Introduction

Your benefits are an important part of your overall compensation. We are pleased to offer a comprehensive array of valuable benefits to protect your health, your family, and your way of life. This guide answers some of the basic questions you may have about your benefits. Please read it carefully, along with any supplemental materials you receive.

The information in this Benefits Guide is presented for illustrative purposes and is intended to assist employees make an informed healthcare decision. It is not intended to be medical advice nor to offer an opinion on which plan may be better for an employee and/or their families. The text contained in this guide was taken from various summary plan descriptions and benefit information.

While every effort was taken to accurately report your benefits, discrepancies or errors are always possible. In case of discrepancy between the guide and the actual plan documents, the actual plan documents will prevail. Common Phrases

Premium: an amount paid for an insurance policy; at Christian Care these are paid through payroll deduction.

Deductible: a specific amount of money that the insured person must pay before an insurance company will pay a claim. Some services, such as preventive care, or those outlined on the SBC may be covered before you meet your deductible.

Copay: a payment made by an insured individual to a provider in addition to the amount the insurance pays the provider for that service.

Coinsurance: your share of the costs of a health care service. Often, this is a percentage. For example, if a service costs $1,000 and there is a 20% coinsurance, the insurance will pay $800 of the claim and the insured person will pay $200.

Out-of-Pocket Limit/Maximum: the maximum amount of money you may pay for health services in a plan year. This may or may not include your deductible, make sure to read the SBC or SPD for more details.

Network Provider (In-Network/Out-of-Network): a network provider, or a provider that is “in- network”, is part of a group of providers that have contracted with your insurance to provide a discounted rate for service(s). Insured individuals will typically pay less for services if they are going to an in-network provider. Some insurance plans will not provide any coverage for visits to out-ofnetwork providers. Many insurance companies maintain a list of network providers on their website. If you choose to ask a provider if they are “in-network” be sure that you get a clear answer that they are “in-network”; sometimes providers will say they “accept your insurance” but this does not mean that they are “in-network”.

Qualifying Life Event: Change in circumstance that will require changes to your insurance. These events allow you to make changes or purchase new policies outside of Open Enrollment.

HDHP: HDHP stands for High-deductible health plan. You pay a larger deductible than most insurance plans in exchange for a lower premium. Typically, standard preventive care is free, but anything beyond that you will have to pay out of pocket or by using your HSA until you reach your deductible.

HSA: HSA stands for Health Savings Account. It is an account that allows you to save up money for your (or your families) health (medical, dental, vision) expenses. HSA funds are withheld from your check pre-tax (before taxes) so that you pay less taxes and save yourself some money in the long run. An HSA is not a “use it or lose it” type of account and money continues to accumulate year after year. In order to enroll in the HSA, you must enroll in the HDHP medical plan. You can only use the funds available in your account at the time.

FSA: FSA stands for Flexible Spending Account. Similar to an HSA, you save money for your health expenses pre-tax - FSA’s also offer a “dependent care” option which allows you to save money for your child-care expenses pre-tax. However, the FSA does require you to use your funds, or you may end up forfeiting them. It is a great idea for someone who knows they will have health or childcare expenses and wants to save money on taxes. An FSA will allow you to use your entire elected contribution right away after your effective date and then pay it back throughout the plan year.

EOB: Explanation of Benefits. A statement sent by a health insurance company that explains what services and/or treatments were paid for on their behalf.

SBC: Summary of Benefits and Coverage. A document required by the government that explains in plain language simple and consistent information about insurance plans.

SPD: Summary Plan Description. A breakdown of all the benefits that are available under a plan.

Benefits Eligibility

You are eligible for most benefits, with the exception of Medical, on the first day of the month following 30 days of employment or change in eligibility. If you are eligible for medical insurance, you are eligible to enroll as of the first of the month following 60 days of full-time employment.

For the purpose of enrollments, dependents are:

You, your spouse, and your dependent children who are under the age of 26 are eligible to enroll in dental and vision insurance as well as AFLAC, Legal Shield, or United Pet Care only.

You are only eligible to participate in the 403(b) Retirement Plan.

■ Biological or legally adopted or stepchildren under the age of 26.

■ An eligible child over the age of 26 who is physically or mentally incapable of self-support may continue coverage according to plan guidelines.

When and How to Enroll

All benefit enrollments for newly hired employees (within 21 days from date of hire), or current employees enrolling or making changes to enrollments during open enrollment are required to be completed online by the employee through ADP Self Service.

Employees must have established an ADP account to access the enrollment platform. All employees are eligible for an ADP account. For assistance with creating or accessing your account, please visit your campus HR department.

Flexible Benefits Plan: Cafeteria/Section 125 Plan

Christian Care’s Section 125/Cafeteria Plan allows employees to elect to have certain benefits paid for with pre-tax dollars. The difference between pre-tax and post-tax benefit premium payments is explained further below. Because some plans are not eligible to be paid “pre-tax” you could have a mixture of plans paid pre-tax or post-tax.

■ Pre-tax: If you elect to have your premiums paid pre-tax, you will be locked into the pre- tax benefit selection for the entire benefit year unless you have a qualifying life event as defined by the IRS. If you know you will not be changing your benefits election, pre-tax may be the best option for you as it can save you on your taxes and increase your take home pay.

■ Post-tax: If you elect to have your premiums paid post-tax, you will not save any money on taxes, but you may drop your post-tax plans outside of open enrollment without needing to have experienced a qualified life event.

You will elect which plans will be paid for “pre-tax” and which plans will be paid for “post- tax” by electing the appropriate plan in ADP when you make your selections.

By enrolling in a “pre-tax” plan, you understand and agree to the following:

■ Restrictions on Election Changes: On or after the first day of the plan year, I cannot change or revoke this Salary Redirection Agreement with respect to pre-tax premiums before the next anniversary date of the plan unless a change in status occurs (as defined under the plan and the Internal Revenue Code), and the change is caused by and consistent with the change in status.

■ Commencement of Coverage and Status of Prior Elections: Execution of this Salary Redirection Agreement does not begin coverage under the component benefit plans or insurance policies. The terms and conditions and actual coverage effective date of the underlying coverage will be determined under the separate benefit plans or insurance policies. Prior to the anniversary date each year, I will be offered the opportunity to add, drop, or change coverage for the following plan year. If I do not complete and return a new Salary Redirection Agreement form at that time, benefit plans or policies currently in effect will continue.

■ Use of Personal Information: In addition to and without limiting in any way the rights my employer; the plan; the service provider; and the respective agents, employees, subcontractors, and assigns may have under applicable state or federal law or regulation, I hereby specifically authorize those parties to use my personal information (including, but not

limited to benefit elections, wages, employment status, number of dependents, marital status, and health and dependent child care information) as is reasonably required to administer the plan (including evaluating and processing requests for payment of claims) and detecting and preventing fraud or misrepresentation. I further authorize my employer; the plan; the service provider; and the respective agents, employees, subcontractors, and assigns to further disclose any such personal information as is reasonably required for such purposes. I hereby expressly waive and release any claims related to the use, disclosure, or release of such information so long as the information is used in furtherance of plan administration, or to detect or prevent fraud or misrepresentation.

■ Effect of Pre-Tax Contributions on Benefits Payments: Paying for coverage on a pre-tax basis may cause insurance claim payments under health and medical coverage to be subject to federal and state taxes if claim payments (combining the total from all health and medical policies/plans) are in excess of medical expenses. Paying for disability income policies with pre-tax premiums will cause the benefits payable thereunder to be taxable. Such coverages may be funded on an after-tax basis to preserve the excludability of policy benefits.

Wellness Credits

To help employees reduce their premiums, Christian Care offers wellness credits to allow employees that lead a healthier lifestyle to reduce their premiums. The two wellness credits available are:

■ One (1) for having an annual physical completed, exercising regularly, and completing a health assessment.

■ One (1) for being tobacco free. Wellness credits are an honor-based system. Employees must complete wellness credit eligibility requirements within the communicated time frame. Failure to do

so, or enrollment in a plan for which an employee is not eligible for, will result in an adjustment to the employee’s enrollment.

Employees that mis-elect their plan and do not enroll in a plan that has a reduced premium based on wellness credits will not be given a retro-active discount. Employees who change their healthy lifestyle (for example, quit smoking) during a plan year are eligible to change their wellness credits outside of open enrollment to take advantage of the premium reduction opportunities.

Christian Care offers two plan options, a Traditional PPO Plan and a High Deductible Health Plan (HDHP) with Health Savings Account (HSA).

Things to Consider...

■ Many preventive services are covered at no cost, this includes immunizations, screenings, annual exams. Be sure to check out a full list of preventive screenings covered by your plan to ensure you are receiving all your recommended screenings at no charge!

■ Always shop around for the best price for your medical services - some providers charge more than others even though they may all be in-network.

■ Remember, some pharmacies charge different amounts for medications. You should always shop around for the best price on a medication.

■ GoodRx is a great website or smart phone app to help you see the price of medications near your location.

■ Be clear that your provider is in-network, and not just that they accept your insurance.

■ Be aware that there is NO Out of State provider coverage.

■ Health Savings Account – Christian Care will contribute $50 per month for active employees.

■ IRS Contribution Limits – The 2025 tax year maximum contribution to an HSA is $4,300 for individual coverage and $8,550 for family coverage. This includes any amount funded by Christian Care). There is a $1 ,000 additional contribution allowance for those over age 55.

Understanding your Medical ID Card

When you enroll in benefits, you will receive an ID card in the mail. You will receive one card for yourself and an additional card for your covered family member(s). You need to present your ID card every time you receive care at the doctor’s office, urgent care clinic, lab, hospital, outpatient facility and pharmacy.

Below is a sample of the information that your ID card may include:

1. Your plan administrator: Personify Health administers your plan, manages your eligibility and benefits, and pays your claims.

2. Eligibility Information: Your name, Group Number (your employer’s assigned number with Personify Health) and your Member ID.

3. Medical Network Information: Identifies the name(s) and logo(s) of the medical network that your employer contracts with to ensure the best discounts for healthcare services.

4. Pharmacy Information: Identifies the pharmacy vendor and contact information.

5. Information for Providers: Contact information that your provider can use to verify your eligibility and benefits with Personify Health.

Health Savings Account (HSA)

A health savings account (HSA) is like a savings account for healthcare. An HSA gives you an easy, safe way to lower your healthcare costs today while saving money for future healthcare expenses.

If you enroll in the High Deductible Health Plan, you will be enrolled in the Health Savings Account. You do not have to contribute to this account, but Christian Care will provide $50 per month to this account to help you pay for your health expenses. You cannot be enrolled in the HSA unless you are enrolled in the High Deductible Health Plan.

If you decide to contribute to your HSA, the funds you contribute will be set aside “pre-tax” which means that you will pay fewer taxes this year. The money in the account is yours to keep even if you change jobs, switch

healthcare plans, or retire. Unused funds are rolled over from year to year.

If you are over the age of 65 and enrolled in Medicare or receiving social security, you can no longer contribute to an HSA. The funds in your account are still yours, but you can no longer add to the account balance. You also have the opportunity to use the funds in your HSA without penalty on non-healthcare related purchases, you’ll simply pay regular income tax on the amount.

However, there is an exception that can be made under the following conditions.

An Employee over age 65 can continue to contribute to an HSA bank account IF:

■ You remain enrolled in the Christian Care HDHP (High Deductible Health Plan).

■ You do not enroll in Medicare A, B or D.

■ You are already enrolled in Medicare A, B, or D but have dis-enrolled.

■ You have not received and will not start to receive Social Security income distributions.

■ You already have been receiving Social Security income distributions, but you pay all of what you have received back to the government.

The money contributed to the account is yours to keep and will roll over year after year – no ‘use it or lose it’ rule!

Flexible Spending Account

A flexible spending account (FSA) can be used in conjunction with the Traditional FPO Health Plan, or without any enrollment in medical insurance. An FSA gives you an easy, safe way to save for healthcare products or services for you, your spouse, and your dependents. There are two options for the FSA program - a healthcare FSA and a dependent care FSA. If you are enrolled in the High Deductible Health Plan with HSA, you are not eligible to enroll in the healthcare FSA.

Simply estimate your annual healthcare expenses and make your contributions through payroll. Your total annual election amount is available on day one of your plan year. Any money left unspent in your FSA at the end of a plan year is forfeited. Christian Care does provide a 2 ½ month grace period after the plan year ends to use your funds. If you decide to enroll in an FSA, the funds you contribute will be set aside pre-tax which means that you

will pay fewer taxes this year and can result in less money spent on healthcare.

■ The 2025 tax year maximum contribution to a healthcare FSA is $3,300.

■ The 2025 tax year maximum contribution to a dependent care FSA is $5,000.

Dental Plan

Dental benefits are effective on the 1st of the month following 30 days of full-time or part-time employment, with a qualifying event, or during open enrollment for a March 1st effective date.

Three dental plans are offered:

■ PPO Low Option 1 is a great option for those looking for an affordable plan, or those who only need minor services. You will need to visit an “in-network” dentist.

■ PPO Value Option 2 is a great option if you want to have the highest coverage level. You will need to visit an “in-network” dentist.

■ PPO NAP Option 3 is the best option if your dentist is out-of-network, this plan costs the same as Option 2, but covers a lower percentage of services so you can visit out-of-network dentists.

Short-term disability (STD) is sometimes called “paycheck insurance”. If you become unable to work due to an illness, injury, or medical condition, short-term disability may pay you up to 70% of your wages while you are out of work. 403(b) Retirement Plan

Christian Care provides an opportunity for ALL employees to defer pre-tax contributions to the 403(b) plan through payroll deductions up to the annual ERISA allowed pre-tax deferral and pre-tax catch-up contribution limits. “Critical Need” Employee Referral Program

On December 5. 2014, The Senior Executive Team determined that it is necessary to update the Employee Referral Program to provide additional incentives for “critical need” employee referrals.
