Now’s the perfect time to have your child’s eyes checked. Undetected vision problems can affect how well a child adjusts to the new school year and performs in the classroom. Children who have vision problems are often misdiagnosed as having learning disabilities. All children, even those with no signs of vision problems, should have their eyes checked at regular intervals. It is recommended that newborn infants have their eyes checked while still in the hospital nursery. This examination can help detect several congenital eye problems, some of which can be very serious and permanently threaten vision. Between six months and one year of age, an Optometrist or a physician should check an infant’s eyesight and many parents seek out an InfantSee provider who will provide a FREE comprehensive eye examination to any child 0 to 12 months of age. Children’s vision should again be tested between ages three and four by an experienced eyecare provider who will evaluate the health of the eyes and the development of appropriate visual skills. Approximately one in 20 pre-school age children will have a vision problem and that number increases as children grow older and enter school. According to the Prevent Blindness America (PBA) organization, more than 12.1 million school-age children, or about one in four, have some form of vision problem. In-school screenings can help detect any severe vision problems, but your child should still see an eye doctor if there are any symptoms or if the child fails the screening test. | YOU ladies night out | september 19, 2010 | www.fdlreporter.com
To help ensure that their children’s eyes are healthy, parents should be on the lookout for: • Eyes that don’t line up, one eye appears crossed or looks out • Eyelids that are red-rimmed, crusted or swollen • Eyes that are watery or look inflamed Other telltale signs of a vision problem, are when a school-age child: • Rubs eyes often or covers one eye to see better • Has trouble reading or doing other close-up work • Holds objects close to eyes to see better • Blinks more than usual or seems cranky when doing close work • Complains about things being blurry or hard to see • Can not sustain attention and ADD or ADHD is suggested Any child experiencing vision problems or showing symptoms of eye trouble should receive a comprehensive professional eye examination, regardless of the results of school screenings or other eye checks. Please remember that your child may still have a vision problem, even if he or she does not complain or show any physical signs. The good news is that most eye problems can be corrected if they are detected and treated early. Some problems, if left untreated, even for a short period of time, can result in permanent vision loss. The more you know about your children’s eyesight, the better you can help protect them. Getting a professional eye exam is the best way to diagnose any potential problems and to ensure healthy vision for your child, now and in the future. Make sure the primary care physician is the one you choose, not the one that is assigned to you. As you get older, body parts don’t function as well as they once did. You could find yourself in the Emergency Department one day with no primary care physician. When that happens, one gets assigned to you. Wouldn’t you rather be the one who researches and chooses a physician who understands your needs and treatment preferences?

Prevent health problems. This is the most important reason. In the West, it seems medicine is geared towards treating health problems and not preventing them. While it’s true that doctors spend most their time treating people who are already ill, more and more doctors are focusing on preventing their patients’ health problems before they start. A regular physical exam is one tool to accomplish this goal. For example, if your doctor notices that your blood pressure is too high, a diet and fitness plan may be suggested to help reduce your blood pressure before you have a heart attack. Also, when you get your physical, your doctor will ask you about your family’s health history. If your family has a history of certain diseases or health problems, your doctor can give you guidance on how you can reduce your risk of suffering those ailments.

Have you ever wondered how you are supposed to eat the four daily servings of fruit and five daily servings of vegetables that experts say we need to maintain healthy bodies? If so, you are not alone.
Part of the problem is that we get “stuck” on portions, according to wellness team Tom and Dian Griesel. The good news, says Tom, is that you don’t need to carry a scale and weigh your foods to measure your portion size. Instead, he provides these very simple visualization tricks for his clients who like to track their calories: • It’s easy to estimate what a “small fruit portion” is if you can visualize a baseball, he points out. An apple or orange the size of a baseball is a mere 65 calories and packs plenty of water, fiber and vitamins.

According to experts, we were designed to be lean and to forage, build, protect and live in the harshest of conditions. “Luckily we no longer have to forage for and hunt down our food,” says Dian, “This makes our present-day food challenges seem easy, doesn’t it?” ■

Many parents-to-be want the security of a hospital setting for the birth, along with getting to know the provider and becoming an informed consumer.” Loree Olson | Certified Nurse Midwife. Aurora Health Center My partner Jackie Pierce, CNM, and I, both certified nurse midwives (CNMs), now offer midwifery services at the Aurora Health Center in Fond du Lac and the Aurora Health Center in Ripon. This column will hopefully shed more light on the mid-level providers know as “midwives.” I’m often asked, “What does it take to become a midwife”? Well, a certified nurse midwife will have a nursing background and a degree in midwifery. After becoming a registered nurse, a CNM has earned a master’s degree from an accredited midwifery program, and passed the National Certification Examination of the American College of Nurse-Midwives to become board certified. Jackie and I are graduates of Marquette University’s nurse-midwifery program. Marquette’s program is the only graduate degree program in the state for nurses to become a CNM. This program teaches students to manage the care of women and newborns during the prenatal period, the delivery and the postpartum period as well as providing routine gynecologic care throughout the life span. Before enrolling in Marquette, Jackie earned a bachelor’s degree in nursing from Marian College. She’s been a midwife for over 5 years. I graduated with a bachelor’s
degree in nursing from the University of Wisconsin-Oshkosh and have been a practicing midwife for over 10 years. We are board certified by the American College of Nurse-Midwives. In addition to certified nurse midwives there are other types of midwives. A certified professional midwife (CPN) has completed a master’s-level midwifery program or an apprenticeship with a practicing midwife, but without having a bachelor’s degree in nursing. There is also the “lay” midwife, a person who has received hands-on training through an apprenticeship, but without any formal schooling. I often hear questions about insurance coverage. Coverage for certified midwifery services should be the same as coverage for medical care from an obstetrician-gynecologist. Coverage may be different or denied for services provided by a lay midwife and a home birth. Fond du Lac women will soon have another choice as to where they can deliver their baby. Some women worry they won’t make it to the hospital in time, so they automatically choose the closest hospital for their delivery. Labor takes time, often several hours or more. We do our deliveries at the new Aurora Medical Center in Oshkosh. The hospital, located along Highway 21, is only
about 20 minutes away from Fond du Lac. Fond du Lac area women who go into labor should have ample time to reach the new hospital before the birth is expected. The Aurora Medical Center features 12 LDRP rooms beds for labor, delivery, recovery and postpartum and six beds in a special care nursery. By all accounts it is a warm and inviting place to have a baby. Finally, why do some pregnant patients choose to see a midwife instead of an obstetrician-gynecologist for their care? The answer may vary, but I think most of our patients want to establish a close, open relationship with the person who will be delivering their baby and they prefer to take an active role in the process. They want to learn as much as possible about pregnancy, delivery and various birthing options. Research shows that patients are very satisfied with the one-on-one care they receive from midwives. Many parents-to-be want the security of a hospital setting for the birth, along with getting to know the provider and becoming an informed consumer. James Kemmerling, MD, FACS | Medical Director, Bariatric Surgery Center In the last ten years alone, the national rate of obesity has skyrocketed to nearly 33 percent of the population in many parts of the United States. While our lifestyles have become less active, our stress levels have increased and food has become more abundant. The result has been steadily increasing waistlines. All that excess weight is impacting the life expectancy of our population. Living with obesity increases a person’s chance of developing diabetes by ten times that of an average weight individual. The good news, however, is that serious weight issues can be successfully addressed for many people through bariatric surgery. I am a fellow of the American Society of Metabolic and Bariatric Surgery, and the medical director of the Bariatric Surgery Center at Aurora BayCare Medical Center. I have received excellent results in over 600 surgeries. Bariatric surgery is intended for individuals with serious weight problems – those with a Body Mass Index (BMI) of 40 or greater. Those with a BMI of 35 to 40 may also be eligible if they have additional health problems like diabetes or high blood pressure. I offer three types of bariatric surgery to assist obese individuals to regain control of their weight. They include: • Roux-en-Y Gastric Bypass which combines the restrictive technique of reducing the stomach size to create a quicker sense of fullness with decreased absorption. • Adjustable Gastric Band which uses a silicone band to create a smaller section in the stomach. Digestion and absorption remains normal and the band can be adjusted or fully removed to reverse the procedure. • Gastric Sleeve which limits the about of food a person can eat by reducing the size of the stomach. A portion of the stomach is removed, but digestion and absorption of nutrients follows a natural course that allows them to be absorbed into the body.
Providing the support needed for success One of the most difficult aspects of bariatric surgery is understanding the insurance expectations for coverage. We have experts who deal with this everyday who can expedite this process and make it very easy for our patients.
Most insurance require a three to six-month period where the patient needs to attempt weight loss. In reality, this can be very valuable. For individuals with very high BMIs over 60, it provides the opportunity to lose weight and lower the risks associated with surgery. We do everything we can to address all of a person’s health issues that may complicate the surgery. For example, if we can correct a sleep issue like sleep apnea, a person’s energy level will go up and make them more motivated to increase his or her activity level. Other services available through the Bariatric Surgery Center include access to a dietitian, sleep medicine specialist, psychologist and wellness supervisor.
Patients often wonder if the Bariatric Surgery Center can help them lose weight without surgery, then why have the surgery. One of the biggest differences between medical weight loss and surgical weight loss is in the longevity of the results. We can help someone lose 100 pounds without surgery, through a very low calorie diet. However, most people will gain all the weight back within five years of going off the diet. Surgery, on the other hand, can provide lasting results for a lifetime. Losing weight has a positive impact on a person’s long-term health by lowering their risk for diabetes and cancer, minimizing the stress on their knee and ankle joints and improving his or her sleep. For most people, the benefits will far outweigh the risks.
