Cause of Chronic Cough Has Doctors Stumped

DEAR DR. ROACH: I picked up a cough while traveling in Europe more than two years ago. No other symptoms have ever presented. Since that time, I have received every test imaginable, including spirometry, multiple breathing tests, CT scans, X-rays and most recently a bronchoscopy. Nothing was discovered through any of these tests. Other than the cough, I am very healthy. The cough is driving me crazy, however. I have been prescribed every type of medication, and nothing helps. My doctors are at a standstill and have nothing further to offer. I don’t know where to go from here, but I know I can’t live my life with this cough. I avoid a lot of personal interaction because it is so disruptive to constantly be coughing. I would greatly appreciate your help with this, and any advice regarding what steps need to be taken next. -- P.M. ANSWER: Chronic cough (lasting more than several months) is most frequently a result of acid reflux, asthma or postnasal drip, although there are many more causes, including infection, which would be a concern given that you have a clear idea of when it started. Despite an extensive evaluation for cough, it is not that uncommon that people are left with no explanation but with a persistent cough. There are other, less-common treatments at this point, some of which your doctors might not have tried. The first is the medication gabapentin (Neurontin). This is used for a condition called laryngeal neuropathy, and I have had a few spectacular cases where the cough disappears entirely. People have written saying that their doctor prescribed the medication for them after reading it in the column and they also had relief, even after decades of persistent coughing, so I think this condition may be more common than I have ever thought. A second has been a cough suppressant. I am very cautious about using opiate drugs, and it is certainly not appropriate for some people, but cautious use of tramadol in selected patients has been successful in reducing, but not eliminating, cough. Just yesterday in the clinic, one of the residents I supervise had a patient with chronic cough who had dramatic improvement through the use of speech therapy. Looking it up today, I find trials showing significant improvement, if modest, with this approach, as well as several ongoing trials. Given the potential side effects of gabapentin and tramadol, I think I will be making more referrals to my speech and language pathology colleagues, and suggest you speak with your doctors about some of these options. *** Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. SENIOR NEWS By Matilda Charles What Lured Us Back to the Senior Center? The too-young staffers at the senior center finally figured out that we’re not coming back until COVID is gone. And it’s not gone. In some areas it’s easing up a bit, but here it’s still going strong. We’re not willing to risk contagion by dashing in the door of the senior center and pretending things are back to normal. At least once a week the center sends out emails and gets few replies in response. Come to the Ladies Lunch served indoors? No thanks. Experiment with still life photography in the meeting room, with vases, statuettes and fruit provided? No. We held out, refusing as a group to be party to any potential COVID-spreading indoor activity. Finally, the staff got it. They went with videos. The art classes will be online. The book club will have an online discussion, with a link to how to log in to the meeting. And the exercise classes will be posted online so everyone can stretch and work out at home. But it was the offer of free haircuts that broke our resolve. Some of us haven’t been in a salon for two years. The rules were specific. Stay in our car until we were handed an at-home COVID test, which we would take in the car. If it proved to be negative, we could proceed to the next step (one person at a time), which was to sit on a chair in the hall inside the right-side door of the center and wait for our turn. When the person ahead of us was finished (down the hall in the kitchen), she would be escorted out the left-side door to the parking lot. It was glorious. We each came out of the left-side door with a spring in our step, freshly relieved of two years of unruly hair. Staff should have thought of this before.

Are Irresponsible Owners the Bane of Dog Parks? DEAR PAW’S CORNER: When I first adopted my Collie mix “Sara,” I went to the dog park near my apartment every morning. I thought it would be a great place for a year-old puppy to socialize with other dogs and humans. A year later, I no longer take her to the dog park if there are any other dogs there -- which means taking pre-dawn walks or bypassing the park entirely. I quickly learned that while many dog owners are responsible people who train and socialize their dogs, others don’t train them at all, or train them inadequately. On Sara’s first day, a huge dog ran up, snarling and barking at both of us. The dog’s owner just sat on a bench, waving and yelling, “He’s friendly,” while Sara cowered behind my legs. A few days later, that same dog got in a vicious fight with two other dogs. Because we continued to have negative experiences with other dogs (and their owners), making it impossible to train and socialize Sara, I’ve stopped going there. I don’t think dog parks are a great idea anymore, even though I live in an urban area and green spaces are important for dogs. What’s your take? -- Hannah J. in Boston

t’s that time again! Mother’s Day is the perfect opportunity to make the mothers in your life feel special, loved, and respected—especially our own mothers who loved us first and continue to love us with unwavering faith. Mother’s Day D is a big event in the United States, but did you know many countries around the world also set aside a special time to celebrate mothers? We gathered fun and unique Mother’s Day traditions from nine countries around the globe, and we hope you find something new to add to your Mother’s Day celebrations!

France

Mother’s Day, “Fête des Mères,” occurs on the last Sunday of May or the first Sunday of June. Fête des Mères originated during the Napoleonic era during which families with a large number of children were presented with medals as part of an effort to promote large families and curb low birth rates. Some parts of France still honor this tradition and present moms with medals on Mother’s Day, known as Médaille de la Famille. It is customary to make a bouquet cake for mom, followed by a family dinner. Children typically give flowers, jewelry, perfumes, handmade gifts, handwritten poems, and chocolates as presents.

Germany

Mother’s Day, celebrated on the second Sunday of May, is known as Muttertag and was first celebrated in 1922. Here, celebrating mothers extends further than the women of the house; it pays tribute to the mothers of the universe. Carried out with a high degree of pomp and circumstance, this day sees children give their moms gifts and cards, red carnations celebrating mothers who are alive, and white carnations honoring mothers who have passed away. Surprises are common, and travel tickets are a traditional present. The family also makes a homemade dinner to give mom a break.

Ethiopia

Ethiopia holds its celebration of mothers in the early fall and hosts festivals that honor its religion. Ethiopia holds a festival called “Antrosht” which is dedicated only to moms. Families come together for large meals and celebrations. Daughters bring vegetables and cheese while sons bring meat, prepare meat hash, and stage performances featuring their family heroes.

India

Like Ethiopia, India also holds its celebration of mothers in the early fall and hosts festivals that honor its religion. In India, this celebration lasts for 10 days. Hindus honor Durga, the goddess of mothers, at the Durga Puja festival. This religious ceremony hosts family reunions centered around the traditional story of how Durga returns to her parents to show her own children off to them. The festivities continue with making food, giving gifts and decorating homes.

Italy

In Italy, mothers are honored on “La Festa della Mamma,” observed the second Sunday of May. The mother is regarded as the most important person in the family, and children must call if they cannot come home that day. While she is honored year-round, on this day, she is pampered from dawn to dusk. Small presents, roses, and handwritten poetry are popular gifts, while cards are not commonly given. Lunch is eaten out to give moms a break, and

at the end of the day, mothers receive a heart-shaped cake made for them by their children.

Japan

Japan’s customs for Mother’s Day are entirely different. Here, the holiday was revived after World War II and was used to console mothers who had lost children during the war. Today, Mother’s Day is observed on the second Sunday of May with red or pink carnations as well as help with chores and cooking. Japanese families make traditional Tamagoyaki or Chawanmushi dishes, while children make handmade gifts or portraits for their mothers.

Mexico

In Mexico, Mother’s Day—“La Dia de la Madre”—is celebrated on May 10th. Each year, women are celebrated with mariachi serenades, special mass services, and children’s performances. They are also treated out to lunches and dinners. No matter what day May 10th falls on, Mexican families always make time to celebrate their mothers.

Peru

One of Peru’s most important events, Mother’s Day, is celebrated for an entire week with much anticipation leading up to the second Sunday of May. This celebration honors not only mothers, but all the women in the home. Families organize lunches, dinners, parties and trips throughout the week, and children prepare events and festivities. Mothers are given the opportunity to visit exhibits, museums, and festivals for free. They are also read poetry and stories, given gifts, flowers, and treats, served dinner, and entertained by their children. Peruvian families also visit cemeteries to honor their dearly departed mothers and grandmothers, bringing flowers, balloons, and other salutations for the graves. No mother is forgotten in this showing of appreciation for a mother’s love and sacrifice.

United Kingdom

Recycling It's pretty sad when you call [local town] Code Enforcement, [local town] Hall, and [others] to find a solution to businesses not recycling, and you pretty much get an answer of "Oh well." No one seems to know or care. So as all these cans, glass, and junk get thrown onto the ground, remember, you have children and grandchildren who have to live on this dirty earth. Why can't people just do the right thing?

Enough already! Am I the only one who’s sick of businesses, companies, services, and retailers overusing the excuse of COVID? Most of these are still trying to increase profits even though supply, laborers, and inflation are affecting all of us. Some still allow personnel to work from home which leaves fewer people to actually provide services to customers. Eliminate the government “freebies” to force people to actually work for their money. Stop giving free money to renters and get them back to work. Quit stealing from the taxpayers and get us back to where we were. The COVID excuse being made is just that: an excuse! Not a Drag Strip! Moving to this beautiful town over 10 years ago was the best decision my family could have made. It’s such a beautiful town with friendly neighbors. However, we couldn’t be more upset with the way most of the younger generations use [local street] in [local town] as a “drag strip." Living on this street is absolutely beautiful, and the 30 miles a hour limit should be enforced. There are many that hit the stop sign on center and then use the straight away as if it’s 390 in a 65mph zone. It’s 30mph or slower. Please slow down. There are autistic children that live in this area as well as family pets. Let’s be safe and slow down. Tomorrows are never guaranteed. We would hate to see something happen to children, walkers, runners, family pets, etc. Slow it down, please!
