FYI: Foster Youth Involved
Entering Foster Care This is a form for the youth who is entering foster care to fill out. He/she should give a copy to whomever they think might need this information, for example a caseworker, a foster parent, a lawyer, CASA or Guardian Ad Litem. Please take my opinions into account when you are creating a plan for me. If you are not able to meet any of my wishes listed below, please inform me and explain why not as soon as possible.
