DEAR DR. ROACH: My husband is tired all the time. He functions, but frequently has to sit to recoup his energy, even after a shower. He is diabetic and has been for over 10 years. He does not take insulin and manages to keep his A1C within 7.1 to 6.9. He weighs 207 pounds, and he’s 6 feet, 1 inch tall and 75 years old. His bloodwork always comes back normal, and a nuclear stress test came back negative. We have discussed this chronic fatigue with our family doctor. His only explanation is possibly low testosterone (the low end of normal). We know that some if not all of his medications list fatigue as a side effect, but we cannot persuade our doctor to do further tests. My husband does have an appointment with a cardiologist in May for a consultation and any suggestions, and also a checkup in six months with our family doctor. My concern is his medications and the fact that they are generic. I have read many stories about generic drugs and problems with their effectiveness. -- J.C. ANSWER: Fatigue, which is the sensation of feeling tired, is a nonspecific symptom and a complaint that’s commonly heard by general doctors. It can be related to many diseases, as disparate as multiple sclerosis to heart failure, anemia to depression, and chronic infection to sleep disturbances. However, your husband notes a severe intolerance to exertion, which makes me suspect systemic exertion intolerance disorder, also called myalgic encephalomyelitis or chronic fatigue syndrome. Poor sleep, difficulty thinking or concentrating, and having worse symptoms with standing also go along with this diagnosis. There is yet no lab test for confirmation. The diagnosis is made after considering other causes, and testing for those that make sense after a careful history and exam. Diabetes, when not well controlled, may lead to chronic fatigue. I’m not so concerned with generic medications as I am about too many or the wrong ones, so a careful review of medications is appropriate. I have previously recommended a graded exercise program for this condition; however, the study that led to this recommendation had several significant issues; many people will actually have worsening of their post-exercise fatigue, so exercise must be used very cautiously and on an individualized basis. Cognitive behavioral therapy has had some benefit, but we desperately need better treatments.

Manufacturers and store owners are saying there will be shortages. There are labor problems, problems with container ships that can’t be offloaded in ports, trucks that can’t deliver the goods, material supply-chain problems ... you get the picture. Here are two suggestions to keep gift-buying simpler this year: toys and books. TOYS -- If you have small children to buy for, shop now. Manufacturers of many of the most popular toys are saying that they’ve seen shortages of required materials on a global scale. Try local toy stores for items they have in stock now. BOOKS -- Call your library and ask for age-appropriate suggestions for young children. Then, list in hand, call your local bookstore. Ask if they have in stock what you need. If you can actually go to the store and pick up the books curbside, you’re ahead of the game. Otherwise try ordering online, but look for things already in stock. For teens and adults, consider buying them a Kindle, an e-book reader available on Amazon. (I’ve had three different versions of Kindle since 2016 and truly appreciate the way I can order a book and have it appear on my e-reader within minutes.) A new Kindle, a $15 case and a $50 gift certificate for e-books would be welcome gifts for readers. Check the delivery dates of the various Kindle colors. Some are available immediately; others aren’t. When it comes to online shopping, experts advise ordering items that are as close to home as possible, not across oceans. Beware of local shipping delays as well. Gone are the days of always receiving boxes in just a few days. If you order something online, track it, and follow up with a phone call if necessary. When asked if he could drive me to a certain destination, he said he was sorry, and that it was beyond where they were allowed to go with their company van. Kindness matters! I have worked about 27 years in this town where my husband grew up. How happy we all were when my daughter, her husband and their 6-year-old daughter bought a home here! My granddaughter was homeschooled last year and hasn’t played with other kids in a long time. She is very bright, happy, friendly, energetic, kind, and eager without a mean bone in her body. This last weekend, I took her to the park. A little girl and her brother came to play. My granddaughter asked to play with them. They ran away, ignoring her each time she followed. Thinking they did not like her, she cried, heartbroken. So, we went to a neighboring playground, but there, another little girl and her brother kept running away. My granddaughter said, “Little girl, little girl…can I talk to you please? I haven’t had any little kids to play with in a long, long time and I want to know if……” The little girl ran away. “Little girl, can I please talk to you?” No response. I hugged my granddaughter as she started crying, and that little girl laughed at us. The welcome my granddaughter got here makes me cry. She starts school for the first time next week and is so excited to “meet some friends.” I pray she gets a classroom with warm, receptive, friendly children who will befriend this wonderful little girl with a big heart.
Social Security Cap Each week, I see comments regarding Social Security. How many of you realize there is a wage-base cap? According to IRS Topic 751 for earnings in 2021, this base is $142,800. Which means that any earnings over this amount are not subject to Social Security withholdings. If your neighbor earns $250,000, only the first $142,800 is subject to Social Security withholdings. That also means that Jeff Bezos pays on only the first $142,800. Also, Donald Trump, Bill Gates, Lady Gaga, George Clooney, Mark Zuckerberg, and the list goes on, only pay on the first $142,800 of income. The powers that be making the decision on the wage-base cap, I am sure, make considerably more than this cap. I’m certain there is nothing we little people can do to change this unfair practice, but it seems a significant raise in this cap could go a long way to making Social Security self-sustaining for years and years into the future. Hero or villain? It is amazing to watch the people who, just a year ago, praised healthcare workers as heroes be the same who are now condemning them as villains for not wanting the COVID vaccine. There were so many signs that praised them. Now, they are the enemy?

Ne Wildlif Reha Launche i Hemlock BY AMANDA DUDEK
M
any people have a soft spot for animals. However, it takes a uniquely compassionate person to go that extra mile to not only rescue animals, but also to rehabilitate them, as well. That is exactly what Rebecca Zielinski and her friend Ashley Mcneill did when they launched ABC Rehab, a licensed wildlife rehabilitation facility in Hemlock. Rebecca Zielinski has spent most of her life helping animals, inspired by her mother who volunteered at a cat rescue. “I have loved animals ever since I was 4-years-old,” said Zielinski. “As a toddler, I was already saving little critters!” A chipmunk enjoys a tasty snack As an adult, Zielinski opened a cat at ABC Rehab! rescue of her own, but she wanted to go further. “People would reach out to me about injured wildlife, and I didn’t know what to do to help them,” she explained. “I wanted to expand on what we were doing to rescue wildlife, as well.” At first, Zielinski didn’t know where to start, but conversations with wildlife rehabber friends soon pointed her in the right direction: a DEC exam. Prospective wildlife rehabilitators must register for this exam on the DEC website. As one would imagine, there is a lot to learn, but Zielinski had an edge. “Luckily, I already knew a lot of the basics that many other new rehabbers have to learn about things like supportive care, feeding, and wound care from rescuing cats and kittens,” she noted. “However, I did have to learn a lot of species-specific information regarding things like feeding and formula blends.” Passing the exam is only the beginning. “If you pass, the DEC sends you a link for your application which you send to Albany for special licensing,” explained Zielinski. “Then, you set up an interview with a DEC officer.” A DEC officer interviews each prospect and examines their rehabilitation facilities. “There are requirements your facility must meet,” Zielinski said. “It cannot be near domesticated animals. It must be in a separate, safe area with sufficient space. It must maintain a constant temperature and have proper cages. It also must have proper lighting with light and dark cycles to mimic the outdoors because we release rehabilitated wildlife back into the wild whenever possible.” In the spring of this year, ABC Rehab opened its doors for the first time. “Early spring is when the babies start coming,” Zielinski said. “Baby squirrels fall out of trees. Baby rabbits run from lawnmowers and animal attacks. When the weather warms up, baby birds fall out of nests. We have helped opossums, squirrels, nonmigratory birds, chipmunks, bunnies, and more.”

While Zelinksi had always received cries for help, their volume definitely increased. “I used to get a tag [on social media] here and there; now I get tags every day,” she said. “People continue to reach out to us, and the DEC and area veterinarians have the numbers of licensed rehabbers, so they call to transfer animals for care, as well.” To Ashley Mcneill, the need speaks volumes. “At first, I got into wildlife rehabilitation by association with Becky,” she recalled. “Then, I realized there just aren’t enough people who are doing this. We have really needed as much help as we could get. There are so many animals in need...we could not say no to them.” Saying yes comes with a cost. “A lot of our funds come out of our own pockets,” Zielinski stated matter-of-factly. “We are not paid nor supported by grants or government funding, so we rely on the community for help. Being licensed, we can ask for and accept donations, and when we post, people have been very generous.” Rehabilitation can become costly, especially with “late babies,” which reach maturity too close to winter and must be cared for until the warmer part of spring before being released. “We currently have about eight red squirrel babies here to stay with us this winter,” she said. “They are in little tree huts which mimic where they would be. We are careful not to handle them so that they do not get used to people.” Their efforts have not gone unappreciated. “A lot of people were excited when we opened,” said Zielinksi. “We received a lot of positive response to our posts, and those who find these injured animals are so appreciative of what we do. It is very rewarding.” In the future, Zielinski and Mcneill hope to expand ABC Rehab to include a larger outdoor facility which would allow them to also rehabilitate larger animals like foxes. Meanwhile, they encourage those who would like to help to donate to a wildlife rehabilitator near them. Wildlife rehabilitators are listed by the DEC at www.dec. ny.gov/animals/83977. “Supplies as simple as blankets, towels, and produce are very helpful,” Zielinksi encouraged. “Every animal deserves a chance, even if it is just a little mouse.”

Forward Thinking Helps Pets in Natural Disasters DEAR PAW’S CORNER: Why do people leave their pets behind when a hurricane or forest fire approaches? And what happens to the pets that survive a disaster? -- Kerry in Cincinnati DEAR KERRY: Pet owners don’t intend to abandon pets to approaching danger, but it happens all too frequently: Pets get left behind when a disaster strikes. Sometimes an owner has to leave the pet behind -- rescue services prioritize human life over pets when a situation is dire. Sometimes, in the panic and confusion of gathering up things to evacuate, a pet will dart out the door. And sometimes, pets get abandoned because evacuation centers won’t allow pets inside. After Hurricane Katrina in 2005, when more than 100,000 pets were left to fend for themselves during and after the storm (www. louisianaspca.org/about-us/hurricane-katrina/animal-rescue-facts/), pet shelters radically changed the way they manage intake and care of animals. They became more proactive. They built bigger, stronger networks with other shelters locally and nationwide, and coordinated their efforts with emergency responders. Pet owners can play a direct role in helping pet rescue efforts by being proactive well before a disaster strikes. Microchip your pets and keep contact info up to date. Keep up with your pet’s vaccinations and health checks. Most importantly, have your own evacuation plan that includes a “go” bag for you and for your pet. A pet’s “go” bag should include an extra leash and collar, a copy of their medical and registration records, pet food, waste pickup bags or pee pads, and extra medication if they take any. I have learned it is best to ask for more information before I give a complete answer. “Did you receive this hydrangea as a gift for Easter or Mother’s Day? Did it have pretty plastic or foil paper around the pot?” If so, the plant may be a florist hydrangea, and it is simply not hardy enough for our area. Mother Nature freezes the flower buds each year destroying the future flower.
Usually, these plants are big-leaf hydrangea (Hydrangea macrophylla). Most varieties bloom on old wood, i.e., wood that grew and produced flower buds during the previous year. Therefore, early, or late freezes or bitter winters may damage the flower buds. The other reason is pruning at the wrong time. If you cut back a hydrangea between the fall and early spring, you have trimmed off the future flower because it blooms on old wood. For further information, go to http://ccelivingstoncounty.org/

A 2017 study published in the journal Cancer Epidemiology, Biomarkers and Prevention found that about one-third of women diagnosed with metastatic breast cancer in the United States live at least five years after diagnosis. Also known as stage IV breast cancer, metastatic breast cancer is the most advanced stage of the disease. Metastatic breast cancer refers to breast cancer that has spread beyond the breast and nearby lymph nodes to other parts of the body. Patients diagnosed with breast cancer also should know that improved treatments may further their chances of surviving a diagnosis, even a diagnosis of metastatic breast cancer. In fact, the American Cancer Society notes that survival rates are based on women who were diagnosed and treated at least five years earlier. In the time since those survival rates were documented, treatments could have advanced even further, potentially improving the five-year survival rates for metastatic breast cancer.
