Introduction Created by industry, for industry, Enform is mandated to serve as the hub of safety for the upstream oil and gas industry. Enform’s vision is no work-related incidents or injuries in the Canadian upstream oil and gas industry. Our mission is to advocate for the continuous improvement of safety performance by providing practices, assessment, training, support, metrics and communication to workers in our industry. Enform’s Training department collaborates with industry partners to develop training courses. These courses evolve to meet the changing needs of a dynamic and progressive oil and gas industry. Enform’s extensive safety training experience allows the Training department to work to provide relevant and effective courses. Our courses respond to changes in industry and environment so that workers are fully prepared to work safely. This Enform Authorized Training Provider Handbook is a guide for Providers of Enform courses. As a Provider, you are a key element in our effort to deliver quality courses. You have a direct impact on workers in the upstream oil and gas industry. This Authorized Training Provider Handbook will help you be efficient in organizing course delivery and will help clarify your relationship with Instructors and with Enform. This Enform Authorized Training Provider Handbook goes through the steps you need to take to prepare for and successfully host any Enform course.

Enform-Authorized Training Provider Business Relationship Enform is responsible for providing industry with the safety training it needs. We need to be flexible and evolve with changes in technology and industry practices. We provide a service for industry: we create and deliver safety training. Enform has a responsibility to industry to make sure that it is providing the best, most up-todate information, in order to work toward our vision of no injuries and incidents. The relationship between Enform and its Authorized Training Providers is a business relationship. This means that both parties have responsibilities to each other. Both Enform and its Authorized Training Providers need to fulfill those responsibilities in order to provide the best safety training possible. Enform’s General Responsibilities In general terms, Enform is responsible for developing and updating course content and overseeing course delivery quality. Enform will also update Authorized Training Providers any time a change occurs. Finally, Enform needs to support its Authorized Training Providers as they host the delivery of its courses. Authorized Training Providers’ General Responsibilities The actions and performance of the Authorized Training Providers and the Certified Instructors are key to students learning what they need to know from Enform courses. Authorized Training Providers’ actions and performance when hosting/facilitating Enform courses also affect the reputation of these courses and of Enform. In addition, the actions and performance of the Providers’ Instructors when instructing these courses affect Enform’s reputation, and the reputation of its courses. This impacts Enform’s ability to carry on its business for the benefit of the oil and gas industry. Enform expects that Providers will abide by the contracts they sign with Enform. Authorized Training Providers are also responsible for ensuring that their Instructors comply with the terms of the Authorized Training Provider Agreements within the scope of the Certified Instructor’s responsibilities, as well as their own Certified Instructor Agreements and Enform’s published course standards. Ensuring successful relationships between Enform and its Authorized Training Providers depends on a commitment by both parties to cooperate. As long as both parties fulfill their mutual responsibilities, the business relationship will work effectively.

Becoming an Enform Authorized Training Provider Application Process If you want to become an Authorized Training Provider, you need to complete the application, which can be found on Enform’s website, enform.ca Training organizations, public and/or private institutions, and individuals can all apply to become Authorized Training Providers. If your authorization is approved, you will sign the Authorized Training Provider Agreement. You will be authorized for three years. Application processing could take a period (approx) 45 days. Authorized Training Provider Representatives You need to designate an Authorized Training Provider Representative. The representative is the person listed with Enform as authorized to order training materials. You also need to identify a single primary contact person. Each Authorized Training Provider Representative is required to take Enform Provider Orientation (EPO). Enform will communicate to Authorized Training Provider when available. If you make a change to the representative(s), you must do it in writing so that Enform records stay up-to-date. If you have multiple approved training locations, you can have one representative for all locations. Additional Licensed Courses If you want to offer any additional Enform courses after you sign the initial contract, submit your request to Enform in writing by filling out the Expansion Request Form. In your Expansion Request, declare any equipment requirements for the additional course. The request will take at least 30 days to review and may not be approved. It is solely Enform’s discretion to decide whether an applicaton is approved. You cannot begin to offer or advertise additional Enform courses until you have received written approval from Enform. Enform will add the additional courses to the existing contract without a change in the contract expiry date.
Notifying Enform of Course Locations Permanent Each Authorized Training Provider is required to include any permanent locations used for training in their Authorized Training Provider Application form. E.g. Company ABC has three locations in western Canada where they have training facilities, all sites would be declared. There is no requirement to advise Enform of course schedules being held at permanent locations approved by Enform.

Temporary Temporary sites do not include permanent sites declared by Authorized Training Providers on their Provider application form. Many Authorized Training Providers conduct business from remote or temporary locations such as work camps or hotels. Enform requires Authorized Training Providers to notify Enform of all courses that are conducted at temporary sites prior to the course being conducted. The Authorized Training Provider is required to give 48 hours’ notice to Enform by sending an email to training@enform.ca with the following information: Email subject line: Authorized Training Provider- [insert company name] Email body: a.

Maintaining Authorized Training Provider Status Each Authorized Training Provider contract is valid for three years. In order to maintain Authorized Training Provider status within that period, you need to meet the established criteria for each course you are licensed to offer. You must also make sure that you only use Enform Certified Instructors, and that they are delivering Enform courses according to Enform’s established standards. Enform will provide periodic notification to remind you of expiring agreements. During the last year of your agreement, you need to reapply to renew your contract with Enform. That request could take up to 45 days to review and process. It is your responsibility to submit your request a minimum of 45 days prior to the end of your existing agreements. If you submit your request later than the 45-day mark, Enform will not guarantee that it can process your request before your contract is up. Submitting your request late could affect the continuity of your contract. Enform is not obligated to accept any or all Authorized Training Provider requests for renewal. You will be informed in writing if your request is denied. If your request is approved, you will be required to sign a new agreement. Enform will provide you with the most current Form of the Enform Authorized Training Provider Agreement upon notice that your renewal is approved. Fees As at the implementation of the new Enform Authorized Traning Provider processes in 2016, Enform is not charging any fees for applications, renewals, evaluations or other activities involved in the program. However, Enform reserves the right to implement reasonable fees as it deems appropriate to the circumstances.

Delivering a Course Facility Requirements Training facilities need to provide a good learning environment for students. They should be away from noise, external activities and, other distractions. Your training facility needs to meet the following criteria:

You must provide your Certified Instructors with an emergency response plan and facility orientation. Do this for each location. You can have multiple fixed training sites identified in your contract. At Enform’s discretion, each site that you identify is subject to a site review by Enform before it will be approved.

If you do not have a previously approved training facility/location, you need to complete the Authorized Provider Training Form. It is located under the Provider heading of the Training section on Enform’s website. You need to fill out this form before you deliver a course.

You are responsible for ensuring that any secondary location(s) meet the standards laid out for the primary location.

Authorized Training Providers will have the option to contract Certified Instructors who work for multiple Authorized Training Providers. It is critical that each Authorized Training Provider have administrative processes in place to track each student kit and exams from distribution to exam submission. Enform will be tracking student kits to ensure declared Certified Instructors and Authorized Training Providers match the kits that are issued. If kits or exams surface from another source other than the designated Authorized Training Provider, Enform will conduct an investigation to determine whether the class can be certified and/ or there has been fraudulent submissions.
Contracting Certified Instructors You can only use Enform Certified Instructors to deliver Enform licensed courses. Enform will provide an up to date active Certified Instructor List in the Portal. If you are not sure if an Instructor is certified or not, contact Enform’s customer service line at 1-800-667-5557 (ask for training) or email training@enform.ca to confirm. If you do not use an Enform Certified Instructor, Enform will not certify the course and the students will have to repeat it at cost to the Authorized Training Provider.
Course Materials and Equipment It is the Authorized Training Provider’s responsibility to make sure that all equipment needed for a course is available to the Certified Instructor. If you offer multiple courses, you need to ensure that you have all the equipment for all the courses. All your equipment needs to be in working order. It must meet Original Equipment Manufacturer (OEM) specifications, as well as the requirements of each course’s specific standard for use. You must inspect and document equipment regularly to meet provincial health and safety legislation. Clean course equipment regularly and carefully. Use the right cleaning procedures for course equipment. You need to have written maintenance and cleaning policies and procedures accessible at all times, according to the manufacturer’s requirements or the Enform Policy Standard. Look on our website for a listing of all equipment requirements for all Enform licensed courses. 14

Enform Student Kits It is the Authorized Training Provider’s responsibility to ensure that Certified Instructors have all required student kits for each course. Every student needs to receive a new student manual for each course that the student takes. When you order student kits:

When you receive student kits from Enform, it is your responsibility to make sure that all materials are complete and not damaged. You can refer to the Enform course material refund policy, which is found on our website. Handling student kits and student kit security: It is your responsibility to make sure that all Enform materials are kept in a secured location. If you delegate this responsibility to your Certified Instructor, you remain responsible to see that the Certified Instructor complies. You also need to ensure that all student information is kept secure and confidential. You are responsible for maintaining the security of your student kit inventory before distribution to your Certified Instructors.

Security is also important during class. Keep exams in a secure location. The Certified Instructors are responsible for the security of the exams during class. You must ensure that they comply with this obligation.
What You Need to do When Students Enroll Student Accommodation During registration make all students aware that, if they need special accommodations for the course, they need to declare their needs to you in advance. Provide a contact phone number and email so they can call you if they need additional special accommodations on the day of the course. Confirming Student Photo IDs Upon registration, inform students of Enform’s mandatory government-issued Photo ID check. Enform requires each student to have their ID validated for every Enform course. It is your responsibility all students are aware of this policy. Acceptable proof of government photo ID can include: driver’s licence, passport, permanent resident card or firearms licence. If the student doesn’t have a photo ID, you can use a combination of alternate proof instead. Acceptable alternative identification would be one primary form of ID plus either one other primary ID, or a secondary form of ID. Primary forms of alternative ID include: Birth Certificate or Social Security Card. Secondary forms of alternative ID include: Certificate of Indian Status, citizenship or immigration papers issued by governmental authorities, Provincial Health Card, and Canadian military identification. Questions relating to accepted ID should be directed to Enform during the registration process. Email training@enform.ca

Dividing Courses We expect that a licensed Enform course will be delivered as it was designed. Continuity of course information is critical for student learning. It’s very important that Enform courses are delivered the same way, all the time, to make sure that students get a standardized learning experience. Any changes to courses proposed by Authorized Training Providers or their Certified Instructors must be authorized by Enform in advance. You must have written permission from Enform to change how a course is delivered. If one of your Certified Instructors want to divide up the course differently than it was designed, you need permission from Enform. The company requesting a course to be delivered over 2 days must submit “Enform’s Course Multi Day Delivery Forms” located on the Enform website. Enform will communicate back its decision to both the client company and the Authorized Training Provider. Only proceed with the divided course when you have received written permission from Enform. Enform may or may not approve the division of a course, at its own discretion. Scheduling Courses

Class Size Enform class sizes are limited, depending on which course you are hosting. You can find a list of maximum class sizes on Enform’s website, enform.ca. This number could be limited further by the equipment available. For example, if there are only 6 SCBA masks for an H2S Alive® course, the standards will not be met for certification of a class of 20.
What You Need to do the Day of the Course This example timeline is based on a full-day course. Appropriate changes should be made when delivering a course of a different duration. If the course begins at 8:00 am: 7:30 am - meet the Certified Instructor when they arrive and go through their plan for the day It is your responsibility to develop, prepare, and distribute a site specific safety orientation for each of your locations. Make sure all Certified Instructors receive the orientation prior to instructing a course in a new facility. Review of the orientation package should occur as required by Authorized Training Provider Assessment. The orientation needs to talk about:

Student participation
Late Policy You also need to discuss an appropriate late policy with your Certified Instructor. Enform grants its Certified Instructors a certain amount of discretion regarding student lateness. Many factors contribute to a student being late. It’s up to the Certified Instructor to decide whether the student can join the class or is too far behind. Enform generally does not allow admission after 30 minutes from the start of class. Note that the Certified Instructor and the Authorized Training Provider must agree on the late policy before class begins. Please be aware that each course has a specific number of hours required for certification. Whatever the reason, late may mean the student cannot be certified and will have to re-register for the course at a later date. Fit for Duty As the Authorized Training Provider, you need to have a policy in place regarding impairment. It is both your responsibility and the Certified Instructor’s responsibility to abide by and enforce the policy. 7:45 am - students arrive Remind students about Enform’s Photo ID policy. You will have notified students about this policy when they enrolled, but remind them on the day of the course, so they have another chance to retrieve forgotten IDs. If they did not bring one to the course. As students arrive, check their fitness to take and understand the course. Enform has a zero tolerance policy regarding drug or alcohol impairment. It is the Certified Instructor’s discretion to remove a student they believe is impaired. 4:00 pm - 4:30 pm - class dismissed When the course is over, work with your Certified Instructor to collect all the necessary documents detailed in class submission on enform.ca.

Document Control and Submission Course-related Documents Student Data Sheets (SDS): an SDS is provided for each student. It is the Certified Instructor’s responsibility to explain the proper way to record information on the SDS to the students in the class. It is the students’ responsibility to make sure they fill out correct information on the SDS.

Mailing Address Student certificates will be mailed to the address written on the SDS. Once a student writes their address on the SDS, you cannot alter it without student consultation and approval. Enform’s policy is to mail certificates to the address written by the student on the SDS. If a company wants students to have certificates mailed to the company that paid for the course, they must make it clear to their employees, so that those employees can write it on the SDS when they take the course. Note: It is the student’s right to decide which address to write on their SDS. Authorized Training Providers must not force students to write the Authorized Training Provider address on their SDS. Putting the Authorized Training Provider’s address on the SDS cannot be a condition of taking a course. However, the student can choose to do so if they want.

To submit all the forms, open each SDS and stack them on top of each other, facing up. Fold them together along the main fold. Include the rest of the forms in the prepaid envelope provided. Be sure to retain the Canada Post tracking number from the pre-printed shipping label. Send the forms back to Enform. All course material must be post marked within 5 days of completion of the class. Performance Management Enform’s goal is to deliver quality safety training. We require professional, ethical Certified Instructors and Authorized Training Providers. As discussed earlier, Enform has responsibilities to its students, which include making sure that Authorized Training Providers act with honesty, integrity and professionalism. We maintain a high standard of Authorized Training Provider performance through our performance management program. Authorized Training Providers are bound by the contractual conditions as written in the Authorized Training Provider Agreement. The information below is a breakdown of the performance management process.

If someone makes a complaint against you, the complaint goes directly to the Enform Evaluations team. The Evaluations team then initiates an investigation, which may include: • Course documentation review • Direct communication with students and/or Instructors • Enform Authorized Training Provider records review

Enform may undertake an investigation on its own initiative, whether or not there is a complaint.
Depending on the results of the investigation, formal action may be taken at the discretion of Enform management. All formal action taken is communicated in writing to the affected stakeholders. Enform maintains detailed records of all investigations it conducts.

Enform’s responsibility to Authorized Training Providers relating to Instructor issues: • Enform will inform the Authorized Training Provider if one of their Certified Instructors is under investigation that may lead to an interim or disciplinary suspension. • Please refer to the Authorized Training Provider Agreement for details of sharing Certified Instructor Performance Management infractions. • I f Enform’s investigation of an Instructor leads to allegations of concerns about the conduct of an Authorized Training Provider, then Enform will also investigate the Authorized Training Provider. • If you receive a complaint about an Instructor, you are required to contact Enform’s Evaluations team at training@enform.ca.

Enform investigations and evaluations: • Authorized Training Provider shall cooperate with Enform personnel who may perform Evaluations from time to time (with or without advance notice) and without limiting the foregoing, shall provide access to all facilities and classrooms used to administrate, support or deliver Enform courses and allow those persons doing an Evaluation opportunity to observe the delivery of Enform courses, inspect equipment and maintenance and storage facilities and to converse with Authorized Training Provider’s Instructors, students and Authorized Training Provider Representatives. • If an investigation or evaluation is underway and an Enform staff member has questions for you, you are obliged to answer them. Do not hold back information during an investigation.

Performance management disciplinary measures
Providers who fail to comply with Enform standards or who engage in misconduct will be subjected to consequences, as set out in the Authorized Training Provider Agreement. Such consequences can include cancellation of the Authorized Training Provider Agreement if the failures are not fixed in a timely manner, or if misconduct occurs. If the business relationship between Enform and a Authorized Training Provider is not conducive to the needs of Enform or of its stakeholders, it is Enform’s discretion to cancel the Authorized Training Provider Agreement.
