On October 6th, the 8th annual Batavia Wine Walk will bring area wine connoisseurs and novices alike the opportunity to get together with friends and neighbors for a leisurely stroll through their community while enjoying several superb, New York wines. This year, 25 local businesses have joined the Batavia Wine Tour to serve as tasting locations. Each local business will represent a different New York winery. Wine Walk guests are provided with a program with a guide to these stops and are welcome to visit them within the three-hour event in whichever order they choose. This year’s Batavia Wine Walk begins with its first stop at the Batavia branch of the Genesee Valley Penny Saver. VIP guests may arrive an hour earlier and receive special perks such as a goodie bag and a free raffle ticket for entry in the Batavia Wine Walk Basket Raffle. Three, quite large baskets are being raffled off this year, and they are filled with very nice products and gift cards from local restaurants and businesses. All participants receive a commemorative wine glass and a program with the tasting locations guide. Participants are then free to weave through local businesses and become more familiar with these establishments, particularly the ones which are stops along the walk. This event is great fun for businesses, too. Downtown Batavia BID Executive Director, Beth Kemp, states that while some businesses may choose to have a simple, straightforward, winery display and tasting space, several others become much more involved with the theme of the walk, providing food, treats, music and other goodies to Batavia Wine Walk guests. An award with accompanying prizes is given to the business with the best participation. This variety of tasting environments from business to business has provided guests with an enjoyable experience, each year bringing new, little surprises. This year will be the second year that the Wine Walk has also had a theme. Last year,

many guests joined in dressing up for the first ever Batavia Wine Walk Theme: the 80s. Several business owners decorated for this theme, as well. This year, the theme is movie characters, and we have it on good authority that there might be some good, old-fashioned, Hollywood red carpet glamour involved, as well. While dressing up is by no means necessary, best movie character awards and prizes will be given, on an individual and group basis, to some of those who choose to do so. The Batavia Wine Walk will sell only 600 tickets, 150 of them VIP passes, to keep the event somewhat more exclusive. VIP passes are $35 while General Admission tickets are $25. Designated Driver tickets are also available for $5, as guests are encouraged always to drink responsibly. Tickets, especially the VIP passes, do tend to sell out for the Wine Walk. They are currently available online at www.downtownbataviany.com. Tickets are also available at three area businesses: The Yngodess Shop, Adam Miller and The Hidden Door/ Pollyanna & Dot. The Batavia Wine Walk runs from 5pm to 8pm and is a fun way to get out and about with friends and family and, of course, great wine.

Gardening benefits the environment in myriad ways. Maintaining natural landscapes and preserving green spaces can reduce the collective carbon footprint of the human race. Trees, flowers and other greenery filter the air and create welcoming habitats for all species of animals and insects. Many home gardeners set out each spring to create landscapes that cater to all of the senses. But choosing plants that are unlikely to thrive in certain climates can lead to dissatisfaction and premature plant demise and may require gardeners to use more fertilizers, pesticides and other not-so-Earth-friendly techniques to help plants thrive. One of the more important steps gardeners can take before spring arrives is to educate themselves about plant hardiness zones. Hardiness zones are defined by the average climatic conditions of the region and are broken down into various zones. The USDA Hardiness Zone map divides North America into 13 separate zones. Each zone is marked by 10 F incremental differences from the last zone. In some versions of the map, each zone is further divided into “a” and “b” regions. The National Gardening Association says the USDA Hardiness Zone Map was revised in 2012. The latest version was jointly developed by the USDA’s Agricultural Research Service and Oregon State University’s PRISM Climate Group. To help develop the new map, the USDA and Oregon State University requested that horticultural and climatic experts review the zones in their geographic areas. As a result, the zone boundaries in the 2012 edition of the map have shifted in many areas. Canada’s Hardiness Zone map, updated in 2010, uses nine different zones. Zone maps are tools that show where permanent landscape plants can adapt. Home gardeners who are looking for shrubs or perennials to last year after year should recognize that such plants must tolerate year-round conditions, including the lowest and highest temperatures and the amount of precipitation. Snow cover and humidity also can impact a plant’s propensity to thrive. While zone maps are not perfect, they can be useful in planning and ensuring the survival of future gardens. Plant and seed manufacturer Burpee says that other factors to consider that also affect the viability of plants include wind, moisture and soil conditions. Addicted seniors a serious problem: Opioid use among the older population The public is quickly learning that drug abuse goes beyond the illegal substances that are purchased on the street. Abuse of drugs extends to the prescription medications sitting in many medicine cabinets. While teenagers and young adults may be the first to be stereotyped as prescription drug abusers, seniors may have unwittingly become mixed up in one of the most misused prescription classes: opioid pain relievers. The problem of opioid abuse has been a growing issue for years. The National Institute on Drug Abuse estimates 2.1 million people in the United States suffer from substance abuse of opioid pain relievers. Older patients are increasingly and repeatedly prescribed opioids to address chronic pain from arthritis, cancer and other problems that become more apparent as people age. Data from U.S. Medicare recipients found that, in 2011, roughly 15 percent of seniors were prescribed an opioid after being discharged from the hospital. When followed up on three months later, 42 percent were still taking the medication. Fast forward to 2015, and almost one-third of all Medicare patients were prescribed opioid painkillers by their physicians, says AARP. The Canadian Institute for Health Information says adults between the ages of 45 and 64 and seniors age 65 and older had the highest rates of hospitalizations due to opioid poisoning over the past 10 years. AARP also indicates nearly three million Americans age 50 or older have started to take painkillers for reasons beyond what their doctors prescribed. Experts from the Physicians for Responsible Opioid Prescribing warn that dependence on opioids can set in after just a few days. Discomfort and side effects can occur when the pills are stopped. Opioids can decrease pain at first, but many people find they can be less effective over time. As a result, patients need to take greater amounts. Although many people can take opioids in small doses for short periods of time without problems, many find

Another possible risk of opioids among seniors is that the medication can cause disorientation that may lead to falls and fractures. The senior care resource A Place for Mom also says that prescription narcotics may increase risk of respiratory arrest. What’s more, an older body may not absorb and filter medicines as effectively as younger bodies can. This means that older adults might become addicted to or have side effects from a prescription drug at a lower dose. Seniors concerned about opioids can discuss other options with their doctors, such as nonopioid medications and alternative therapies for pain management, like massage or acupuncture. If opioids are prescribed, ask for the lowest dose and don’t exceed the time frame for taking the medicine. Only take the pills when absolutely necessary, and never mix opioids with alcohol or other substances.

--DEAR DR. ROACH: I am a 44-year-old man with chronic headaches and migraines. Should I worry about the long-term effects of taking an NSAID (such as diclofenac) at least once a week? -- G.W. ANSWER: Nonsteroidal anti-inflammatory drugs such as aspirin, ibuprofen and naproxen are among the most commonly taken classes of drugs in the world, with 17 million Americans alone taking them daily. Although these drugs are generally safe, any medication has the potential for side effects. Typically, the higher the dose and the more frequently it is taken, the increased risk of side effects. So, a once-aweek dose would be expected to have a low side effect risk. Adverse effects from this class are among the most frequently seen in clinical medicine. Although they can affect multiple systems, let me briefly go over the side effects of NSAIDs on the GI tract, kidneys and heart. NSAIDs might affect the stomach, and although we think of ulcers, stomach pain without ulcers is more common. Ulcers happen most often in people who take higher doses for long periods of time. A history of ulcers, older age and use of other medications (steroids, warfarin, antiplatelet drugs like clopidogrel, or Plavix) increase the risk of ulcers and bleeding. NSAIDs also might affect the small bowel and colon, causing bleeding and diarrhea, among other symptoms. Again, these are most common in high doses for extended periods. NSAIDs can affect the kidneys, causing both acute and chronic kidney failure, but only in a few percent of people. Periodic blood and urine examination is appropriate for people who are on long-term NSAIDs. The risk of heart disease among people who use NSAIDs regularly is increased, but in people without known heart disease, the risk is small -- about one bad event, such as heart attack, stroke or episode of heart failure, per thousand people taking the medication for a year. I would advise you that the risk of any of these side effects is unlikely in someone taking the drug only once a week, but people who take these drugs every day should know about the potential harms. *** DEAR DR. ROACH: When I was 64, a cyst was found in my bladder. When the cyst was removed, it was found to be cancerous. My urologist wants me to continue to have an annual cystoscopy, which I do. I’m now 76 and in good health. Do you feel the annual cystoscopy is still necessary? -- C.K. ANSWER: Now that more people have had successful treatment of cancer, we are starting to learn about the risks they face in subsequent years. The risk of another cancer is generally higher, but it’s the exact form of cancer that determines how much risk there is, and consequently, how aggressive follow-up must be. In the case of bladder cancer, I have read some studies that the risk is quite small after five years. However, the American Urologic Association has clear guidelines: Because of the increased risk for recurrence, it recommends annual cystoscopy indefinitely. Cystoscopy is a pretty safe procedure, so I think your urologist’s recommendation is reasonable. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. THE FLAP OVER WISDOM TEETH When the third molars (“wisdom teeth”) do not have sufficient room to erupt through the gums, a partial eruption can lead to inflammation and infection of the soft tissue around the tooth. Be patient. Lifting too much weight or pushing yourself too hard on the treadmill, exercise bike or elliptical machine will increase your risk of injury.
