoy. This guide outlines our range of services that you can use to develop your own personal support package.

Your support made easy Care and support helps people to live independently and stay healthy. The way support services are delivered has changed to a more personalised approach that puts you in control to make decisions about your support. This new approach is called personalisation and we can help you make the most of it. You don’t need to use us for all your support decisions. You might want our help to apply to your local council for support funding, or you might only want us to deliver services you already know you want. By listening to what you want and working closely with you to make informed choices, we can help you get where you want to be.

How we can help You can use our support services for as little or as much help as you want. The key areas we can help you with are: 1. Working with you to create your support plan 2. Help you arrange funding for your support through either your local council, personal funds or other means 3. A brokerage service, where we help you choose who provides your support services

Value for your money We charge a competitive hourly rate for our support services that varies depending on the level of expertise you need from our staff and where you live. We can offer you a free consultation to understand your needs and provide you with an estimated cost for your support package.

1 – Help with your support planning To start your support planning process, we will work with you and those important to you to complete a life planning questionnaire. The questionnaire will help you identify the changes you want to make, and:

Your plan will cover both practical issues (such as money management and getting healthy) and emotional issues (such as relationships and building confidence). Your plan will be specific about:

2 – Help with arranging funding

for your support plan

We can help you work out how much your support plan will cost and how it could be paid for, so you can make the right choices. In some cases, you might need to pay for the support yourself. However, with the personalisation system, some people can get an individual budget from their local council to fund their support.

If you are entitled to an individual budget, we will help you get council funding by making sure your support plan meets their requirements. If you already have an individual budget, you can use some of this funding to cover the cost of our help to develop and manage your support plan. We charge a competitive hourly rate for our support services – ask us for our latest prices.

Are you eligible for an individual budget? Anyone who is assessed as needing a social care service may be able to apply for an individual budget, including:

• Older people (65+) • Carers • People aged 16+ who have: physical and/or

learning disabilities, mental health issues or a sensory impairment

• People living with HIV or AIDS

• People with alcohol and

substance abuse issues

• People with long term health conditions

3 – Finding the best way to

provide your support plan

We can help you find the best way of meeting your care and support needs, as outlined in your support plan. If you are interested in using, or continuing to use our services in some way, we can arrange this for you. There is a full list of our support services listed over the page. We can also act as a broker and help you arrange support from other organisations, and involve your friends and family if you want. Ways we can help you:

• You can choose from a wide range of

local providers using our network links and partnerships

• We can help you get the best price for your support services

• We can also help you make use

of free local training, services and community groups

• If you want to employ someone to

provide a service (such as a personal assistant) we can help you understand the role and responsibilities of becoming an employer

• Once your support is arranged, we will

help you put agreements in place to ensure you get what you have paid for.

5 – Help to review & monitor your service It is important that you are happy with the support you receive. Our regular reviews will help you check that you are getting a good service and value for your money. It’s also important to review your plan regularly because as you build your independence, your needs will change, along with your goals.
