INTRODUCTION

Whether you are a new employee enrolling into your benefits for the first time or considering your benefits during open enrollment, this guide is designed to help you through the process.

Lohman Company is proud to offer you a broad range of benefit options. You can choose from a number of plans including medical, dental, vision, life insurance and voluntary supplement programs. In addition, we provide health care and dependent care reimbursement accounts to assist employees in managing their out-ofpocket expenses with before-tax dollars.

Please take the time to read this information and ask questions so you can make the best benefits decisions for both you and your family.

Enrollment Information

OPEN ENROLLMENT

Open Enrollment elections are due in Employee Navigator no later than December 15, 2024.

BENEFIT ELIGIBILITY

Active, full-time Partners & All Other employees are eligible for benefits the first of the month following 30 days from full-time employment. All Active, full-time Project Professionals are eligible for benefits the first of the month following 60 days from full-time employment. If you do not enroll within 30 days of your eligibility date, you will not be able to enroll until the next annual open enrollment period.

Open Enrollment is your annual opportunity to enroll. Unless you have an IRS qualified Life Event during the plan year, you cannot make changes to your benefit elections until next open enrollment.

Qualifying Life Event

The elections you make during Open Enrollment or at your initial benefits eligibility will remain in effect for the plan year (January 1, 2025 – December 31, 2025). During that time, if your life or family status changes according to the recognized events listed below, you are permitted to revise your benefits coverage to accommodate your new status. You may make benefits changes by contacting Human Resources and providing the proper documentation.

IRS regulations govern under what circumstances you may make changes to your benefits, which benefits you can change and what kinds of changes are permitted.

■ All changes must be consistent with the qualifying life event.

■ In some cases, you can change your benefit plan and modify the level of coverage (such as add or delete dependent).

Any changes in benefit levels must be completed within 31 days of the qualifying life event.

HEALTHCARE REFORM REMINDERS

■ Your medical plans are in compliance with the Healthcare Reform legislation.

■ Pre-existing conditions will not disqualify employees from coverage.

■ Preventive services are covered 100% under your medical plans, including men & women’s health benefits, annual physical, immunizations, etc.

■ There is no longer a lifetime limit per covered individual.

COBRA

In most cases, if your employment ends, benefits will terminate on the last day of the month in which you worked. Benefits will end on the day of termination in cases of employee fraud.

Through federal legislation known as the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA), you may choose to continue coverage by paying the full monthly premium cost plus an administrative charge of 2% (if applicable).

The right to continuation of coverage ends at the earliest of the date:

■ your COBRA Continuation Period expires

If you ever misplace your ID card, you can visit the carrier website listed below or download their mobile app.

Members may register at azblue.com. Once you register, members may print or request additional ID cards (if you are enrolled in medical coverage, an ID card will be mailed to your home address). You will also have access to copies of your EOB’s and educational information.

Employee Navigator

Employee Navigator is your 24/7 Online Benefits Site. This portal will allow constant access to anything and everything benefits related. You can find your enrollment summary as well as copies of your Summary of Benefits and Coverage (SBC’s).

You will receive an Open Enrollment email from Employee Navigator which will allow you to register for the portal (if you haven’t already) and login to confirm your benefits. If you do not receive this email, please check your junk mail. It is not required that you register via the link in the email, you can also go directly to employeenavigator.

Dental & Vision Only

Members may register at MetLife.com. Once you register, members may print an ID card. (MetLife will not mail a physical ID card to your home for dental and vision)

Open Enrollment is passive this year. This means you are only required to participate if you are a new hire or would like to make a change. If you do not participate, existing elections and declinations will rollover into the replacement plans.

Medical Plans

Lohman Company offers Medical Coverage through BCBS of AZ, utilizing the Alliance & Statewide Networks. You and the company share the cost of the healthcare benefits. Your contribution depends upon the plan you choose and the number of covered dependents, if any. Group # 039962

Enter a valid email address, password, the best number for our doctors to reach you, your preferred language, and accept terms and conditions.

What is HealthiestYou?

HealthiestYou is a healthcare service that offers convenient, confidential access to quality doctors 24/7, anytime, anywhere.

By scheduling a visit with one of our U.S. board-certified and licensed medical doctors, you can be diagnosed, treated, and prescribed medication if necessary.

What can I use HealthiestYou for?

HealthiestYou can help you with everyday, non-emergency healthcare issues, including sinus problems, allergies, flu symptoms, and much more. Skip the waiting room and the trip to the ER. We’re here to help you feel better, faster, and get you back to living your life.

Does HealthiestYou replace my doctor?

No. HealthiestYou doesn’t replace your primary care doctor. HealthiestYou should be used for non-emergency illnesses when it is not convenient to get to the doctor or it is outside of regular office hours.

How do I set up my account?

Download the HealthiestYou app, visit the website, or call the number below to set up your account.

Do I need to have my insurance information available?

No. HealthiestYou is a separate benefit, and your insurance information is not required to have a visit.

How do I access HealthiestYou?

The service can be accessed by app, web, or phone, and visits are available by phone or video.

Can my family use HealthiestYou?

This varies depending on your specific HealthiestYou plan. Most plan designs allow you to use the HealthiestYou service for you, your spouse, and your dependents. Dependents over 18 years old must call our service center to request a visit. For dependents under 18 years old, the primary account holder must request a visit on their behalf through the app, website, or by phone.

How much does it cost?

Free—there are no visit fees when using HealthiestYou for everyday medical care.

Is there a time limit when talking to the doctor? And am I charged more for taking longer?

There is no time limit for visits, and there is no extra charge for longer doctor visits.

Who are the HealthiestYou doctors?

HealthiestYou doctors are U.S. board-certified internists, family doctors, and pediatricians. They average 20 years of experience and are licensed to practice in your state.

Can HealthiestYou physicians prescribe medications?

Yes, when medically appropriate, doctors can prescribe medications. If a prescription is not required, the doctor may provide the member with instructions for managing symptoms or following up with their primary care doctor.

Can my primary care doctor get a record of my HealthiestYou visit?

With your consent, we’ll send an electronic copy of your HealthiestYou visit to your primary care doctor.

Can I use HealthiestYou while traveling?

HealthiestYou is available in all 50 states, so you can use the service while traveling within the United States. Some restrictions may apply.

Health Savings Account (HSA)

With any of the plans offered, you will have the option of opening an HSA provided by Lively. An HSA is a tax advantaged savings and spending account that can be used to pay for qualified health care expenses.

THERE ARE TWO COMPONENTS TO AN HSA-BASED COVERAGE PLAN:

1. A qualified health plan is the insurance component that provides medical coverage for you and your family.

2. An HSA bank account with Lively which can be funded by pre-tax payroll contributions from you. Lohman Company will contribute $100 per month into your HSA bank account.

HOW AN HSA WORKS:

1. Enroll in an HSA Compatible health plan

2. Contribute to your HSA by payroll deductions on a pre-tax basis:

■ Up to $4,300 for single coverage

■ Up to $8,550 if you enroll a dependent

■ An additional $1,000 if you are age 55 or older

The money contributed to the account is yours to keep and will roll over year after year –no ‘use it or lose it’ rule!

3. With your HSA debit card, use your HSA funds to pay for qualified expenses such as:

■ copays

■ deductibles

■ chiropractor

■ dental treatment

HSA DEBIT CARD

■ Lohman Company has partnered with Lively Bank. You will use your HSA debit card to pay for qualified medical, dental or vision expenses out of your Lively HSA Bank account.

■ hearing aids

■ glasses/ contacts

■ prescriptions

■ over the counter drugs and medications

■ If you are newly enrolling in the HSA medical plan and would like to receive the bank account contribution from Lohman Company, you will need to be enrolled in the Lively HSA Bank account.

HSA ELIGIBILITY

To make tax-free contributions to an HSA, the IRS requires that:

■ If you have an HSA Account from a prior employer, you are still able to use those remaining funds.

■ You are covered by an HSA qualified plan (such as the HDHP plan)

■ You are not enrolled in any other plan that is not considered a High Deductible Health Plan.

■ You are not enrolled in Medicare, Medicaid, medical FSA, or have other military health benefits.

NOTE: Under the CARES Act, over-the-counter medicines are NOW an HSA qualified expense regardless of having a Doctor’s prescription. (See IRS Publication 502 for details).

Lohman Company offers a Vision plan through MetLife. If you choose an out-of-network provider, benefits are reimbursed based on the service and materials.

Employer-Paid Basic Life and AD&D

BASIC LIFE AND AD&D

Lohman Company provides employees with Basic Life & AD&D through MetLife. This benefit come at no charge to all full-time benefit eligible employees, excluding Project Professionals.

1x ANNUAL SALARY UP TO $100,000

Benefits reduce by 35% at age 65, 50% at age 70, and 65% at age 75.

POLICY FEATURES

■ Living Care/Accelerated Death Benefit: 80% of the amount of the life insurance benefit is available to you if you become terminally ill (not to exceed $500,000).

■ Waiver of Premium: If disabled, your life insurance benefits will continue without payment of premium. (disabled prior to 60, waiting period 9 months coverage continues to 65)

■ Portable: Allows you to continue this insurance program if you leave the company for any reason, without having to provide evidence of insurability. (Rates based on Attained Age).

■ Conversion: If your employment ends, you may apply for an individual life insurance policy. (Rates based on Attained Age)

Are Your Beneficiaries Up to Date?

■ Beneficiaries are individuals or entities that you select to receive the benefits from your policy.

■ You can change your beneficiary designation at any time.

■ You may designate a sole beneficiary or multiple beneficiaries to receive payment in the percent you indicate.

■ To select or change your beneficiary, please login to Employee Navigator, select “Benefits” and then “Beneficiaries”.

Voluntary Life and AD&D

Eligible employees have the option to purchase additional Term Life and AD&D coverage with MetLife. Coverage is available for employees as well as their dependents.

VOLUNTARY LIFE AND AD&D BENEFITS

■ Employee: Increments of $10,000 up to 5x Salary or $500,000. (Guaranteed Issue $100,000)

■ Spouse: Increments of $5,000 up to $100,000, not to exceed 50% of EE (Guarantee Issue $25,000)

■ Child(ren): Flat Amount of $1,000 / $2,000 / $4,000 / $5,000 / $10,000 (Guarantee Issue $10,000)

*No reduction in benefits due to age

Please note:

■ In order to enroll dependents, you must also enroll yourself in Voluntary coverage.

■ Guarantee Issue is a predetermined dollar amount that the policy allows you to elect for Voluntary Life Insurance without the completion of a Statement of Health form. GI is only available when you are a new hire or first become eligible for Life Insurance.

■ If you elect more than the Guarantee Issue amount, you will need to complete a Statement of Health form for additional coverage to be approved.

■ If you are currently enrolled in Voluntary Life and AD&D and would like to increase your coverage, you will be allowed to increase your coverage by $10,000 as an employee at Open Enrollment without the completion of a Statement of Health, up to the Guarantee Issue amount.

Employer Paid Short-Term Disability

Lohman Company provides employees with Short Term Disability through MetLife. A disability can happen to anyone—a back injury, pregnancy, or serious illness can lead to months without a regular paycheck. If you’re unable to work for a short period of time due to a non-work-related condition, illness or injury, Short-Term Disability Insurance offers financial protection by paying you a portion of your earnings. This benefit come at no charge to all full-time benefit eligible employees, excluding Project Professionals.

SHORT-TERM DISABILITY BENEFITS

Benefit Amount: Up to 60% of weekly salary with a maximum benefit of $1,000/week

Elimination Period: 0 Day Injury / 7 Day Illness

Benefit Duration: 13 Weeks

This benefit come at no charge to all full-time benefit eligible employees, excluding Project Professionals.

Benefit Amount: Up to 60% of monthly earnings with a maximum benefit of $10,000/month
