Children with Disabilities Team Family Support Team
To qualify for support from the Integrated Children with Disabilities Team (DCT & FST) the child must meet the requirements in the profound or severe labelled columns in at least two of the dimensions as outlined in the Eligibility Criteria: Health Education Communication Behaviour Associated with the Disability Family and Social Relationships Self-Care Safety Mobility Children under the age of 5 VI & HI More information and the criteria can be found on the website https://www.afcinfo.org.uk/local_offer
Eligibility DCT to FST transfers: After the Social Care assessment has been completed and: • There are no current safeguarding concerns. • There is a package of assessed need support in place. • The care package does not contain more than 24 overnights of respite in a year. Cases can transfer back to DCT should there be any new safeguarding concerns or significant changes which require a social work reassessment and oversight.

Children With Disabilities Team -
Family Support Team • Early Help assessments (6months) • Visits to the child every 8 weeks (alternate visit at home) • Direct work to address specific issue (cowork) • RSE Specialist within team.
