Amazing Staff at Batavia’s Perry Pet I am overdue in thanking the amazing staff at Perry Pet in Batavia. We went through an incredibly difficult time this past April when we had to say goodbye to our sweet girl. Our dog was almost 17 years old and a huge part of our family. The staff at Perry Pet were kind, helpful, considerate, and so supportive during this time. Because of their kindness, we were able to have our whole family together one last time. I can’t tell you how much this meant to us. We cannot thank you enough for helping us through one of the most difficult times our family has gone through and for treating our girl with grace and dignity. The beautiful card and the touching memento that were sent to us afterwards were such an amazing surprise. Now we have her paw prints framed and a touching reminder of our sweet girl. Thank you from the bottom of our hearts.

Mind your own business To the person who put a face mask on, entered the fitness room, and tried to belittle me and make fun of the fact that I wear a mask.(You never wear a mask.) I am vaccinated, but I still wear one to protect myself and others, unike you. Then, you ask me what I’m doing on the treadmill and tell me that I should be outside walking. You then replied that you walk 4 miles a day outside. So what? Who are you to judge me? Who are you to know why I have to use a treadmill? I guess you don’t know I have titanium in three different parts of my body (rods, plates and cadaver bone). I’ve had two surgeries on my spine and a botched foot surgery. Stay in your lane, buddy, and mind your own business! A sad week for our country! After 20 years our time in Afghanistan has ended. After great cost in terms of human life and money spent the people that supported us there have been left in many cases high and dry. It might have always ended like this but it’s still a shame that things came to a close the way they did. Nonetheless thanks to all the vets that served there overseas. To those who are unvaccinated. In case you haven’t heard, hospitals are overrun with Covid patients, children are struggling to breathe (they don’t have the choice to get vaccinated) and thousands of people are dying. All because you “know your rights” and want the government to butt out. How many of you refused your stimulus check? Your unemployment? Your Social Security? It’s time to start thinking about someone other than yourself. It’s not too late to do the responsible thing and get vaccinated.

DEAR PAW’S CORNER: Our dog “Boadicea” is a sweetheart, but when she wants a treat she turns into a little monster. She drools on my arm, chews on my roommate’s shoe (with his foot in it), barks loudly, yips ... whatever it takes. We eventually break down and give her a treat. This happens several times a day. How can we stop this behavior? -- Darren in Manchester, New Hampshire DEAR DARREN: There are two steps I’d recommend to curb this behavior, because you know the downside of Boadicea’s begging: spiraling behavioral problems, future digestive issues and potential obesity. First, reinforce her basic obedience training. Work with her at least once a day on the “sit,” “stay,” “lie down,” “leave it” and “come here” commands. I recommend not working with her during the time that you’re normally relaxing in the living room with your roommate, because you want to try to reinforce that certain places and times are for training, play or treats. Obedience training alone won’t resolve the problem, though, because she’s developed a habit of begging whenever you’re trying to relax or socialize. So, you’ll need to employ the “ignore and divert” strategy as well. Ignore her attention-seeking behavior for a specific amount of time ... say, five minutes. Next, command her to either sit or lie down, and give her a chew toy. When she inevitably gets up to beg again, repeat the process. Don’t give her a treat at all. This will take time, but eventually she will get it. Many dog owners add a structured treat time to the day. For example, after your dinner and dishes are done, she can have her treat. We’ve Gotten Careless If you read the news, you know there are serious questions about how well the vaccines we’re been taking actually protect against COVID. Especially worrisome is the protection they offer against the Delta variant that’s sweeping the world. Those in authority seem to be flipping a coin each day, depending on who you listen to. Some days we can throw caution to the wind and go about our business, and other days we need to continue to beware. There’s talk of going back to the indoor mask mandates, and that’s likely one step away from again limiting the number of people allowed in stores at one time, or so I imagine. If those things happen, it won’t be like it was in March 2020. Back then we were all scared to death. We’d spread out in the grocery aisles, staying far away from other people. We wore that mask at all times when out and about, and we respected all those 6-foot markers on the floor. We certainly didn’t sit down at a restaurant table for four and take off our masks, or head to the gym. Now, having lived through that whole horrible year, people are tired of it all, and we’re eager for a normal life. We think that because so many people have had the vaccination, that surely we’ll be safe. I fear that this time, however, if the mask mandates come again, it will be even more dangerous than it was last year, because we’ve gotten too casual, careless and passive about our safety. My short to-do list involves buying new masks and more hand sanitizer, renewing my grocery curbside pickup account ... and saying no to an invitation to join a dance group. How about you? Non-Stop Acid Reflux? Look at Surgical Options ---
DEAR DR. ROACH: I am a 78-year-old female who has suffered from acid reflux for many years and has been taking Prilosec (omeprazole) for at least 15 years. I have heard that it is not meant for longtime use. If I don’t take it, I have such heartburn I can’t sleep and am in misery all day and night. I recently attended a seminar on the new LINX surgery and hope to have it done. Do you have any information on the success of or problems after this surgery? -- J.B. ANSWER: The LINX device is a magnetic ring, placed by laparoscopic surgery, that helps increase the strength of the lower esophageal sphincter, the valvelike muscular structure at the junction of the esophagus and the stomach. This reduces reflux (backward movement) of stomach acid up into the esophagus. The procedure was very effective at reducing both heartburn symptoms (89% before procedure to 12% after) and the need for daily medicines like omeprazole (100% before to 15% after). The device has not been in use for a very long time, so long-term safety data is unavailable. Some people had to have the devices removed because they eroded into the esophagus, but only 0.3% of devices were removed at four years. There are other types of surgeries used to treat reflux, most of which are effective at reducing both symptoms and the need for medication. There are now options for treatment being done endoscopically (with an instrument placed through the mouth), without the need even for laparoscopic surgery (with the instrument being placed by small incisions through the skin). My personal practice is to discuss medication treatments as well as surgical treatments for people with moderate to severe acid reflux. Surgery in younger people may have more benefit by preventing a lifetime of daily medication. People who do not get relief with medication are those who most clearly benefit from these kinds of procedures.
Many people find it impossible to think about summer without conjuring visions of spending endless hours outdoors from morning until evening, whether beachside, on the open water or even floating in a backyard pool. Although a certain measure of sun exposure is required for some natural functions of the body, it’s well documented that too much time in the sun can be hazardous to one’s health. That’s why summer frolickers need to exercise considerable caution each time they step outside. Taking sunburn for granted can be a big mistake. Many people wouldn’t risk burns from a hot stove or open fire, but they won’t think twice about being unprotected under the very hot rays of the sun. The Centers for Disease Control and Prevention says more than one-third of adults and nearly 70 percent of children admit to suffering from sunburn within the past year. Depending on the intensity of the sun and the amount of time spent outside, sunburn can be a first- or second-degree burn. In first-degree burns, damage affects the topmost layer of skin. However, sunburn can even affect deeper layers and cause blistering in addition to redness and pain. Sunburn also can cause some irreparable damage that goes unseen. According to WebMD, ultraviolet light from the sun can alter DNA, prematurely aging skin or even contributing to skin cancers. It can take years before symptoms become noticeable. Therefore, it is best for people of all ages to exercise caution when spending time in the sun. Sunburn is one of the most easily prevented summertime ailments. It’s also important to note that sunburns are not just limited to the hot weather or when it is sunny outside. Ultraviolet damage can occur at any time of the year, and also from artificial UV sources, such as tanning beds. Preventing sunburn is simple.

Mary Cariola Students Participate in 4-H Public Presentations BY SUSAN COYLE
4
-H Public Presentations are held each year to encourage youth to develop their communication skills. The program supports youth to create a presentation and develop the skills needed to deliver it to an audience. With public speaking known as one of the biggest fears that people have, it is easy to appreciate that this is a challenging and important project for youth to complete. For a group of youth at Mary Cariola Center, this challenge is magnified as the students are non-verbal and non-ambulatory. However, these youth, and their supporters, meet the challenge head on and succeed! Sean, Maddie, DeAshaney, and Shamell are hard workers and brave to try out and improve an important skill! A cornerstone of 4-H is to create experiences that all youth can participate in. According to author, Liz Fosslien, “Diversity is having a seat at the table. Inclusion is having a voice. Belonging is having that voice be heard.” Tricia Coleman, Speech Language Pathologist, embraced that quote and set out to make public presentations possible for these students. Access to technology, on-going educational and therapeutic support, motivation and dedication made 4-H Public Presentations a reality for the 4-H Busy Bees. The evolution and adaptation of technology has made communication increasingly more accessible for students with disabilities. All four youth utilize a speech generating device, which involves dynamic technological equipment programmed with communication software to allow access to voice output communication. Each system is accessed differently and all programs are customized to meet the needs of each individual. Moving from the technological aspects of Public Presentations, the youth explored Dream Boards for 2021. The youth organized their thoughts and answered questions on their dream boards that included “What do I want to learn more about?”, “What do I like to do with my friends and family?”, “What do I want to accomplish this year?”, “What will keep my mind and body healthy?”, and “Where do I want to go?”. Based on the ideas that were gathered, each student created a visual dream board and presented them to both a live as well as a virtual audience for the annual 4-H Public Presentations event. Two of the youth shared in their planning that they dreamed to be in a fashion show and a live concert. A week before the presentation, music and speech therapists collaborated to make those dreams come true. Music Therapist, Jason Willey, was at the ready to support the musical part of the youths’ dreams. As the students collaborated to plan their event, entitled “Melodies and Accessories” they used their speech generating devices to select music that would be performed and how each song would be presented. They also co-wrote and recorded an original song to premiere at the event. After they wrote the lyrics to this song, one of the students composed the primary melodic hook on an electric keyboard, and the other student added her own vocal stylings. The event featured modeling accessories and the debut of “Dream It-Be It,” the original song composition that

highlighted the fact that we can all achieve our dreams just as we are Joanne Fisher, Mary Cariola Center Communications Specialist, shares that every Cariola student has goals. The team of educators and clinicians work together to support students to achieve those goals. Using purposeful work, fun, and encouragement, the Mary Cariola Center celebrates all successes as the Center works to transform the lives of individuals with disabilities. Kathy Lee, classroom teacher and leader of the Busy Bees 4-H Club, shared how much it means to the youth and their families to be able to participate in the 4-H Public Presentations Program. The youth were thrilled with the certificates they earned and loved the 4-H goodie bags they received. Parents loved the bragging rights to share their childrens’ success in the public presentations and amazing fashion show. The Monroe County 4-H Program is offered through Cornell Cooperative Extension to the youth of Monroe County. 4-H is a worldwide youth development program open to all youth ages 5 to 19, who want to have fun, learn new skills, and explore the world. In return, youth who participate in 4-H find a supportive environment and opportunities for hands-on or “experiential” learning about things that interest them.
