No Screening Test for Bile Duct Cancer ---
DEAR DR. ROACH: A family friend has just been diagnosed with bile duct cancer. She is a breast cancer survivor and is 75 years old. The tumor is said to be the size of a quarter, and it was discovered after she complained of heartburn symptoms. Are there screening tests that could have detected the condition before the heartburn symptoms? Are there things that she could have done to avoid the cancer? My family members are frightened and want to avoid her condition. -- Anon. ANSWER: Cholangiocarcinoma, cancer of the bile ducts, is a rare cancer that is, unfortunately, associated with a high mortality rate. There are no symptoms in most people until it is advanced. Since the cancer is in the bile ducts, the most common symptoms are caused by obstruction of the bile ducts: jaundice (yellow in skin/ eyes), generalized itching, dark urine and light-colored stools. Abdominal pain, fever and weight loss are other symptoms. Cholangiocarcinoma is not a disease that’s amenable for screening; it is rare in North America (it is more common in East Asia, where there are screening programs). Further, the available screening tests, such as blood tests and imaging studies, are not very sensitive, meaning the tests will miss cases. And they are not very specific -- a positive result on the screening test does not necessarily mean cancer. For these reasons, screening is not currently recommended for people at average risk, although it may be considered in people at high risk, such as people with primary sclerosing cholangitis, a chronic liver disease that puts people at high risk for cholangiocarcinoma. Other risk factors for cholangiocarcinoma are mostly beyond a person’s control: other liver diseases, parasites and genetic conditions. There is some evidence that obesity and diabetes increase the risk of this cancer; however, it does no good for you or your friend to look backward. Never blame the victim. She should concentrate on getting treated. I don’t have enough information to comment on her prognosis, but the fact that it was diagnosed early -- apparently before obstruction of the bile ducts -- and with a tumor only the size of a quarter, are favorable. Treatment may include surgery and chemotherapy (before or after surgery).

She was given a prescription for a physical therapist to work with her for her condition. Unfortunately, she has hit dead ends. How can she find a physical therapist to work with her? -- A.F. ANSWER: Physical therapists, like most other health care professionals, have individual skill sets. Pelvic-floor physical therapy requires a level of expertise that not all physical therapists have. I suspect she may have just been calling physical therapist offices near her and not finding a therapist with that expertise. A quick internet search found a good number of specially trained therapists in the area where she lives. There are many online reviews of physical therapists, just as there are reviews of doctors and dentists. Later, interstate highway numbering would be improved by colored signs and the odd-even demarcation that distinguishes between north-south and east-west travel.
(NAPS)—Calls and e-mails from scammers pretending to be government employees are widespread. Social Security phone scams are the #1 scam reported to the Federal Trade Commission. Chances are you, a friend, or a family member have received a call like this. You don’t have to be receiving benefits to become a victim. You may get a call saying there is a problem with your Social Security number or account. Everyone, regardless of age, income, and geography, is at risk. Scammers will try to scare and trick you into giving them your personal information or money.
On March 3, 1931, President Herbert Hoover signs a congressional act making “The Star-Spangled Banner” the official national anthem of the United States. Francis Scott Key had composed the lyrics as he witnessed the overnight British bombardment of Fort McHenry during the War of 1812.
Is It A Scam? The best way to protect yourself and your money is to recognize a scam. Scammers use intimidating language and often offer a “solution” to fix what they say is a serious problem with your Social Security number or account. How can you tell when it’s a scam? Social Security will not:

Scammers prey on your fears. The stories they tell you would scare anyone. No matter how horrible the story, if they do any of the above, it’s a scam. What Should You Do? If you receive a suspicious call, the safest thing for you to do is: 1.

Don’t share personal information or make a payment.
3. Report the scam to the Social Security Administration’s Office of the Inspector General at https://oig.ssa.gov. And, if you receive such threats via e-mail, delete the e-mail and do not click on any links or download any attachments. Even if the e-mail or an attachment contains Social Security’s seal or names of real people, ignore it. Then, report the scam. Other Tips How about if Social Security needs to contact you? Generally, they will mail you a letter and only contact you by phone if you have requested a call or have ongoing business with them. Usually, Social Security will mail you a letter that contains telephone numbers for contacting them. You can also contact Social Security by calling 1-800-772-1213 or visiting SSA.gov. Scammers are always looking for the next way to trick someone. No matter how someone might try to scam you, learning the warning signs shared here can go a long way to protecting yourself and someone you care about from identify theft and financial loss. If you think you have been scammed, don’t be embarrassed. BriPen Senior Rides was started in 2017 and is sponsored by the Town of Penfield. It is a wholly volunteer, donation-based service that seniors, without any private transportation, may request one round trip ride or two one-way rides per week. Clare Van Durme of Dansville Says, “Together, We CANN!” By Amanda Dudek
J
ust over 15 years ago in Dansville, Clare Van Durme opened her home to launch C.A.N.N., the Community Answers Needs Network, and bring hope to hundreds of people across Livingston County and beyond. C.A.N.N., a nonprofit endeavor serving under the auspices of the Holy Family Catholic Community, provides necessities such as available clothing and furniture to those with nowhere left to turn. “I started it in my garage,” said Van Durme. “I saw that people had a need for goods, and I thought I could collect items from others and give them to the people who had need of them. I am the middleman!” Once seen, Van Durme’s vision became impossible for her to ignore. “God told me to do this, literally,” she insisted. “I prayed, kept getting this nudge, and I kept balking.” It didn’t take long, however, for Van Durme to embrace her calling, and once she did, she had to figure out exactly how to help. “I didn’t want to start a ministry that was already being done,” she said. She decided to focus on what many of us would consider the basic necessities. “I provide needs, not wants,” said Van Durme, matter-of-factly. When people have been through an emergency situation like a house fire, are displaced, are starting out in a new apartment or have no other resources, C.A.N.N helps provide things like dishes, pots and pans, linens, blankets, furniture, baby items and clothing, including winter items like coats and boots. They also fundraise to purchase such items as new cribs, diapers and underwear. Of course, since C.A.N.N. is completely dependent on donations, all items are subject to availability. “Sometimes, we have beds; sometimes we don’t,” said Van Durme. “Sometimes, we have refrigerators; sometimes, we don’t.” C.A.N.N. quickly outgrew Van Durme’s garage, moving into two stationary semi-trucks before finally moving into the St. Mary’s School building in Dansville, after the school closed. “I am Catholic; that’s my church,” she explained. “So, I work with that.” Van Durme finds her inspiration in Mother Teresa. “She is my person,” she shared. “If I could even emulate one millionth of the person she was, I would be doing well.” In Mother Teresa’s giving spirit, C.A.N.N. does what it can to help everyone, regardless of income, without judgement. “I ask for name, address and how many are in the household,” said Van Durme. “I am doing what God wants me to do, and the rest is up to them. These things were never mine to begin with.” Van Durme noted that C.A.N.N is also keeping items out of

landfills by giving them a second life in a new home. “When I receive a call, I create a bundle of the items needed,” she said. “The person in need is welcome to take or leave whatever is in their bundle.” C.A.N.N. has certainly grown over the last 15 years, assisting over 450 people in need last year. “People used to just show up,” said Van Durme. “Now, I receive phone calls and referrals as other organizations, nonprofits and government services have become familiar with what I do.” Still, she is amazed by how many people do not know about C.A.N.N. Clare Van Durme encourages people to not only spread the word about C.A.N.N. but also to consider volunteering and serving their community. “This world is not just for taking,” she observed. “Serving your community opens your eyes to needs you never knew existed.” On April 6th, C.A.N.N. will host a Penny Social at the Dansville Moose Club. “People love it,” Van Durme enthused. “They say it is the best Penny Social in the area!” C.A.N.N. will also hold a big yard sale on May 15th and 16th, of items beyond necessities which are donated. “With the money we fundraise, we can purchase some necessary items which cannot be used secondhand,” Van Durme stated. In large pot over medium-high heat, heat oil. Add beef and garlic. Cook, gently stirring until meat is evenly browned. Season with salt and pepper. Add onion, carrots and parsnips. Cook 3-4 minutes. Stir in broth and bring to a boil. Reduce heat to low and simmer about 75 minutes, or until meat is tender. Stir in potatoes and simmer another 30 minutes. Add rosemary and leeks. Continue to simmer, uncovered, until potatoes are tender. To avoid potatoes falling apart, do not overcook. Serve hot and garnish with parsley, if desired. DID YOU KNOW? Brief breaks during the work day can dramatically improve one’s ability to focus on a task. A 2011 study published in the journal Cognition found that brief mental breaks can improve focus on a prolonged task. The study’s authors tested participants’ ability to focus on a repetitive computerized task for 50 minutes under various conditions. The control group performed the task without breaks or diversions. The switch group and non-switch group memorized four digits before performing the task and were directed to respond if they saw one of the digits on the screen while performing the task. The switch group was the only group to be presented with the digits twice during the experiment, and both groups were tested on their memory of the digits at the end of the task. Most participants’ performance declined considerably over the course of the 50-minute task. However, the performance of those in the switch group did not decline at all, as the two brief breaks to respond to the digits allowed them to maintain their focus throughout the experiment. The researchers behind the study concluded that the switch group’s performance was steady because the brain is built to detect and respond to change, and its performance will actually suffer if it must maintain prolonged attention on a single task without taking a break. Professionals with a lot on their plates may see their performance improve by taking frequent mental breaks throughout the workday.
