• With weak immunity (ex. Elders, on dialysis or cancer treatments)
• Positive COVID-19 test in the past 5 days

Our approach to quality and safety:
Gila River Health Care uses evidence-based medicine and clinical decision support tools, engages in performance measurement and improvement, measures and responds to patient experiences and satisfaction, and practices population health management.
Coordinated care:
Care is coordinated across the broader health care system, including specialty care along with community and support services
The role of your Patient-Centered Care Team is:
• Explain medical care and medications so you understand.
• Listen to your concerns, answer questions about your care, and assist you (the patient) with decision-making.
• Arrange additional testing or evaluation if needed.
• Arrange referrals for specialty care if needed.
• Provide clear instructions about care and treatment plans that you agree on.
• Assist with health education, health promotion, and basic assistance management.
• Help you find the appropriate team member for a specific medical service.
Your role as the patient:
• Schedule and attend your appointments.
• Call ahead if you are unable to make your scheduled appointment.
• Tell us your health history, symptoms, goals, and other important information about your health.
• Tell us about any changes in yourhealth.
• Keep a list of all medications you routinely take and bring list to your appointments.
• Follow your provider’s instructions and take your medications as prescribed.

FOOT PROBLEMS > 75% of people will experience foot problems in their lifetime
COMFY SHOES > Shoes should fit comfortably right away. Always leave a whole thumb’s width between your longest toe and the end of the shoe.
TRIMMED NAILS > Nails should always be trimmed straight across and avoid trimming into corners you cannot see and avoid ripping toenails
GROWING FEET > Feet often flatten as we age - even though the bones do not grow, the fallen arch makes our feet longer
FOOT PROBLEMS > Only elders have foot problems
COMFY SHOES > It is normal to need to “break in” a pair of shoes
TRIMMED NAILS > Cutting a ‘V’ notch in the middle of the toenail will prevent/treat an ingrown nail
GROWING FEET > Feet always stay the same size

TIPS FOR HEALTHY FEET
1. Inspect your feet daily, including the bottoms of your feet. A good time to check is after bathing.
2. Use a handheld mirror to see those hard-to-see/hard-to-reach places.
3. Dry well in between toes after bathing to reduce the risk of athlete’s foot
4. Use lotion on dry skin areas to avoid cracks/fissures. It is best to apply within 15 minutes after bathing, sparing the spaces between toes.
5. Replace walking shoes every six months or 500 miles whichever comes first
6. Do not walk barefoot. Well fitted shoes serve as armor to protect your feet from the environment.

Life Center Services are for everyone not just for patients with diabetes, most services are available without a referral.
All Life Center Services: Available to 10 years and older and with or without diabetes.
Thought of exercising but thought personal training is too pricey?
• Life Center Fitness Centers offer personalized 1:1 training programs for kids (10 +) and adults at no cost to GRIC Members
• Prefer working out independently but would like some guidance, our fitness staff can create an individualized workout routine for you.

TO SHOW SOLIDARITY AND SUPPORT for survivors of sexual assault (The campaign began after a ruling by the Italian Supreme Court where a rape conviction was overturned because the justices felt that since the victim was wearing tight jeans, she must have helped the person who raped her remove her jeans, thereby implying consent. The following day, the women in the Italian Parliament came to work wearing jeans in solidarity with the victim)

Trauma and Addiction

Prenatal Trauma: Raising Children Who Have Been Exposed to Substances in Utero

RN DIALYSIS
LOCATION:
HU HU KAM MEMORIAL HOSPITAL - DIALYSIS EAST
Evaluates patient conditions and collaborates with health care team members and patient family members to deliver care safely and effectively. Maintains and updates patient’s medical records with accurate and timely entries to ensure health care team members have access to relevant information. Supervises assigned team members and delegates work as appropriate to optimize care delivery.
QUALIFICATIONS:
• Graduate of an accredited Registered Nurse program from an accredited university or college; Bachelors’ Degree in nursing or a closely related field of study preferred.
• Registered Nurse Licensure in AZ or qualified Compact Licensure.
• Hemodialysis certification preferred.
• 1 year nursing experience required; Medical/Surgical floor experience preferred.
• Current and continuing BLS training and certification.
• Employment at The Caring House (TCH) requires: AZ Department of Public Safety fingerprint Clearance Card.
