To Parents/Guardians of Ninth Grade Students: Welcome to high school! As part of the School District of Philadelphia Nursing Program, I will be working with the Father Judge faculty and staff to provide for the health services of your sons during the next four years. I can be reached at the school office: 215-338-9494 (ext. 1116). During the summer please leave messages with the main office receptionist. Pennsylvania State Law states that all ninth grade students have a complete physical examination. The Report of Physical Examination must be completed and returned to the School Nurse by September 8, 2014. If your son intends to try out for any sport during the school year, please have your doctor complete the PIAA form Section 4 at the time of his physical exam. This form should be dated on or after June 1, 2014 to be valid for the 2014-2015 sports season. Parents must complete Sections 1, 2 and 3. As tryouts for fall sports begin before the opening of school, please give the completed PIAA forms to the Main Office during the summer or to the coach at registration. The Pennsylvania Health Department now requires ALL students to have TWO doses of the Varicella (Chickenpox) Vaccine or documentation of having had Chickenpox. Please have your physician administer the Varicella Vaccine if needed and also attach a copy of all of your son's immunizations with the physical form. It is recommended that you keep a copy of your son's immunizations and physical forms. Thank you for your cooperation.
