Thigh Numbness Likely Due to Trapped Nerve --DEAR DR. ROACH: I am a 65-year-old male who has developed a lack of sensation in the lateral front half of my right thigh. The numbness involves strictly the skin. There is no loss of strength, function or balance in my leg. My gait is normal, although I find that if I go on a long walk, the skin will begin to “tingle.” When I went to an orthopedic surgeon, he suspected that the condition could be caused by a disc problem. Although an X-ray did show some stenosis, it was not definitive. I suspect I caused the problem by years of sleeping on my right side in a curled-up position, resulting in entrapment of the nerve. The orthopedist, however, says he generally sees this condition only in obese people, which I am not. How likely is it that my condition is of spinal origin rather than an entrapped nerve? If the condition can resolve itself gradually by avoiding activities that contribute to the problem, what does “gradually” mean? Do stretching exercises help relieve the entrapment? -- J.C.B.

*** DEAR DR. ROACH: Can you please give me some information on Elhers-Danlos syndrome? My daughter and two granddaughters have it. -- E.E.H. ANSWER: Ehlers-Danlos syndrome is actually a group of uncommon genetic disorders, with very stretchy skin, flexible joints and tissue fragility as common features. There are six different kinds of Ehlers-Danlos, and each is different in regard to what tissues and organs it most affects. Most are caused by mutations in genes for collagen and related structural proteins. They can be inherited or not (in which case they are new mutations). Flexible joints (often but wrongly called “double-jointed”) and excessively stretchy skin are almost universal in EDS. The excess joint movement can cause dislocations and chronic joint pain resembling fibromyalgia. Other commonly associated conditions include prolapse of the mitral valve, hernias and, in the vascular type of EDS, aneurisms. The prognosis in EDS depends to a great extent on the subtype. ANSWER: What you are describing is meralgia paresthetica, the compression of the lateral femoral cutaneous nerve, which can be compressed as it passes under the inguinal ligament, exactly as you described. Your orthopedist is correct that this condition is more common in the overweight or obese; however, I have seen it in both people who are losing weight and people of normal, stable weight. It is more common in conjunction with diabetes, and it has been reported after long-distance walking and cycling. Because of the area involved in your numbness, it is very likely to be meralgia paresthetica and not spinal in origin. It does usually resolve, but it does so over the course of months, typically. Stretching would seem to make sense, but as far as I know, it hasn’t been shown to work. If it isn’t getting better, an injection into the nerve usually is effective. This is commonly done by an anesthesiologist or a painmanagement specialist.

Make a household favorite more healthy Many beloved dishes might be enjoyed more often if they were only a little healthier. Author and former personal chef Michelle Dudash was once asked by a client if the she could make a healthier version of chicken parmesan. The result of that request is the following recipe for “Skinny Chicken Parmesan with Spinach,” which Dudash ultimately included in her book, “Clean Eating For Busy Families” (Fair Winds). The recipe was a hit with Dudash’s client and will surely be a hit at home cooks’ dinner tables as well.

Skinny Chicken Parmesan with Spinach Makes 6 servings For the sauce: 2 teaspoons extra-virgin olive oil 2 cloves garlic, sliced 1 28-ounce can whole tomatoes 1/4 teaspoon dried basil 1/4 teaspoon dried oregano 2 pinches salt 2 pinches freshly ground black pepper 1/2 teaspoon honey For the chicken: 3 tablespoons grated Parmesan cheese 2 tablespoons whole-wheat flour 1/2 teaspoon salt, divided 1/4 teaspoon freshly ground black pepper 1/4 teaspoon dried basil 11/2 pounds chicken breast, cut into 6 pieces, pounded to 1/4 inch 2 tablespoons plus 2 teaspoons extra-virgin olive oil, divided 4 cups baby spinach 1 teaspoon lemon juice 3/4 cup fresh mozzarella cheese, sliced into 6 medallions (or shredded) 6 cups cooked whole-grain thin spaghetti tossed in 2 teaspoons olive oil Preheat oven to 375 F and coat the bottom of a 9 x 13-inch pan with olive oil spray. To make the sauce: Heat a medium saucepan over medium heat. Add oil and garlic and cook gently for 30 seconds. Do not brown. With your hand, squeeze each tomato into the pan and add basil, oregano, salt, pepper, and honey. Reduce to medium heat and simmer for 20 minutes, lowering the heat as needed. To make the chicken: Combine parmesan cheese, flour, salt, pepper, and basil in a medium dish and coach chicken with cheese mixture. Place a large skillet over medium heat and add 2 tablespoons of oil. When oil is shimmering, add chicken, rounded-side down, and cook until golden on one side, about 5 minutes. Arrange chicken in a baking pan. Pour remaining 2 teaspoons of oil into skillet and add spinach. Cook spinach for 1 to 2 minutes until wilted and swirl in lemon juice. Gently press spinach to release water and divide spinach on top of chicken. Spoon sauce around and over the chicken, place mozzarella on top, and sprinkle with 1/4 teaspoon salt. Bake for 15 minutes until chicken is cooked through. Serve over spaghetti.

DID YOU KNOW? Many people are familiar with the term “ozone layer.” But some may not know that the Earth’s atmosphere consists of several layers. The lowest layer of the Earth’s atmosphere is the troposphere, which the U.S Environmental Protection Agency notes extends from the Earth’s surface to roughly six miles in altitude. The troposphere is where nearly all human activities occur. Even climbers who make it to the peak of Mt. Everest are still in the troposphere, as Everest is less than six miles high. But anyone who has ever boarded a commercial flight has likely left the troposphere and entered the stratosphere, where the majority of commercial airplanes fly. The stratosphere begins at six miles above the Earth’s surface and extends to about 31 miles above the surface of the planet. The EPA notes that most atmospheric ozone is concentrated in a layer in the stratosphere that’s roughly nine to 18 miles above the Earth’s surface. The ozone layer in the Earth’s stratosphere absorbs some of the sun’s radiation, including the UV and UVB radiation that has been linked to cancer. The third layer of the Earth’s atmosphere is the mesosphere, which extends from 31 miles above the Earth’s surface to roughly 53 miles above the surface of the planet. The height of the next highest layer, the thermosphere, varies due to changes in solar activity. The exosphere, which contains many of the satellites that orbit the Earth, is the outermost layer of the Earth’s atmosphere.

Research suggests that a combination of nutrition and mental, social and physical activities may have a greater impact with regard to maintaining and improving brain health than any single activity. Harvard Medical School also states that volunteering, caring for others and pursuing hobbies may benefit the brains of older adults.

REQUIRED MINIMUM QUALIFICATIONS: • Bachelor’s Degree in Early Childhood Education with NYS certification in birth-grade 2 • One-year experience in preschool/day care setting • One-year experience in supervising staff

EDUCATION SPECIALIST MAJOR RESPONSIBILITIES 1. Assists in oversight, planning, coordination, implementation and evaluation of Early Childhood Education services, in accordance with Head Start Program Performance Standards and NYSOCFS Day Care regulations for Head Start (birth-5) program. 2. Provides education staff training and coaching in areas of responsibility; monitors records; ensures curriculum implementation, ongoing child assessment, weekly classroom planning and parent engagement across program. 3. Works directly with leadership, staff, children and families. 4. Typically spends majority of time at various sites supporting classrooms. 5. Reports to Education, Disability and Family Services Manager. Coordinates and communicates with all Head Start staff.
