Understanding the Four Levels of Hospice Care

It includes pain and symptom management, medication and equipment needs, emotional and spiritual support, as well as grief support for families.
The Hospice team consists of a medical director, nurse practitioner, registered nurse (RN), chaplain, social worker, a certified nurse assistant (CNA), grief specialist, as well as volunteers for additional support. Hospice of the Red River Valley also offers an after-hours team, staff members specially trained to help patients and families for needs that may arise during nights and weekends. The entire Hospice team cares for patients and their caregivers and family using specialized skills, compassion and sensitivity.
The Hospice team comes to you, wherever you call home. That could be a single-family home, apartment, hotel, skilled nursing facility and more— anywhere you call home.
There four levels of hospice care:
1. Routine Home Care
2. Continuous Home Care
3. General In-patient Care
4. Respite Care
Routine Home Care
Routine Home Care takes place in a patient’s home and involves the Hospice team visiting their residence on a routine or planned visit basis.
This level of care is similar to scheduled visits with a primary provider at the clinic, but it is available 24/7 in the person’s own home. This schedule is flexible and changes based on an individual’s needs. An RN schedules a visit at least once per week, or more depending on every person’s unique needs.
Routine Home Care may also include using our dedicated after-hour team for emergent needs, increased symptoms management or a crisis.
Continuous Home Care
If symptoms become difficult to manage and the patient requires frequent evaluations and assessments from their nurses or doctor, they have access to Continuous Home Care.
Symptoms that may trigger Continuous Home Care include:
• Sudden deterioration requiring intensive nursing interventions
• Uncontrolled pain, nausea or vomiting requiring concentrated nursing support
• Respiratory distress requiring aggressive assessment and medication management
• Uncontrolled restlessness, agitation or delirium needing continuous skilled nursing assessments
Continuous Home Care is meant for the time frame when symptoms are being actively controlled by onsite hospice staff. This includes frequent assessments by the Hospice nurse, a minimum of every hour, but it can be more often based on the person’s needs and symptoms. A variety of interventions may take place, including consultations with the medical director, medication changes, the use of medical equipment incorporated and more. Continuous Home Care also includes a monitoring period after the interventions take place to make sure the measures were successful in getting the patient comfortable at home. While under this level of care, a dedicated hospice nurse or CNA is present at the bedside in the patient’s home as often as possible to make sure symptoms and needs are met and managed.
The goal of this level of care is to get a patient back to their baseline wellbeing by actively solving emergent issues.
After it is determined that current interventions are successful, the patient returns to Routine Home Care services. If the interventions were not successful in the home a hospital or hospice house admission may be needed.
General Inpatient Care
General Inpatient Care (GIP) is available when symptoms cannot be managed at the patient’s bedside at home, and shortterm hospital placement is required. The goal is to stabilize symptoms and make sure the patient is comfortable before returning home.
During this level of care Hospice of the Red River Valley staff supports the hospital team by providing active, hands-
on care during portions of the hospital stay. Hospice staff will not remain in the room with the patient 24/7 as they did at home; rather, they will continue to be available to the patient, family and hospital staff as needs arise. Once the patient is comfortable and ready to return home, they will be discharged with a detailed care plan to make sure their symptoms are managed without the need for hospital staff and resources.
Respite Care
While the other levels of hospice care are directly for the patients’ benefit, Respite Care is designed for caregivers. Being a caregiver is a labor of love, but it’s also fulltime, challenging commitment. Respite allows caregivers to take a break, so they don’t get burned out and can continue to care for the patient through their end-oflife journey. Caregivers can use Respite Care for personal appointments or chores, or traveling, attending family weddings or funerals, or to simply rest and recharge themselves.
During respite, the patient is admitted into a Medicare-certified skilled facility for five days. This temporary move allows the patient to continue to receive aroundthe-clock care and attention from staff, so their full needs are met. The Hospice team helps with the transfer and admission to the facility and continues to provide care and visits to the patient while in the
facility.
You can be admitted to hospice at any level of care, depending on what is necessary at that time, and you can transfer between levels of care as the progression of your illness changes. The Hospice team monitors each patient to determine if they require a different level of care.
Hospice care is about helping people live comfortably and with a sense of normalcy, at all levels of care, regardless of their terminal illness.

How to pack your vehicle for a road trip
Trunk
Load heavy objects lower in your vehicle, and pack larger items at the back and smaller ones near the front. Fill in the gaps with bags and soft pieces. If your trunk isn’t separate from the cab, don’t stack items above the height of the back headrest. This will prevent things from falling over the seat if you suddenly hit the brakes.

Cab Keep your vehicle’s registration, itinerary, tickets, sunglasses, tissues, games, bottled water, and snacks in the cab. Of course, try to leave enough space for everyone to sit comfortably. Have a great holiday!

4 symptoms you shouldn’t ignore
As you age, you may notice minor changes in your overall health. While some symptoms are a normal part of aging, others require immediate attention. Here are four changes in your health you should never ignore.
1. A wound that won’t heal
Do you have a wound that’s slow to heal? If so, it may be a good idea to see your doctor. If you have a low-grade fever or the injury becomes red and increasingly painful, seek immediate help.
2. An abnormal presence of blood
Unless you have a known medical condition, you must alert your doctor if you notice blood in your stool, urine or spit. If the bleeding is new or recurring and accompanied by other symptoms, go to the emergency room right away.
3. Unusual stomach pain
Abdominal pain that increases gradually, lasts a long time or appears suddenly should be taken seriously. If this is happening to you, visit your doctor. They may refer you to a gastroenterologist for further testing.
4. Increasing shortness of breath
Shortness of breath that prevents you from doing everyday activities like climbing stairs and going on walks should be investigated. This could be a sign of
a serious health problem, especially if accompanied by a fever or chest pain.
If you have any doubts, don’t hesitate to consult a health care professional.
Eating healthy, nutritious foods gives you the energy you need to go about your daily life and stay in good shape. For seniors, it’s essential to have a varied diet to help maintain independence and quality of life. Here are some healthy eating tips to follow as you age.
50 to 59 years old
At this age, women should focus on eating foods that help reduce the unpleasant symptoms associated with menopause. For example, they should reduce their sugar intake and consume foods like soy and flaxseed, which are good sources of phytoestrogens.
On the other hand, men should reduce their consumption of red meat and saturated fats to help prevent prostate cancer. They should also include plenty of vegetables, lean proteins and fish in their diet.
Both aging men and women can benefit from eating more protein to help prevent their muscle mass from deteriorating.
Nutrition tips for seniors
60 to 69 years old
As you age, your metabolism gradually slows down. Therefore, you don’t need to consume as many calories as you used to. However, you must ensure you’re still getting enough calcium, vitamins and nutrients by eating the appropriate foods. If you lack the necessary appetite, you may need to supplement your diet with quality mealreplacement bars or drinks. Consult a health care professional for advice.
70 years and older
Hunger is less of an issue at this age. Therefore, the goal is to consume enough protein to meet your nutritional needs while still eating plenty of healthy fruits, vegetables and starches. You may also need to take vitamin and mineral supplements to strengthen your immune system.
On top of maintaining a healthy weight, fuelling your body with healthy foods can help prevent muscle and bone loss and the onset of disease. Consult your doctor or a dietitian-nutritionist to help you adopt healthy eating habits.

How to get started in photography
First, purchase a high-quality camera or a smartphone with a built-in camera.
Make sure you buy equipment that suits your needs and current knowledge of photography. Then, join a group of amateur or professional photographers or sign up for a workshop to learn the basics.
Let yourself be inspired by your environment and other photographers during your picture-taking sessions. Don’t hesitate to try different themes, contexts and lighting to discover the style that suits you best. Finally, practice makes perfect. 1. Get back to nature
The thing about the outdoors is that there are many ways to enjoy it. Birdwatch in your backyard, stroll through the woods or bike around the countryside. Alternatively, enjoy a picnic in the park or pack your fishing rod and spend a weekend camping by the lake.

How to avoid heat stress
As you age, it’s more difficult for your body to regulate heat, which can result in health complications. Here are some tips to help you survive the hot summer days ahead.
Keep your space cool
If you don’t have an air conditioner, use fans to circulate the air in your home. Close curtains and windows during the day to block out the hot sun and open them on cooler nights to lower the temperature inside.
Hydrate regularly
Seniors are less likely to feel thirsty than younger people. Therefore, it’s important to prevent dehydration by drinking frequently, even before you feel thirsty. Drink cool water, eat juicy fruits and vegetables and avoid drinks that can dehydrate you, like alcohol and coffee.
Refresh yourself
Try to keep your body at a healthy temperature. You can do this by taking refreshing showers or baths, swimming or covering your skin with wet towels.
Limit your activities
Avoid strenuous activities that raise your body temperature and create discomfort. On hot days, do outdoor activities in the shade and wear appropriate clothing and accessories like a hat and sunglasses.
Heat stress shouldn’t be taken lightly. If you or a loved one are affected by it, contact a health professional immediately.

Who’s Got Your Back? Hospice Chaplain Explains Role of Support Systems
BY PASTOR RANDY SPAHR, chaplain with Hospice of the Red River ValleyA story I often like to share is about love and support. In the story, a teacher asks her kindergarten class what they’d like to be when they grow up. Most students give “typical” answers, like firefighter, nurse, astronaut or farmer. But one student walks to the front of the room and proudly proclaims “lion tamer.” He goes on to explain the fearlessness and skill he would show in that role before pausing and saying, “And, my mom will be right there with me!”
I love that story. It reminds me we all need someone to have our backs, especially during tough times or grief. We all have times when we really need to know that someone is there to support us—whether it be a debilitating medical diagnosis, losing a job or the death of a loved one.
Our society often tells us that to be strong, we need to be independent and manage tough times by ourselves. Our natural tendency can be to pull away from people when we’re experiencing a loss or difficult time. That’s exactly when we need the most support. Not only do we need others in those times, but we also can benefit from different perspectives. Pain and loss can narrow our focus and make the “big picture” harder to recognize or accept. The people we trust can help bring the picture back into focus for us.
Independence alone does not make us strong. Rather, friends, family and other support systems bring us strength, reassurance and comfort. We need others in our lives to provide balance for our emotional, spiritual and even physical well-being. Others help us be better, and we, in return, can help them. A lesson can be learned from geese every spring: The geese fly over our heads in a strong v-pattern, reminding us that if we flock together, we can overcome obstacles and journey forward together.
It reminds of a verse from Romans 12:15: “Rejoice with those with those who rejoice; mourn with those who mourn.” Others may not be able to fix all our problems, but they can give us encouragement and assistance when we need it most.
During my freshman year of college, I was in the best shape of my life because my roommate and I got up early every morning and ran 3-5 miles If I would’ve
done that on my own, I would’ve slept in a lot of mornings, but we encouraged each other and challenged each other, so we were successful.
So, who’s got your back? Who is supporting you? Often, the people keeping an eye out for us are the same people we hold the closest. Our family
and friends, who we celebrate wins or life events with, are the same people we can lean on in life’s darker moments. It’s important for our overall well-being to develop a comfortable, dependable support system, so we can share sharing our story and feelings.
It’s never too late to develop friendships and relationships. Even if you find yourself in a new city or far from family, you can seek out community and companionship anywhere; there will always be people waiting with open arms to welcome you.

INGREDIENTS
Salad
• 1 box (16 ounces) Campanella (or other short pasta of your choice), cooked and drained
• 2 cups romaine lettuce, torn
• 16 cherry tomatoes, halved
• 1 cup store-bought croutons
• 2 chicken breasts, cooked and diced
• 4 tablespoons grated Parmesan cheese, for garnish
• 1/4 cup dried cranberries
Dressing
• 1/4 cup store-bought mayonnaise
• 1/4 cup plain Greek yogurt
• 1 clove of garlic, minced
• 1/4 cup grated Parmesan cheese
• 2 tablespoons Dijon mustard
• 2 tablespoons olive oil
• 2 tablespoons lemon juice
• Salt and pepper, to taste
DIRECTIONS
1. Combine the pasta, lettuce, cherry tomatoes, croutons and chicken in a large bowl.
2. In a separate bowl, whisk together the dressing ingredients.
3. Pour the dressing over the pasta and toss well. Add salt and pepper to taste.
4. Arrange pasta salad in serving bowls and top with grated Parmesan cheese.
Sweet and juicy, fruit makes a great summer dessert. This delicious glaze enhances the natural flavor of the fruit to create a crowdpleasing favorite.

INGREDIENTS
Skewers
• 10 strawberries, halved lengthwise
• 10 kiwi cubes
• 10 blueberries
• 10 pieces of pineapple
• 10 pieces of cantaloupe
• 10 wooden skewers
• Simple syrup
• 1/2 cup water
• 1/2 cup brown sugar
DIRECTIONS
1. Combine the water and brown sugar in a saucepan and cook over medium heat until the sugar is completely dissolved. If the mixture starts to boil, lower the heat slightly.
2. Once the sugar has completely dissolved, remove from the heat and allow to cool for 45 minutes.
3. Thread the fruit pieces one after the other on the skewers.
4. Brush the skewers with the simple syrup.

Barbecue grilled sweet corn
Do you usually boil your corn on the cob and slather it with butter and salt? Dare to think outside the box at your next barbecue with this recipe.
INGREDIENTS
• 6 ears of fresh sweet corn with husks
• 1/2 cup sour cream
• 1/2 cup mayonnaise
• 1 clove of garlic, minced
• Salt and pepper, to taste
• 1/4 cup crumbled feta cheese, for garnish
• 1 teaspoon paprika, for garnish
• 1 bunch of fresh coriander, chopped, for garnish
DIRECTIONS
1. Preheat the barbecue on high.
2. Soak the cobs for about 10 minutes in a bowl of cold water. Keep the husks on. Drain.
3. Grill the cobs for 12 minutes or until the husks are charred. Peel the cobs and continue cooking until they’re toasted in some places. Turn the cobs often to prevent them from burning. Remove from the grill and allow to cool for a few minutes before serving.
4. In a bowl, combine sour cream, mayonnaise and garlic. Add salt and pepper. Brush the ears with this mixture. Sprinkle with feta cheese, paprika and cilantro.

DIRECTIONS
This poke bowl is packed with tasty and nutritious ingredients and makes the perfect healthy summer treat.
Servings: 4
INGREDIENTS
• 2 sweet potatoes, diced
• 2 cups baby spinach
• 4 cups quinoa, cooked and cooled
• 20 cherry tomatoes, cut in half or quarters
• 2 cups arugula
• 1 can chickpeas, drained
• 2 ripe avocados
• 1 tablespoon white sesame seeds
• 1 tablespoon black sesame seeds
• 1/2 cup maple syrup
• 1/4 cup olive oil
• 1/4 cup soy sauce
• Salt and pepper, to taste
• 1 tablespoon sriracha sauce (optional)
1. Preheat the oven to 350 F.
2. Place the sweet potatoes on a baking sheet with a little bit of olive oil and bake for 30 minutes or until tender. Season with salt and pepper.
3. Place the baby spinach in the bottom of four serving bowls. Arrange the quinoa, cherry tomatoes, cooked sweet potatoes, arugula and chickpeas in separate sections. Set aside in a cool place.
4. Cut the avocados in half lengthwise. Remove the pit and skin. Place one avocado half on a cutting board. Cut into thin
slices crosswise, taking care to maintain the shape. Using your hands, fan out the slices. Make sure to keep them stuck together to create a straight line. Take one end́ of the line and roll it over itself until it becomes an avocado flower. Repeat with the other halves.
5. Gently lift each avocado flower and place one in the middle of each bowl. Sprinkle a quarter of the white and black sesame seeds over each bowl.
6. Combine the maple syrup, olive oil and soy sauce in a separate bowl. Pour a quarter
of the sauce over each bowl or serve separately.

Roasted tomato and goat cheese bruschetta Recipe
Are you having guests over for a casual meal? These cheesy starters are sure to be a hit.
Servings: 8
INGREDIENTS
• 20 cherry tomatoes, cut in half
• 1/4 cup olive oil
• 8 slices of baguette bread
• 8 tablespoons goat cheese
• Fresh basil, coarsely chopped
• Salt and pepper, to taste
• Honey, for garnish (optional)
Drizzle some honey over the bruschetta before serving for a subtle sweetness.
DIRECTIONS
1. Preheat the oven to 350 F.
2. Toss the cherry tomatoes with olive oil and spread them into an oven-safe baking dish. Season with salt and pepper. Bake for about 15 minutes or until the tomatoes start to pucker.
3. Lay the bread slices on a baking sheet and toast in the oven until crisp.
4. Spread a tablespoon of goat cheese on each slice of bread.
5. Add some fresh basil and the roasted tomatoes.

The Grief You’re Feeling is Normal & Tips for Coping With It
Is there a “normal” way to feel grief?
Grief is a normal, natural part of the human experience. Every person grieves a loss in their lifetime—whether it’s a loss through death or a loss of a relationship, a job, sense of normalcy or some other facet important to the person. Understanding how grief can affect us and being aware of the commonalities in all our grief experiences can help us cope in more practical, less isolating ways.
Grieving is not a linear process, and the grief journey can be filled with peaks and valleys. Grief can impact people emotionally, physically, cognitively, behaviorally and spiritually. It can also challenge a person’s belief system. It’s normal to go through a range of reactions with varying intensity.
While everyone responds to grief in different ways, it’s helpful to know there are many common grief reactions.
Common grief responses may include:
• Behavioral: Forgetfulness, avoidance of people or situations, increase or decrease in activity, loss of interest in work or social activities
• Physical: Sighing, fatigue or exhaustion, hollow feeling in stomach, sobbing, change in appetite, headache, sleeping more/less or experiencing interrupted sleep
• Cognitive: Difficulty concentrating or making decisions, confusion, increase or decrease in dreams
• Emotional: Anger, sadness, mood swings, guilt, relief, loneliness, numbness, fear, helplessness, longing/yearning for loved one
• Spiritual: Loss or strengthening of faith, need to find meaning in the loss, asking “why” questions, anger at God or other religious figures
Grief can be painful and overwhelming; the experience can also feel frightening and worrisome.
Mourning is the process involved in how you adapt to the loss and your grief reactions. Many bereaved describe this experience as a roller coaster ride with a lot of ups and downs and twists and turns.
Grief is a unique experience. Everyone will have different reactions and journeys when navigating and living with grief.
Some factors that can affect how you deal with grief are your relationship to the person who died, belief systems, circumstances of the death, your own personal coping skills and the resources available to you. Working through grief involves accepting the loss, experiencing the pain of that loss, adjusting to life without your loved one and creating a different connection or relationship with the person who has died.
Here are six helpful activities to do while grieving a loss:
• Reach out to your support system. Stay connected with your family, friends, faith community, social group or therapist. It’s important to look at all the support systems in your life and stay open to new systems that might not have been in
your life before the loss. Talking regularly about grief and sharing your feelings and memories with people you trust can be helpful in coping with your grief.
• Join a support group. You may not feel ready to join a support group immediately, but over time, you may be open to it and find that sharing your experience with others can offer comfort. You might also learn from others in the groups—sharing coping mechanisms and talking through the difficulties of grief could help you in your own grieving process.
• Take care of yourself. Get a good night’s rest, 7-9 hours of sleep. Take care of your nutritional health by eating healthy balanced meals and drinking plenty of water.
• Be patient with yourself. You can’t rush grief. It’s going to take as long as it needs to settle. Try not to be too critical of yourself. The grieving process is normal and personal—and you will grieve in your own way and that’s OK. Treat yourself with the same gentle care and kindness that you would offer to a friend going through a similar situation.
• Practice self-care. Some techniques that can help include journaling,
meditation, daily affirmations, creating a list of things for which you are grateful. You can also try learning a new skill or expressing yourself creatively through a hands-on approach (sewing, woodworking, knitting or other crafts).
• Get moving. Be active and participate in your favorite physical activities. You can try yoga, golf, pickle ball, jogging or biking. Yardwork and gardening are also great ways to stay active. Spend some time outdoors by going for walks or jogs. You can even just sit outside to enjoy the sunshine and fresh air.
There are many resources available to help you through your grief. Hospice of the Red River Valley offers an extensive grief support program—not only to family members of the individuals who were cared for by the organization—but also for community members.
The grief support team at Hospice of the Red River Valley offers free grief support to individuals and families moving through the grief process by providing support groups, education and training, and individual grief support for community members. The services extend to providing guidance to schools and workplaces, as well. Anyone may attend a grief support group or class regardless of whether or not your loved one was served by Hospice of the Red River Valley.
