Ask the

Canterbury Vet Q: We had a really long, cold winter this year. Does that mean there will be fewer ticks this summer? A: April is designated Lyme disease Awareness and Prevention month. As the weather begins to improve, everyone will be getting outside more, taking walks in the woods, the park and down the street. During those walks, dogs love picking up things like sticks and leaves, but they also can pick up unwanted hitchhikers like ticks. Ticks do not fly or jump, but instead do something called “quest”. Basically, they wait around in the grass and leaves or on low brush and when something warm-blooded walks by, they grab a ride. Deer ticks are the ones primarily implicated in carrying the lyme disease organism (a bacteria called Borriella borgdorfori). This particular tick is very small, especially the early life stage called a nymph, which is about the size of the head of a pin. You can imagine how difficult it would be to find something that small on a dog’s coat, even if the dog has short hair. Ticks do not stay on the dogs indefinitely. They attach to the skin using their mouthparts and stay for only a few days at most. They then drop off on their own to lay eggs. Most research indicates that the tick has to be attached for at least 24 hours and maybe as long as 48 hours to transmit lyme disease. Obviously, this means that if you do find a tick, you should remove it as soon as possible. The best way to remove a tick is to use a commercial tick remover or come to our office and we’ll remove it for you. You do not want to burn, smother it in Vaseline or squish a tick with your fingers. All of those things can actually make the tick regurgitate. The lyme bacteria is stored in the tick’s salivary system and when it regurgitates, it can inject the bacteria into the dog more readily. Once the lyme bacteria is inside the dog, it can cause a number of different problems. The most common form of lyme disease affects the joints. Affected dogs suddenly become very lame or have intermittent lameness and often have swollen joints that are painful. They often run a fever and don’t feel well. These dogs usually respond very well to antibiotics. There is also a very serious form called lyme nephritis that causes kidney failure. Unfortunately, dogs with this form often do not respond to antibiotics because the damage to the kidneys is irreversible. This form is usually fatal. To diagnose lyme disease, we have a blood test that is run right in the office and takes about ten minutes. The test we use also looks for heartworm, anaplasma and ehrlichia- the last two are also tick-transmitted diseases – which appear to be on the rise in the Capital District. We recommend this test be run annually to screen dogs for exposure to lyme. Unfortunately, many dogs that get lyme nephritis never show signs of lameness, so we hope that 12

by finding these dogs early, we can prevent them from coming down with this devastating form of the disease. We treat dogs that have lyme disease with antibiotics. If the dog is in pain, we also will usually put him on an anti-inflammatory. Dogs with lyme generally respond very quickly and feel better in 1-3 days. However, the antibiotics are continued for a month. With lyme nephritis, we have to treat the kidney failure, which involves hospitalization for IV fluids and monitoring of the blood work. Unfortunately, as was mentioned previously, many of these dogs will die even with intensive treatment. Because of the potential seriousness of lyme, it is better to prevent it than treat it. There are several things you can do to prevent lyme in your pet. Firstly, keeping ticks off will prevent lyme. Remove any ticks you find as soon as possible. Use a veterinary approved topical tick preventative. The other important way of preventing lyme is the lyme vaccine. It is very effective and very safe. You’ll notice we did not discuss cats and lyme disease. That is because not much is known about lyme in cats. There is no test or vaccine for cats. We do however know that it is not the common, potentially serious disease that it is in dogs, so cat owners can breathe a sigh of relief over this one. As you travel along Delaware Avenue over into Albany, do you notice the waters of the Normans Kill flowing many feet below you? You are passing over a vital part of Bethlehem’s history. Petanock, Secktanock and Tawasentha are early Indian names of the creek we know today as the Normans Kill. In Dutch, Normans Kill means Norwegian’s Creek. Albert Andriessen Bradt is that Norwegian. Bradt, his wife Annetie and their three children arrived at Fort Orange on April 7, 1637. One of those children was born aboard ship during the Atlantic crossing. Albert and Annetie named him Storm. They would eventually have five more children. Bradt made his living through a variety of endeavors including saw milling (at one time his mills were producing 6000 broad feet a day), farming, fishing, raising cattle, and dabbling in the fur trade. Much of this activity happened in the environs of the creek that would bear his name. The precursor to Delaware Avenue, the Albany and Delaware Turnpike Company, was chartered in 1805. The company’s road building efforts included a 100 foot long wooden bridge over the Normans Kill. A tollgate was located just over the river on the west shore and Peter Esmay was the toll collector for many years. The hamlet of Normansville soon began forming on both sides of the creek near the bridge with various mills making use of the water power. A hotel, church, and school were established. According to Howell & Tenney’s BiCentennial History of Albany County in 1886 there were 17 dwellings, 22 families and 100 inhabitants. The low, wooden bridge was carried away by a “freshet” in 1868. By this time the turnpike company 22

had abandoned the road. It fell to the Town of Bethlehem to build a new, two- truss iron bridge in 1869. The concrete bridge that is in place today was built in 1913. In those early days, Delaware Avenue traveled a different path starting at the high ground near Graceland Cemetery and winding down into the ravine, over the bridge and then up the hill on the other side. This Yellow Brick Road, so named for its distinctive paving, was a main thoroughfare. In December 1928 the Normanskill Viaduct was completed. This new, high bridge certainly made travel between Albany and Bethlehem easier. Interestingly, the river did not become the dividing line between Albany and Bethlehem in this area until 1967. Before that, Bethlehem included much of the Whitehall Road area. Graceland Cemetery did not get annexed to Albany until 1977.

Looking upstream from the bridge about 1940 you can see one of the dams that used to be on the creek. The building next to the dam is Papalau’s ice house. Message from the Supervisor As I mentioned in January, much of the Town’s focus this year will be on economic growth as well as carefully overseeing residential development. We want to do everything we can to maintain the quality of life residents enjoy in Bethlehem. It is important that we continue to grow our commercial tax base as the pressure for residential growth increases. In h first ffew months h off 2014, I have taken several steps to the strengthen and expand the Town’s economic development efforts. In January, the town hired an Economic Development Coordinator, Liz Staubach, to work within the Department of Economic Development and Planning, under Director Rob Leslie. This new position serves as the initial point of contact for business inquiries, promotes the Town’s Economic Development Strategy, and acts as a liaison between the Town and the Bethlehem IDA (Industrial Development Agency), which is a separate public benefit corporation created under and regulated by state law. The IDA is helping to fund this position, and IDA Board members participated in the recruitment and interview process. Essentially, the Coordinator will organize the Town’s economic development efforts and serve as a resource to businesses, both large and small. Liz Staubach will also serve as a business ombudsperson of sorts, because we want Bethlehem to be as business friendly as possible. Currently, she is exploring ways for the Town and its businesses to access New York State Economic Development programs, including the Start-Up NY Program. With this new approach, the Department of Economic Development and Planning will be a central location for information on state and local programs, and other available resources for businesses. The Department will help new and existing businesses understand and navigate the various loan and incentive programs that may be available to them. Another task of the new coordinator is to develop and implement strategies to market commercial and industrial properties. The first step of this marketing effort will be to raise the Capital Region’s awareness of Bethlehem’s assets. This spring, the IDA will host a tour of Bethlehem for regional developers, real estate agents, stakeholders and State economic development representatives. Bethlehem has so much to offer within the Capital Region, and this will be our opportunity to shine. The tour will include stops at existing businesses, vacant commercial buildings, and commercial and industrial land available for future development, such as the Vista Technology Park, Route 9W Corridor, and the Selkirk Yards Industrial Area. The tour is a chance to show the rest of the Region that Bethlehem is not just a great place to live, but that it is business friendly and has what businesses need to grow and succeed in New York. One of the great economic and civic assets of the Town is the Delaware Avenue Corridor, which helps define and distinguish Bethlehem among other suburban areas in the region. In order to further promote and enhance this resource, the Town Board created the Delaware Avenue Improvement Group. This is a public-private cooperative 26

effort including Delaware Avenue business owners, civic organizations, and residents. With the assistance of the Bethlehem Chamber of Commerce, the Group will evaluate, promote and support Delaware Avenue Improvements. The overarching goal is to maintain and improve this Main Street corridor for commercial, social and civic benefit. Improvements will include beautification, commercial success, traffic flow, safety, pedestrian convenience, and civic improvement. All Delaware Avenue businesses are invited to participate, and information is available through the Chamber of Commerce or my office. While the Group is currently focused on the Delaware Avenue Corridor, I anticipate that the ideas and recommendations that are generated will be replicated throughout the Town. Delaware Avenue is just the starting point. Where feasible, the improvements will be duplicated in other areas of the Town, including the Route 9W and New Scotland Avenue corridors. Many businesses are already realizing the potential and opportunities that exist within the Town and we must continue to market the benefits of doing business in Bethlehem. Within the year, Bethlehem will be home to new retail establishments and I anticipate making many more exciting economic development announcements throughout the summer and fall. John Clarkson,

CAR CARE TIPS Is Your Engine Rough? Maybe It’s Time For A Tune Up by: Matt Ban

Every year, Americans spends thousands of dollars repairing their vehicle. This includes maintenance and basic tune up. However, many have refuse or neglect to take care of their car. Some people think they only need to take their care for service when there is a problem. But the problem here is that those “problems” may be big problem. So, are you one of those Americans? Maybe it’s time for a tune up. The question here is, is your engine running rough? I’m sure many of you guys do not know why or how your engine is running the way it is. Let’s examine here for a bit shall we? When was the last time you brought your car in for a basic oil change? I hope no more than 3,000 miles or 3 month. If that’s the case, it’s time for an oil change. Now, the importance here is, how many miles do you have on your car? If your vehicle is over 100,000 miles, maybe it’s time to switch the regular oil to synthetic oil. After all, synthetic oil adds that extra protection for your car. The next question is, when was the last time you changed your air filter and spark plug? I’m sure it has been a while. Many people do not know that air filter must be changed every 10,000 miles on average. And spark plugs needs to be changed at least every 50,000 to keep engine running in optimum condition. Now, are you one of those people who did not know this? Now in days, a lot of cars use sensors. Sensor purpose is to monitor your engine’s condition and to keep it running efficient. Some sensor are importance to check periodically. For example on a vehicle, the MAF which is mass air flow and IACV the idle air control valve can make your engine run rough. Both monitor the air to adjust the air/fuel ratio on your vehicle. The mass air flow sensor should be cleaned once every 20,000 miles or so and IACV should be cleaned every 60,000 miles or so to keep them both running in optimum condition.

Market values have changed dramatically over the past several years and many, many property assessments are outrageously inaccurate. Town assessors simply don’t have the ability to personally inspect the interior of each home as a real estate professional does. Towns also can’t afford to complete town wide valuations each year. It really isn’t the assessor’s or town’s fault. They have a daunting task of valuing thousands of properties with limited time and resources. However, when the market changes often the homeowner is left paying hundreds if not thousands more each year in property taxes than they should. If you are thinking of selling your home or just concerned that you are over paying on your property taxes you owe it to yourself to review your property assessment. Homeowner’s often spend weeks to months preparing their homes for sale. They will paint, remove clutter, and remodel to get top dollar when they sell. However, the property tax assessment is one area most homeowners overlook prior to sale. There is a direct correlation between the tax burden on a home

and what a buyer can afford or is willing to pay. A lower tax burden simply allows a buyer to afford more home and allocate funds from the tax payment to the mortgage payment, which is obviously preferable to the seller. We recently listed a home for $99,000 that is grossly over assessed and has a tax burden of $7,200. Buyers qualifying for a $100,000 mortgage can’t buy this home because the tax burden per month is more than the mortgage payment and more than any other $100,000 home in the area. In another example, we recently listed a $600,000 home with an assessment over $800,000. The result was a tax burden $6,500+- more than competing $600,000 homes. It makes it hard to compete when a buyer can buy a similar home with $6,500/year lower property tax bill. If you have a luxury home valued over $500,000 and your town has not done a revaluation in the past year or two there is a high probability that your assessment is not in line with current market values.

But if you’re going to get the full benefits from your retirement years — which could last two, or even three, decades — you’ll need to have a vision for what you want to do. Many people think Medicare will cover everything, but that’s not the case — in fact, you could easily spend a few thousand dollars each year, out of pocket, for health care costs. Even beyond normal health care costs, though, you’ll need to be aware that you could eventually need some type of long-term care, such as a stay in a nursing home or assistance from a home health aide. These costs can be enormous; to cope with them, you need to prepare well ahead of time, so you may want to consult with your financial advisor for possible solutions. •How can I protect my family? Your retirement vision can’t just involve yourself, or even just yourself and your spouse. To fully enjoy your retirement years, you’ll want to know that you are helping to protect your grown children from financial and emotional burdens that could fall on them should you become incapacitated in some way. Among the steps you might consider taking is establishing a durable power of attorney, which allows you to appoint an agent to manage your financial affairs, make health care decisions or conduct other business for you during your incapacitation. All About Pharmaceutical Compounding What is pharmaceutical compounding? • Pharmaceutical compounding refers to the preparation of a pharmaceutical product that meets the unique needs of a patient. Prepared by a licensed pharmacist, compounds can be used for patients who are unable to use commercially available products. Compounding does not replicate commercially available products. Why is pharmaceutical compounding necessary? • Compounding is necessary for many patients and for many reasons. Compounding allows for custom dosing for a specific patient, and reformulation of medication to eliminate ingredients a patient may be allergic or sensitive to. • Compounding is helpful for products used for children and animals that often require doses of medication much lower than what is commercially available, and special flavoring to make dosing easier. • Compounding can also be used to change the dosage form of a medication. For example, if someone has difficulty swallowing tablets, the medication can be made into a liquid for easier administration. • Compounding can help patients receive medication that is on shortage or has been discontinued, and allows for preparation of a product not available, such as custom dosed hormone replacement therapy. How does compounding differ from drug manufacturing? • Compounded preparations achieve a prescriber’s exact instructions of a medication for a specific patient. It is completed or overseen by a pharmacist who has been licensed by the state board of pharmacy. The product is not for resale by patients or prescribers. • Drug manufacturing is composed of large scale production of a drug product, not for a specific patient. What pharmacies can compound? • All pharmacies, by law, can do certain types of compounds. Common preparations include ointments, creams, liquids, or capsules. Some pharmacies also have the proper requirements for making sterile injectable forms of compounds. Licensed pharmacists are trained to perform basic compounding, and all pharmacies have compounding tools (a mortar and pestle for grinding materials, graduated cylinders for measuring liquids, scales for weighing solids, spatulas for mixing materials, and pill tiles to work on.) With the tools and knowledge, most pharmacists prepare compounded products when requested by prescribers. • A small percentage of community based pharmacies specialize in compounding services, which means the pharmacists spend most or all of their time compounding special preparations for patients. Some pharmacies, with a primary focus on compounding, may provide a larger variety of compounds, as they are usually more equip to do so. What is the difference between sterile and non-sterile compounding? • Sterile compounds are prepared to be injected into blood or body tissue. Sterility means no living organisms are present on the prepared product, or anything that touches the product. This needs to be maintained to prevent introducing harmful organisms into the body, such as germs and bacteria which can make a 40

patient sick. Compounding of sterile products is also completed in sterile environments, with use of special hoods and clothing. Sterile preparations include IV bags, commonly used in hospitals. • Non-sterile compounding refers to preparations being used in areas of the body where absolute sterility is not necessary. Nonsterile preparations include items such as creams, ointments, capsules, or liquids. • The United States Pharmacopeia (USP) has two guidelines, UPS 797 and USP 795, which are standard to be followed for preparation of sterile and non-sterile compounds, respectively. Who regulates compounding pharmacies? • Both federal and state authorities can regulate compounding pharmacies, but each state board of pharmacy regulates the practice of compounding. The FDA is the regulating authority over the drugs that are used to make compounds, also referred to as the active ingredients. If controlled substances are used in the preparation of compounds, the Drug Enforcement Agency (DEA) oversees. In addition to the governmental regulatory agencies, an organization known as the USP Convention specifies standards for preparing compounds. Compliance with USP is considered a standard of practice in pharmacy. Do compounding pharmacists have special training? • Compounding is an important part of a career in pharmacy. All graduates are trained on how to properly prepare compounded medications, including IV bags, ointments, capsules, and liquids. In addition, some states, including NYS, require graduates be tested on compounding skills and knowledge prior to obtaining a license. • Pharmacists who specialize in compounding have more advanced training in the area. However, no state currently requires a particular type of training, and there is no nationally recognized specialty for pharmaceutical compounding. Will my prescription insurance cover compounded prescriptions? • All insurance plans are different. Depending on the benefits covered in your insurance plan, and the compound being prepared, it may or may not be covered. By contacting the insurance directly, you will be able to find out if a compound is covered. The pharmacy may be able to let you know if a compound will or will not be covered. Should compounded medications be handled differently from other prescription medications? • Compounded preparations have dates for which they should not be used beyond, and may have specific storage instructions, depending on the preparation being used. When obtaining a compounded preparation, always be sure to check with the pharmacy what the expiration date is, and if there are specific storage requirements. This information is also commonly

displayed on the item label.

Dear Fellow Readers, As I write this article, it is officially Spring! The sun is shining, there is a screaming blue sky, the sunrise bird chorus has begun and daylight savings has begun. April has always been a special month for me. It is filled with family birthdays (including mine), Easter, the Easter break, daffodils, forsythia, sometimes even lilacs & azalea. When I lived in Pennsylvania, outside Philadelphia, April was an explosion of green lawns, bright azaleas, tulips and the most magnificent dogwoods & weeping cherry trees. I am not going to attempt to predict what our April weather will be like, but, I can assure you that it will be perfect for reading! Imagine this, taking a stroll after dinner, just before sunset appreciating the sky streaked in purple, pink and orange. Coming home, putting on soft music and settling in with a good book! Or…it rains every day and you curl up with a hot cup of tea and a great book. In my case, I will have a wonderful little long-haired dachshund either on my lap or snuggled next to me while I read a few chapters of my latest book. Currently I am reading “Black House” by Peter Straub & Stephen King in the bookshop and “Gone Girl” by Gillian Flynn at home.
