Does Hole in Heart Increase Stroke Risk? --DEAR DR. ROACH: Articles on strokes or TIAs rarely mention that they can be caused by a congenital heart defect. I had a family member with a patent foramen ovale, found at age 54, after he’d had several strokes. He had surgery to repair it and has been well ever since. I would like to know why a PFO isn’t often mentioned or considered. When I see an obituary for a young person who died suddenly, I wonder if there was an undiagnosed hole in the person’s heart. One in 6 people has a hole in the heart; that is a high percentage. I think the public should be educated and made aware of this congenital condition. -- J.R. ANSWER: A patent foramen ovale (which literally means “open oval-shaped window”) is a remnant of our embryology. The foramen ovale is a small open flap that is necessary to send oxygenated blood from the placenta to the body of the developing fetus. In about 25 percent of people (that’s 1 in 4, even higher than you thought), the “hole” doesn’t completely close. PFO is the most common of the “holes” in the heart (ventricular septal defect and atrial septal defects are the other common ones). A stroke is caused by the death of brain cells. PFOs are certainly implicated in strokes. In general, the younger and healthier the person, the more likely it is that a “cryptogenic” stroke (one with no obvious cause) may be due to a PFO. What is likely is that a blood clot can pass through the foramen ovale and go to the brain’s blood vessel, blocking off blood supply to an area of the brain, causing a stroke. The absolute increase in stroke risk is hard to quantify. For people who have never had a stroke, it is generally not recommended to close the PFO. This surgery has risks, and these risks probably outweigh the small potential benefit. For people who have had a stroke, the risk of recurrent stroke from PFO is higher. One group has created a model (the RoPE score) to help predict the likelihood of recurrent stroke. This can help the clinician examine the benefits of surgical repair of the PFO. Surgery is most likely to benefit younger people without traditional risk factors for stroke. *** DEAR DR. ROACH: I heard that letting vinegar touch your skin causes arthritis. Is there any danger in using vinegar as a cleaning agent? -- N.J.A. ANSWER: I often get asked whether apple cider vinegar cures arthritis (it doesn’t) or can help relieve symptoms (it might, but there is no good evidence that it does), but I have never heard anyone ask about vinegar touching the skin causing internal problems. Vinegar is a mild acid (most are about 5 percent acetic acid), which normally isn’t particularly irritating to the skin, and shouldn’t be absorbed. Even if it were, acetic acid is found in the body (it’s an important molecule in several metabolic pathways), and your body can use it as an energy source. Thanks to all those who came to my husband’s rescue when he fell October 31 in the driveway at the Livingston Skilled Nursing Facility. What a bunch of caring and helpful people. Thanks again to all. Have a blessed Thanksgiving and Christmas. My daughter and I were walking on the Trolley Trail on Saturday, November 10. It was very cold and very windy, and our orange, white, and blue BB8 Blanket fell out of the stroller. We didn't notice until we got home, and it was too cold to take her out again. The next morning, we went out looking
There are certain stores in my area that always have only one (or on rare occasions, two) cash registers open. It drives me absolutely NUTS that people will be in line with at least fifty items and will very clearly see me or someone else behind them in line with only one item but not offer to let us ahead of them. Please consider this the next time you are shopping and the person in line behind you has only an item or two. I promise, it takes mere seconds to for us to get through the line.

The people who used to go and get food were people who needed that extra help once a month. ONCE A MONTH! And the [local dept.] will not even allow this. I thought that it was the town's taxpayers who paid for the parking lot, so shouldn't they be able to use it once a month? [Local dept.] ought to be ashamed of themselves. [Local Town] a Few Hints; 1) The stick on the left-hand side of the steering wheel is called a turn signal. USE IT! 2) Left Lane is for Passing, NOT TRAVELING even if you are

Our lunch runs from Noon to 3 pm. Our ladies were all just humbled and grateful for the generosity of this gift that will keep on giving! They said sorry we were swamped this morning (they close between 12-1pm for lunch). When I finally was brought to the examination room, I was told the doctor would be in shortly. 35 minutes later, my doctor came into the room and said, “good news, your blood work came back great!" Once she took the time from my 5-minute visit to glance at her computer, she noticed my thyroid was off the charts. She thought her mistake was funny! I did not! Needless to say, I’m in the process of finding a new doctor.

On Sunday, December 2nd at 5pm, the 6th Annual Holiday of Lights Parade will begin just north of the canal near the hospital and continue down Main Street in Brockport. This thoroughly enjoyable, family-friendly, holiday event will once again be sponsored by the Brockport Police Department’s Stetson Club. “Each year, we have had a really good turn out,” said Stetson Club President, Joshua Sime. “Several families and friends come out to see the parade.” This year is planning to follow proudly in this holiday tradition with many gorgeous floats and several visitors. As in years past, the parade will be hosted by MCs. This year’s MCs will be Caurie Putnam of the Democrat & Chronicle and The Mayor Pete Kennedy of the Mix 100.5. Everything participating in the parade will be covered in lights, from firetrucks to ambulances to snow plows to floats. Many of the floats will be sponsored and created by local businesses who go above and beyond in their attempt to create the most elaborate and festive float in the parade. Not only do local businesses compete with their floats because it is good fun, but they also compete because the floats are in competition and are judged at the end of the parade with prizes for the winners. It’s not just the floats and vehicles that are decorated, however. People and bands participating in the parade will be decked out in holiday finery, as well.
The Holiday of Lights Parade has grown to include anywhere from 25 to 40 floats, and the Stetson Club, the local businesses and the community put a lot of work into getting the event together. As the last floats come into view, Santa and Mrs. Claus can be seen riding along in the parade, waving to visitors and greeting the children. The parade concludes in front of the Brockport Diner for the Tree Lighting Ceremony. Village shops, restaurants and other businesses offer warm and inviting spaces for parade goers to visit and explore, as well, which adds greatly to the feel of an entire community coming together. At the end of the parade, as the tree is being lit, a spectacular fireworks display will commence, courtesy of Skylighters. This parade in the heart of Brockport certainly celebrates the season with color, light, sound, flash and spectacle. Come on out and join the holiday fun!
