Rare Disease Causes Hard-to-Diagnose Rash ---

DEAR DR. ROACH: My daughter was diagnosed with lymphomatoid papulosis. It’s not a bad case, but it’s enough to make me worry. I know it’s rare for this to turn into cancer, but there is always a chance. Doctors often misdiagnose it, but thankfully I have the best pathologist in the world. My daughter is being treated with UVB light, which worked in the past, but now is not helping. I don’t want to do methotrexate. I worry about every dot that pops up on her, and if it doesn’t go away, I think the worst. -- M.W.M. ANSWER: Lymphomatoid papulosis is a rare disease, most common in people in their 40s, but can happen in children and older adults as well. The cause is unknown, and although a virus is suspected, it hasn’t been found. It is not contagious. The rash of LyP is not specific and needs to be confirmed through biopsy. Spots are relatively large (but less than an inch), and usually red but with a white center that can turn black. The rash comes and goes. A skilled pathologist in combination with an experienced dermatologist is necessary to make the diagnosis. Adults are treated with methotrexate if the lesions are in a cosmetically important area, especially the face. UVA light is better studied than UVB light, especially in children, but an expert would be the most appropriate to recommend the best treatment. Your concern about cancer is understandable. Rates of associated cancer -- specifically, lymphoma, a blood and bone marrow cancer -- have been reported in 5% to 50% of people with LyP. Unfortunately, apart from people with a specific gene rearrangement, there’s no way yet known to predict who is likely to get lymphoma nor any way of preventing it. She will need to be vigilant. Fortunately, the lymphomas, if they do develop, are usually effectively treated. ***

DEAR DR. ROACH: I recently opened up some stored tuna that my son had saved for me. He loves to hunt his own food. When I ate it, I realized that when he had gone to Alaska, he had not only hunted tuna but also black bear. He had used the black bear grease to cure the tuna. I was mortified that he had used a protected animal to preserve his tuna. I reluctantly ate it anyway. Now, I am worried about my health. Will anything happen to me health-wise because I have eaten the grease from a black bear from Alaska? The bear was wild, so could it have had a disease, like rabies? I’m sure it has never been vaccinated. -- Anon. ANSWER: Black bears are hunted in many states, and although it is regulated (you need a license) in Alaska, they are not a protected species under the Endangered Species Act. Black bears are hunted for both sport and meat. The major health risk from consuming bear meat is trichinella, a muscle parasite. The meat must be heated to a high temperature to be sure of killing the parasite. Grease from the bear should not have trichinella risk. Another concern is rabies. Rabies is possible but rare in bears. I doubt it could be transmitted from grease, though, as rabies virus lives in nervous tissue, not fat.

Cancer and COVID-19

Keep Your Spirits Up Depending where we live, many of us are still in lockdown. Still, there are ways to keep our spirits up. Plants can make all the difference in how we feel in our environment. Check your local nursery's website for small plants you can either have delivered or pick up at the curb. Some of the easiest to grow are pothos, spider plant and philodendron. Beware, however, if you have pets. Be sure you can either hang up your plant or put it up on a bookcase where the pet can't reach it. Books we enjoyed a long time ago can take us back to a time and place that was calm and safe. Look on your own bookshelves or consider getting an e-reader if you don't want to take a chance going into your own library, assuming it's open. Baking can fill the house with lovely smells. Since finding flour is impossible in many parts of the country, look online for hundreds of recipes that don't require flour. No-flour banana bread, flourless chocolate cake ... you can even make oatmeal muffins. Just be certain you can acquire all the ingredients before you set your heart on making a recipe. Zoom is the online conference program that so many companies are using for business meetings. We can learn Zoom, too, and stay in touch with family and friends if we have a computer or device with a camera. Explore the basic free version of Zoom. You'll be limited to 40 minutes in group meetings, with no limit on one-toone meetings. Your first step will be to read online instructions and get comfortable with the steps to set up a connection. The COVID-19 outbreak of 2020 caught many people, particularly those in the western hemisphere, off guard. As the virus spread, the lives of hundreds of millions of people throughout North America and Europe changed dramatically and seemingly overnight, prompting many to wonder if they or their loved ones were going to get sick. No one was thought to be immune to COVID-19, though many people were unfortunately believed to be more vulnerable to the disease than others. According to the Centers for Disease Control and Prevention, older adults and people with serious chronic medical conditions are at the greatest risk of getting sick from COVID-19. The American Cancer Society notes that cancer patients, like groups with an elevated risk of getting sick from COVID-19, must do all they can to avoid being exposed to the virus. That’s because the immune systems of cancer patients, particularly those in active chemotherapy and bone marrow transplant patients, can be severely weakened. In fact, in March 2020 officials with the Fred Hutchinson Cancer Research Center warned that early data from China and Italy, two countries which experienced early and widespread outbreaks of the disease, indicated that cancer patients would be at increased risk of getting the virus. Are all cancer patients at risk? Dr. Steve Pergram, the medical director of infection prevention at Seattle Cancer Care Alliance, said that patients with blood malignancies are most at risk for COVID-19. Blood malignancies include non-Hodgkin lymphoma, chronic lymphocytic leukemia, acute myeloid leukemia, acute lymphoblastic leukemia, and multiple myeloma. While people with these forms of cancer may be most at risk of getting COVID-19, it’s important to note that all cancer patients, including those who recently concluded treatment, are vulnerable. Those who concluded treatment should know that the aftereffects of treatment can linger long after treatment programs have ended, and those aftereffects may continue to compromise patients’ immune systems. What can cancer patients do to protect themselves from COVID-19? Cancer patients should embrace the safety protocols recommended by the CDC, including social distancing measures and limiting direct contact with other people. Seniors in cancer treatment should take advantages of opportunities to limit their exposure to COVID-19, such as visiting grocery stores during seniors-only hours and opting to participate in digital religious services instead of in-person services. In addition, patients can work with their cancer care teams to devise ways to continue receiving treatment without elevating their risk of exposure to COVID-19.

The Partnership for Ontario County: Filling the Gaps By Amanda Dudek

S

ometimes the best treasures can be found in one’s own backyard. This is certainly the case when it comes to The Partnership for Ontario County, serving residents from centrally located Canandaigua. Since 1998, the Partnership has been an independent nonprofit, pursuing its mission to “create, support and administer alliances to cultivate positive social change” with a coalition of local nonprofit programs under its organization. “More than 20 years ago, a group of committed people realized there were gaps to be filled in Ontario communities,” explained Tracey Dello Stritto, Executive Director of the Partnership for Ontario County. “They created this umbrella agency to have these various programs while providing administrative infrastructure and support to them,” she continued. “Their vision was to create ‘an environment where all people are hopeful, engaged, and thriving.’” Currently, the Partnership is comprised of six active programs. Suicide Prevention Coalition: “We work together with Ontario County Mental Health,” shared Dello Stritto. “We are a group of concerned community members and agencies who really want to be a part of this conversation, and we focus on preventative measures.” The Suicide Prevention Coalition recently received a grant to support its preventative work in the community. Substance Abuse Prevention Coalition of Ontario County: This community coalition meets regularly to implement strategies, utilizing the strategic prevention framework developed by the Substance Abuse and Mental Health Services Administration (SAMHSA). In addition, this coalition provides community education, as the opioid crisis continues, and vaping has become a concern. “In collaboration with community partners, the coalition hosted community medication take-back days,” shared Dello Stritto. “Five years ago, we received a rather substantial grant, and we developed an initiative called, ‘Monitor, Secure, Dispose.’ Originally, we had three medication drop boxes in our county; now we have 29.” Ontario County Youth Court: Celebrating its 20th year, this volunteer alternative to traditional juvenile justice offers a diversion program for nonviolent, nonsexual young offenders. Young people appear before a court of their peers, all trained volunteers, to constructively take responsibility for their actions. “This program focuses on restorative justice,” stated Dello Stritto, noting that successful completion results in a sealed record which is destroyed when the individual turns eighteen. “This court is not here to establish guilt but to make things right.” Consequences might include online prevention courses, counseling, apology letters and community service. “This is a very valuable program where we see change and offer leadership opportunities for those who participate,” Dello Stritto said. “Even through COVID-19, youth court members have been willing to participate each Thursday night!” Nowhere to Go: This program works to monitor and mitigate homelessness in Ontario County. “Our volunteers conduct Point in Time counts twice a year, counting the homelessness and providing resources for those in need,” shared Dello Stitto. “We help those facing homelessness to connect with resources. If there are still gaps, we try to meet those needs.” EPIC Zone – A Youth Clubhouse: In the wake of COVID-19, this youth drop-in center transitioned to digital platforms. A crucial, positive support network, EPIC Zone has continued to support area youth throughout these challenging times. “All of the directors have been amazing,” Dello Stritto praised. “Within two weeks, these programs were up and running virtually, a pivot and shift driven by the knowledge that kids still need support.” EPIC Zone continued to provide many opportunities for youth to connect through ZOOM and other platforms. “We check in with kids, drop of goodie bags, offer homework support and more,” explained Dello Stritto. “We want to come alongside these kids and

make sure they do not feel alone. Still, we are all very excited to get back together, even socially distanced.” Community Support Center: This program is based on the Partnership’s Family Support Center program, a successful prevention and intervention counseling program housed within schools. Its approach of Solution Focused Brief Therapy was ultimately so successful in area schools that Wayne-Finger Lakes BOCES brought the program onboard as one of its Tracey Dello Stritto, Executive services. One of the program’s Director of The Partnership for therapists believed the model Ontario County would also be effective at the community level. “This therapist realized that community members consistently reported feeling at their best when they were moving more and had people around them,” shared Dello Stritto. “The Community Support Center started spontaneous walks, hikes and partnered with Canandaigua CrossFit to offer yoga and movement.” The Center currently provides a support group: Managing Anxiety, and it has brought back kayaking this summer. The Partnership has brought a new program under its umbrella this past year: Leadership Ontario. “We are excited to launch this program after COVID-19 delays,” stated Dello Stritto. “Leadership Ontario will facilitate the growth and development of Ontario County’s leadership, focusing on its government, health report card, history, tourism, agriculture and more!” “We are able to see how positive our programs are for the community,” Dello Stritto affirmed. “We engage youth, collect thousands of pounds of unwanted medication, and lower recidivism rates among youthful offenders. We encourage our community to check out our services and get involved!”

Heck, the American Revolution was one big protest against injustice! Of the few hundred people at a protest in [local town], everyone was respectful, peaceful, and wore masks. Fostering a Pet vs. Adopting DEAR PAW’S CORNER: What is the difference between pet adoption and fostering? -- Jess in Knoxville DEAR JESS: An adopted pet is going to his or her forever home, a successful story every animal shelter wants to see replay again and again. But many pets aren’t quite ready to be adopted. They may be too young. They may be recovering from abuse and injuries. They may need to be observed to figure out what home environment is best for them. Sometimes, a shelter runs out of space. And some rescue organizations don’t have shelter facilities at all. In all these circumstances, this is where foster families come into play. A foster is a temporary arrangement, either for a predetermined amount of time or until a pet is adopted. Rather than being stuck in a chilly kennel with little interaction, a foster pet stays in a warm, loving home -- an arrangement that vastly reduces their stress and helps them heal. Some pets are never ready for adoption and remain a foster for the rest of their life. Others are adopted by the foster family if approved by the rescue organization. Caring for a foster pet takes dedication, and it helps to have experience caring for pets. If you’re interested in being a foster parent to a pet, check with the local shelter or rescue group. They will have an application for you to fill out, and then will evaluate you and your living space to determine if you are a good fit for the foster program and which pets would do best in your care. Richard B. Hensler June 6, 2020 Canandaigua/Bloomfield- Richard B. Hensler, age 79, passed away on June 6, 2020. Richard was a wonderful and special father and grandfather. He is survived by his three children, Timothy (Penny), Carol (Michael) Dotterweich, and Christine; six grandchildren, Morgan and Courtney Lyon, Ryan Dotterweich, Noelle, Clarissa, and Owen Hensler. He is also survived by his sister, Kathleen (Max) Albertson and many nieces, nephews, and friends. Richard was born in Rochester to Bernard and Mary and predeceased by his brother, Gerald. He grew up in Irondequoit and attended St. Thomas the Apostle, Aquinas High School and Alfred State College. He worked at Onondaga Litho and Southco. Richard enjoyed walks along Canandaigua Lake, birdwatching, his daily visits to Wegmans and McDonald’s and playing the Lotto. He watched the Yankees and NASCAR. He loved car shows, music, sailing, and spending time at Marble Lake, but most of all spending time with his family. He had a gentle spirt and a big heart with a great sense of humor and enjoyed being around people. He taught us you may not be able to control every situation in life, but you can control your attitude and how you deal with it. Richard believed in always being humble and kind. Contributions can be made to Golisano Children’s Hospital and Foodlinkny.org in memory of Richard Hensler. Private burial at the convenience of family. Will lavender grow in and around small stones if it has some soil under it? Lavender is a sun worshiper, loves heat, and grows best in infertile, welldrained and slightly alkaline soil. Of course, lavender plants need soil, but will do just fine in an area where there are many small stones. Avoid heavy clay soil that does not drain well. Plant in the spring when danger of frost has passed, mixing some compost into the soil to get your plants off to a good start. Some lavender growers recommend putting a layer of crushed oyster shells or limestone

in the bottom of the planting hole to improve drainage and to increase the alkalinity of the soil. Also, some suggest mulching around lavender plants with pea sized gravel. The theme, good drainage is essential. Fertilize sparingly or you will get leaves at the expense of flowers. Lavender thrives in dry conditions so be careful not to overwater to prevent root rot. Also, dry heat helps to release lavender’s strong and wonderful fragrance and because of that aroma, deer generally stay away.

Garden stores generally stock two cultivars, ‘Hidcote’ with silvery foliage and deep purple flowers and ‘Munstead’ with green foliage and a violet or blue-purple flower. In the past few years, one called ‘SuperBlue’ has become more popular and is one that I find does very well here. It is a little shorter and heavier blooming cultivar than the two mentioned above.
