TERMS TO KNOW AS YOU READ THROUGH THIS BOOKLET:

Copay

The fixed dollar amount you must pay directly to a medical provider at the time certain covered services are received, such as office visits or prescription drugs .

Deductible

The fixed dollar amount you must pay for covered services each coverage year before the insurance company begins to pay benefits .

Coinsurance

Sharing, between yourself and the medical plan, of the cost of covered services after you have paid the deductible . Out-of-Pocket Maximum

The maximum dollar amount you have to pay under the terms of the insurance company’s contract in a coverage year for covered services Note that you would be responsible for anything exceeding the Plan’s Allowable per Benefit Maximum or Coverage Year Limit should you reach that limit within a Plan Year

What can I expect from Fairos?

How do I access medical care?

Your health plan is an open network This means every doctor / provider is eligible to deliver services to you If the front desk staff has any questions about your insurance that you cannot answer, advise them to call HealthComp at 800-843-3831

How do I pay my medical bill?

1 Compare the price of your medical bill to your Explanation of Benefits (EOB) before making any payment

2 You will receive your EOB in the mail

3 . If the price of your medical bill matches your patient’s responsibility on the EOB, you can pay the bill

4 If the price of your medical bill does not match your patient’s responsibility on the EOB, this is a balance bill

What do I do if I receive a balance bill?

1 Call HealthComp at 800-843-3831 to confirm you have a balance bill

2 HealthComp will confirm & then transfer you to your personal member advocate at Fairos

3 You will know your Fairos Advocate’s name and have direct access to them via phone & email

4 Your Advocate will set you up on the Fairos portal so you can track the status of your balance bill

5 . Expect frequent updates from your Fairos Advocate at a minimum of every 15 calendar days until it’s resolved

What do I tell the provider if they do not recog nize my insurance or Member ID card?

Fairos is an open network that allows you to access the provider of your choice They can call HealthComp at 800843-3831 for any questions regarding your Fairos benefit plan HealthComp’s number is on the back of your Member ID Card

What if my doctor or hospital has questions about my insurance plan?

What happens if the provider requests payment at time of service?

Confirm with the provider that they called HealthComp to verify benefits Any applicable co-pays will need to be paid at time of service

How do I find a provider that will accept my insurance?

Fairos is an open network, meaning you can go to your provider of choice If you’re having difficulty finding a provider to accept your insurance, you can call HealthComp at 800-8433831 HealthComp will assist in finding an alternative provider that can provide the medical services you need

What happens if the provider won’t accept my insurance?

Your doctor or hospital should call HealthComp HealthComp will handle any questions regarding your Fairos benefit plan

Who is FairosRx?

FairosRx is a Pharmacy Benefit Manager (PBM) located in Amarillo, TX We partner with employers to administer prescription benefits for their covered enrollees and dependents

Who do I contact with questions about my prescription benefits?

The Member Services team at FairosRx is here to assist you by answering questions related to your prescription benefits such as drug coverage, copays and out of pocket amounts, prior authorizations, network pharmacies, mail order and more! How do I create a FairosRx member portal account?

Creating a FairosRx member portal account is easy! Please have your prescription/medical ID card available as you will need information from your card for account registration

Please follow these simple steps:

1 Go to fairosrx com and select Member Login

1 Enter the subscriber’s last name, date of birth and member ID number The subscriber is the employee who carries the benefits

1 Select the member for whom you are creating the account and verify their date of birth

1 Enter a username, email address and password

1 You’re done!

How do I find pharmacies in my network?

FairosRx has over 67,000 pharmacies in our nationwide network . Members can view a listing of participating pharmacies by going to the Pharmacy Lookup tool on the FairosRx Member Portal or by calling member services at 833-4649600 Pharmacies can be filtered by zip code and 24-hour locations

How do I determine my copay or out of pocket amount?

To determine your copay or out of pocket amount, please refer to your benefit documents, use the Medication Lookup tool on the FairosRx member portal or call member services at 833-464-9600

How do I know if my drug is covered?

To determine if a drug is covered under your prescription benefits, please refer to your benefit documents, use the Medication Lookup tool on the FairosRx member portal or call member services at 833-464-9600

What is a formulary?

The formulary is a list of generic and brand name medications used to help you determine your copay A group of doctors and other experts choose the drugs on formulary based on their e ectiveness, safety and cost The formulary can be accessed by logging into your FairosRx member portal account and selecting Benefit Documents

What if my medication is not listed on the formulary?

Depending on your benefits, if a brand medication is not listed on the formulary, the brand is considered non-preferred or may be excluded For lower cost and formulary alternatives, please contact member services at 833-464-9600

What is a prior authorization?

Certain medications require an approval before they are covered To determine if a medication requires prior authorization, please go to Benefit Documents on the FairosRx member portal or contact member services at 833-464-9600 .

How do I know if my medication has quantity limits?

How do I sign up for Mail Order?

Members can register for mail order by completing one of the following:

■ Go to fairosrx com to create a member portal account Select the My Prescriptions feature and then click on Visit Mail Order under the Mail Order Prescriptions tab

■ Print, complete and mail your Mail Order Registration Form . The form can be found at fairosrx com under the Member section

How can I order refills?

Mail order refills can be ordered through the FairosRx member portal or automated phone system at 833-464-9600

How do I file for reimbursement if I paid out of pocket for my prescription?

If you paid out of pocket for your prescription(s) and need to file for reimbursement, please complete a Prescription Reimbursement Request Form The form can be found at fairosrx com under the Member Resources section Please note that your original pharmacy receipt must be submitted with your reimbursement request .

International Alliance Group offers LifeShield Group Term Life Insurance Plan at no cost to the Member

Coverage for each full-time benefit eligible member is $25,000

Life insurance provides protection for those who depend on you financially Your need varies greatly due to age, number of dependents, dependent ages and your financial situation Accidental Death and Dismemberment (AD&D) benefits provide a benefit to you or your beneficiary if you are seriously injured or die in an accident

HOW IT PAYS

100% benefit amount if your death is the result of an accident, in any year You also have the option to convert this term life policy into a whole life policy up to age 70

Year 1

If a death is to occur in the first year the benefit payout is 120% of all premiums paid

Year 2

If a death is to occur in the second year, the benefit payout is 20% of the full amount

Year 3

If a death is to occur within or after the third year, the payout is the full benefit amount

Zero

International Alliance Group enrolls members in health insurance at no cost to the member, unless the member’s District partner chooses to offer coverage . Members enrolled in IAG’s health benefits are only responsible for any out of pocket expenses they may incur

Your best defense against cancer is awareness and prevention.

HealthComp’s Cancer Awareness program is designed to develop a culture of health, create awareness, and ultimately reduce your risk of cancer through early detection and intervention.

What

you can expect from the program

If you, or a covered dependent fall into one of the screening categories below, you will be receiving some helpful information in the mail and a phone call from our Wellness staff to answer any questions that you may have about the screening.

Let’s not stop there! Pass it on, encourage someone else to get their screening done. We can do great things for our health and wellbeing.

Colon cancer

Colorectal cancer almost always develops from precancerous polyps (abnormal growths) in the colon or rectum. Screening tests can find precancerous polyps, so that they can be removed before they turn into cancer. Screening tests can also find colorectal cancer early, when treatment works best.

You should begin screening for colorectal cancer soon after turning 50, then continue getting screened at regular intervals. However, you may need to be tested earlier than 50 or more often than other people if:

• You or a close relative have had colorectal polyps or colorectal cancer

• You have inflammatory bowel disease

• You have genetic syndromes such as familial adenomatous polyposis (FAP) or hereditary non-polyposis colorectal cancer

Speak with your doctor about when you should begin screening and how often you should be tested.

Breast cancer

The best way to find breast cancer early is with regular screenings:

• Women age 35

• Bi-annually 40– 49

• Women age 50 years or older, be sure to have a screening mammogram every year

You can help lower your risk of breast cancer in the following ways:

• Get screened for breast cancer regularly

• Control your weight and exercise

• Know your family history of breast cancer

Cervical cancer

Can cancer of the cervix be prevented?

Most cervical cancer can be prevented. There are 2 ways to prevent this disease. The first way is to find and treat pre-cancers before they become cancer, and the second is to prevent the pre-cancers. Finding and treating pre-cancers before they become cancer A well-proven way to prevent cancer of the cervix is to have testing (screening) to find pre-cancers before they can turn into cancer.

You should start getting regular Pap tests at age 21, or within three years of the first time you have sex—whichever happens first. The Pap test, which screens for cervical cancer, is one of the most reliable and effective cancer screening tests available.

Prostate cancer

Prostate cancer screening means looking for cancer before it causes symptoms. This helps to find cancer at an early stage when it may be easier to treat. Informed decision making occurs when a man:

• Understands the nature and risks of prostate cancer

• Understands the risks, benefits, and alternatives to prostate cancer screening

• Participates in decision making at a level he desires

• Makes a decision consistent with his preferences and values, or defers the decision to a later time

Discuss with your doctor to make screening decisions that are best for you.

Nurturing Together

HealthComp has partnered with your employer to offer FREE access

During pregnancy, we sometimes need someone to turn to for advice and information.

HealthComp’s Nurturing Together Program provides you with access to prenatal nurses and their expertise in the care of parents and newborn babies. Your prenatal nurse remains available to you throughout your pregnancy and six weeks postpartum to assist you in obtaining information and care you need such as:

•Pregnancy & Newborn Baby Care

•Support for Pregnancy Related Issues (nausea, vomiting, diabetes, high blood pressure)

•Finding Network Providers (Obstetrician and/or Pediatrician)

•General Parent Health Topics

• Community resource referrals

• Obtaining a breast pump
