--DEAR DR. ROACH: I am a 59-year-old woman who was recently diagnosed with hepatitis C and cirrhosis. I have never been a drinker. I was treated with Harvoni for three months. I understand the medication is intended to cure the hepatitis C. What about the cirrhosis? My doctor hasn’t explained any of my long-term effects from taking this. -- C.J. ANSWER: Hepatitis C is a common infection of the liver. It is caused by a virus, which can be transmitted by blood and blood products, as well as through sharing needles for injection drugs. It is rarely transmitted sexually. Anyone with a history of injection drug use, who received blood transfusions or organ transplants before 1992, or who received clotting factors before 1987 should be tested, as should anyone with a known blood or sexual exposure to someone with hepatitis C. The Centers for Disease Control and Prevention also recommends testing all adults born in the United States between 1945 and 1965, anyone who is HIV-infected, who received hemodialysis or was incarcerated. The treatment you have taken (Harvoni) is a combination of ledipasvir and sofosbuvir, and has an excellent cure rate. The cure rate is 99 percent in people who had never been treated before and who did not have cirrhosis. Even people like you, with cirrhosis, had a cure rate of 94 percent with 12 weeks of therapy in the trial that got the medication approved. Cirrhosis is liver damage and scarring as a result of liver disease. Once it has developed, it is not reversible; however, a successful cure of the hepatitis C virus greatly reduces the risk of further damage, specifically reducing the need for liver transplant and the rates of death, liver cancer and other liver-related complications. These medications are so new that I can’t estimate how much lower your risk of further damage to your liver is post-treatment; however, there is little doubt that your likely successful treatment will substantially improve your expected length and quality of life. But, since you still have cirrhosis, you will need to take some important precautions. These include continued abstinence from alcohol, careful monitoring of prescription and non-prescription medications, avoiding raw shellfish and making sure your immunizations are up-to-date. You should keep asking your gastroenterologist questions until you are satisfied. I recommend you make a list of questions to ask, and bring a friend to help make sure you understand the answers you get. *** DEAR DR. ROACH: Should your blood pressure go higher as you age? Is the top number or the bottom more important? I don’t like medicine, and my blood pressure was 145/82, but the top number sometimes goes to 150-158. -- J.S. ANSWER: Both the systolic (top number) and diastolic blood pressures are important, and the difference between them is occasionally important (for example, a blood pressure of 160/60 is more worrisome than 160/85). Blood pressure does go up with age, but even in people over 60, the recommendation is still to get the BP down below 150, and I try to aim for 140 if there aren’t too many side effects. It often takes several tries to find a regimen that gets the blood pressure where it should be without affecting quality of life. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. On October 1st I was shopping at Honeoye Falls Marketplace. A nice couple was in front of me paying for their groceries. They bought two baked chickens. I said "your dinner is ready". The man smiled at me and waved to the cashier. He pointed at my groceries and said "add those on my bill" What a wonderful experience! Thanks again! Grateful to the City of Batavia for the scarecrows on Main Street. So fun and festive! It's nice to drive down Main Street and see how creative people are!

To the "boy" who was behind me on his motorcycle in the middle of the busy [local town] intersection going to the left. I'm not turning quickly rushing the already gone arrow just because you need to get where you're going. Then you make a scene and scream at me about being a bad driver and calling me disgusting names. You're clearly the one who needs to get off the road. I hope your license gets revoked or you get arrested for road rage because that was shameful and rude. Don't assume she knows the neighbors can see her. When the sun is shining it reflects off the windows making it hard to see inside. Why don't you try talking to her about it? If all else fails, you could just not look into her yard. People have a right to be nude in their own homes.

Some clarification is needed. Feeding vermin is actually frowned upon by the NYS DEC. Feeding (baiting) deer is illegal in NYS. Look it up. As for the Bible quote, have you ever heard "do unto others as you would have them do unto you"? Feeding squirrels causes damage to home and property, plain and simple. By the way, our neighbor has more money than the Lord himself and far more than we do. We do our gardening ourselves and buy what we can afford. Fences do not keep vermin out, and twenty foot fences would be necessary to keep deer out which is against village code. Last note, flowers are a gift from God too that we have tried to nurture also and those won't eat holes in screens and cause property damage. I do believe trained service animals serve an important role. But they don't belong in grocery carts! And that nasty pad used to line the cart isn't cutting it. I do applaud [local store], they post at entrances service animals CANNOT be placed in carts. My wife and I were shopping in [local town] in 2 different stores. The first store we were in there was a couple in front of us

In the second store, we watched a couple take their dog and load it in a shopping cart. Now the shopping carts are meant for your food items and your young child. In both cases, these dogs weren't service dogs, and they did not belong in the stores shopping with their owners. I like dogs, but there is a time and place for your pets, and shopping in a store is not the place. Your truck passed me and another car on a double solid line. You think you are so smart? You're going to be the ones in a coffin someday, when you don't get back in time of oncoming traffic. You people who pass our wide trailer and my car and the truck pulling the trailer, I've seen you barely have time to get back in on time from traffic in opposite direction. You seem to have a death wish because one of these days, you being in a huge hurry, you're going to get nailed from you taking stupid chances. Your choice. You think your lawlessness is permissible, I'm sure one day you will pay the price.

You were lucky your fireworks stopped. I was going to call the cops. It's illegal at this time of year and in our county it isn't legal, period. Being so dry out as it has been, it's not a smart thing to do. Anyhow putting all this aside, if you haven‚'t noticed it's not the Fourth of July. But if I ever hear them again, first boom, I will be calling the sheriff's office Not even four months ago myself, as well as other residents of this small hamlet received notice that our sidewalk would be extended. This was joyous news, for children walk on this road, people walk their dogs, and people in general are out walking or exercising. This is a busy route and this was long overdue. However, here we are now the summer is over, yet no sidewalk. I find it frustrating and embarrassing that this around 1/2-1 mile sidewalk extension is so hard to produce. Whether it's the town to which this hamlet belongs is at fault, or the community itself taking it's time to "take the right or low bid" it's inexcusable. It's been four months and it's a sidewalk! So if you are in charge of this project and are reading this, please hurry it up and lets extend it before winter comes.

FAIRPORT David Marsh, A Life of Courage Born in 1940, David Marsh contracted rheumatic fever at age six leading to many hospital stays and a life bound to a wheelchair. However, due to the compassion of the Fairport community, he was able to realize many of his dreams before his 1964 death. Recently, his sister donated many of his favorite items to the Perinton Historical Society and are on exhibit through 2018 at the Fairport Museum (18 Perrin St.), helping visitors remember and honor his short life. Pick your destination Anywhere with deciduous trees provides the opportunity to witness blazing autumn color. During leaf-peeping season, travelers can go coast to coast and see awesome vistas. Some regions are particularly known for their leaf displays. New York’s Catskill and Adirondack regions are prime spots for visitors in the autumn. Those who live further north will find that leaves in Vermont are such an attraction that routes are published indicating where to enjoy the best views. Further south, Central and Eastern Virginia peak in late October. And don’t forget the shores of the Great Lakes, which are awash in reds, oranges and yellows by mid-September. Festival of Ales Provides Local Craft Brewers on Tap By Amanda Dudek On Saturday, October 6th, CB Craft Brewers of Honeoye Falls held their 20th Annual Autumn Festival of Ales at the Honeoye Falls Fireman’s Field. The festival featured 25 different brewers, including CB Craft Brewers themselves, and over 40 different craft beers. The focus of this event was on local breweries and their products. Smokin’ Pete’s BBQ, Duke’s Donuts and others provided food, and “pretzel necklaces” abounded. On stage at the back of the festival, Big Mike & the Motivators gave guests a reason to dance, and some guests took them up on their offer. Surrounded by such diverse local brewers, it is hard to believe that only a fraction of today’s local breweries even existed just a decade ago. In our Finger Lakes Region of New York, creating beer has become big business. The Finger Lakes Region leads the state in craft breweries, second only to the Hudson Valley Region. New York State saw a surge of interest in craft brewing in 2012. Since then, the number of local breweries has more than doubled. Bolstered by assistance from the state, many Finger Lakes residents took the opportunity to pursue their dreams and establish their own breweries, microbreweries and farm breweries. Given this very significant upswing in the number of local craft brewers, these breweries have become an even greater small business component in our communities. The 20th Autumn Festival of Ales embraced these local brewers, providing a fun opportunity not only to sample various craft beers but to support these local businesses, as well. Of the craft beers we sampled, we particularly enjoyed CB Craft Brewer’s own Krys2Toff, a heavy, dark Baltic Porter with a

caramel start and a multi-finish, as well as their Hefeweizen, a curiously flavored beer with a melon-fruity start and a light, crisp finish. Other standouts included three additional brews. Hailing from Rochester, Fairport and Webster, we were surprised by Napa Woodfired Pizzeria’s Harvest Basil Brown Ale, a unique, heavy ale with a floral start and a nutty finish. In from Mumford, the Genesee Country Village & Museum’s Fat Ox Ale, a full-body ale with an expresso start and a smooth finish was a welcome, albeit different, addition. Finally, visiting all the way from Hamburg, NY, Mammoser’s Oktoberfest, a medium-body brew with notes of nut and spice and a very smooth finish, won us over. The wide variety of beers put on draft by local craft breweries, the relaxed good natures of the men and women pouring into adorable, mini tasting mugs, the delicious food, good music and pretzel-necklaced crowd all made this an enjoyable event. The weather, which had been rainy, held up with warm sunshine for the duration of the festival. Importantly, local brewers had the opportunity to show off their craft, apparently to great success.

How you can benefit from having a pet Pets have been kept for centuries. Statistics from various sources indicate North Americans own millions of dogs, cats, birds, small mammals, and reptiles. Pets’ affable nature and loving looks are enough for many people to welcome them into their homes, but the benefits of having a pet extends beyond their appearances and temperaments. • Pets can help prevent loneliness. Loneliness affects people of all ages, but it is particularly problematic among seniors. Older adults who may be isolated can benefit from having a pet around. According to a study published in Aging & Mental Health, older adults who owned pets were 36 percent less likely to say they were lonely compared to those who didn’t have an animal companion. • Pets can save lives. Pets can be trained to perform various tasks around the house and in the community. Rescue animals assist in finding people after natural disasters. Medical alert pets can help people with debilitating illnesses and assist physically impaired people with everyday tasks. • Pets help lower allergy risks. Keeping pets around can reduce a child’s likelihood of developing allergies by as much as 33 percent, according to a study by pediatrician James E. Gern that was published in the Journal of Allergy and Clinical Immunology. People exposed early on to animals tend to develop stronger immune systems overall. • Pets provide socialization opportunities. A pet can be the common denominator to strike up new friendships and connect with others. Whether walking around the neighborhood or being part of a pet obedience class or interest group, pets can help their owners expand their social circles. • Pets can help combat stress. Talking to or stroking a pet can

12 whole fresh mushrooms, washed 1 T. vegetable oil 1 T. garlic, minced 1 (8 oz.) pkg. cream cheese, softened 1/4 c. grated Parmesan cheese 1/4 t. ground black pepper 1/4 t. onion powder 1/4 t. ground cayenne pepper Preheat oven to 350 degrees F. Carefully break off mushroom stems and chop until extremely fine. Heat oil in a large skillet over medium heat. Add mushroom stems and garlic and fry until no moisture remains, taking care not to burn garlic. Set aside to cool. Combine cream cheese, Parmesan cheese, black pepper, onion powder and cayenne pepper. Stuff each mushroom cap with cheese mixture and arrange on a greased cookie sheet. Bake for 20 minutes, or until mushrooms are piping hot and liquid starts to form under caps.
