VEN BEFORE the inception of national Breast

Cancer Awareness Month in 1985, Dr. Kamilia Kozlowski, a Clinical Breast Radiologist, launched Knoxville Comprehensive Breast Center, or KCBC as it is commonly known. Recognizing the challenges that women with breast related health issues faced, Dr. Kozlowski founded KCBC in 1983. A woman friendly, vertically oriented, streamlined, and cost-effective alternative to the fragmented and inefficient system of breast health care then in existence, Knoxville Comprehensive Breast Center has been at the forefront of providing women with the dedicated education, screening, diagnosis, and treatment resources needed to combat breast cancer. A pioneer in her field, Dr. Kozlowski and her team of 73 compassionate and caring team of health care professionals have served as an inspiration of other independent medical centers across the country dedicated exclusively to women’s breast health. During the month of October and the 32nd anniversary of Breast Cancer Awareness Month, it is indeed our pleasure to feature Knoxville Comprehensive Breast Center. EK: Dr. Kozlowski recognized early on in her career that women’s breast health care was being delivered in a fragmented and inefficient manner. How was that impacting women with breast related health issues? KCBC: In the late ’70s or early ’80s, modern mammography began with the development and use of mammographic machines with better resolution and lower radiation doses than the general radiology machines that were being used for breast imaging at the time. These machines were located in the general radiology department at the hospital, so women had to sit with other patients having all types of x-ray procedures. There were no breast centers in Knoxville at the time, and less than 10 independent centers existed across the United States. In those early years, women with an abnormal mammogram report were referred to a general surgeon for further evaluation. Generally, a physical exam was performed and they were scheduled for surgery. The period of time between their appointments and the period of time for women to get the results of their testing could sometimes take weeks. Dr. Kozlowski realized there was a better, more vertically oriented way to provide breast healthcare to women and established KCBC in May of 1983 in order to provide it to the women of Knoxville. Dr. Kozlowski found that if there was an indeterminate mammographic finding, breast ultrasound was very useful in evaluating whether it was benign or suspicious enough to warrant a fine needle biopsy. If the biopsy was essential, she performed it at the time of the initial visit. Women received the results of their breast imaging studies before leaving the office. This placed breast health care in the hands of the clinical breast radiologist, who was then responsible for examining the patient, communicating their negative results, and making the diagnosis of breast cancer. The previous way of referring the woman from one doctor’s office to another with a delay in diagnosis only increased the patient’s anxiety. Dr. Kozlowski was one of the first dedicated clinical breast radiologists in the country who took on the role as the primary doctor for the diagnosis of breast disease. These clinical breast radiologists were no longer just sitting in the reading room; they were also examining patients and interacting with them, taking care of their breast health needs and questions. Breast cancer diagnosis was no longer a surgical specialty, but rather an imaging specialty for breast cancer diagnosis 6

with the clinical breast radiologist accepting the role of the patient’s primary care breast doctor. EK: How has KCBC’s concept of combining education, screening, diagnosis, and treatment within a single medical center dedicated to women’s breast care helped further women’s breast health in our community? KCBC: Within 18 months of opening KCBC, the hospitals in the area updated their own mammographic equipment to state-of-theart mammographic machines available at that time. Soon afterwards the hospitals began to move their mammographic equipment into a specified area separate from the general radiology department. Dr. Kozlowski was the primary agent in changing breast health care delivery in Knoxville not only with the development of KCBC, but also by indirectly influencing change in the surrounding hospitals. EK: There are a variety of other area medical

facilities that offer similar health related services. At KCBC, however, you are dedicated 100% to women’s breast health care. Why should your commitment to remaining focused exclusively on breast health be important to women? KCBC: Of all the cancers that afflict women, breast cancer is the most emotionally charged cancer. It is closely interconnected with a woman’s physical appearance and her sensuality and intimacy. It is a very sensitive issue for women to undergo yearly imaging and possibly be faced with being told she has indeterminate findings or, worst yet, that she has breast cancer. At KCBC, although we offer similar services to other medical facilities in our area, we offer a much better experience in the delivery of those services. Our clinical breast radiologists specialize in, and read, only one type of imaging all day long. We specialize in breast health care and we are not distracted from that… ever. Women are treated in a

kindhearted manner from the moment they open our doors and enter into our “spa-like” setting. At KCBC, patients are treated compassionately throughout every stage of their visit. KCBC will always be dedicated 100% to women’s breast health care. Because we are exclusively devoted to this field and remain an independent breast center, we are, and always have been, free to incorporate the newest technology and treatments in the realm of breast health care. Through the last 34 years, KCBC has led the way in Knoxville with being the first to institute the latest improvements in breast health care. EK: Many of our women readers delay (or ignore altogether) the need for screening. What encouragement would you offer them? KCBC: It is so very important for them not to delay in having their yearly breast imaging studies. Aside from skin cancers, breast cancer is the number one cancer that affects women. More than 200,000 women are diagnosed every year in the United States. What they often do not know is that 80% of all breast cancers diagnosed every year afflict women who have no family history of breast cancer! What is also not commonly known is that our chances of getting breast cancer increase as we get older. One in 227 thirty year olds get breast cancer, one in 68 forty year olds, one in 42 fifty year olds, one in 28 sixty year olds, one in 26 seventy year olds, and up to one in eight eighty year olds. The best screening interval is one year as the average doubling in size of a breast cancer is one year. It is best to find and treat a breast cancer when it is small, thus giving women more options to have conservative breast surgery and reducing adjuvant treatments. EK: KCBC offers its patients a variety of screening technologies, including screening mammograms, ultrasound, and MRI. Tell our readers a little about each of these diagnostic tools. KCBC: A screening mammogram uses special x-ray technology specially designed for breasts to detect abnormal growths or changes in breast tissue. It is currently the best defense against breast cancer, as the x-rays can detect tumors and growths in early stages. A breast MRI, or Magnetic Resonance Imaging, involves a specially-designed machine that provides high-resolution imaging while maintaining breast sensitivity and comfort. The MRI technology takes several images throughout the breasts at less than 1mm of thickness. Those images are combined to generate detailed pictures. MRI machines do not use x-ray but rather a combination of radio and magnetic waves. A magnetic field is put around you while radio waves are directed at specific areas of your body. In the AURORA® Dedicated Breast MRI machine that we have in-house at KCBC, both breasts are scanned simultaneously. The machine also allows for feet-first entry, which can alleviate some of the anxiety associated with having an MRI. Breast ultrasound uses sound waves to make images of the breast. Breast ultrasound is noninvasive and is often used as a follow-up test after an abnormal finding on a mammogram, breast MRI, or clinical breast exam. Studies have shown mammography combined with breast ultrasound finds more breast cancers than mammography alone in women with dense breasts. EK: Why is it important that KCBC have clinical breast radiologists on your staff? KCBC: It is important to have clinical breast radiologists on staff who are solely engaged in breast imaging because it has been proven in the medical literature that dedicated breast radiologists have the highest sensitivity for 8

breast cancer detection. Additionally, by being clinical, our radiologists interact face-to-face with our patients, addressing all their breast concerns and also explaining in a clear way what may or may not be of concern when reviewing their images with them. This is not a common practice in breast centers, as the quality of service varies from breast center to breast center. Dr. Kozlowski truly believes that it is an important feature of a breast center to educate women in breast health care while alleviating their fears of having breast disease. EK: In addition to those services you’ve already described, what other services do you offer in-house? KCBC: For the past 16 years, we have also offered genetic high-risk counseling and screening services. Genetics has become increasingly more important in cancer prevention, as well as in alleviating the concerns of those patients who were once overwhelmed with the thought they were at high risk. In addition to detecting breast and ovarian cancers, the hereditary cancer panel also tests for other mutations for cancers such as gastric, colorectal, pancreatic, melanoma, endometrial, and prostate. KCBC also has the Unique Boutique, which is located in our main lobby. We have a mastectomy fitter who will fit patients for prosthetics and assist them in obtaining the products they need. The Unique Boutique also provides the latest products for women who have had breast surgery or are currently having breast surgery such as an array of lotions, post-mastectomy garments including swimsuits, and head coverings such as scarves and wigs. We also offer a wide variety of other retail items and gifts such as jewelry, clothing, candles, and more. EK: Education and outreach are important components of Knoxville Comprehensive Breast Center’s overall mission to our community. Tell

us about your upcoming 12th Annual Mission Mammography. KCBC: One of KCBC’s missions to our community was to provide mammography screening to all women who needed it. We discovered that many women had difficulties in finding access to quality mammography services due to their busy schedules, work duties, lack of transportation, and family responsibilities, so in 1986, we started our mobile screening program, which was the first such program in Knoxville and one of only a few nationwide. While traveling to provide services to women, we realized that a number of them lacked insurance coverage or had a very limited means with which to pay for screening mammography or diagnostic services. The Kim Rowden Breast Cancer Memorial Fund was established by KCBC to help these women. Every year we hold a “Mission Mammography” event where we provide mammograms for free to women in Knoxville and surrounding communities who don’t have health insurance. This year we will be hosting our 12th Mission Mammography starting with a kickoff event on October 4th from 7 am to 1 pm. Women without insurance can come at any time during the kickoff event or any time throughout the month of October, and we will provide them with a mammogram funded by the Kim Rowden Breast Cancer Memorial Fund. We encourage uninsured women to take advantage of these free mammograms during October, which is Breast Cancer Awareness Month. EK: In closing, is there anything else you would like to add? Dr. Kozlowski: It has been a special, and at times a difficult journey, which all started in 1983 when I had the passion to find early breast cancer and believed it was time to change the delivery of breast health care. It has been personally gratifying for me to accomplish this goal. In closing, to try in an endeavor is to risk failure, but to risk and succeed is most rewarding with a peaceful sense of fulfillment and gives encouragement to others to risk pursuing their dreams.

Reconstructing the Reconstruction IN THE PAST

few years, we have seen a quantum shift in the treatment of breast cancer. Medical treatments for breast cancer – chemotherapy, hormone treatments, and genetic treatments – have changed significantly, and although no cancer treatment is “easy,” newer therapies are much more effective and patient-friendly than in the past. Likewise, radiation treatments have become more targeted, meaning less “normal” tissue is affected. Surgical treatments for breast cancer have also undergone significant changes. Here are just a few: » Mastectomy techniques have changed for the better. Gone is the “breastbone to armpit” scar of the “old” mastectomy; a modern mastectomy respects the anatomy and shape of the breast instead of destroying it. Contemporary mastectomy incisions are designed like breast reduction or breast lift incisions with the assistance of the plastic surgeon responsible for reconstruction. » Nipple-sparing mastectomy techniques have been proven safe and effective. In the right patients, a mastectomy that spares the nipple can result in a reconstruction that is difficult to differentiate from a normal breast. » The “lumpectomy,” or partial mastectomy, is becoming more common. In women who are able, removal of the tumor itself, sparing the rest of the breast, is becoming a more common form of surgical treatment. The lumpectomy must be combined with radiation treatment but with newer forms of radiation (such as implantable radiation treatments or “partial breast” irradiation). This is quickly gaining favor among breast cancer patients. » Lymph node removal is rarely performed anymore. With the advent of targeted “sentinel” lymph node biopsies, removal of the lymph nodes of the armpit are no longer routinely performed. Removing the lymph nodes can cause severe arm swelling, which is difficult to treat without sophisticated surgery. » Breast reconstruction is being planned as part of the cancer treatment process. The most important aspect of a cosmetic breast reconstruction is that as much normal tissue (skin and the fat immediately under the skin) is left behind as possible. Collaboration 16

between the general surgeon and plastic surgeon BEFORE surgery is essential for this to occur. The benefit of breast reconstruction for a woman’s psychological and physical wellbeing has been well documented. Proper tumor removal is the most important factor in a successful reconstruction, and “newer” techniques that respect the anatomy of the breast are the best first steps to achieving a satisfying reconstructive result. Once we, as plastic surgeons, are left with a healthy skin “envelope,” any number of techniques can be used to recreate the breast itself. A few of the newer techniques include shaped silicone implants, fat grafting, and perforator flaps. Shaped silicone implants. Newer, breast-shaped implants allow us to recreate the normal shape of the breast and avoid the “stuck-on” look that was common in older reconstructions. When combined with nipple-sparing mastectomy techniques, these newer implants can allow us to reconstruct a breast in a single stage instead of the three stages that were common in the past. Fat grafting, where fat is harvested from one area of the body with liposuction and reinjected somewhere else, has allowed plastic surgeons to further recreate the natural shapes and curves of a beautiful breast. Perforator flap techniques, such as the DIEP flap, allow for reconstruction of the breast without the need for implants. A DIEP flap is a lengthy, complex operation that requires a few days in the hospital to recover. Every year more plastic surgeons are adding it to the list of techniques they use for breast reconstruction. If you are a woman (or know a woman) who had breast reconstruction already, you may have a reconstruction that isn’t satisfying or that you feel you have to “live with” for the rest of your life. This is not the case. Fortunately, asymmetric, unnatural breast reconstructions can be salvaged or, in some instances, completely transformed using newer reconstructive techniques. » Hollowness in the upper part of the breast can be eliminated. A hallmark of older breast reconstruction techniques, this can be eliminated using fat grafting, shaped implants, or a combination of both. » Breast crease asymmetries can be fixed. Using newer, shaped implants or acellular dermal matrix, the crease below the

breast can be recreated in an even position. » Old transverse scars can be corrected, either surgically or with the use of laser treatments or fat grafting (which, in addition to adding shape, can significantly improve the quality and texture of overlying skin). » “Lumpectomy” defects can be treated. Previously ignored by general and plastic surgeons alike, “lumpectomy” defects (such as hollow breasts, distortion of nipple position, indented or deformed areas of the breast) can be partially, if not totally, corrected with fat grafting. » Nipple reconstructions can be rejuvenated or can be made to look natural with modern 3D tattooing techniques. If you have suffered through breast cancer treatments and are not happy with the results of your reconstruction, or are facing a new diagnosis and are uncertain about the appearance of your breasts after surgical treatment, we would be happy to have you come in for a complimentary consultation to go over your options with you.

A reader asks: “Justin, financial markets are recording record highs. What should I do?”

Average and S&P 500 have reached all-time highs in recent weeks, which may have sent your stocks skyrocketing, but as we’ve seen in the past, stock market highs and lows are anybody’s guess. In May 2000, the S&P closed at 1,537 and then more than two years later bottomed out to 777. As the markets recovered, we saw yet another big gain in October 2007, only to watch the bubble burst yet again a year and a half later when the S&P closed at 677. Now with the S&P breaking 2100 in July of this year, you may be asking yourself what you should do. 1. Review your risk tolerance: After a big climb or fall in our markets, the media, our friends, and family all start talking about these swings. Take the opportunity to re-examine your risk as it relates to your investment goals. 2. Review your investment allocations and rebalance if needed: As the various markets move, our portfolio allocations often

inflate or deflate out of proportion. Simply selling the asset classes that have gained in value and buying those asset classes that have decreased in value will reposition your portfolio to the desired allocation. 3. Review your broader financial plans: There’s nothing like the movement of markets to cause us to take pause. Review your specific objectives with your CFP® and gain clear understanding on how these movements in the markets can, may, or will affect your specific plans. 4. Check your emotions: This is perhaps the hardest thing for us mortals to do. Money is inherently emotional. This is the reason why I don’t manage my own personal funds. When the markets move drastically, our emotions often tell us to do the exact opposite of the disciplines that have proven to yield the best results. Fall Recipes Creamy Pumpkin Seed Soup » 2 cups water » 4 cups cauliflower cut into 2-inch pieces » 2 cups coconut milk » 2 cloves garlic » 1 cup diced onion » 1 cup pumpkin seeds, ground » 1 Tbsp arrowroot » 1 Tbsp curry » 1 tsp sea salt or to taste » Pepper to taste To pan add water, cauliflower, garlic, and onion. Bring to boil and simmer for 30 min. Let cool. Blend until smooth. Place back into pan and add coconut milk, ground pumpkin seeds, curry, salt, and pepper. Let cook for 5 min. Place arrowroot in bowl and add 2 Tbsp cold water. Mix and slowly add to soup to thicken. To get desired consistency you may need to add more arrowroot. Serve warm with scallion garnish.

Pumpkin Pie Crust: » 1/2 cup pecans » 1/2 cup walnuts » 6 dates, pits removed » Pinch cinnamon Blend in blender until crumbly. Press down in pie pan. Filling: » 2 cups raw macadamia nuts » 1 cup raw cashews » 1 cup cooked pumpkin » 1 tsp vanilla » 1 Tbsp allspice » 1 tsp orange zest » 1 tsp cinnamon » 1 Tbsp fresh squeezed lemon juice Place nuts in blender and blend until smooth. Add cooked pumpkin and continue to blend. Add remaining ingredients and blend. Pour into pie pan and let set overnight. Note: Walnuts may be substituted for macadamia nuts, but macadamia nuts will help firm the pie.

Chocolate Delights » 1/2 cup coconut oil » 3 Tbsp cocoa powder » Pinch sea salt » 1 tsp vanilla » 1 Tbsp coconut shavings » 3 Tbsp agave » 1/2 tsp cinnamon Mix ingredients together. Place in freezer for 15 min to begin to harden. Remove and make round balls. Roll in more cocoa powder. Place in freezer to harden. Add more cocoa if desire a more chocolate taste or more agave for more sweetness. Also, 2 Tbsp almond butter may be added for variety.

for national pumpkin days. There is a national day for pumpkin seeds, national pumpkin cheesecake day, and, of course, a national day for Halloween filled with pumpkin Jack O’Lanterns. Sugar intake for this month may reach an all-time high for most. According to surveys, teachers dread the sugar highs resulting in changes in behavior the day after Halloween. Is there a way to counteract the sugar highs? Or can substitution be an important aspect of balancing sweet cravings? Take a national holiday approach to celebrating this October and try including pumpkin delights. Pumpkin seeds are extremely important for health, especially male health, because they are loaded with zinc. One ounce of pumpkin seeds supplies 2mg of zinc. Zinc improves immunity, proper cell growth and division, sleep, mood, and insulin regulation, as well as reproductive health in men. Magnesium is another benefit of pumpkin seeds. One half cup of pumpkin seeds gives the daily requirement for magnesium. It is estimated that 80% of Americans are deficient in this mineral. Magnesium helps with proper functioning of the heart, proper bone and tooth formation, relaxation of blood vessels, and proper bowel function. It also benefits blood pressure and may help to prevent heart attack and strokes. Pumpkin seeds should be an important part of your daily snacks or as an added ingredient in recipes. They can be creamed and added to soups, made into pate, crumbled for a breading type coating for vegetables or tofu that can then be pan fried, ground into flour that can be included into bread recipes or corn bread recipes, and used as thickening for gravies. To counteract sweet cravings, add cherries or raisins to raw or roasted pumpkin seeds as a trail mix.

Act Pumpkin cheesecake day is another national holiday during October. Let’s face it: You can’t think of fall without thinking of pumpkins. Pumpkins have many healthful benefits that include the antioxidant beta carotene which, according to the National Cancer Institute, plays a role in cancer prevention. It also contains vitamin C, fiber, and potassium. One cup of cooked pumpkin contains 564mg of potassium. Potassium regulates many functions of the body, including helping to lower blood pressure and keeping arteries more flexible. The benefits of pumpkin are too many to ignore. Pumpkin is very easy to add to recipes – just add to soups, sauces, breads, pies, or stuff in ravioli. Trick-or-treat can be a fun time but a dilemma as well when it comes to watching sugar intake for our children. This year try some different ideas that add other sweeteners to the treats. Make fudge with coconut oil and agave. Blend coconut milk, chocolate, agave, and cinnamon and then freeze into popsicles. When the child comes back from Halloween festivities, try exchanging the sugary treats with homemade treats. It turns out that my child gladly gave up the basket of sugar for more delicious, healthful fare. Start early when a child is young, and this will impact their future. My child is now 21 years old and rarely craves sweets. If your child is older and starting out, increase the amount of healthy sweeteners in recipes, then slowly decrease the amount over time. Eventually their sweet cravings will decrease as well. October is a time for festive fall events. Using the bounties of fall by including pumpkin in recipes makes balancing food healthy and delicious. By eliminating simple sugars from our diet with better quality sweets, we can live a more balanced life. After all, life is pretty much a balancing act.

“Designed for Drama:

Fashion from the Classics” Coming to Biltmore in 2017

A COLLECTION of classic literary works

and the popular movies that brought them to life will be celebrated in “Designed for Drama: Fashion from the Classics,” Biltmore’s newest exhibition of award-winning costumes and objects from the Biltmore collection. Set for February 10 through July 4, 2017, the exhibition is inspired by favorite stories in George Vanderbilt’s 22,000-volume library. More than 40 elaborate costumes from recent films will be displayed in Biltmore House, including a costume worn by Robert Downey, Jr. in “Sherlock Holmes” and Nicole Kidman’s Isabel Archer costume from “The Portrait of a Lady.” The film fashions will be displayed along with the original books from Vanderbilt’s library that served as cinematic inspiration. The clothing reveals the attention to detail involved in period costume design. Vanderbilt began to read at a very young age, and years later he was once dubbed by New York media as “one of the best read men in the country.” At age 12, he started keeping

a record of books read, numbered consecutively, including the title and author of each work completed. These journals log 38 years and 3,159 books and will be on display during this exhibition. The exhibition will also give insight into the eclectic and varied interests that led Vanderbilt to collect more than 22,000 volumes before his death. GUILD of the Great Smoky Mountains Heritage Center is proud to announce the sixth annual Fall Home Tour to be held on November 4. The three homes showcased on this year’s tour represent a variety of architectural styles and décor, along with spectacular views that round out the possibilities found in the Townsend area. The Little River Railroad House Set on the banks of the Little River just upstream from the swinging bridge, the “Little River Railroad Station House” offers convenience, beautiful river views, and the soothing sounds of water flowing over rocks steps from the home. The house features interior exposed wood framing, a large stacked stone fire place, four bedrooms and four full baths on the ground floor, a loft with a half bath, a porch all the way around the house, with the river side screened, a

balcony on the front, a deck on the river side, and stacked stone features at the front and rear entrances. All the stone came from fence rows in Dry Valley. The heart pine floors and fireplace mantle were cut onsite from columns and beams removed from a 125 year old mill in southern Alabama. The floors have no stain and are the natural color of the wood. It is a great place to relax and enjoy life on the peaceful side of the Smokies. The Lindgren Home From the river to the ridge, the Lindgren home, “Hawk’s Nest,” is mountain living at its best. Soaring ceilings, walls of glass, and magnificent views have made the dreams of two Florida transplants a reality. Indoor and outdoor living spaces are designed to bring nature to the front door. Dual level screened porches and a lower level designed for relaxation and recreation are key elements. On early morning walks, passersby can enjoy the

sounds of the baby grand piano wafting on the breeze. The “Mountain 6” Home Tucked away on a cul-de-sac in Laurel Valley is “Mountain 6,” a home that is full of hidden surprises. Any required vaccinations should be complete at least eight days prior to boarding, and give copies to the boarding resort. Bring a sufficient amount of food, supplements, and any prescribed medication to the resort. The same food and supplements that are eaten at home should be going into the tummy at the resort. A little extra is best, just in case your trip plans change. Ms. C’s provides the beds and bowls. For the comfort of the dog, we request that you bring an old, sweaty t-shirt, which we put on top of the bedding. It brings the scent of the owner into their suite, making it “home” for the dog while at camp. Ms. C’s also asks for phone numbers where you may be reached while away in addition to

emergency contact information of a local friend or family member in the event we cannot reach you. Knowing special concerns, medical and characteristics (such as epilepsy or fear of loud noises) is important. Our kennel staff can make special accommodations for various special needs of each dog. SMOKY MOUNTAIN SERVICE DOGS INAUGURAL GOLF CHALLENGE

HE INAUGURAL Smoky Mountain Service Dogs (SMSD) Golf Challenge will take place on Saturday, October 8, at the Tellico Village Toqua golf course. The Golf Challenge is a fundraising event for the SMSD. SMSD is a Tennessee chartered 501(c) (3) non-profit organization headquartered in Tellico Village. SMSD was founded in the Tellico Village home of Mike and Suzy Kitchens in 2010, and its mission is to enhance the physical and psychological quality of life for veterans with service related disabilities. Typically recipients of a mobility assistance service dog from SMSD have suffered traumatic brain injury, limb loss, or other debilitating injuries where balance and stability are

compromised. The SMSD Golf Challenge is a four person scramble format, and the $65 per person fee includes continental breakfast, buffet lunch, and prizes. Greens and cart fees are the responsibility of each player. DON’T KNOW about you,

but I’m ready for some fall temperatures. It got warm early this year, then went to hot and stayed there! Our nights are starting to cool down at least, and I hope the daytime temps follow soon. If we don’t get cooler weather and rain soon, there won’t be much of a fall leaf show. No matter what the weather does, October is still one of the best gardening months. » Have you walked around your yard yet to check on things? If not, you need to ASAP, and you need to be prepared for some surprises. I can almost guarantee that some of your plants have been damaged by fungus, insects, or drought. If you can’t tell what’s wrong, put some leaves or a branch in a bag and bring it in. It’s very hard to diagnose “brown leaves” over the phone. You can also email photos to me if you aren’t going to be out this way soon. » Fall is a great time for new beginnings. It’s when you can really see the strengths and weaknesses of your yard and contemplate the possibilities. In fact, October is generally the best month for planting (or re-planting, as the case may be) trees, shrubs, and perennials. 85% of root

development is done during the fall and winter, so anything you can do now has a good head start on spring. I have one caution: Hot weather lovers like crepe myrtle and perennial grasses need to be planted as soon as you can. They’ll be unhappy if the soil turns cold early and will refuse to put down roots. » Is your yard boring now that your deciduous plants are dropping their leaves and summer annuals are done? Add plants with fall and winter appeal! Some trees, shrubs, and perennials are at their best this time of year. Burning bush and nandina turn fiery red, beautyberry and winterberry hollies put on clusters of berries, red twig dogwoods have branches that practically glow, and fall annuals and perennials are showing off their blooms and fall color. » Our average first frost is between October 15-27. This means that annuals such as impatiens and begonias need to be pulled up before then, because they turn to mush. Geraniums and petunias can usually handle a light frost just fine. Pansies can still be planted now and will last all winter and on through spring. Mums that are planted in the ground this late are iffy

at best, so consider them a good addition to your compost heap. » Don’t forget containers! Consider putting mums, cabbage, kale, mustard, or grasses in with pansies for interesting colors and textures. I’ve also seen some wonderful planters that used pumpkins, gourds, and even statuary as accents. Don’t be afraid to play around, and use some of those Pinterest ideas. » Do a good cleanup of your yard. Sick, infested, or dead plants don’t just look bad; they also harbor pests that will re-emerge in the spring to attack your plants all over again. You can leave annuals and perennials with nice seed heads for the birds, but get up the damaged leaves as they fall. » Speaking of leaves, your cleanup includes the dreaded leaf piles from your trees. If you don’t have many trees, this is easy: Leaves can be shredded and used as mulch or compost. If you have a yard full of trees like I do, it’s a big deal. A couple of inches of leaves are good. A couple of feet are bad! Get them up or they will suffocate everything underneath. » Since I mentioned compost, I’ll tell you that this is a terrific time to add it to your flower beds. If you spread it now, it has all winter to break down and add nutrients to your soil. When spring gets here, all those nutrients will be ready to make your plants grow. » Let’s talk about water. I will point out that last October we got no rain. None. Zilch. Zero. Nada. If you aren’t watering, your plants are dying. Sprinklers are fine for grass and flowers. They don’t do diddly for trees and shrubs. Hand water at least a couple of times each week unless you want to replace plants. » Don’t prune. » Do mulch. Mulch will help preserve moisture and stabilize ground temperatures during the winter. Don’t bury your plants! Three inches is enough. Donuts, not volcanoes, around your trees, please. » Pull any weeds and apply a pre-emergent to keep them from popping back up during the winter. It’s never too cold/wet/miserable for weed seeds to germinate. » Fill your birdfeeders with good birdseed. Don’t use the cheap kind or you won’t have pretty birds and you’ll have ugly weeds. Don’t forget the birdbaths either! Keep them full and clean. Glass and ceramic baths should be brought in before a freeze, but metal and plastic baths can stay out all year. N OCTOBER 8, history will come to O life as more than 175 presenters showcase skills, music, and art that were popular

in the 19th century. Visitors can shop for unique handmade crafts, sample savory foods, and enjoy a long lineup of string bands playing old-time music. There are lots of activities for children, including cotton picking and goat milking! Many of the activities take place in Ames’ Heritage Village – a replica of a 19th century settlement that includes several log cabins, a one-room school, and the nationally renowned Stencil House, so named for the elaborate stencil designs that cover the home’s interior walls. In the Heritage Village, visitors can see demonstrations of brickmaking, tobacco curing, cotton spinning, and much more. From the village, it’s a short stroll to a

reproduction of a Civil War camp, where visitors can chat with re-enactors in period dress and watch artillery demonstrations. Returning this year are the cannon blasts at the top and bottom of each hour! Some of the other features returning this year include storytelling in the schoolhouse, an archeological dig at an antebellum site, horse shoeing, woodcarving, and Native American flint knapping. Homemade ice cream will be made on-site with the help of a hit-and-miss engine. Visitors will also again be able to purchase pumpkins from a local farmer. Admission is $5 for adults, $2 for ages 4-16, and free for ages 3 and under. I

T’S DIFFICULT TO BELIEVE

that we are entering our 10th year of business with Surface Doc. I remember my husband, John, missing out on so much happening in our family because he was busy working in restaurant management 60+ hours a week. I was frazzled and actively looking for a new career path – something we could possibly do together. That’s when I found some videos from a company in Arizona demonstrating cleaning equipment that was so dramatic. I was immediately intrigued but skeptical. If this equipment was “for real,” I wanted to know more. So when John got home that night, we watched in awe one video clip after another. We saw carpet, tile and grout, concrete, stone, and most every surface we could think of undergo dramatic transformation. A few days later we traveled west to see this equipment firsthand. When we saw in person what we had seen on the videos, we were

EW BANDS HAVE IMPACTED

the ’70s and ’80s like Styx. Their music still permeates airwaves and stereos all around the world, as well as being prominently placed in movies and TV shows. I r e c e n t l y s p o ke w i t h t h e b a n d ’s keyboardist and vocalist, Lawrence Gowan, to discuss the band’s new DVD, “Styx Live at Orleans Arena Las Vegas,” as well as the latest with the band. Gowan joined the band 18 years ago. When I asked him if, in his wildest dreams, he ever imagined joining a band like Styx, he replied: “It’s been everything that I could have anticipated and far more, because we’ve played around the world so many times. This line-up of the band has played more shows than any previous era of the group, and I’ve seen younger and younger people coming to the shows I’d say during the past 10 years. Now, on any given night, half the audience is under 30 years of age and wasn’t even born when some of the biggest Styx records were at the top of the charts. It’s a fantastic thing to be a part of and to witness and to enjoy and see the smiles on so many people’s faces every night. “As I look back on it, on the one hand I am surprised, on the other hand I just think ‘this just seems really natural to me’. Who would ever want to give up the experience of some of the greatest moments of my life just standing in the audience listening to Pink Floyd or being at a Paul McCartney

show? It’s an amazing thing to experience, so why would you stop that?” Turning our chat to the band’s new DVD, Gowan had this to say: “We’ve done a couple of live DVDs throughout the years. This one, ‘Styx Live at The Orleans Arena Las Vegas,’ was recorded about a year, getting close to two years ago, that we actually recorded that show. But we wanted to kind of show a lot of the behind-the-scenes machinations or the way the whole thing comes together. So we included a lot of our crew, and a lot of the experience of touring is included in this DVD. Did you get to see some of that? “I think that people are getting more of a behind-the-scenes view of what it is to be a band that is effectively touring as Styx is to this day. That’s really what the DVD focuses on – as much on that as on the show and the overwhelming response of the audience and just how the whole lifestyle and this life commitment and devotion that all of us have that’s driving this forward is captured on this ‘Styx Live at The Orleans.’” As for similarities and differences fans will see between that show on the DVD and seeing the band during this tour at the fair, Gowan said: “Well, I don’t think it will be difficult for them to kind of connect the dots between the two. But, ultimately, I think that because we live in a world now where so much of it is connected to small screens. Having said that, there’s nothing like the live experience of having something

happen in real time with a few thousand people around you and experiencing this great, instantaneous communication of music in a fantastic arena. I’ve touched on that before. I think it’s the ultimate form of entertainment and it’s too large to be contained on a small screen. You go get a taste of it and then maybe it will whet your interest. But there’s nothing like that experience of seeing a band live and getting swept up in it like we do along with the audience.” As for what’s on the band’s radar for the next year, Gowan said: “We always have little projects going on behind the scenes that we don’t want to overly focus on, so there’s nothing imminent that I would like to make any pronouncements or announcements about other than the fact that I’m looking at my itinerary now and noticed we’ll finish up in December. We will have played 112 shows through 2016, and I’m already seeing about 40 or 50 on the itinerary for next year. We try to keep our focus as close to that and over the next few months as possible. But, as I say, there are great things going on behind the scenes and projects that we’re working on that will, hopefully, see the light of day sooner than later.” The band will be performing at Harrah’s Cherokee on October 15th. Pack your bags and head to the Lone Star State for some nationally recognized fun. There’s a place in far West Texas where night skies are as dark as coal and rivers carve temple-like canyons in ancient limestone. This magical place is Big Bend National Park. While exploring more than 801,000 acres, visitors can take a rafting trip down the Rio Grande, hit the trails to see some of the best panoramic views of the state, search for hidden mines on the Lost Mine Trail at the summit of the Chisos Mountains, or take an overnight backpacking trip. Visitors will also see Texas wildlife, as there are many different species in the park including mountain lions, deer, Mexican black bear, javalina, and more than 450 species of birds. Big Bend also supports a remarkable diversity of plant life; there are 1,200 plant species, some of which cannot be found anywhere else in the world. A few hours away in Salt Flat is the world’s premier example of a fossil reef from the Permian Era, Guadalupe Mountains National Park. From protecting travelers from the Mescalero Apaches who were defending their lands to exploring and mapping the region, Buffalo Soldiers are entwined in the history of the Guadalupe Mountains. The Guadalupe Mountains are still an important cultural and spiritual sanctuary for the Mescalero Apaches with members of the tribe returning to the area to harvest agave for ceremonial purposes. Aside from its rich historical value, the park 62

is known for its extensive hiking and backpacking opportunities in one of the nation’s most pristine wilderness areas. Eighty miles of trails welcome hikers to Guadalupe Peak, the highest point in Texas (8,749 feet) and to mountaintops with scattered but thick conifer forests typically found in the Rockies hundreds of miles to the north. While exploring the Guadalupe Mountains, visitors are bound to encounter some of the 60 species of mammals, 289 species of birds, and 55 species of reptiles present. These are just a few reasons why Guadalupe National Park is one of America’s best kept secrets. In Central Texas, San Antonio’s five Spanish colonial missions (including the Alamo) represent the largest concentration of Spanish colonial missions in North America and the first UNESCO World Heritage site in Texas. The missions include the state-owned Alamo, or Mission San Antonio de Valero, and the four federally run missions – Concepción, San José, San Juan, and Espada – that each have an active Catholic parish. Once you finish exploring the Missions of San Antonio, take a road trip south to Padre Island National Seashore, the longest section of undeveloped barrier island in the world. Stretching more than 130,454 acres, Padre Island National Seashore separates the Gulf of Mexico from the Laguna Madre, one of a few hypersaline (saltier

than the ocean) lagoons in the world. The park protects 70 miles of coastline, dunes, prairies, and wind tidal flats teeming with life, including a variety of sea turtles. In fact, five sea turtle species – Kemp’s ridley, green, loggerhead, hawksbill, and leatherback – have been documented along the seashore. All of these species are federally listed as threatened or endangered. The Division of Sea Turtle Science and Recovery at Padre Island National Seashore is the only division of its kind in the entire National Park system. Padre Island National Seashore began efforts to save and recover sea turtles in the 1970s, when the National Park Service led the formation of a bi-national program to restore the Kemp’s ridley. This species, which once nested from Mustang Island, Texas to Vera Cruz, Mexico, is the most endangered sea turtle in the world. Staff and volunteers at Padre Island National Seashore have worked diligently to recover the Kemp’s ridley for decades, and the program has now expanded to include protection and conservation measures for the other four species. Today more Kemp’s ridley turtles nest at Padre Island National Seashore than at all other locations in the U.S. combined. Four Common IRA and 401(k) Mistakes 1 . B or row i n g money from your 401( k). You should avoid this, as you may jeopardize your future retirement. Usually you take a five-year loan on your 401(k). If you can’t pay back the loan, the unpaid balance is treated as a withdrawal and is subject to a 10% penalty and is taxable. The other disadvantage of a 401(k) loan is you’re paying taxes twice. You pay the loan back with after tax money and later, when you withdraw the money, you pay taxes again. 2. Taking a lump sum IRA inheritance. One of the biggest mistakes is taking a lump sum IRA inheritance that may result in losing as much as 30-40% of the inheritance, depending on the size of IRA, due to taxes. The IRA’s “Stretch IRA” provision allows non-spouse beneficiaries to stretch their distribution over their life expectancy. The strategy here is to maintain the inherited IRA as an investment, taking the required minimum distributions and allowing the remaining assets to continue growing. 3. Keeping company stock in your 401(k) plan. Distributions from 401(k) plans are taxed as ordinary income, but if you roll your company stock over to an IRA, you can take advantage of an IRS provision called “Net Unrealized Appreciation.” This allows you to withdraw money from your IRA, paying ordinary taxes on the basis but only capital gain taxes on the gain. The maximum capital gain tax rate is 20% versus the highest tax rate for ordinary income of 39.6%. 4. Not updating your beneficiary designation. Many people are unaware that regardless of what your will states, IRA proceeds are distributed to the beneficiary listed on that IRA. I recently met with a widow whose deceased husband had a 401(k), but he neglected to update his beneficiary designation. Upon his death, the proceeds were paid to his nephews, who were the original beneficiaries listed. Neither his wife nor minor children would receive the benefits from his 401(k). History

Turning Soil and Turning Tides for the Historic Mossy Creek District THERE IS A LOT of history in

downtown Jefferson City, but you may not know it with just a glance at the decaying facades of what was once the heart of the city. If you look closely though, you will see signs of hope popping up all over what is now known as the Historic Mossy Creek District. In 2011, thanks in part to a grant from the Tennessee Downtowns Program, a group of concerned citizens joined together to find a solution to save their dying downtown. Now the fruits of their labor are beginning to show in several exciting projects for the district. A recent celebration was held to kick off the groundbreaking of a new train depot-inspired park in a vacant area on Main Street. Optimistic developers have purchased some of the key properties, and new businesses are beginning to emerge. To top it all off, a new historical walking tour is being developed to highlight the city’s rich history that has slowly been fading away in the last of downtown’s historic

structures. The Historic Mossy Creek Walking Tour is a project of Evon Design Co. and is supported by a list of dedicated local sponsors. The tour will work to bridge the gap between the current state of downtown and the hope of future development by filling vacant windows with large format prints that illustrate historical stories of the once vibrant downtown. “I think people often drive by these old buildings and never realize the stories these walls could tell,” said Michael Evon, Principal of Evon Design Co. and founding President of the Mossy Creek Foundation. “This tour is a way of giving a voice to these buildings and relaying the history of the people who have called this place home for generations.” The historic walking tour is set to be unveiled later this month. Preservation Power Summit to Be Held in Morristown on November 4 KNOX HERITAGE and the East Tennessee

Preservation Alliance are proud to bring the East Tennessee Preservation Power Summit to our region on November 4 in Morristown. This one day “power summit” is an evolution of the popular East Tennessee Preservation Conference designed to better fit busy schedules while still serving as an impactful educational and networking event. For the past seven years, the conference has been hosted by communities throughout our region: Townsend, Rugby, Jonesborough, Gatlinburg, Maryville, Oak Ridge, and Knoxville. Attendees include preservation professionals, architects, planners, tourism professionals, government and community leaders, economic developers, real estate professionals, and others who have found that advanced knowledge of historic preservation principles and practices have value. It is a great event for learning, networking, and experiencing historic sites distinctive to the host community. This year the Summit will be held in Morristown at the historic Rose Center. 64

Attendees will benefit from education sessions focused on proven historic preservation applications, heritage tourism, and downtown revitalization. “Preservation Crash Courses” will be introduced for the first time, as well as a “Preservation Master Class.” Historic site tours will visit Crockett Tavern, General Longstreet’s Headquarters, and Bethesda Church. The event will end with the East Tennessee Preservation Awards reception at historic Arrow Hill, a c. 1859 residence once used as headquarters by both sides during the Civil War. This year’s keynote speaker and special guest will be Marsh Davis, president of Indiana Landmarks. Mr. Davis has developed initiatives in African American heritage and rural preservation, teamed with writer Brad Shaw to produce 99 Historic Homes of Indiana, and was recognized by Indiana Governor Frank O’Bannon for his work in historic preservation. Mr. Davis has served as the executive director of the Galveston Historical Foundation and was elected in 2008 to chair the Statewide and Local Partners of the National Trust for Historic Preservation

October Featured Artist Exhibit and Reception R ECENT WORKS

by painter Brenda Mills and clay sculptor Karyn Kyte will be on display October 4-30 at the Art Market Gallery. An opening reception for the featured artists will begin at 5:30 pm on October 7 during downtown Knoxville’s monthly First Friday Art Walk, with complimentary refreshments and music performed by John Kueny. Brenda Mills is a water-media painter and pastel artist with a Cum Laude BFA from the University of Tennessee. A self-described “country girl” originally from Kingsport, Tennessee, now residing in Knoxville, Brenda has won many national and regional awards. Her work was most recently accepted into the Tennessee Watercolor Society’s 35th Juried Exhibition. When painting in watermedia, Brenda uses liquid acrylics, mainly for their intense color and glazability. Trained as a printmaker in college, she begins her paintings on glass, printing the image onto paper or yupo, a tree-free synthetic alternative to traditional art paper. Then she proceeds to manipulate the paint. Karyn Kyte began sculpting as a child, using “mud pies” as her medium; however, her real passion for art came about while she was studying Medical Technology in college and then took a painting class. Soon her interests moved back to clay, and she changed her major to Fine Arts. Karyn is a sculptor whose creations are spontaneous. When she sees a lump of clay, images start to form in that lump, willing itself into a certain form. Her pieces are inspired by her love of nature, and she uses shape, color, and movement to honor earth and all its creatures. She tries to bring awareness to the fragility of earth and of man’s impact on it through her art. Not only is sculpting Karyn’s way of showing gratitude to nature, to her, art is a way of seeing, being, and thinking. Karyn received her Bachelor of Science from the University of South Dakota. The Art Market Gallery is owned and operated by more than 60 professional regional artists. The gallery is wheelchair accessible, and parking in the abutting garage and on the street is free on weekends and after 6 pm weekdays.

Local Blogger to Lead Workshops A LAN SIMS, author of the award-winning blog “Inside of Knoxville,” will lead back-to-back workshops for the Knoxville Writers’ Guild on Saturday, October 8, on writing and caring for a blog and maintaining a social media presence. Sims will lead “Blogging Basics” from 10 am to noon and a more advanced session, “Branding Your Blog,” from 1 to 3 pm. Both workshops will be held at the Central United Methodist Church, located at 201 E. Third Ave. Attendees should enter off of the large parking lot behind the church. Keep the Little Ones Entertained at Your Upcoming Holiday Events T HE

season is around the corner, and this often means getting together with others. When family and friends get together for a meal or other special event, it can be great fun. However, children can easily become bored, fidgety, and ruin the event if they become unhappy – especially young children who have trouble sitting still for long periods of time. If you’re hosting, you should have items and activities available that will keep them occupied. Keeping the little ones happy makes it easier for adults to visit. Assign Little Helpers Children generally rise to the occasion and are happy and behave when you make them feel important. Help pass the time while children are waiting on mealtime by assigning chores to all of your little helpers. This can include having them put ice in glasses, setting out utensils, greeting people at the door, and so on. Even little ones can feel included by being told to go sit on somebody’s lap because that person “looks lonely.”

Paper Tablecloths If you’re hosting a meal or having snacks, have a children’s table. Place a white paper tablecloth on it with packs of crayons for each child. Give the children permission to color in the area in front of their seat while they wait on their food to arrive. Even older children enjoy drawing pictures. Games Assign older children in the group as the “leaders” and let the children play games. If the children grow tired of board games and cards, have them play games such as I Spy, Mother, May I?, or Simon Says. Special Surprises As the event is winding down and cleanup time comes around, tell the children that you have special surprises for everyone that can help with clean-up. This not only helps get your home back in order, but it also gives children something to do with all their pent-up energy that was kept pentup while they remained seated at their table for so long. Examples of clean-up tasks can include throwing paper plates and cups in

the garbage, putting dirty utensils in the sink, putting condiments in the refrigerator, and more. Play Area and Rest Area If you have guests that will be lingering, make sure you have a designated area in your home where the children can congregate. This could be a den, sun porch, or an extra bedroom. Be sure to remove any breakable items from this area prior to your guests arriving. Having a television with children’s movies in this area is good, as is coloring books, a few toys, and games. In addition to a play area, you should provide a rest area for little ones who may fall asleep. This can be your own bedroom or a guest room. Be sure the area is roomy enough for parents to set up a portable crib, if needed, and that you have extra blankets on hand. Wendy Faulkner is a freelance writer based in Blount County, Tennessee. The Problem with Expecting Instant Communication THINGS HAVE REALLY CHANGED

with communication, and I know you know this. As you know, we all assume that everyone has a cell phone (mostly), and we also assume that everyone keeps their cell phone with them at all times. With these assumptions come expectations of instant or very quick responses. Lately I’ve had several friends and colleagues discuss the pressure, and even the annoyance, of these expectations. Most of us have experienced leaving someone a voicemail or text and expecting or wanting that person to respond within the hour or definitely the same day. When and if that response doesn’t come, we start thinking things like “something is wrong,” “they’re ignoring me,” “they must be mad at me,” etc. These feelings happen because of our expectations of immediate or quick response times. Likewise, if we are the receivers of a text or voicemail, as soon as we receive it, 66

EVERYTHING KNOXVILLE October 2016

the pressure to respond starts mounting. Perhaps we’re not ready to respond, perhaps we need more information, or perhaps we’re just not in the mood; however, the longer we go, the more anxiety most of us feel regarding our response. Wow, it really has changed! What can we do to just help with all of the expectations? Perhaps we can make our expectations more clear. Let me give you a few examples. Leaving a voicemail: » Hey Sara, it’s Monica. Just wanted to say hi. No need to call back but would love to talk when you have some time. » Hey Sara, it’s Monica. Could you give me a call by tomorrow? I need to ask you a question before I buy those tickets tomorrow. Texting responses: » Got your text. Will respond when I have more time to think about it. » Thanks for your text. I appreciate hearing from you. Give me a few days to think about it.

It’s rude not to return calls and texts, but it’s also rude to expect other people to stop what they’re doing and respond instantly to our correspondence. I think we all would benefit from being more patient with each other and realize that we each have very busy lives and sometimes we simply cannot respond. Perhaps if we lowered our expectations with regards to instantaneous responses, we would avoid hurt feelings and wrong assumptions. Ladies and gentlemen are patient, kind, and avoid making assumptions about other people’s behavior. Let’s do our best to respond to correspondence in a timely manner, even if that response is just a request for more time.

often have areas of their bodies that make them feel self-conscious of their looks. One of the most common areas we hear complaints about is submental fullness, also known as the dreaded double chin. In this era of selfies, patients have coined a new term for this problem – “tech neck.” Unlike other areas of the body, excess fat in the neck cannot be easily concealed. The cause is often due to weight gain, but genetics seem to also play a big role. Unfortunately, a double chin can make patients look heavier than they actually are.
