Pre-Approval commenced and routed to HoS – when approved it is sent back to the OT to commence application excursion # created. General details collected. Routed to Head of Primary or Faculty HoD upon submission Commence PreApproval

Head of Primary or Faculty HoD confirms budget availability. Routed to PA to HoS (Primary or Secondary) upon submission

Step 1: HOD/HOS Budget Available?

PA to HoS confirms date with HoS in negotiation with OT Routed to OT when date approved and entered onto calendar. PA to provide copy of previous event review to OT

PA to HoS assess calendar, confirm date with HoS (as appropriate), enter on calendar.

Step 2 (PA to HoS): Confirm Date

Advise Year Advisor

Advise dep. HoP

OT complete Risk assessment and receive approval from WHS Officer. OT submit leave variation. E&EO create parent note, complete bus and event bookings, Parent lounge etc. Upon submit, application routed to HoS.

WHS Officer approves RA

Organising Teacher completes RA, Parent Note, bus bookings, leave variation. Setup Parent Lounge tasks

Step 3 (Organising Teacher) Complete Documentation

Step 4 (HoS) Approval

Advise Deputy Principal

HoS approve. Upon submit, OT receives confirmation to proceed and access to complete review questions.

OT reviews the excursion and submits to the WHS Officer – relevant information given to HoS and event is closed

Step 4 (Organising Teacher) Excursion Proceeds

Step 5 (Organising Teacher) Excursion Reviewed

Organising Teacher accesses rolls, first aid kit and other equipment

Organising Teacher completes review questions (curriculum, risk)

E&E Officer confirms completion
