REGISTER Registrations are accepted on a first-come, first-served basis. You can register online or by phone. ONLINE: You can register online at enform.ca. If you havenâ&#x20AC;&#x2122;t done so already please set up an account by clicking on the login button on the homepage. This will provide the instructions for setting up your account. Once your username and password is set up you can add courses to your cart and pay with your credit card. Confirmation of your registration will be emailed to you. BY PHONE: You can also register by calling 1.800.667.5557 and one of our customer service representatives will be happy to help. A credit card will be taken over the phone to reserve your spot.

BEFORE THE COURSE Remember to bring government issued photo identification to your course as it is required for admission. Some courses may also require safety equipment, calculators or other materials. Please review the notes and equipment listing for each course to ensure you have everything you need.

CANCELLATION Cancellations received by phone or in writing a minimum of five business days before the course start date will be fully refunded. Cancellations received less than five days before the class start date will receive a 50 percent refund. Where no written notice is received before the class start date no refund will be granted. Additionally, the full cancellation policy can be found at www.enform.ca CERTIFICATES Certificates are issued upon course completion. Only one certificate, per course, is valid. Your wallet certificate will be mailed to the address supplied at the time of registration or on the student data sheet submitted. It can take a few weeks to receive a ticket due to processing and mailing time, however copies of Records of Training can be obtained sooner, if needed. Please note that some courses provide 90 day temporary certificates. If you have not received your permanent certificate prior to the end of the 90 days contact Enrolment Services at customerservice@enform.ca or toll free at 1.800.667.5557.

REPLACEMENT CERTIFICATES AND RECORD OF TRAINING Replacement certificates can be ordered online by logging into your Enform account at www.enform.ca or by calling customer service at 1.800.667.5557. A Record of Training is also available online. The Record of Training can be provided to an employer as proof of certification.

BURIED FACILITIES LOCATOR – SEISMIC Duration: 2 days Contract Only Certificate Expiry: 3 years This course is designed for locators working in the seismic industry. Participants will learn how to prevent damage to buried facilities through safe and effective planning of locates and the identification and marking of buried facilities. The hands-on training will emphasize the correct use of locating equipment and proper marking and documentation of locates in the seismic industry. DESIGNED FOR

CANADA’S OIL AND GAS INDUSTRY (COGI) – ONLINE SERIES Cost: $130 This online series features five courses dealing with the upstream, midstream, and downstream sectors of the oil and gas industry. CAODC SERVICE RIG COMPETENCY ASSESSORS Duration: 1 day Cost: $185 Learn how to become an assessor and gain an understanding of the guidelines for completing a competency assessment for a floorhand, derrickman or driller. DESIGNED FOR Field superintendents and rig managers in the well servicing division of the Canadian Association of Oilwell Drilling Contractors (CAODC). PREREQUISITES The CAODC Service Rig Assessors Committee has set the following requirements for becoming an assessor: »» Must be currently employed with a service rig contractor »» Must be at the level of field superintendent or rig manager »» Must complete this one-day Service Rig Competency Assessors Program »» Must submit at least one assessment every three years If an employee of a service rig contractor does not meet these prerequisites and wishes to become an assessor, Part B of the Service Rig Assessors application form must be filled out by the HSE Manager of the service rig contractor. CERTIFIED HEALTH AND SAFETY AUDITOR RENEWAL Duration: 1 day Cost: $250 Certificate Expiry: 3 years Enform Certified Health and Safety Auditors must take this program once every three years prior to the expiry of their certification. DESIGNED FOR Enform auditors wishing to renew their auditor certification or to learn about updates to the audit process. PREREQUISITES »» A current Enform Certified Health and Safety Auditor Certificate »» If your certificate has expired, the standard allows up to six months from the expiry to complete the renewal program. Note: the standard does not allow any new audit registrations until recertification is complete. »» Maintenance of your auditor certification by having performed two audits or maintenance audit options in the previous threeyear certification cycle. »» If this requirement has not been met, the auditor will be required to conduct and submit a regular Certificate of Recognition (COR) audit within six months of the renewal program before recertification can be granted. CHAINSAW FALLER COMPETENCY PROGRAM

COMPLETION

GRANDE PRAIRIE:

The oil and gas industry adopted the BC Faller Training Standard as the standard level of training for workers falling trees over six inches in diameter. Endorsed by WorkSafeBC, this training program progresses in a graduated manner with on-the-job work experience required between each level. 75% of the time is spent in the field practicing skills.

Upon successful completion of this course, you will receive a Chainsaw Competency Record Logbook and a wallet certificate that is valid for three years. After successful completion of this course, participants are qualified to find work as a bucker (maximum six inches in diameter at breast height trees (DBH). CERTIFICATE RENEWAL

DESIGNED FOR Individuals who would like to become fallers in the oil and gas industry.

If your certification is still valid, you can contact a certified instructor to challenge the course to renew your certificate.
