Introduction Created by industry, for industry, Enform is mandated to serve as the hub of safety for the upstream oil and gas industry. Enform’s vision is no work-related incidents or injuries in the Canadian upstream oil and gas industry. Our mission is to advocate for the continuous improvement of safety performance by providing practices, assessment, training, support, metrics and communication to workers in our industry. Enform’s Training department collaborates with industry partners to develop training courses. These courses evolve to meet the changing needs of a dynamic and progressive oil and gas industry. Enform’s extensive safety training experience allows the Training department to provide relevant and effective courses. Our courses respond to changes in industry and environment so that workers are fully prepared to work safely. This handbook is a guide for all Certified Instructors of Enform courses. As a Certified Instructor, Enform considers you a key element in our effort to deliver quality courses. You have a direct impact on workers in the upstream oil and gas industry. This handbook will guide you through your responsibilities as a Certified Instructor, including administration and classroom management. Following our structure and standards will create a consistent learning environment for all Enform courses and enhance students’ learning experience. This Enform Certified Instructors Handbook goes through the steps you need to take to prepare for and successfully deliver any Enform course.

Enform-Certified Instructor Business Relationship Enform is responsible for providing industry the safety training it needs. We need to be flexible and evolve with changes in technology and industry practices. We provide a service to industry: we create and deliver safety training programs. Enform has a responsibility to industry to make sure that it is providing the best, most up-to-date information, in order to work toward our vision of no injuries and incidents. The relationship between Enform and its Certified Instructors is a business relationship. This means that both parties have responsibilities to each other. Both Enform and its Certified Instructors need to fulfill those responsibilities in order to provide the best safety training possible. Enform’s General Responsibilities In general terms, Enform is responsible for developing and updating course content and overseeing course delivery quality. Enform will also update Certified Instructors any time a change occurs. Finally, Enform needs to support its Certified Instructors as they deliver our content. Certified Instructors’ General Responsibilities The actions and performance of Certified Instructors are key to students learning what they need to know from Enform courses. Certified Instructors’ actions and performance when instructing Enform courses also affect the reputation of those courses and of Enform. The reputation of our courses affects Enform’s ability to carry on its business for the benefit of the oil and gas industry. Enform therefore requires that Certified Instructors sign and abide by Enform’s Certified Instructor Code of Ethics. Certified Instructors should consider entering into contracts with Authorized Training Providers that confirm the Authorized Training Providers’ support for the Certified Instructor. Ensuring successful relationships between Enform and its Certified Instructors depends on a commitment by both parties to cooperate. As long as both parties fulfill their mutual responsibilities, the business relationship will work effectively.

Becoming an Enform Certified Instructor Application Process Once you have gone through the application process, fulfilled the pre-entrance requirements, and taken the Instructor Skills Workshop course (ISW), you are ready to become an Enform Certified Instructor. A detailed explanation of that process is on our website, enform.ca. Note: In order to become a Certified Instructor, you also need to sign the Enform Certified Instructor Code of Ethics. It is signed after you take the Enform Instructor Orientation (EIO) course, and before you conduct any co-teaches. Before becoming an Enform Certified Instructor, you are mentored by an experienced Certified Instructor to make sure that you understand how to properly teach an Enform course. The mentorship takes the form of observations and co-teaches, where both Certified Instructors are in the room and teaching a course. Co-teaches are helpful because they allow new Certified Instructors to teach an entire course. The mentoring Certified Instructor helps support the new Certified Instructor by giving tips, correcting procedures, and giving ideas for improvement. You will be required to have at least two co-teaches. If your mentor Certified Instructor thinks it’s necessary, you might also have to conduct a third co-teach to make sure that you are ready to teach on your own. The last step is your Accreditation Evaluation. It is not a co-teach. You must teach the entire course on your own under the supervision of an Evaluator. If your Evaluator confirms that you are ready, the Evaluator will recommend that Enform approve you as a Certified Instructor. If Enform approves you to be a Certified Instructor, you must then sign the Certified Instructor Agreement and Certified Instructor Certificate, which is specific to the course you are ready to teach. At this point, you become a fully certified Enform Certified Instructor. The application process can take a number of weeks to complete.

Maintaining Certified Instructor Status All Certified Instructor status is granted for a three-year period. In order to maintain your Certified Instructor status during those three years, you need to meet certain criteria, depending on the course you teach.

To maintain Certified Instructor status, you need to teach at least two courses a year.

If your three years pass and you do not reapply, you will be automatically decertified.

If you reapply past one year of being decertified, you will have to submit a new, fully completed Certified Instructor Application.

It is Enform’s right to deny any application without notice or explanation. The decision to approve an application for recertification is solely at Enform’s discretion.

Within the third year of your certification, (and at least three months before the end of the year) you need to let Enform know which courses you teach, and that you intend on re-applying for certification. If your request is approved, you need to schedule a formal evaluation. If you pass your evaluation, you will sign a new three-year contract or agreement.

If you reapply within one year of being decertified, you need to complete the course-specific evaluation. If your evaluation is approved, you will sign a new three-year contract or agreement.

Fees As at the implementation of the new Enform Certified Instructor processes in 2016, Enform is not charging any fees for applications, renewals, evaluations or other activities involved in the program. However, Enform reserves the right to implement reasonable fees as it deems appropriate to the circumstances.

Pre-Course Responsibilities Before you instruct a course, make sure you are aware of the following information. The purpose of the Enform Certified Instructor Code of Ethics is:

If you have concerns about Enform’s administrative procedures, course content, or course design, do not voice them while you are teaching a class or while in conversation with students. Instead, contact the Enform Evaluations team at training@enform.ca to let them know about your concerns. Certified Instructor Fitness You need to be fit for duty when you show up to teach an Enform course. Fit for duty means you are:

Dress Code When you are teaching an Enform course, your conduct and appearance reflect on Enform. We expect that, while you are instructing Enform courses or doing business in regard to Enform courses, you will wear appropriate attire. This depends on the situation, especially if you have to go to a jobsite instead of a classroom. Appropriate personal protective equipment (PPE) must also be worn when required. Dress in a business casual manner when you teach Enform courses. This means:

Class Size Enform class sizes are limited, depending on which course you are teaching. You can find a list of maximum class sizes on our website, enform.ca. This number could be limited further by the equipment available. For example, if you only have 6 SCBA masks for an H2S AliveŽ course, you will not meet the standards for a full class of 20 students. . Course Duration When Enform develops a course, it determines a minimum number of hours required to properly cover the material. Courses may not be offered in less time than what Enform requires Dividing Courses We expect that a licensed Enform course will be delivered as it was designed. Continuity of course information is critical for student learning. Enform must authorize any changes. For example, if a Authorized Training Provider asks you to teach only a part of a course, or to teach course content over several days/evenings, make sure you ask to see a written statement from Enform allowing that change. If you do not have a written approval from Enform, you cannot teach the course with any changes to it. It’s very important that Enform courses are delivered the same way, all the time, to make sure that students get a standardized learning experience. Do not alter course content or the way you teach a course without permission from Enform. It is essential that all Certified Instructors follow a uniform model of instruction. Although each Certified Instructor has a unique delivery style, the core material and essential information must be standard and consistent. All Enform licensed courses are designed on the Review, Overview, Present, Exercise, and Summarize (ROPES) method for delivering individual lessons. Make sure you use the ROPES delivery model in your classroom, so that the course is being taught as it was designed. Using the ROPES model keeps Enform course delivery standard, and allows Certified Instructors to teach a uniform message to all students, no matter their background or learning style. Special Test-Taking Requirements and Verbal Exams Assess special test-taking requirements before the exam starts. Although standard practice is for students to write their exam, some students might need an alternative form of learning assessment. You can administer a verbal exam for any student who requests this service. If you give a student a verbal exam, make sure you record it on the Attendance/Exam Record form. You cannot give a verbal exam in the presence of other students. If a student needs a verbal exam, the student must wait until the other students are dismissed. If more than one student needs a verbal exam, they have to go one at a time. One waits outside the room while the other exam is in progress.

On the front page of the Student Data Sheet (SDS), there is a section called “Photo ID Confirmed.” Acceptable proof of government photo identification can include: driver’s licence, passport, permanent resident card or firearms licence.

Check “Yes” or “No”: “Yes” if they have their photo ID, “No” if they do not or if they don’t appear to match the person on the ID.

If the student doesn’t have any photo ID, you can use a combination of alternate proof instead. Acceptable alternative identification would be one primary form of ID plus either one other primary ID, or a secondary form of ID. Primary forms of alternative ID include: Birth Certificate or Social Security Card. Secondary forms of alternative ID include: Certificate of Indian Status, citizenship or immigration papers issued by governmental authorities, Provincial Health Card, and Canadian military identification.

If a student fails to produce appropriate ID, the Certified Instructor must mark “No” and ask the student to take the course on another day.

Let students know that they will be filling out the Instructor Evaluation on the SDS after the exam.

Enform Late Policy You also need to discuss an appropriate late policy with your Authorized Training Provider. Enform grants its Certified Instructors a certain amount of discretion regarding student lateness. Many factors contribute to a student being late. It’s up to the Certified Instructor to decide whether the student can join the class or is too far behind. Enform generally does not allow admission after 30 minutes from the start of class. Note that the Certified Instructor and the Authorized Training Provider must agree on the late policy before class begins. Please be aware that each course has a specific number of hours required for certification. Whatever the reason, late may mean the student cannot be certified and will have to re-register for the course at a later date. Student Fitness and Dealing with Disruptive Students As you instruct, pay attention to your class. It’s important to notice your students’ fitness to take and understand the course. At the beginning of class, invite students to privately and confidentially disclose any medical conditions that may cause them to appear unfit to complete the course. If you have reason to believe that a student is impaired in any manner, you must ask the student to leave the course. If you dismiss a student, record the incident on the Attendance/Exam Report. It is recommended that you consult with the Authorized Training Provider prior to removing a student from a class. Sometimes students can be disruptive. Make sure you discreetly address any disruptive behaviour directly with the student as early as practical. Speak to them in private, and maintain a respectful tone. If the student continues to be disruptive, it is up to you to decide whether to allow them to stay or to remove them from the class. If you decide to ask a student to leave, talk to the Authorized Training Provider first and let them know why the action is required.

Breaks Give students a chance to eat, stretch and walk around. Short breaks (15 minutes) can happen once every two hours, while longer breaks can be scheduled after four hours. Don’t try to move the day along faster by skipping lunch. Including a morning and afternoon break, as well as lunch, will help students focus and improve their learning experience. 8:30 am

Exam Policy Delivering the exam is a crucial part of any course. It’s important that you follow Enform’s policy to make sure exams are administered in a consistent and unbiased manner. Keep exams secure at all times. You can use a personal briefcase or lock them in a cabinet. Don’t leave the exams unattended if they are not secured. Maintain the security of the exams until it’s time to write. When a student is given an exam, make sure the exam doesn’t leave the classroom. All exams for Enform licensed courses are closed book. This means that the student does not have access to the Student Manual, class notes, mobile phones, computers, or other sources of information in the learning environment. Before the exam:

Make sure that all students are seated quietly Check that every student has a pen or pencil to write the exam Erase notes from the whiteboard, and make sure no books, notes or papers are on the desks Direct students to turn off their mobile phones and put them away until exams are submitted

Before the exam, go through expectations with the students. Let them know that the exam is closed book. Advise them that cheating is unacceptable, (including copying off other students). Explain that mobile phones and other devices must be turned off during the exam so that no students gain any advantage over the others. Explain what happens if a student is caught cheating. Cheating If you catch a student cheating on the exam:

You cannot fail a student unless you have evidence that they cheated. Evidence of cheating could include such things as:

Consulting the Student Manual during the exam Searching for answers on an electronic device during the exam Directly attempting to copy another student’s answer or consult another student

Explain the outcomes of cheating. If a student is caught cheating, they must re-register for the course and a full registration cost will have to be paid again. It is recommended the Certified Instructor communicate with the Authorized Training Provider if any such incident occurs. Certified Instructor and Course Evaluations Ongoing assessment of training courses helps Enform maintain course quality. The course evaluation is on the back of the SDS. Explain the course evaluation at the beginning of class. Students fill out the evaluation before the exam is given at the end of the course. Every student who takes an Enform course is encouraged to fill out the course evaluation. Enform will follow up on any quality or other issues identified by the course evaluation. Note: You are the only one who can grade your class’s exams. If the course is a co-teach or evaluation, and you are not yet fully certified, you cannot sign off on the course. The Evaluator has to certify the course and sign the temporary certificates. The Evaluator also has to check that exams are marked properly. If a student fails an exam:

Do not give a temporary certificate to a student who has failed. Keep the temporary certificate attached to the exam, and give it to the Authorized Training Provider, who will send it in to Enform with the rest of the paperwork. There are no re-tests allowed. If a student fails, they have to take the entire course again. Time Limit All exams are limited to one hour. However, you have discretion to grant some reasonable extra time if necessary. If a student does not finish an exam, write it on the Attendance/Exam Record form, regardless of whether they pass or fail.

Temporary Certificates Temporary certificates are valid for 90 days from the date of the course. Enform sends permanent certificates to the address provided on the Student Data Sheet. As the Certified Instructor, you need to:

Sign the temporary certificate Write down your Instructor ID number legibly Write the student’s name (the one displayed on the photo ID) Record the date the certificate is being issued

Everything you write on the temporary certificate must be in ink. Do not let the students fill in the information themselves. Only fill in the temporary certificate when the student has passed the exam. Don’t fill anything in beforehand. Now the exam portion of the day is done.

Document Control and Submission Course-Related Documents Student Data Sheets (SDS): an SDS is provided for each student. It is your responsibility to explain the proper way to record information on the SDS to the students in your class. It is the students’ responsibility to make sure they fill out correct information on the SDS.

If the Authorized Training Provider submits paperwork missing the above information, Enform may return it to be corrected and resubmitted.

Mailing Address Student certificates will be mailed to the address written on the SDS. Once a student writes their address on the SDS, you cannot alter it without student consultation and approval. Enform’s policy is to mail certificates to the address written by the student on the SDS. If a company wants students to have certificates mailed to the company that paid for the course, they must make it clear to their employees, so that those employees can write it on the SDS when they take the course. Note: It is the student’s right to decide which address to write on their SDS. Authorized Training Providers must not oblige students to write their address on their SDS. Putting the Authorized Training Provider’s address on the SDS cannot be a condition of taking a course. However, the student can choose to do so if they want. Attendance/Exam Record This document collects information required by Enform to certify a class. Enform keeps it on record for a limited time. Make sure that students hand print their own information on the Attendance/Exam Record. This will ensure proof of identity. Remind your students to:

Performance Management Enform’s goal is to deliver quality training. We need professional, ethical Certified Instructors and Authorized Training Providers. As discussed earlier, Enform has responsibilities to its students, which include making sure that Certified Instructors act with honesty, integrity and professionalism. We maintain a high standard of Certified Instructor performance through our Enform Certified Instructor Performance Management Program. Enform’s Certified Instructor Performance Management Program is based on the Enform Certified Instructor Code of Ethics you sign when you become a Certified Instructor. If you violate any of the terms set out in the Code, then you are no longer performing at an acceptable level. Enform has a responsibility to hold Certified Instructors accountable for their performance. Depending on the situation, consequences may have a serious impact on a Certified Instructor’s ability to deliver Enform courses. The information in this manual is not intended to go through every possible performance management scenario. It is just to give you a guide and an understanding of how Enform reacts to performance management issues. As a condition of maintaining Enform Certified Instructor status, you must agree to be bound by the disciplinary process indicated within the Enform Certified Instructor Performance Management Program, Enform Certified Instructor Handbook, and Enform Certified Instructor Code of Ethics by signing the Enform Certified Instructors Agreement. Certified Instructors are assessed by means of mentored co-teaching, formal Evaluations, and student feedback.
