or many rheumatoid arthritis patients, the changing seasons can affect the signs and symptoms of the disease, but arthritis flare-ups can be managed. “While some patients clearly do have weather or seasonal triggers, this is not a given, and for many patients the seemingly random nature of their flares can be frustrating,” said Dr. Arthur M. Mandelin II, associate professor of medicine at Northwestern University’s Feinberg School of Medicine. “Most patients learn quite quickly whether they personally have a weather/seasonal trigger or not.” “Colder temperatures and changes in barometric pressure may trigger worsening joint symptoms,” said Dr. Elena Myasoedova, a rheumatologist at the Mayo Clinic in Rochester, Minnesota. Usually these episodes are self-limiting, Myasoedova said, meaning they will resolve themselves without treatment. Rheumatoid arthritis is the most common form of autoimmune arthritis and affects about 1.5 million Americans. This common chronic condition can significantly impact a person’s well-being and quality of life. Ups and downs Rheumatoid arthritis can be unpredictable, with good days when joints are feeling pretty pain-free to bad days when it’s hard to get out of bed. Symptoms differ from person to person, Mandelin said. “In general flares involve increased pain and swelling in the same joints that the patient finds are usually the most troublesome anyway,” he said. “A more serious flare can also involve joints that had previously improved during the course of treatment.” In addition to swelling and joint pain, a flare-up may
For some people the weather can cause a flare-up, for others stress can be a trigger, Mandelin said. “Not taking medication as prescribed can cause a flare of your rheumatoid arthritis,” Myasoedova said. “A major stress physical or mental — such as surgery, infection or loss of a loved one — can trigger a flare-up. Generally, the flare is less likely if a patient is taking appropriate treatments.” Dietary triggers are more rare. “The frustrating thing with diet triggers is that they are personal to each patient if they happen at all, so there is no one single diet recommendation that works for everyone,” Mandelin said. “Sometimes, rheumatoid arthritis can become more active without any apparent trigger, requiring change in medications,” Myasoedova said. Finding relief Work with your primary care provider or rheumatologist to create a plan of action to keep your symptoms in check, Myasoedova said. Be open about your symptoms, especially any changes. “Keeping a diary of your flares and discussing them with your provider can be helpful. If you think you are in a flare of your rheumatoid arthritis and it is not responding to your usual treatment, contact your rheumatologist’s office to discuss your symptoms,” she said. If possible, bank a few sick days so you can take time off from work to get relief when most needed. “Frequent flare-ups can be a warning sign that the overall control of your disease isn’t what it should be,” Mandelin said. “Uncontrolled disease can cause more trouble than just flare-ups, so if your arthritis gets out of control often you should be asking your rheumatologist about stronger medication.”

t’s an all-too-common occurrence: A simple trip on a rug or a wet floor leads to broken bone. While a younger person may heal quickly, a fall can be the start of more serious problems for older people. After age 50 weak bones lead to breaks in 1 in 3 women and 1 in 5 men, according to the International Osteoporosis Foundation. Good bones should be a goal at any age, but as we get older strong bones should become more of a priority. While men have more bone strength because their bone size and width is greater, both men and women can suffer from osteoporosis, said Dr. Philip Bosha, a physician with Penn State Sports Medicine. Life stages and calcium The best time to build strong bones is during childhood. Peak bone mass (the strongest bones you’ll ever have) is determined by genetics as well as a combination of lifestyle factors such as diet and exercise, he said. It’s established during puberty and early adulthood. Half of the total body calcium in women and two-thirds in men are established during puberty. About 90 percent of a woman’s peak bone mass is present by age 20. Men make gains in bone mass until 30, Bosha said. A slow decline in bone mass begins around age 40. Everyone will lose bone mass with age, but people who develop a higher peak bone mass when they are younger will be better protected against fractures later in life, Bosha said.

Nutrient needs The amount of vitamin D a person requires varies based on several factors. Greater amounts may be needed in people who have limited sun exposure, have darker skin or are obese. But in general: • Children between ages 1 and 3 need 700 milligrams of calcium per day. This can be reached easily with a glass of milk, vegetables and other dairy products. • Between 4 and 8, children need 1,000 milligrams of calcium per day (two cups of yogurt and one glass of milk) and 600 to 1,000 international units of vitamin D. • During puberty teens should take in at least 1,300 milligrams of calcium. After puberty, 1,000 milligrams per day is recommended. Vitamin D supplements are needed to reach the recommended 1,000 IU per day. • Pregnant and breastfeeding women, people ages 30 to 50 and men over 50 should get 1,000 milligrams of calcium per day. • Women over 50 and women who are entering or past menopause should increase their calcium intake to 1,200 milligrams per day. • After 70 both men and women should take 1,200 milligrams of calcium per day and 800 IU of vitamin D per day.

Surgery may be necessary to repair fractured bones, and orthopedic hardware or implants may be the best way to ensure everything heals as it should.
What is orthopedic hardware? Orthopedic hardware or implants are substances that are placed inside or on the surface of the body. The American Orthopaedic Foot and Ankle Society states that, in the field of orthopedics, and particularly in regard to foot and ankle surgery, implants
refer to hardware that is used to hold bones together, strengthen tendons and ligaments, or attach them to bone. Some implants replace the bone itself. The process of surgical implementation of implants for the purpose of repairing a bone is called internal fixation. Although this technology may seem futuristic, according to the book “Surgery, Science and Industry: A Revolution in Fracture Care, 1950s-1990s,” this type of surgery originated in the 19th century and has been evolving ever since. Common hardware types Implants may be biologic and nonbiologic. Biologic materials are similar to human tissue and strengthen areas. Many operations make use of nonbiologic materials, which can be comprised of titanium, stainless steel and plastics. Among the most common types of medical implants are pins, rods, screws, and plates used to anchor bones while they heal. Wires and nails may
also make up an orthopedic surgeon’s catalog of hardware options. Depending on a person’s age and health status, bones can generally take anywhere from six to 12 weeks to heal to a significant degree, advises the Cleveland Clinic. Orthopedic hardware, in addition to casting, can help keep bones in place as they heal. Removal of hardware Orthopedic hardware was once routinely removed after the fracture had healed. Then physicians realized that it is not always easy to remove hardware. As a result, the decision of whether or not to remove implants is now one doctors and patients discuss. There are mixed opinions on whether or not it is necessary to undergo further surgery to have rods or pins removed. Typically, it is a case-by-case decision based on some key criteria: • Severe pain in the area where hardware is located.
• Impaired function in the area the hardware helped repair. • Loss of mobility. • Infection in or around the hardware connection site. However, the AOFAS states there is no evidence that the current implants used in orthopedic surgery result in an increased risk of infection. isconceptions about the seasonal flu and the flu vaccine can be dangerous to your health. Current vaccines are not perfect, but even in years where the vaccine has had low effectiveness in preventing illness, vaccination dramatically reduced deaths and hospitalization especially in children, said Dr. Gerald Fischer, president and chief executive officer of Longhorn Vaccines and Diagnostics and adjunct professor of pediatrics at Uniformed Services University of the Health Sciences, a school that prepares graduates for service in the U.S. Armed Forces’ medical corps. Myth 1: Flu shots can cause the flu. “Injectable flu vaccines are made from small portions of the flu virus and are not infectious,” Fischer said. The vaccine is made either with inactivated (killed) flu viruses

EMOTIONAL EATING

I
f a bad day at work or a broken heart makes you reach for a gallon of ice cream or a bowl of macaroni and cheese, you may be an emotional eater. Finding comfort in food when you’re at a weak point emotionally is not a solution to stress or problems, said licensed psychotherapist Arlene Englander, author of “Let Go of Emotional Overeating and Love Your Food: A Five Point Plan for Success.” “The key concept of how to describe emotional eating is eating neither for enjoyment or to satisfy hunger but to distract from painful feelings,” she said. Common causes of emotional eating could be conflicts at work or in relationships, financial stress, health problems or simply feeling overwhelmed. Struggling with emotional eating can sabotage your health and lead to long-term weight gain, Englander said. It may offer a temporary fix but often produces negative feelings of regret. “It can lead to self-berating and thoughts of ‘I shouldn’t have done I’m a loser. I’m fat,’” Englander said. Find the cause The odd advantage of emotional eating is that a person can use it to pinpoint a problem and figure out what their triggers are. “We can realize we’re drifting to the refrigerator or pulling up to fast food, and that can help us be aware that we’re
experiencing painful emotions,” Englander said. Keeping a food diary can help people understand not only what they ate but also how they felt when they ate it, Englander said. To break a pattern of unhealthy eating, it’s necessary to first recognize the pattern exists. “Awareness is key. We have to be compassionate to ourselves and practice selfcare,” she said. Practice mindful eating Instead of gulping down food in front of the television or while staring at a screen, “tune in to the pleasure of eating,” Englander said. “Tune in to the taste, the texture and the scent of what you’re eating.” Distract yourself It only takes a few minutes to distract yourself from emotional eating, Englander said. Instead of eating, take a walk around the block, take five minutes to clean up the room you’re in, phone a friend or meditate. Often a person then will realize they were not even physically hungry, she said. Overcome setbacks Don’t blame yourself if you have a lapse. “Learn from it. Forgive yourself and see it as a gift for next time,” Englander said. Periodically evaluate how you’re doing. Ask what’s working and what’s not. Consider how you’re handling stress. Don’t forget to exercise and fill up on healthy food, Englander said. “Both mindful eating and cognitive behavioral therapy are skills that can be broken down into simple steps and learned, and once learned can become lifelong habits that make our lives not only healthier but more enjoyable,” Englander said.
