Route 66 Motorsports is proud to offer you a broad range of benefit options. You can choose from a number of plans including medical, dental, vision, life insurance and voluntary supplement programs. In addition, we provide health care and dependent care reimbursement accounts to assist employees in managing their out-ofpocket expenses with before-tax dollars.

Please take the time to read this information and ask questions so you can make the best benefits decisions for both you and your family.

Enrollment Information

OPEN ENROLLMENT

Open Enrollment must be completed via Employee Navigator between June 4th - June 13th. This is your one time per year to make changes.

BENEFIT ELIGIBILITY

Active, full-time employees are eligible for benefits the first of the month following 60 days from full time employment. If you do not enroll within 30 days of your eligibility date, you will not be able to enroll until the next annual open enrollment period.

Open Enrollment is your annual opportunity to enroll. Unless you have an IRS qualified Life Event during the plan year, you cannot make changes to your benefit elections until next open enrollment.

Qualifying Life Event

The elections you make during Open Enrollment or at your initial benefits eligibility will remain in effect for the plan year (July 1, 2024 – June 30, 2025). During that time, if your life or family status changes according to the recognized events listed below, you are permitted to revise your benefits coverage to accommodate your new status. You may make benefits changes by contacting Human Resources and providing the proper documentation.

IRS regulations govern under what circumstances you may make changes to your benefits, which benefits you can change and what kinds of changes are permitted.

■ All changes must be consistent with the qualifying life event.

■ In some cases, you can change your benefit plan and modify the level of coverage (such as add or delete dependent).

Any changes in benefit levels must be completed within 31 days of the qualifying life event.

HEALTHCARE REFORM REMINDERS

■ Your medical plans are in compliance with the Healthcare Reform legislation.

■ Pre-existing conditions will not disqualify employees from coverage.

■ Preventive services are covered 100% under your medical plans, including men & women’s health benefits, annual physical, immunizations, etc.

■ There is no longer a lifetime limit per covered individual.

COBRA

In most cases, if your employment ends, benefits will terminate on the last day of the month in which you worked. Benefits will end on the day of termination in cases of employee fraud.

Through federal legislation known as the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA), you may choose to continue coverage by paying the full monthly premium cost plus an administrative charge of 2% (if applicable).

benefit plan immediately preceding the employee’s COBRA event has the right to continue his or her medical, dental, vision, or Flexible Spending Accounts (FSA) plan.

The right to continuation of coverage ends at the earliest of the date:

■ you, your spouse or dependents become covered under another group health plan; or,

■ you become entitled to Medicare; or,

■ you fail to pay the cost of coverage; or

■ your COBRA Continuation Period expires

If you ever misplace your ID card, you can visit the carrier website listed below or download their mobile app.

Medical

Members may register at mycigna.com. Once you register, members may print or request ID cards through the myCigna employee portal. Additionally, you will have access to copies of your EOB’s and educational information. (Cigna will not mail a physical ID card to your home for medical)

Employee Navigator

Employee Navigator is your 24/7 Online Benefits Site. This portal will allow constant access to anything and everything benefits related. You can find your enrollment summary as well as copies of your Summary of Benefits and Coverage (SBC’s).

You will receive an Open Enrollment email from Employee Navigator which will allow you to register for the portal (if you haven’t already) and login to confirm your benefits. If you do not receive this email, please check your junk mail. It is not required that you register via the link in the email, you can also go directly to employeenavigator.com,

Dental & Vision Only

Members may register at MetLife.com. Once you register, members may print an ID card. (MetLife will not mail a physical ID card to your home for dental and vision)

To download the mobile app, search MetLife US App on your smartphone App Store.

click ‘Login’ and then select ‘Register as a new user’. From there, you will be required to enter personal identifying data as well as the following company identifier: R6M

The Open Enrollment window opens at 12am PST on Tuesday, June 4th and closes at 11:59pm PST on Thursday, June 13th. Open Enrollment is passive and you are only required to login and confirm your elections if you are a new hire or want to make changes to your plans/enrollment tier.

Route 66 will now offer your Medical Coverage through CIGNA utilizing the nationwide Open Access Plus network. You and the company share the cost of these healthcare benefits. Your contribution depends upon the plan you choose and the number of covered dependents, if any. Group # 650465

Health Savings Account (HSA)

If you choose to enroll in the High Deductible Health Plan (HDHP), you will have the option of opening an HSA provided by HealthEquity. An HSA is a tax advantaged savings and spending account that can be used to pay for qualified health care expenses.

THERE ARE TWO COMPONENTS TO AN HSA-BASED COVERAGE PLAN:

1. A qualified health plan is the insurance component that provides medical coverage for you and your family. This health plan includes a deductible of $1,600 for individuals and $3,200 for family coverage

HOW AN HSA WORKS:

1. Enroll in an HSA Compatible HDHP health plan

2. Contribute to your HSA by payroll deductions on a pre-tax basis:

■ Up to $4,150 for single coverage

■ Up to $8,300 if you enroll a dependent

■ An additional $1,000 if you are age 55 or older

2. An HSA bank account with HealthEquity which can be funded by pre-tax payroll contributions from you.

The money contributed to the account is yours to keep and will roll over year after year –no ‘use it or lose it’ rule!

3. With your HSA debit card, use your HSA funds to pay for qualified expenses such as:

■ copays

■ deductibles

■ chiropractor

■ dental treatment

HSA ELIGIBILITY

To make tax-free contributions to an HSA, the IRS requires that:

■ You are covered by an HSA qualified plan (such as the HDHP plan)

■ hearing aids

■ glasses/ contacts

■ prescriptions

■ over the counter drugs and medications

■ You are not enrolled in any other plan that is not considered a High Deductible Health Plan.

■ You are not enrolled in Medicare, Medicaid, medical FSA, or have other military health benefits.

Dental Plan

Route 66 offers a Voluntary Dental plan through Metlife. You can choose any Dentist, however you will pay less out of pocket if you use a Metlife PPO Network contracted dentist. Group # 5396295

Vision Plan

Route 66 offers a Voluntary Vision plan through MetLife utilizing the Superior Vision Network which includes Costco Optical, LensCrafters and Visionworks retail clinics. If you choose an out-of-network provider, benefits are reimbursed based on the service and materials. Group # 5396295 METLIFE

Employer-Paid Basic Life and AD&D

BASIC LIFE AND AD&D

Route 66 provides employees with Basic Life & AD&D through MetLife. These benefits come at no charge to all full-time benefit eligible employees. Group # 5396295

$25,000 IN LIFE AND AD&D

Benefits reduce to 65% at age 65 and to 50% at age 70.

POLICY FEATURES

■ Living Care/Accelerated Death Benefit: 80% of the amount of the life insurance benefit is available to you if you become terminally (12 months or less to live).

■ Waiver of Premium: If disabled, your life insurance benefits will continue without payment of premium. (disabled prior to 60, waiting period 9 months coverage continues to 65)

■ Conversion/Portability: If your employment ends, you may apply for an individual life insurance policy. Must apply within 30 days of coverage termination.

Are Your Beneficiaries Up to Date?

■ Beneficiaries are individuals or entities that you select to receive the benefits from your policy.

■ You can change your beneficiary designation at any time.

■ You may designate a sole beneficiary or multiple beneficiaries to receive payment in the percent you indicate.

■ To select or change your beneficiary, please make changes through Employee Navigator.

Voluntary Life and AD&D

Eligible employees have the option to purchase additional Term Life and AD&D coverage with MetLife. Coverage is available for employees as well as their dependents.

VOLUNTARY LIFE AND AD&D BENEFITS

■ Employee: $10,000 increments up to 5x earnings or $500,000 benefit maximum. (Guaranteed Issue

$100,000)

■ Spouse: $5,000 increments up to $100,000 not to exceed 50% of employee benefit. (Guaranteed Issue

$25,000)

■ Child(ren): $10,000

*No reduction in benefits due to age

Please note:

■ In order to enroll dependents, you must also enroll yourself in Voluntary coverage.

■ Guaranteed Issue is a predetermined dollar amount that the policy allows you to elect for Voluntary Life Insurance without the completion of an Evidence of Insurability (EOI) form. GI is only available when you are a new hire or first become eligible for Life Insurance.

■ If you elect more than the Guaranteed Issue amount, you will need to complete an Evidence of Insurability (EOI) form for additional coverage to be approved.

■ If you previously waived Voluntary Life and AD&D coverage when you were initially eligible, you will be required to complete a Statement of Health form.

■ If you are currently enrolled in Voluntary Life and AD&D and would like to increase your coverage, you will be allowed to increase your coverage by 1 increment ($10,000) at Open Enrollment without the completion of an EOI up to the Guaranteed Issue amount.

Voluntary Short-Term Disability

Route 66 provides employees with the option to purchase Voluntary Short-Term Disability insurance through MetLife. A disability can happen to anyone—a back injury, pregnancy, or serious illness can lead to months without a regular paycheck. If you’re unable to work for a short period of time due to a non-work related condition, illness or injury, Short-Term Disability Insurance offers financial protection by paying you a portion of your earnings.

Benefit Amount: Up to 60% of weekly salary (base + commissions) with a maximum benefit of $500 per week

Elimination Period: 0 Day Injury / 7 Day Illness

Voluntary Long-Term Disability

Route 66 provides employees with the option to purchase Voluntary Long-Term Disability insurance through MetLife. Long-Term Disability insurance helps protect your paycheck if you’re unable to work for a long period of time after a serious condition, injury or sickness.

401(k)

Route 66 offers a 401(k) plan to their full-time employees upon completion of their 1 year anniversary.

The 401(k) policy is written through John Hancock and your Employer will provide you with a 3% contribution, which is based off of your annual compensation.

Access your 401K account at myplan.johnhancock.com or download the John Hancock Retirement App in the app store.

Medicare Support

Strategic Growth Insurance Associates (SGIA) provides free personalized assistance to find a healthcare supplement that best fits your individual needs.

SGIA services include:
