taying active is key to good health. Don’t let common sprains and strains get in the way of your workout. “While names sound the same, these are two very different injuries. A sprain affects ligaments, while a strain affects tendons and/or muscles,” said Dr. Frank Benedetto, a doctor of physical therapy and founder of ProVere Health, with four locations in New Jersey. “Ligaments connect one bone to another bone. For example, the most famous ligament in popular media, the ACL (anterior cruciate ligament), connects the femur (thigh bone) to the tibia (the shin bone). Tendons connect a muscle to a bone. The well known Achilles tendon connects the calf muscle to the heel bone.” Other expert tips for avoiding workout injuries:

Focus on hamstrings While many sports tend to have a particular injury related to it — think “tennis elbow” — hamstring injuries are perhaps the most common across all sports, said Dr. Pablo Costa, an associate professor in the department of kinesiology at California State University, Fullerton. Hamstrings deserve special attention. By strengthening the hamstrings you’re not only preventing injuries to those muscles, you’re helping protect your knees, Costa said. Hamstring curls and knee flexion exercises — in which you slowly straighten and bend your knee from a seated position — are particularly beneficial for strengthening the hamstrings, he said.

Go slow and low When exercising start slowly, with a few repetitions or low mileage, and make sure any progression or increase follows the same principles, said Dr. Jerome T. Nichols, sports medicine physician at

Listen to your body “One of the worst phrases ever coined was ‘no pain, no gain,’” said Nichols. “Aches and pains in muscles are normal to some degree with any change in activity. Pain that gets progressively worse or is isolated to a bone or joint, however, may be a sign of something more serious developing. As you get more experienced exercising you’ll learn to recognize what kinds of pain are worrisome and which aren’t. Until or unless you have that experience, you should back off if your exercise routine is causing you significant discomfort and consider having (the painful area) checked by a physician.”

Hydrate “Adequate consumption of water can help reduce tendon strains,” said Dr. Bruce Pinker, associate with the American College of Foot and Ankle Surgeons. It is recommended adults drink eight to 10 glasses of water per day at 8 ounces per glass, Pinker said.

Rest when needed “The human body operates on a very simple formula: Stress plus rest equals growth,” Benedetto said. “In order to encourage adoption of this mindset, I like to redefine what I mean by ‘rest’ to the avid exercisers I work with. Rest can mean strength training, yoga, Pilates, cycling and any number of things that just aren’t replicating the same ‘stress’ that the client typically puts on the body.”

he average American spends more than $10,000 a year on personal health care, according to the most recent estimates from the Centers for Medicare & Medicaid Services, but there are ways to keep health care expenses down. Be healthy While staying healthy is the most straightforward way to minimize health care expenses, that’s easier said than done. Chronic health problems cost Americans big bucks, and many of them are the result of poor health choices, said Angela Snyder, director of health policy and financing at the Georgia Health Policy Center and associate professor at Georgia State University. Diet and nutrition matter in the prevention of chronic diseases so follow these tips: Eat healthfully and get moderate exercise, regular checkups and adequate sleep. Doing those simple things can go a long way to minimize long-term health expenditures, Snyder said. Be a smart shopper “Shopping around for the lowest cost is an important part of being a health care consumer,” said Dr. Raffi Terzian, senior medical director at HealthAdvocate Solutions, which works with companies and organizations to communicate insurance benefits to employees. To avoid surprise bills, find out how much procedures will cost before having them done, Terzian said. Price-shopping can pay off big when it comes to medications. Request generics, which are required to have the same active ingredients and must work the same as their brand-name counterparts to obtain FDA approval. “Other ways to defray costs include taking advantage of drugstore discounts and manufacturers’ coupons,” Terzian said. Stay in-network While how much you pay depends on your

health plan, it’s always better to stay within your health insurance network. It’s also in your best interest to know what your plan covers, Terzian said. “Certain fixed amounts are set, but out-ofpocket costs may be independent. Find out how much (a procedure or test) will cost, and have a conversation with your provider,” Terzian said. The best time to ask for a discount on a noncovered procedure is before service is rendered. “You have a better opportunity to negotiate on the front end,” Terzian said. Be proactive If your health plan offers preventative services, be sure to use them, Terzian said. For example, the Affordable Care Act requires that most health plans cover blood pressure, colorectal cancer and cholesterol screenings and flu and tetanus shots for free. If you’re taking advantage of these screenings, it’s more likely a physician will notice a serious condition earlier, which may help reduce health care costs long-term. Use a health spending account Health spending accounts allow a person to put money aside for future health care costs, Terzian said. But, it’s a use-it-or-lose-it program. “You need to anticipate what expenses will be ahead of time and put money aside to cover them. It’s good for a whole host of medical, dental and vision expenses,” Terzian said. For people with high-deductible accounts, a health savings account is a great way to save for medical expenses and reduce taxable income, but a user must qualify for the program, Terzian said. The government sets the limits, and for 2018 the limit is $3,450 for singles and $6,900 for families. Read your bill Mistakes happen. Protect yourself from overpaying by checking your bill. If you see something you don’t understand or think there’s an error, contact your provider and ask about the charge.

How to train for a
MARATHON

M
arathon runners are endurance athletes pushing their bodies to the limit. Whether you’re a first-timer or an experienced marathoner, all runners need to build up for the big day. “The No. 1 piece of advice is to create a game plan,” said Dr. Joshua Blomgren, a sports medicine physician at Midwest Orthopaedics at Rush University Medical Center in Chicago and aid station medical lead for the Bank of America Chicago Marathon. Game plan A plan of attack will help keep athletes on track as they gradually build up mileage. One of the most common mistakes is to build up mileage too quickly, which is a “recipe for overuse injuries,” Blomgren said. Plenty of opportunities exist for creating a personal training plan either online, with a team or through personal coaching, he said. Get checked out Going from being a regular exerciser to a marathoner is a big jump. If you’re not a regular runner, Blomgren advises an evaluation by a sports medicine specialist to see if your body is capable of tolerating the strain it will need to endure. A physician can also offer guidance for training and injury avoidance. Stretch it out Create a routine stretching program that also includes days off to help avoid injuries, Blomgren said. When warming up, use dynamic stretching, which actively takes your body through a range of motions and increases blood flow and muscle temperature to get your body ready to move. After a workout, use static stretching, such as the classic
hamstring stretch, where you hold the stretch for up to 30 seconds. Cross-train Focusing on running alone can create muscular imbalances, especially in the hips, Blomgren said. Cross-training helps build core and hip strength and adds variety, Blomgren said. Gear up Getting fitted by a professional at a running store will ensure that your shoes fit properly. How long a shoe lasts depends on an individual, but in general shoes should be replaced after 350 miles, a number that can be reached while training for a marathon, Blomgren said. After that, shoes will still be wearable, but the foam cushioning may wear down and affect its form and function, Blomgren said. Shave time If you’re interested in setting a personal record or simply shaving off some time, you’ll need to enter marathon training in good shape. Get ahead of the competition by training in the cold. “At the end of October or early November, put in the time to increase speed and strength,” Blomgren said. Most rivals are taking time off in cold conditions. Avoid the expo “Have a well-thought out plan for race day. Think about pace, meal situation, parking situation and other variables. Remove as much stress as you can and focus on your run,” Blomgren said. Pre-marathon, don’t get sucked into the vendor expo. If someone hands you a free energy gel, now is not the time to try something new, Blomgren said. “Use what you know, because you know how your body will tolerate it,” he said.

f you’re always feeling hungry, your body is telling you something, and there are many science-backed reasons for what it’s saying. “Our bodies often confuse the signals of hunger, thirst and fatigue,” said registered dietitian nutritionist Lauren Harris-Pincus, author of “The Protein-Packed Breakfast Club.” “We tend to choose to focus on hunger because eating is so rewarding and pleasurable. However, you may just need a glass of water or a power nap to satisfy the feeling of discontent. Try those first and save the calories.” Blood-sugar swings are one of the major culprits behind why a person might always feel hungry, said Dana Ellis Hunnes, senior dietician at Ronald Reagan-UCLA Medical Center. “If someone uses a lot of juicing or other simplecarbohydrate meals, their blood sugar will spike up immediately after the meal and then will precipitously drop soon thereafter, leaving them feeling dissatisfied and hungry,” Hunnes said. “Another potential reason that a person may feel hungry is that they are simply not eating enough fiber and/or volume of food.” Both of these problems has a simple and similar solution. “Eat meals that are nutrient-dense and full of water and fiber,” Hunnes said. “For example, instead of eating white toast with jam, have whole-grain toast with either avocado or a nut butter as the fiber — plus, the protein from the whole-grains and nuts or avocado will satisfy you more. Add chia pudding (chia seeds soaked in water or non-dairy milk) to add soluble fiber, bulk and omega-3s to the diet. This can help with fullness.” Other factors Stress and anxiety can also affect a person’s hunger. “Our minds and gut are undeniably connected,” Harris-Pincus said. “We all react to stress and

NGRY anxiety a little differently. Some stress-eat to feel better; others are unable to eat when anxious or stressed. Unfortunately, the foods of choice when eating as a result of stress are high-calorie, highcarb comfort foods. Kale isn’t exactly the go-to in these situations.” Sleep can absolutely affect how hungry we feel. “If we are feeling very tired because we didn’t get enough sleep for a day or two, our entire digestive system — our second brain — overreacts or is hyperactive. If we are tired, we will seek the energy we crave from foods,” Hunnes said. A lack of sleep can make us feel hungrier than we might usually be, and when we are tired we are less likely to make healthy food choices, going instead for what is convenient or cheap, Hunnes said. Dieting healthfully Restricting the amount and type of food consumed, such as when on a diet, can affect hunger in a few ways, Hunnes said. By significantly reducing our calorie intake, our body will react by saying, “Hey! I’m not getting enough nutrition here. I’m hungry!” If a dieter is not replacing highly caloric foods with less-dense, more water-rich and highfiber foods, he will feel hungry, Hunnes said. Because most people try to lose weight quickly by cutting calories significantly, they are fighting the urges to eat as well as fighting the food cues that surround them in grocery stores, restaurants and at home. “We can only make so many decisions in one day, and having to extend willpower can wear you down,” Hunnes said. “For most people, it takes years to put on the weight. The average person gains about 3 to 5 pounds per year, yet most of us expect to take off 20 to 30 pounds in six months. These big changes that people are doing affects not only the psyche but also the hormones that regulate hunger — ghrelin and leptin.”

How to juice for health, not weight loss

uice cleanses are all the rage, especially when people want to drop some weight before a special event or just for swimsuit season. But are they healthy, safe or effective? “Most of the claims associated with juicing to aid in weight loss or cleanse the body are unsupported by science,” said registered dietician Shannon Weston of the UTHealth School of Public Health in Houston. More research is necessary to determine the safety, health benefits and effectiveness of juicing, Weston said. “It is really a temporary fix and does not keep weight off,” said Sonya Angelone, registered dietitian nutritionist and spokeswoman for the Academy of Nutrition and Dietetics. “You have to maintain a change in order to keep weight off. There is no evidence that it actually works in the short-term or longterm for weight loss.” Juicing isn’t sustainable because it creates a diet that is deficient in protein, fiber, fat and many other nutrients, Angelone said. “A day of juicing may help with momentum, but being
overweight isn’t a lack of juice, it is an overconsumption of calories, not enough exercise and just eating the wrong foods,” Angelone said. Juicing strips nutrients One primary reason juicing is not a healthy weight-loss option is because pure juice is just a step up from a soda, said nutrition and weight loss expert Dr. Lori Shemek, author of “How to Fight FATflammation.” Juicing strips away the fiber and concentrates the sugars from many servings of fruit into a single serving of juice, Shemek said. While juices can be good sources of vitamins and minerals, juicing fruits removes fiber, which is beneficial for a healthy diet. “Fiber is one of the most important components of foods,” Angelone said. “Highfiber diets have been associated with decreased chronic diseases. Fiber feeds good gut bacteria, which keeps your gut healthy. It is also filling.” Fiber, especially insoluble fiber, helps with digestion, lowers cholesterol levels and supports glucose (blood sugar) control, Weston said. “In addition, the majority of the vitamin, mineral and antioxidant property of the produce
is lost when the outer layer and pulp is removed,” Weston said. Healthier alternatives Eating a well-rounded, common-sense diet and taking part in daily exercise is the prescription for good health. “Eating a balanced diet full of whole fruits and vegetables, lean protein, healthy fats and whole grains and engaging in physical activity at least 30 minutes daily is the best way to obtain sustainable weight loss,” Weston said. “A registered dietitian can create a customized plan to help individuals reach their desired health goals.” Juicing is a better snack or part of a meal, Angelone said. “Juicing can be a good addition to a healthy diet as long as the majority of the juice is derived from vegetables,” Weston said. Include vegetables such as cucumbers, leafy greens and bell peppers while adding a healthy fat such as avocado, mediumchain triglycerides — or MCT — oil or coconut oil, and a protein source such as hydrolyzed collagen powder or powdered amino acids, Shemek said.

As part of his fellowship at the TRIA Orthopaedic Center in Minneapolis/St. Paul, MN, Dr. Cowan provided medical care for the NHL’s Minnesota Wild, MLB’s Minnesota Twins, Gustavus-Adolphus college football, and area high schools. His interests are in athletic injuries to people of all ages including ACL reconstruction, multi-ligamentous knee injuries, rotator cuff repairs, and many more procedures for the knee, shoulder, and elbow. When not taking care of patients, Dr. Cowan enjoys spending time with his wife, three children, and extended family. He also enjoys fitness, travel, and cooking.
