Over the past few years, the federal government announced, and carried through, sweeping changes to healthcare services. The move to Care Coordination Organizations (CCOs) is perhaps the most important and consequential change for special needs individuals and their families. So, what’s the full story on this transition?
Naturally, this shakeup has raised concerns for many families statewide. Now that the system has completed its transition, people want to know—
Before CCOs, people with intellectual and/or developmental disabilities (I/DD) were assigned a Medicaid Service Coordinator. That MSC was an employee of an actual service provider, an arrangement that offered both advantages and disadvantages to the service recipient. A big challenge of the old system was that recipients and their care team had to coordinate with several agencies to access various kinds of services.

Their successes include: Enlisting Care Managers to create individualized Life Plans, which outline the unique supports an individual needs to achieve their desired outcomes, as well as a network of providers who will deliver the services. How will my loved one benefit from a CCO? What is the CCO’s ultimate objective? How does the Care Manager ensure individualized care? And what kind of life-changing supports and services can we expect to receive?

In mid-2018, Medicaid rolled out a new and improved system of care to enable people with I/DD to plan all their needs in one place—including disability services, community-based supports, health and wellness services. Part of that new system was the formation of seven CCOs across New York State. MSCs, now renamed Care Managers, are hereby employed directly by these CCOs, creating a unified, simplified structure for coordinating all provided services.
The truth is that every family with a loved one who is eligible for services under I/DD needs more help than has been offered to date. This is why CCOs consider their responsibility not a one-time service but a lifelong commitment to maintain a deep, honest, people-first relationship with the Care Manager, care recipient and their family.
For people with I/DD, the support of a CCO means more holistic, comprehensive care; more options and personal choice; more care flexibility; and fewer barriers to access.
To ensure CCOs do right by their clients, the state put the power to choose in the hands of the care recipient: a person-centered approach. This led to CCOs that are

Coordinating healthcare, hospital discharges, homecare, education and other services for individuals as they become integrated in new settings. Encouraging people to embrace independence by mastering activities of daily living, building friendships and cultivating hobbies. Granting individuals with I/DD opportunities for meaningful contribution to a community in transition, while assuming more control over their lives. Most importantly, these new service developments come as a welcome reminder that our communities are full of good people doing good work. Care can be a confusing thing, but CCOs are here to remove doubt, answer all questions, coordinate services on the individual level, and make life the best it can be.

LIBBY’S LAST HOPE — AS TOLD BY SHONI NAGEL
Do you know what’s worse than finding out your child has cancer? It’s finding out she has cancer again. Our story started when Libby was 3 and a half years old. She was lethargic, and our pediatrician urged us to get it checked out. A minute into the ultrasound, the doctor turned off the machine and diagnosed her on the spot. “Your life is about to be turned upside down. Your child has neuroblastoma.” There was a 50% chance of survival. She responded beautifully to treatment. Eight rounds of chemo later, along with a bone marrow transplant, radiation, immunotherapy, and surgery, the tumor was gone. I was on the road when the oncologist called again. “This is not the phone call I wanted to have with you,” she said. A brain tumor the size of a golf ball rested at the front of Libby’s head. I opened the car door and threw up. The chances of survival were slimmer this time–much slimmer. The doctors had no hope for us. “Just go home,” I remember them telling us sympathetically. We didn’t go home. Instead, we transplanted our family from our hometown in Sydney, Australia to New York. She went into radiation with Sloan-Kettering immediately. Followed by more chemo, surgeries, and hundreds of platelet infusions.

When you have a sick child, you do a lot of things differently. You eat more takeout because you can’t possibly come home from the hospital and have hot food ready. We were worried about food, nannies, babysitters, parking, rent and medical insurance. My husband left his business behind in Australia. We spoke to the hospital and as an international patient, they wanted 1.4 million dollars upfront before they would even treat us. We heard about RCCS. They kept phoning us. Eventually, I gave them the time of day—I’m so glad I did. They told us, “This is what we can do for you. Just tell us where to send the check.” They offered us housing support, helped pay our rent, and gave attention to our other children, making them feel important when all of the focus was on Libby. A MIRACLE OF SCIENCE—AS TOLD BY YOCHEVED I was a kallah. Eighteen years old. I’d just tried on the wedding gown of my dreams when I felt a sharp, stabbing pain in my shoulder. “It’s probably nothing,” my mother reassured me. We stopped at the family doctor between meeting with the sheitel macher and choosing flowers for the hall. (Roses and hydrangeas, if you must know..) The doctor wasn’t worried, but he referred us to another doctor, who referred us to another. Meanwhile, I tried ice packs and heat packs and endless Motrin. I noticed a lump forming on my shoulder. It was getting harder and harder to concentrate on my wedding details. “We’ll do an MRI, and that might give us some information,” my doctor finally said. We took the tests and waited. “Watch,” I said to my mother, “I’ll wake up the day after all these fancy tests and the pain will be gone.” Three days later, we got our results. The doctor was blunt. “It’s a tumor,” he said, looking directly at me and then at my mother. I was in denial. I waited for my mother to explain that it was a sore shoulder, nothing serious. But her face had turned pale, and she shook like a leaf as the doctor spoke again. “We’ll take a biopsy to figure out what kind of tumor it is, and then we can discuss treatment.” The reports showed a very rare form of cancer growing in my shoulder bone. Because it was so rare, my parents had no idea which doctors to turn to. They called RCCS for guidance, finding doctors, and figuring out a treatment plan. For days I locked myself in my room and cried. My mother, sitting at the edge of my bed one night, gently explained how we’d proceed. Because of the size and location, it would be a risky surgery. I could lose my arm. Or my life. The days leading up to the surgery passed in a blur of prayers and tears. My parents called my chasan, Mordechai’s parents to break the news. He decided to commit to marrying me–no matter what the future would bring. I cried tears of joy and gratitude. It was Mordechai’s loyalty and commitment to our future that kept me sane throughout the saga. With his image in my mind, I was

wheeled into the operating room, and he was the first one I thought of when we were told it had been a success. The tumor had been completely removed, and my arm had not been damaged in the process. Once again I busied myself with the wedding preparations, no longer a naive bride. My future had almost been taken from me, and now that I had it back, it was the only thing that seemed to matter. We began our married life together, falling into an ordinary routine. For almost a full year my scans were clear and I believed that the cancer was gone for good. But then one day, about ten months after the wedding, I woke up at dawn to sudden pain in my shoulder. I didn’t need any scans to tell me what it was; the cancer was back. I sobbed all morning. I had come to accept that my brush with a terrible illness had helped me grow. But I didn’t want to do it again. It was different this time around. Because the area had already been operated on, a repeat surgery would likely mean an amputation. It was such a rare form of cancer that there were no other known nonsurgical treatments that had been proven to be successful. In their search for an alternative to surgery, RCCS had my records distributed to world-renowned oncologists and held conferences with top sarcoma institutions. They introduced us to genetic profiling, which involved testing the original tumor that was removed to see if it had certain genetic mutations found in other cancers. While I tried to imagine living without an arm, the research team matched my tumor to other cancers–and a drug. A miracle pill that treated those cancers–and could potentially treat mine. Many doctors told us the pill was a waste of time, but others insisted it was worth a try. Gedolim, including our Rebbe, encouraged us to try the alternative option, and with their blessings, we decided to wade into unknown territory. Miraculously, within the first two months of treatment, the scans showed that the tumor was shrinking. The turnaround was shocking for everyone; no one had anticipated such quick and successful results. We left Baltimore amid much fanfare. My story was written up in medical journals, and the doctors kept telling us how fortunate we were. I didn’t have to be told.

I was 24–just starting my career postgraduate school, newly married, prepared to live a perfect life. I was never expecting to get a brain tumor. Several months after my wedding, I’d been waking up with terrible headaches. I didn’t think much of them, although it made my wife nervous. “Go to a doctor!” she urged me, but I couldn’t bother with the hassle of it all–spending two hours in a waiting room to hear that nothing was going on. Sometime later, I started noticing that the right side of my face was losing muscle movement. Okay. I’m not the worrying type. Then, I lost some hearing in my right ear. I finally went to get tested. It was summer, so as we waited for the test results, we flew down to Dallas, my hometown, so my son–a one-year-old– could bond with my parents. One morning, I was enjoying breakfast with my family when I get a phone call from my doctor’s office. They found a brain tumor. It was embedded four inches deep into my brain. It was in a difficult location, and there were only three doctors in the country willing to perform the surgery. The surgery would cost 80-100k, out of pocket. The only insurance that would cover it had a premium of at least 10k.The last thing you want to think about when preparing for a lifesaving surgery is, how can we afford this? RCCS stepped up to the plate right away. Our insurance bills

were taken care of, just like that. We just filled out a couple papers and then we got a check in the mail. Everything was fast, discreet, impeccable– unbelievable. I left my wife at the door of the operating room, not knowing what was going to happen. You don’t know what to expect when you have brain surgery. It was cold in the room. I heard utensils clinging and rattling. They were opening my brain. There were drilling holes into my skull. I was scared that they would hit a nerve that would take away the use of my arm, leg or eyesight. Everything was in Hashem’s hands. 10.5 hours later, they completed a successful surgery. Tumor: the size of a golf ball. I was home for 2 months trying to learn how to walk again, how to talk again. But after that? It was like nothing ever happened. I’m walking, working, volunteering again for Hatzolah. When RCCS came through for us, it took away three fourths of the stress. You’d think, when it comes to health, who cares about money? But when it comes to complicated surgeries, to survive, you need the best. And if you need the best, you need the best insurance. A CHANUKAH MIRACLE — AS TOLD BY MALKY BERKOVIC
It was late September, the beginning of Yom Tov season, when the air turns crisp and the house smells like honey cake and the little ones keep borrowing Totty’s shofar to “practice”. As the mother of 9 children, I didn’t have time for anything except cooking, making sure the kids had enough clothes for Rosh Hashana, and preparing myself for Yom HaDin.
But illness doesn’t come politely, making sure it’s a good time for you. The diagnosis, cancer, came swiftly and suddenly, thrusting my happy, boisterous, spirited life into one of turmoil, sadness, and dread. Where’s Mommy? My innocent children watched their mother morph from a loving mother into a living zombie, floating from one round of chemo to the next. My mind was in a constant, relentless haze. Some mornings I would wake up, listen to the sounds of the kids grabbing breakfast bars and heading to the bus while my husband was in shul, and dive further under my cover, disappearing into the only respite from fog and nausea: sleep. By December, I’d gone through six rounds of chemo. As surgery loomed closer, I was totally depleted–physically, emotionally, spiritually. My home was in total disarray, and my children felt like they had lost their Mommy already. Our normally busy, happy household was now in total shambles instead of joy and excitement. Stress and

neglect hung in the air. I was scheduled for surgery right before Chanukah. The house was definitely not in the spirit of the day–instead of lightness and excitement, everyone was on edge, nerves frayed, waiting anxiously for Mommy to come home– hopefully in time to see the Chanukah lights. I traveled home from the hospital on the third day of Chanukah. The surgery was a success, Baruch Hashem, but I was in terrible pain. I felt quite sorry for myself–but I felt much worse for my poor children. I knew I’d have no energy to fry latkes, cheer them up, and show them that Mommy was back.
I got home–and my jaw dropped. Sitting on my front porch were seven absolutely humongous packages, all beautifully wrapped. When my children came home from school, instead of letting them get distracted by my lack of energy, I directed them toward the incredible gifts RCCS had sent us. Every mission has its movers and shakers. While many people must be shown the way, others jump into the thick of it—like the remarkable hostesses of this year’s RCCS home parties. It’s easy to cheer on the action. It’s another thing to plan, arrange, bake, and host for the cause.
This book is the culmination of hundreds of workhours and barrels of midnight oil. The credit belongs to the dedicated, brilliant creatives who poured their ingenuity and best efforts into this roaring success. From the first stages of ideation through the last moments of printing and delivery, they kept true to the mission. We are thankful beyond words, just as their contributions went beyond the printed page.
We don’t have a clue how they managed to do it all, on top of their full family lives, bloated calendars, and the duties of the everyday. We also know they never asked for recognition, applause or credit. Their mission is to rally the community around helping Cholei Yisroel. That makes it our duty to do just that, to honor their devotion with this page.
