Essential Tremor and the Emerging Role of Medical Cannabis
ESSENTIAL TREMOR (ET), a neuro-logical disorder affecting nearly 7 million Americans, is characterized by involuntary and rhythmic shaking, most commonly of the hands. It is worth noting that essential tremor can run in families, a hereditary form known as familial tremor. While ET can occur at any age, the disorder is particularly prevalent among older adults and often worsens over time. Essential tremor is distinct from Parkinson’s disease tremor, which typically occurs at rest and has different underlying mechanisms. ET primarily manifests as a postural tremor, occurring when a person holds a position against gravity. This type of tremor has been recognized for hundreds of years and is known to be temporarily suppressed by alcohol consumption. However, recommending alcohol as a treatment option is not viable due to the risk of developing alcoholism and other long-term negative health effects. In contrast, cannabis offers a potential treatment solution that may suppress tremor without the associated risks of addiction and long-term damage linked to alcohol. The endocannabinoid system (ECS), a complex cell-signaling network present throughout the body, plays a key role in regulating motor control. Cannabinoid receptors, particularly CB1 receptors found in the central nervous system, modulate neuronal excitability, which could influence tremor activity. By interacting with the ECS, cannabinoids like tetrahydrocannabinol (THC) and cannabidiol (CBD) offer a novel mechanism to address the dysregulated neural activity underlying ET. Cannabinoids have been documented to facilitate muscle relaxation and provide neuroprotective effects. By helping to relax muscles, cannabinoids may reduce
involuntary contractions, while their neuroprotective properties may help preserve normal function and delay disease progression
CURRENT EVIDENCE AND CHALLENGES
Although the scientific basis for using medical cannabis in ET is compelling, clinical research in this area is still in its infancy.
A notable early study by the University of California, San Diego, examined the effects of inhaled cannabis on ET patients. Results demonstrated significant tremor reduction in the majority of participants, underscoring the potential therapeutic benefits of cannabinoids. Other research efforts are investigating alternative delivery methods, such as oral formulations and sublingual sprays, to enhance efficacy and minimize psychoactive side effects associated with THC.
Research regarding the use of cannabis as a treatment for essential tremor has been limited. However, if one were to extrapolate results of studies on other diseases in
which tremor is prevalent, such as MS and Parkinson’s, these would support the use of cannabis as a plausible treatment option in one’s comprehensive care plan. While we have not yet conducted a study at DENT on the use of cannabis in essential tremor, we have studied the use of cannabis on other conditions where tremor is prevalent, such as Multiple Sclerosis (MS) and Parkinson’s disease. In our Parkinson’s study, we found that 48% of patients suffering from tremor reported a noticeable decrease in tremor activity after initiation of medical cannabis therapy. Patients most often used a 1:1 ratio tincture with total daily mg consumption of THC and CBD ranging from 2mg to 15mg.
A Cannabuff reader had recently inquired if a 4.2mg THC gummy could help to ease essential tremor. Based on available studies, one cannot definitively state that a 4.2mg gummy will help mitigate the symptoms of essential tremor. However, it is plausible that, on an individual basis, when incorporated into a comprehensive treatment plan, cannabinoid-based therapies may help with essential tremor. It is important to remember that each person is unique and has different needs with respect to their medical treatment as well as their endocannabinoid system. As with any condition one wants to treat with medical cannabis products, remember to start low, go slow, modifying dosing after 2 weeks of consistent use, and making sure to involve a knowledgeable healthcare professional in any treatment decision.
LOOKING AHEAD: A PATH TO LEGITIMACY
Despite these promising developments, barriers to widespread clinical adoption persist. The federal classification of cannabis
as a Schedule I substance in the United States continues to restrict research funding and limit the scale of clinical trials. Additionally, many healthcare providers remain hesitant to recommend medical cannabis due to limited standardization in dosing, formulation, and long-term safety data.
To fully realize the potential of medical cannabis in ET management, we must address these challenges through rigorous, large-scale clinical trials that evaluate the safety and efficacy of cannabis-based therapies. Partnerships between academic institutions, biotech companies, and advocacy organizations can drive innovation and funding to fill the current knowledge gaps. Moreover, regulatory reforms at the federal level are critical to facilitate research and ensure patient access to high-quality, standardized cannabis products. It is also essential to educate clinicians and patients about the
potential role of cannabis in ET treatment. This includes providing resources to guide proper dosing and product selection while emphasizing the importance of using cannabis under medical supervision. As more data emerges, we anticipate the development of targeted cannabinoid therapies tailored to the specific needs of ET patients, potentially transforming care for this underserved population.
CONCLUSION
For individuals living with essential tremor, medical cannabis represents more than a potential therapy— it offers hope. While significant hurdles remain, the convergence of scientific innovation and patient advocacy has the potential to unlock a new era of treatment options. By embracing evidence-based research and fostering collaboration, we can empower patients with ET to regain control over their lives and improve their quality of life through cannabis-based medicine.

A WITHERING INDUSTRY: The Decline of Medical Marijuana in Buffalo
THE ONCE-PROMISING medical cannabis industry in Buffalo is facing a bleak reality. The recent closure of The Botanist, downtown Buffalo’s first medical dispensary, marks another significant loss for patients seeking relief.
This follows the closure of MedMen’s Buffalo store earlier this year, leaving a dwindling number of options for those in need. MedMen closed their store on Transit Road in the town of Clarence after the parent company, a Multi-state cannabis operator or “MSO” declared bankruptcy. This was just 6 years after MedMen achieved a whopping $3 billion valuation after its Canadian IPO.
Why can’t medical dispensaries stay open in Buffalo?
The illicit market continues to siphon off potential customers. Despite the legalization of adult-use
cannabis, the black market remains a persistent challenge. Several unlicensed stores continue to operate in the region with impunity. Decreasing penalties for possession and legalizing home cultivation makes this problem even harder to reign in. Additionally, the proliferation of adult-use dispensaries, particularly those located on Native American lands, has further intensified competition.
Trips to “the res” are normal for WNYers. Many people in the region already go to the res to buy tax-free cigarettes and gas. Dispensaries on Native land now offer another reason to make the trip.
The impact of these closures on medical patients is profound. With only one medical dispensary remaining, access to this medicine is becoming difficult. Patients may face longer wait times, limited product availability, and potential disruptions to their treatment. Moreover, the closure of these dispensaries has led to significant job losses and economic hardship for our cannabis community. The future of medical marijuana in Buffalo is uncertain.
