Anyone with diabetes can develop diabetic eye disease, though some groups are more affected than others. According to the National Institute of Diabetes and Digestive and Kidney Diseases, African Americans, American Indians and Alaska Natives, Hispanics/Latinos, Pacific Islanders, and older adults are at an elevated risk of diabetes-related vision loss or blindness. It’s also important that pregnant women with diabetes recognize the risk of eye problems. The NIDDK reports that women with diabetes can develop eye problems very quickly during pregnancy. Women who have already been diagnosed with diabetic retinopathy, the most commonly diagnosed diabetic eye disease, may have their condition worsen during pregnancy. The link between diabetic eye disease and pregnancy has to do with the changes a woman’s body undergoes when she is with child. The NIDDK notes that changes the body undergoes to support a growing baby can put stress on the blood vessels in the eyes. Damaged blood vessels can harm the retina, thereby increasing a woman’s risk for diabetic retinopathy. Individuals with diabetes who are at a heightened risk for diabetic eye disease, including pregnant women, can speak to their physicians about what they can do to protect their vision in the short- and long-term.

My hostas have a milky substance coming out of the new shoots. This is the first time I have ever seen this. I’ve cut off the affected shoots. Any ideas as to the cause of this?
Without seeing the plant to identify exactly what it could be,a white “milky substance” around your hostas could be a number of things, including two of themostcommon:anthracnoseand slime mold.

spot on. Removing the affected area is the best treatment, being careful not to allow spores to spread. Very often mulch fosters such growth and spores spread even with the best of intentions. I have found the removal of all debris around hostas to be the best practice in preventing slime mold. Larger growth concentrations, though, may call for the use of a fungicide, which I recommend not using without consulting your local Cornell Cooperative Extension Office. Anthracnoseisalsoafungusandisthe
most common and most widespread in cultivating good hosta growth. The good news with anthracnose is that it won’t kill the plant, but treating it can be tricky. To rid your garden of this fungus could involve a multi-step approach, particularly if the fungus goes unnoticed for some time. I would begin by removing the affected plant and any fungal growth noticeable on the mulch or ground surrounding it. Then, contact your local Cornell Cooperative Extension office. The extension office has at
its disposal a number of resources, including a team of Master Gardeners who can assist with identifying the problem and recommending specific resources for further assistance.

Memorial Day in our community is a heartfelt celebration combining remembrance with the joyful start of summer. From what I saw, so many towns waved their flags and patriotic decorations, honoring the brave souls who served our nation even through the rain. The annual parade is a highlight, featuring local marching bands, veterans, and community groups proudly making their way down Main Street. Thank you to those involved.

Went to local DMV with son to get his permit. He lost his glasses at work so went to have eye appointment to get new ones. With all paperwork in hand from eye exam DMV said there’s no proof of passing eye exam even though he has new prescription for glasses. Had to go back to eye glass place 45 min drive to have eye Dr. Sign paper saying he passed eye test. Then still given hassle because he didn’t have his glasses. What is an eye test paperwork for then. Tired of people wasting time even eye place said same paper they gave him. Just a waste of time on both ends.

DEAR PAW’S CORNER: I have a dog and a cat, and my last vet visit cost over $300. It’s incredibly difficult to pay for vet care on top of the ridiculous cost of housing, grocery prices and my own health care costs. How can I keep pet care affordable? -- John in Newton, Massachusetts
DEAR JOHN: Pet care can be quite pricey, and I know you want the best for your dog and cat. I just laid out a few hundred dollars for my senior cat’s checkup and tests, so I feel your pain.
e Humane Society of the United States has some good information about cost-saving options, discounted veterinary services and ways to stretch out payments: www.humanesociety.org/resources/are-you-having-trouble-affording-your-pet.
One interesting avenue is veterinary colleges. Today, almost every state has at least one school of veterinary medicine, and many operate lowcost clinics.
Check the town hall or municipal website for upcoming shot clinics. ese are held in many communities at least once a year at an easy to access location (like a pet store) and offer required vaccinations at a very affordable cost. Some clinics also offer microchip services and pet wellness exams for a few dollars more.
Finally, if you have the means and just want to get your budget under control, take a look at pet insurance. ere are a number of companies providing this service, with several coverage options -- including reimbursing the cost of annual exams and vaccinations, as well as care for unplanned injuries and illnesses. Coverage varies, and it’s important to read the fine print and compare plans closely, but it can really help keep unexpected costs from derailing your checking account.

Spring cleaning encompasses a host of tasks, and homeowners who want a truly clean house should not overlook smaller tasks that can produce big results. they might be most often associated with dogs, ticks can affect cats as well. According to Blue Cross for Pets, cats are most likely to encounter ticks if they live near areas with lots of deer and sheep or in regions where other wildlife, such as hedgehogs and rabbits, are present. While ticks are active throughout the year, cats are most likely to encounter them between spring and autumn. That’s due in part to the climate during these seasons, which tends to be warmer and therefore more inviting to cats, compelling them to spend more time outdoors than they do in winter. Ticks, which are egg-shaped, are big enough to spot, so cat owners should conduct a careful examination of their cats’ fur each time they come into the house after spending time in the yard. Blue Cross for Pets notes that ticks feel like small bumps on a cat’s skin. Ticks are most likely to attach themselves to particular areas of a cat’s body. These areas include the head, neck, ears, and feet. Routine brushing, especially whenever cats come inside after time spent outdoors, can help cat owners spot and remove ticks from their cats. It’s important to recognize that indoor cats are not invulnerable to ticks, as other pets and humans can track ticks indoors. Periodic inspections and brushing of indoor cats’ can protect them from ticks and the diseases ticks carry.

COLD, PAINFUL FINGERS CAN MEAN A SERIOUS DISEASE
(NAPSI)—“Zombie Hands.” That’s what can happen to an estimated 5 to 10 percent of the population whentemperaturesstarttofall.
In a typical case of Raynaud’s phenomenon, also known as Raynaud’s disease or syndrome, sufferers experience numbness and pain in their fingers, toes and other extremities. Fingers turn white, blue or red as the small blood vessels go into spasm within minutes of exposure to cold or stress, and they appear to be “dead”asbloodflowisconstricted.
Named for the French physician Maurice Raynaud, who first recognized the condition in 1862, it causesaninterruptionofbloodflow to the fingers, toes, nose or ears. There may be associated tingling, swelling or painful throbbing. The attacks may last from minutes to hours. In severe cases, the area may develop ulcerations and infections, which can lead to gangrene.
Raynaud’scanoccurasa“primary” disease—thatis,withnoassociated disorder—or as a “secondary” conditionrelatedtootherdiseases, such as scleroderma, lupus, and rheumatoidarthritis.
“Ninety percent of Raynaud’s sufferers don’t seek treatment and too many physicians pay short shrift to those who do,” says Lynn Wunderman, founder and chair of the Raynaud’s Association. “Treatment is important because some sufferers may have an underlying condition such as systemic scleroderma or lupus. Awareness of such a problem may allow for earlier medical intervention.” Simple blood tests can rule out the presence of antibodies associated with diseases that have Raynaud’s as a component.
What’s Being Done
Tohelp,theRaynaud’sAssociation, a 501(c)(3) nonprofit health organization, launched a new and assertive campaign,”Don’t Turn a Cold Shoulder to Painful Fingers,” to urge those with the disorder— and their doctors—not to dismiss the pain that Raynaud’s sufferers endure,orthelifestyleadjustments they make to minimize exposure to coldorstress.
Although there is no known cure as yet, treatment options such as calcium channel blocker drugs have been clinically proven to alleviate symptoms by opening up the blood vessels so blood circulatesmorefreely.

Patient Seems Unsure of ALS Diagnosis
DEAR DR. ROACH: My question is about amyotrophic lateral sclerosis (ALS). I have been diagnosed with this horrible, debilitating disease, and I don’t know where I am headed. I went to a hospital for a second opinion and additional testing to rule out other possible causes of this numbness. Nary a test was done -- the neurologist looked at the electromyography (EMG) done by a neuromuscular doctor at another facility, and he just agreed with the diagnosis.
I wanted to get tests done to see if it was perhaps a concussion, if it was from using yard sprays or from COVID vaccines, or if it was due to my having a terrible bad back for years, but none of these were even considered. I am deteriorating rapidly. Would you please give me some suggestions? Am I wrong for seeking other causes? -- J.B.
ANSWER: I am very sorry to hear that you have been diagnosed with ALS. I understand why you would like to believe there are other possibilities for this because there are no treatments for ALS that can stop the progression of this fatal disease.
The diagnosis of ALS is made after a thorough examination and history review. Electrodiagnostic studies, like an EMG or a nerve conduction test, can help support the diagnosis, but they do not make the diagnosis themselves. The nerve conduction study is normal for ALS, while the EMG shows the nerves that are not connecting to the muscles. Other diseases of the muscles and muscle nerves should be considered. MRI scans are usually performed, as they can identify conditions that mimic ALS. The terrible back you mention could include a condition called cervical myelopathy, which has some features that can be mistaken for ALS. Routine blood and urine testing should be done to exclude other conditions, such as Lyme disease, and the spinal fluid is sometimes analyzed if the neurologist suspects immune-medicated nerve disease. However, none of the other possibilities that you mention is likely to be confused for ALS by an experienced neurologist who does a complete exam.
I have three concerns: The first is that you are suggesting you may not have had a thorough evaluation so far. Second, the only symptom you mention is numbness, and although this can occasionally happen with ALS, you are much more likely to have weakness and muscle twitching without numbness. Third, ALS tends to progress slowly, not rapidly.
For all these reasons, and because you need to be sure of your condition, I recommend getting another opinion from a neurologist who doesn’t rely on someone else’s history review and exam.

A SWEET TREAT FOR GET-TOGETHERS
Yields 64
Crust
21/2 cups all-purpose flour
3/4 cups powdered sugar
1 teaspoon baking soda
3/4 cups butter or margarine, melted
Filling
3 cups pecan halves, divided 2 cups packed brown sugar
3 eggs
4 tablespoons butter or margarine, melted
2 teaspoons vanilla extract 1/4 teaspoon salt
Powdered sugar, optional
Preheat oven to 350 F. For crust, combine flour, powdered sugar and baking soda in a batter bowl. Add butter; mix until crumbly. Lightly press crumb mixture over bottom of stoneware bar pan; roll lightly. Bake 15 minutes; remove from oven to a cooling rack.
Meanwhile for filling,reserve 1 cup of the pecans for garnish; chop remaining pecans. Combine chopped pecans, brown sugar, eggs, butter, vanilla, and salt; mix well. Pour filling over warm crust, spreading to edges of pan. Arrange remaining pecan halves over filling. Bake 17 to 19 minutes, or until filling is set in center. Remove from oven; cool completely.
Sprinkle with additional powdered sugar, if desired. Cut into 32 squares using a utility knife; cut each square in half diagonally.

A Slice of Life: Alessio and Sarah Santiago Take Over Salvatore’s Fairport Location

In the heart of Fairport, where the scent of freshly baked pizza mingles with the bustling energy of the community, lies a cherished establishment - Salvatore’s. But this isn’t just any pizza joint; it’s a story woven with familial ties, childhood memories, and a passion for serving others. Meet Alessio and Sarah Santiago, who took over the Fairport location in April 2024.
eir story begins over a decade ago, as East Irondequoit high schoolers working at Salvatore’s E Ridge/Culver location. A er pursuing different career paths - Alessio at Wegmans and Sarah in the banking industry, they have now been together for ten years, married for three, and with their daughter, Alessiana, and three “crazy dogs, Atlas, Ink, and Enzo,” the Santiagos are excited to blend their professional backgrounds in operating the Fairport Salvatore’s location. Alessio runs daily operations and Sarah coordinates marketing and accounting.
Over the past few weeks, the Santiagos and their parents have cleaned and repainted the shop, and Sarah says they’re very thankful for the help of their parents, who have helped without hesitation. eir siblings even traveled across the country for the monumental first day.
Sarah particularly recognizes Ray and Maria LoRe, owners of Salvatore’s E Ridge/Culver and e Garage Door. She says they owe a lot of who they are as employees and now owners to Ray and his dedication to his business: “Ray was our first call when we considered becoming franchisees,” and since then, “Ray and Maria have been by our side from training to reorganizing the kitchen and bringing us supplies from their stores. eir children, who also work at their locations, have been a ton of help as well. To say we are lucky to have this family in our corner is an understatement - we are blessed.”
e Fairport village location was once home to Arrivederci’s Pizza, owned by Sebastian with his mom Connie by his side. Sarah says Sebastian and Mama C are still very much involved, Sebastian owning the building and offering a wealth of community information to the Santiagos, and Mama C still works at the pizzeria at 83 years young! ( ank her for the perfectly baked cookies you’ll see at this location.) Sarah says, “To us, it feels like we’ve known Sebastian and Mama C our entire lives, the way they’ve shared their memories and knowledge with us. We want to make them proud.”
Central to their vision for Salvatore’s Fairport is a commitment to putting employees first, echoing the supportive environment they experienced as young employees. By fostering a culture of camaraderie and mutual respect, Alessio and Sarah aim to create a workplace where every team member feels valued and empowered to excel. Sarah says, “We’re where we are today in large part due to those who invested their
time in us throughout our careers. We want our employees to feel like an extension of our family like we did 15 years ago and still now within the Salvatore’s family.”

The U.S. Department of Agriculture says birds, bees, bats, butterflies, moths, flies, beetles, and small mammals all can work as pollinators. Examples of crops pollinated by pollinators include apples, squash and almonds.Animals and insects help pollinate more than 75 percent of the world’s flowering plants, and nearly 75 percent of all crops, according to the U.S. Fish & Wildlife Service.
