Can Nothing be Done for Severe Back Pain?

DEAR DR. ROACH: I have severe lower back pain and can’t stand up straight or walk very well. I saw a back specialist, and he took X-rays and reviewed an MRI. He informed me nothing could be done for me because I have diffuse idiopathic skeletal hyperostosis (DISH). I would like to know what this is and why nothing can be done for me. -- Anon. ANSWER: DISH is essentially a disease of bone spurs. The ligaments and their attachments to bone themselves become calcified and bonelike. The cause is unknown. Symptoms are commonly back pain (more often, middle or upper back, not the lower back pain you have), but pain also may occur in the neck. Problems with walking are not common, but can happen if a nerve is being compressed. Surgery is not a usual treatment for DISH. It is occasionally necessary if the bone spurs press on nerves or if the bone spurs are so large that they interfere with eating. That doesn’t mean nothing can be done for you. Physical therapy may be very beneficial; judicious pain medication and regular gentle exercise are the other effective treatments. If the difficulty standing straight and walking persist, you should get re-evaluated.

by the Food and Drug Administration, in people with interstitial cystitis, where it is directly instilled into the bladder. It is also used (off-label, meaning without an official FDA indication) as a recovery treatment when the tube delivering some types of chemotherapy comes out of the vein. DMSO may have a pain-relieving effect, both topically when applied, as well as in the brain, though the mechanism for this is unclear. DMSO is an antioxidant and opens pores in lipid membranes, but again it’s unclear how this relates to its possible effects on people’s symptoms. It is an interesting and difficult-to-study compound. It is exceedingly permeable: Within five minutes of being applied to skin, it is detectable in the blood. It has a distinct flavor and odor that can be noticed after application to the skin, which makes doing a controlled trial difficult; subjects in a study find it very easy to know whether or not they have been given DMSO. It’s estimated that people have spent billions of dollars on DMSO for other conditions, especially arthritis and inflammatory diseases. However, the evidence that it is effective is poor, largely because of the difficulty in doing good studies on this compound, but also because large-scale studies are very expensive and there is little enthusiasm on performing such studies on a cheap compound. The risks appear to be small. Apart from the taste and smell changes, there are rare reports of allergies, discomfort in the bladder after instillation there and possible damage to the eye lens (at least in animals). You didn’t tell me what condition you are considering DMSO for, but except in the case of interstitial cystitis (where there are other therapies more likely to be effective), given uncertain benefits, I don’t recommend DMSO despite the low risk of harm.

Pets viewed from above should display an hourglass figure that includes an indentation near the midsection. The absence of this “waist” may indicate a problem. When observing a pet from the side while he or she is standing, a slight upward slope of the stomach should be visible. A low-hanging abdomen means abdominal fat may be present.

Springwater Voters Beware The Row A candidates are not really for the benefit of our Town. Some of them were in office before and some of their results at that time are carried over to the present situations. If they were really interested in the Town they would have come around throughout the year, and not just election time. We seldom see them at any local organizations, or fundraising programs by the Church, Fire Company, Legion, or Historical Society. Row 1A says no one is watching the hen house. There are checks and balances - Board members, Lawyers and Auditors for that purpose. Our board members are involved with the Town’s operation and they are invested in the Town organizations. There are personalitites, and we all have one, some good and some bad. Irritations by some bring about bad results. Simple change just for change is not beneficial. Re-elect the present Town Officials, they are doing their jobs. The World Sauna Championships have taken place in Heinola, Finland, annually since 1999. The starting temperature is 110 F, and a liter of water is poured on the stove every 30 seconds. The winner is the last person who is able to leave the sauna without assistance. 1. Melt the butter in a large skillet over medium heat. Add the apples, turn the heat to high and sauté until they begin to brown on the edges, about 5 minutes. If some are getting too well done, remove them and place on a plate while the rest continue to cook, then return them to the skillet when all are done. 2. Reduce the heat to low and add the honey, wine, lemon zest, lemon juice, and 1/2 cup water. Cover and allow to cook until the apples are tender but still firm; you don’t want them to turn into applesauce. 3. Serve this in a bowl, tuck in a couple sprigs of fresh sage from your garden, and tell your guests all about sage honey.

Removing The Mystery From A Prostate Cancer Diagnosis (NAPS)—According to the American Cancer Society, about one in nine American men will be diagnosed with prostate cancer. More than 173,000 new diagnoses are expected in the U.S. this year, fortunately, this disease can be treated successfully, especially if caught early. What To Do If you’re 50 or older or have a family history of prostate cancer, you should speak to your doctor about whether screening for the disease is right for you. Understanding Prostate Cancer “In the early stages, the most common symptoms of prostate cancer are no symptoms at all,” says Deepak A. Kapoor, M.D., founder of the non-profit, Integrated Medical Foundation, and President of Advanced Urology Centers of New York. There are four stages of prostate cancer. Stage one is diagnosed very early and confined to the prostate. At this stage, the patient is unlikely to be experiencing any symptoms. He may not need treatment beyond regular follow-up blood tests, exams and possibly biopsies. The disease is very treatable. When diagnosed early, the five-year relative survival rate is almost 100 percent. Why Do Some Men Resist Routine Screenings? Many men are embarrassed by the digital rectal exam, concerned about treatment or fear they can’t afford it. Some Answers Testing: Since the 1980s, prostate cancer was being diagnosed with a simple blood test, the PSA. It checks the level of prostate-specific antigen in your blood. In the past, there was controversy about whether having a prostate cancer screening done was beneficial or if it produced more harmful effects due to complications from over-testing. PSA testing was the best thing available for a long time. New tools now exist that provide much more information, giving predictability about the aggressiveness of the cancer and data to help urologists safely manage their patients’ disease. With this information, urologists have higher numbers of appropriate candidates on active surveillance regimens. Using tests such as the Gleason grade score, overall patient health and risk factors—age, race, ethnicity, family history and exposure to Agent Orange—urologists can determine with confidence how aggressive the cancer is and which patients will do well on active surveillance. They will also know which therapy options will be the most optimal for the patient. Making it easier for doctors and their patients to do this testing is the full-range of diagnostic equipment and supplies available through the trusted advisors at Henry Schein Medical, a provider of medical and surgical supplies to health care professionals. Treatments: There are many ways to treat prostate cancer, including hormone therapy, surgery, chemotherapy, radiation and cryoablation. The newest innovation being successfully utilized is immunotherapy, which uses your own immune system to identify, target and destroy the cancer cells without harming the body’s own “good cells.” Your doctor can help you decide what’s best for you. It is important to remember that you have the most options available when prostate cancer is diagnosed early and is in the most treatable stage. It is important for men to be vigilant with their routine screenings. Paying: Many insurance policies will pay for diagnostic tests and in some places, such as New York State, there’s no co-pay or co-insurance cost sharing responsibility for diagnostic prostate cancer screenings (with such policies). This puts PSAs on the same level as mammograms, thanks to efforts of advocates and doctors such as those at Advanced Urology Centers of New York, one of the largest urology group practices in the country. Cohocton-Merrill, WI-Rita G. Oestreich, 83, formerly of Cohocton passed away Thursday, August 8, 2019 at Ascension Good Samaritan Health Center, Merrill, WI. Rita was born July 3, 1936 in Cohocton, the daughter of Loris and Laura (Brown) Graves. Rita was a secretary in both New York and Wisconsin and was employed with the Private Industry Council of Marathon County. At retirement, Rita was working for the Steuben County Sheriff ’s Department. Rita enjoyed knitting, gardening, cooking, baking and singing in the church choirs of Cohocton and Wisconsin. Rita was predeceased by her husband Edward Howard; her sister and brother-in-law Marjorie and Ralph Keifer; and her brother-in-law Stanley Newfang. Rita is survived by her three daughters, Lorie ( John) Warren of IL, Lisa Howard of Appleton, WI and Lizabeth (Mark) Heckendorf of Merrill, WI; her son, Lynn (Lisa) Howard of Stoughton, WI; her grandchildren, Ashley, Leslie, Emily and Megan; and her sister Janet (Allan) of FL. 1. Let time be on your side. When planning your trip, take into account the time of day you intend to travel and likely traffic patterns. Always leave lots of extra time and be aware of possible bottlenecks along the way. 2. Read the agreement. We had a small electrical problem at our house. We called the local electric company, and they recommended four electricians. Three of them came to our house and gave estimates. They all seemed eager for the job and told us to contact them. Unfortunately, when we called and then emailed each of them, we received no replies. They ignored our messages and made us wait to see if they would reply to us. It would have been nice and respectful if they could have at least sent us a simple message saying, “Sorry, but I am unable to take this job because my schedule is busy right now.” That is the proper way to do business!

Responding to the comment that a handicapped person should welcome any assistance they are offered, whether requested or not: I am shocked! I am not an object for your pity. I am a person, endowed with the same rights as anyone else. I am quite capable on my own, but if I require assistance, I will ask for it. Touching me or my wheelchair, without my permission, is assault. It is against the law! Before you offer to “help” me, please wait for me to ask. Otherwise, you could find yourself explaining things to a policeman. I repeatedly asked for a VIP Empire Pass at my library, but it’s always out. I have been told a man checked it out in mid-June and still has

priced $5000 higher. I am a person that when I see a business treating people well, I tell others about them.
Thank you to the townships and the state for keeping the sides of the roads mowed. It helps to see deer and wildlife when driving at night and helps deter them from feeding at roadside. And in doing so, prevents deer-to-car accidents. It’s so nice to know our safety and the safety of the wildlife is still being kept in mind
it. Phone calls have not been returned; emails ignored. You may have saved some money, but all you really accomplished is to be known as a patron inconsiderate of others. It’s a privilege offered by NYS and the library system. It’s a shame you feel you’re owed this benefit more than others. You should be ashamed, but I doubt you are. Featured Nonprofit: Light Hill End of Life Care Training Light Hill in Canandaigua, New York, is looking for volunteers interested in caring for terminally ill residents, their families and loved ones. We provide end of life care training that includes seven classroom sessions (approximately 6 weeks to complete) followed by hands-on “shadowing” prior to caring for our resident guests. Light Hill is a two-bed comfort care home, full of light and positive energy. Our home is an alternative to a nursing home, hospital, or home care. Our primary focus is providing a peaceful quality existence at endof-life guided by the hospice philosophy. Our goal is to support our resident guests as they experience physical, emotional and spiritual changes, as well as support their loved ones. St. Agnes Harvest Tastings Offers Flavors and Fun By Amanda Dudek
O
n Saturday, November 9th, novices and connoisseurs alike will revel in the rich and sumptuous flavors of the Finger Lakes region at the St. Agnes School Harvest Tastings and Silent Auction. This annual fundraising event is much anticipated for its variety of culinary delights and for the fun to be had participating in auctions and draws. Held again this year at the historic Avon Inn, St. Agnes Harvest Tastings will be open from 12pm until 3pm to guests 21 and older. “Every year, we gather together to enjoy the area’s best wine, beer, hard cider and delicious food, including everything from desserts to chilis and homemade pastas,” said event cochair Kathryn Boor. “Arts and crafts are also available from local artisans. This is my 10th year helping fellow SAS parents create this event. We have about 20 vendors each year and always add something new.” This year, the Rabbit Room, located in the heart of Honeoye Falls, and Star Cider, located in Canandaigua, will be joining as new tasting participants. Also new to the event are the Avondale Pub and No BS Brewing. Returning favorites include The Cake Place, Shoreacre Wines, and the Beachcomber to name a few. The event typically draws about 200 guests and has been held at Camp Stella Maris and the Lakeville Training Grounds. “Guests come to eat, drink, bid on items in our silent auction and enter our raffles and drawings,” shared Boor. “It is one of the biggest fundraisers for our school.” 2018 was St. Agnes School’s first year holding Harvest Tastings at the beautiful Avon Inn. “Owner Jenn Kraus and her staff were amazing at helping us pull off such a large event,” Boor recalled. “We love being at the Inn. It is absolutely stunning, and many of our guests had never been inside to see all of the renovations.” For only $25, guests are welcome to sip and sample everything vendors have to offer. Upon entering the Avon Inn, guests will be ushered into its elegant ballroom filled with vendors, the tantalizing scent of tasty samples and fabulous auction items. “Our committee really thought outside of the box to offer some unique auction items for our guests,” said Boor. “We even feature experiences, like getting a ride in an Avon PD patrol car, or firetruck as great gifts for children.” Items also include such things as baskets from many local businesses, tickets to a Rochester Broadway Theatre League show, a vacation, jewelry, memorabilia such as a signed Sabres puck, goods and gift certificates from area eateries and services like family photo sessions. “The room A chalkboard topped desk up for auction, comes alive,” donated by Anne Barrett explained Boor.

“Friends and neighbors set the atmosphere buzzing as they move from one booth to the next. Everyone is welcome to participate in the silent auction as well as to purchase tickets for a Chinese auction. The beautiful creations by St. Agnes School Pre-K students are a huge hit every year,” Boor said. “Bidding can get intense. Every year, Mrs. Cindy Zhe bakes a homemade apple pie, and the bidding war commences! It gets crazy and is a lot of fun.” A wine pull will also be offered. Guests will be asked to donate $20 and are guaranteed to walk away with a bottle of wine worth up to $75 dollars, and some bottles come with a gift card, as well.
