--DEAR DR. ROACH: My wife, age 70, complains frequently of a sour taste in her mouth soon after eating. I thought she had esophageal reflux, but I did not find that sour taste is one of the symptoms. She has no heartburn, but the taste occasionally improves after taking an antacid. -- B.P. ANSWER: Gastroesophageal reflux disease can have no symptoms, or many. Sour taste is, in fact, a common symptom. This is the “reflux” of the condition’s name, since it refers to the backward movement of stomach acid -- all the way into the back of the throat or even into the mouth. Heartburn is perhaps the most common symptom, but so is difficulty swallowing. This is more common in people with longstanding symptoms. Antacids neutralize the acid in the stomach but do not prevent the reflux of stomach contents. Widely prescribed medications, including H2 blockers like Zantac or Pepcid (“H2” is a type of histamine receptor, a stimulus for acid production) and proton pump inhibitors (like Prilosec or Nexium, which directly reduce acid secretion), also act on the acid in the stomach, and do not reduce reflux. In order to reduce reflux, I recommend eating smaller meals, not reclining for two to three hours after eating, losing weight if appropriate and avoiding products that weaken the lower esophageal sphincter, the valve-like muscle at the bottom of the esophagus. Alcohol and tobacco are the biggest offenders. Raising the head of the bed (traditionally with a brick, but foam wedges also are good) uses gravity to help the refluxed acid get out of the esophagus. Reducing reflux or acid at night is most important. While the body is in a horizontal position, the acid can stay longer in the esophagus and cause damage. *** DEAR DR. ROACH: I am very concerned about my 81-year-old sister. She is 4 feet 10 inches tall and weighs 78 pounds. This is a new low weight for her. She recently had bloodwork done and was told she has an underactive thyroid and should take medication for it. She also has a colon problem and COPD. I thought that with an underactive thyroid, you gained weight, not lost. Can you explain the difference between underactive and overactive thyroid? -- C.K. ANSWER: The thyroid gland in the neck produces thyroid hormone, which affects every tissue in the body. People with an underactive gland (called hypothyroid) tend to gain weight, may have dry skin, and slow bowel movements, and can be lethargic or sluggish. People with overactive thyroid (hyperthyroid) usually are the opposite: full of energy, thin and intolerant of heat. The diagnosis is confirmed by blood tests of thyroid levels. However, these common conditions occasionally look very different from the normal. I have seen people with severe hyperthyroidism be lethargic to the point of apathy. Weight gain or loss isn’t inevitable with thyroid disease. However, with severe COPD, weight loss is almost universal. The COPD may be a bigger contributor to her weight loss. But with a body mass index of 16.3, she is dangerously underweight and needs further evaluation and treatment. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Learn to downsize before a move Aging men and women often take inventory of their lives in an effort to focus on activities or lifestyle changes that can ensure happy retirements. Data from the U.S. Census Bureau shows there are some 76 million baby boomers — those people born between 1946 to 1964 — across the country. With the youngest of the boomers in their mid-50s and the majority having already reached retirement age, many boomers are trying to decide if it’s time to move out of their family homes and into smaller, more manageable abodes. Many older adults find they do not need the same amount of space as they did when they had children living at home. Retirees and those on the cusp of retirement may find that downsizing is a smart financial move that frees up more time for recreation. However, it can be challenging to cut down on living space and then deal with figuring out how to make furniture, belongings and stored items fit in more condensed areas. Moving can be stressful even without having to cut down on prized items. Taking an inventory of belongings can help the process go smoothly. Before moving, men and women can go room by room, making piles of items that will be kept, donated, sold, or discarded. This can be a tedious task, but it is necessary to avoid clutter in a new home. People downsizing can attempt to sell items they do not need via newspaper classified sections or online classified sites. Appliances and furniture in excellent shape may fetch good prices. Any extra cash can be put toward buying new items that are size-appropriate for the smaller home. Another way to clear out clutter is to sort duplicates from the stock of items. A person may no longer need multiple sets of dishes or silverware. If the move involves switching from a kingsized to a queen-sized bed, donate or trash bed linens that will no longer fit. Pay close attention to kitchen and bathroom items,

which tend to accumulate over time but might not be discarded when clearing a home of clutter. People moving from a detached home to a condominium or a townhouse may learn that homeowner’s association fees cover everything from snow removal to lawn maintenance to pool upkeep. If so, it’s unnecessary to bring lawn and garden supplies. Homeowners are advised to look at the floor plan of their new dwellings and pay attention to storage space. This can make it easier to plan ahead for what may fit, what will need to be purchased new and which storage solutions may be needed. Having a plan in place can make unpacking and settling in go smoothly. The organizing company Organize Me says that homeowners should consider how cabinets and closets will be used before moving in. Downsizing can free up time and money. When done right, downsizing can make retirement easier and create more leisure time for retirees.

Fairport to Host Overnight Stop on 2018 Great Race Fairport, N.Y., will host an overnight stop on the 2018 Hemmings Motor News Great Race presented by Hagerty Saturday, June 23, race organizers have announced. The Great Race, the world’s premiere old car rally, will bring 120 of the world’s finest antique automobiles to town for the $150,000 event, with the first car rolling onto South Main Street downtown starting at 4:30 p.m. In all, the participants in the event will cover more than 2,300 miles in 9 days. The start will be at the Pierce Arrow Museum in downtown Buffalo, NY, on June 23. The race will finish July 1 in Halifax, Nova Scotia, on the banks of Halifax Harbour as part of that city’s Canada Day celebration. Teams and cars from Japan, England, Germany, Canada and every corner of the United States will be participating in their vintage automobiles dating back as far as 1913. “There are more than 500 people just in our entourage from all around the world taking part in this incredible adventure,” director Jeff Stumb said. Along the route, competitors will travel through four states - New York, Vermont, New Hampshire and Maine - and two Canadian provinces - New Brunswick and Nova Scotia. The Great Race, which began 35 years ago, is not a speed race, but a time/speed/distance rally. The vehicles, each with a driver and navigator, are given precise instructions each day that detail every move down to the second. They are scored at secret check points along the way and are penalized one second for each second either early or late. As in golf, the lowest score wins. Cars start - and hopefully finish - one minute apart if all goes according to plan. The biggest part of the challenge other than staying on time and following the instructions is getting an old car to the finish line each day, organizers say. Each stop on the Great Race is free to the public and spectators will be able to visit with the participants and to look at the cars for several hours. It is common for kids to climb in the cars for a firsthand look. Cars built in 1972 and earlier are eligible, with most entries having been manufactured before World War II. In the 2017 Great Race from Florida to Michigan a 1932 Ford Cabriolet won the event when it crossed the finish line in downtown Traverse City. That vehicle was driven by Jody Knowles of Tyrone, GA, and navigated by Beth Gentry of Newnan, Ga. They won $50,000 for finishing first overall.

A 1913 Bugatti, a 1916 Hudson Pikes Peak Hillclimber, a 1916 Chevrolet Phaeton, a 1917 Peerless Racer and a chain-driven 1918 American LaFrance Speedster are the oldest cars scheduled to be in the 2018 Great Race. The winners will again receive $50,000 of the $150,000 total purse. Over the decades, the Great Race has stopped in hundreds of cities big and small, from tiny Austin, NV, to New York City. “When the Great Race pulls into a city it becomes an instant festival,” Stumb said. “Last year we had four overnight stops with more than 10,000 spectators on our way to having 250,000 people see the Great Race during the event.” The event was started in 1983 by Tom McRae and it takes its name from the 1965 movie, The Great Race, which starred Tony Curtis, Jack Lemmon, Natalie Wood and Peter Falk. The movie is a comedy based on the real life 1908 automobile race from New York to Paris. In 2004, Tony Curtis was the guest of the Great Race and rode in his car from the movie, the Leslie Special. The Great Race gained a huge following from late night showings on ESPN when the network was just starting out in the early 1980s. The first entrant, Curtis Graf of Irving, Texas, is still a participant today. The event’s main sponsors are Hemmings Motor News, Hagerty, Coker Tire and Reliable Carriers. The families that own Knuckleheads Craft Brewery, in West Webster, put on a fundraiser for The Cystic Fibrosis Foundation this past Tuesday. Ten different small craft breweries donated beer and other establishments donated food and prizes. Knuckleheads raised over 6,000! Everything brought in, including tips, went right to fighting CF. Knuckleheads definitely deserves a lot of praise! You hear a lot of talk today about how the generation coming of age now are spoiled and self-centered and apathetic. I attended a dinner last month to honor student volunteers from a local college, and I want to say that I was so impressed by the caring and selflessness of these young

We had a sold out number of guests and they donated a full carload of items for the shelter. Our guest speaker Karen Tremmer did an outstanding presentation about the shelter and who and how it
