TO YOUR GOOD HEALTH By Keith Roach, M.D.
Should Women Still Take Estrogen? --DEAR DR. ROACH: I have a question concerning estradiol. My gynecologist has me taking 1 milligram of estradiol daily. My internist recommends that I not take it. Which recommendation should I follow? I have never had hot flashes, so that is not a problem. My internist says that I need to gradually stop taking the estradiol. He says that this medicine has more problems than benefits. It is hard to choose what to do. What do you recommend? -- H.L. ANSWER: Back in the 1990s, we often recommended that women take estrogen in order to prevent problems, in addition to treating hot flashes. Two important studies came out showing that estrogen appeared to cause problems as well as prevent them. The definitive answer as to whether estrogen causes more problems than it prevents still isn’t known, and it probably depends on many factors. The term “estrogen” as I am using it refers to a group of related compounds. Estradiol is sometimes referred to as “bioidentical,” since it is the same hormone that a woman’s body makes, as opposed to conjugated equine estrogen (Premarin), which was most commonly used in the 1990s and earlier. The type of estrogen may make a difference in terms of risk. Also, estrogen is seldom given without a progesterone compound in women who have not had their uterus removed, and the type of progesterone appears to make a major difference in risk. Briefly, the evidence suggests that estrogen reduces the risk of colon cancer and of osteoporosis and fractures. However, it increases the risk of blood clots. The largest areas of confusion are in breast cancer and heart disease. For conjugated estrogens with medroxyprogesterone, which has the best information, for 1,000 women in 10 years there were seven more heart attacks, eight more strokes, eight more blood clots in the lungs and eight more invasive breast cancers, but six fewer colon cancers and five fewer hip fractures. Recent data suggest that women who start estrogen immediately at menopause have a lower risk of heart disease than those who start five or 10 years after menopause (in fact, they may have an overall lower risk, compared with women who don’t take estrogen). Women who took estrogen alone did not have an increased risk of breast cancer. Because of the known harms of estrogen, at this time neither I nor any expert body recommends taking estrogen just to prevent problems. Until new studies become available, since you have no hot flashes, I agree with your internist. *** DEAR DR. ROACH: I wanted to tell you of the unbelievable relief I’ve gotten from the small, very painful cracks that used to develop at the ends of my fingers next to the nails, usually in the winter. Since I started taking 1,000 mg of fish oil once a day, I have had only one crack. -- R.L.S. ANSWER: I couldn’t find any studies on this, but anecdotal reports say fish oil may be helpful and is unlikely to harm. It’s reasonable to give it a try. I would recommend a supplement that contains both EPA and DHA. The Costco brand was found to be of high quality. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. HEALTH BENEFITS OF AVOCADOS Avocados can benefit vision. Avocados contain lutein and zeaxanthin, a pair phytochemicals concentrated in the tissues in the eyes. Lutein and zeaxanthin are believed to block blue light from reaching structures in the retina, thereby reducing a person’s risk of developing macular degeneration. In fact, studies published in the American Journal of Epidemiology, the American Journal of Ophthalmology and The Archives of Ophthalmology found that diets high in lutein and zeaxanthin are associated with a lower risk of macular degeneration, which the American Macular Degeneration Foundation notes is the leading cause of vision loss in the United States.

ECO-FRIENDLY WAYS TO CELEBRATE VALENTINE’S DAY Valentine’s Day is second only to the holiday season with regard to gift giving. Although this day devoted to love and exchanging gifts can be wasteful, that does not mean those with a mind toward green and simple living should ignore the holiday altogether. Valentine’s Day celebrations can be enjoyed in an eco-friendly way that can make this special day even more meaningful to eco-conscious couples.

D id you k now ? Fireflies are often seen lighting up the night sky on w arm evenings. They are particularly abundant during the summer months. One reason that fireflies glow is to attract mates. M ales w ill flash, and females w il l respond to mates they find interesting. Fireflies also may glow to avoid predators. The luciferase enzyme in fi refly cells causes a chemical reaction that stimulates light, a phenomenon called bioluminescence. Fireflies are filled w ith lucibufagins, a poor-tasting chemical that turns off predators from attacking the blinking bug. Some fireflies cannot light up at all and use pheromones to attract mates. Fireflies can be fun to catch and releas e and also beneficial to have around. In the larval stage, fireflies w ill eat destructive insects. Adult firefl ies may feed on nectar and could help to pollinate plants. Despite their name, fireflies aren’t really flies. They a ctually are a type of beetle. Fireflies are difficult to spot during the day because they’re often resting on leaves or plants. It’s only at night w hen their brilliant light show comes alive.

VALENTINES TRADITIONS FROM AROUND THE WORLD In South Korea, men get to enjoy the spotlight on Valentine’s Day, as women bestow gifts of chocolate on them. In return, a month later men reciprocate with gifts for women on White Day. South Koreans take Valentine’s Day a step further on Black Day, which falls on April 14. This is an opportunity for all single people who may not have received Valentine’s Day gifts to gather at restaurants and eat a dish called “black noodles” as they celebrate their singleton status.
Keep Your Eyes Open for Antlers! Provided by the New York State Department of Environmental Conservation Every year, starting in late December and continuing into March, male whitetailed deer shed their antlers as their testosterone levels drop in response to the lengthening days. By late winter and early spring, eager shed hunters head out into the woods and fields to look for these hidden treasures. Finding a single shed is a thrill, but finding a matching set, or those of a purely impressive adult buck, is worth more than their weight in gold to many. For the inexperienced, it might seem like looking for a needle in a haystack, but for the avid shed hunter with a trained eye it’s worth the effort. And, if your dog enjoys accompanying you in the woods, you can learn about shed hunting with a dog in the February 2017 issue of the Conservationist, a New York State Department of Conservation publication. Winter deer yards are good places to start. During bouts of extreme cold and snow, deer will congregate in lower elevation areas with good southern exposure and evergreen cover to break the wind. Experienced shed hunters scout
these areas pre-season in hopes of increasing their odds. Shed hunting is a great way to get outside as the weather breaks in late winter/early spring. It can also provide the experienced hunters with clues as to what kinds of bucks will be around in the fall during the next hunting season. Remember, “Let Young Bucks Go and Watch Them Grow” to see bucks with bigger, more impressive antlers -- a treasure worth looking for.

Assistive devices help people remain mobile Disabilities affect people from all walks of life. The Centers for Disease Control and Prevention estimates that around 55 million Americans have a disability of some kind. Of these people, 33 million have a disability that makes it difficult for them to carry out some daily activities. Statistics Canada states that, as of 2012, 13.7 percent of the population age 15 years or older reported having a disability that could impact daily life. People with disabilities may need assistive devices to reclaim some measure of their independence. For example, millions of people rely on wheelchairs or walking aids to get around. These are called assistive technology and rehabilitative devices, which include tools, equipment or products that can help people with disabilities get around more easily. These devices may be as small as magnifying glasses for reading to as large as wheelchairs. The National Institutes of Health state that 2.2 million people in the U.S depend on wheelchairs for day-to-day tasks and mobility. More than six million use canes, walkers or crutches to assist with mobility. The following are some of the common types of mobility devices available for purchase. • Canes: Canes are handheld devices that provide a little extra stability or support to weakened limbs. Canes are easily transported and can assist those who need only a little help. • Standard walkers: Also known as medical walkers, standard walkers offer substantial support and must be lifted to move. That requires upper body strength, which can be difficult for some to maintain on long trips. • Rolling walkers: Rolling walkers are similar to standard walkers. But rolling walkers have wheels, either on the front only or on both sets of legs. They are somewhat less stable than standard walkers. Rolling walkers with wheel-locking devices may offer stability when needed. Some rolling walkers are called “rollators.” • Wheelchairs: Standard wheelchairs must be powered by the
Rolling walkers are assistive devices that can keep individuals mobile despite cognitive or physical disabilities.
person in the chair or by someone pushing it from behind. This can be difficult for people with minimal upper body strength. Mechanical wheelchairs are powered with a lever or button. Some people may opt for motorized scooters over bulkier wheelchairs when possible. Assistive devices can be highly effective, but only when they are used properly and safely. Assistive devices should be measured for each user. Many walkers are height-adjustable. The width of a device also is important. A walker or another device that is too wide can affect mobility or require modifications to be made around the home. Weight is another consideration. The device should be lightweight so it can be maneuvered, but also weigh enough to provide enough stability to keep users upright. Users should proceed slowly and favor their stronger sides when relying on canes or walkers. Devices should not be used to climb atop other items like step stools. Children should not be allowed to play with or ride on assistive devices. If there are safety belts or locks on any assistive technology, they should be put in place before use to prevent further injury.
SENIORS AND EXERCISE: TIPS TO AVOID INJURIES, GET HEALTHY Pick a partner. Whether it’s a spouse or a friend who is physically active or wants to be, try exercising with a partner, at least initially. Doing so can provide the motivation you need and partners can serve as safety nets should you need assistance completing an exercise or suffer an injury and require medical attention. Personal trainers can serve as your partner, and many gyms offer discounts to seniors on personal training services.

EMPLOYMENT: THE NEXT STEP AFTER GRADUATION Graduation is an exciting time in the lives of students. After years in the classroom preparing for life after school, graduation marks a time when students are finally ready to enter the “real world” and land their first professional job. The U.S. Bureau of Labor Statistics advises that earning a college degree can greatly improve a person’s chance of landing a job. The more education a person receives, the lower his or her prospects of being unemployed become. The BLS said that, as of 2014, individuals with a bachelor’s degree could earn on average $1,101 per week, compared to $668 for persons with high school diplomas. Those with bachelor’s degrees had a 3.5 percent unemployment rate compared to 6 percent for those with only high school diplomas. The National Center for Education Statistics estimates that roughly two million students earn bachelor’s degrees each year. Many others will go on to earn master’s or doctorates before entering the workforce. As the economy continues to improve, job prospects follow suit. According to a job outlook from the National Association of Colleges and Employers, employers had plans to hire 8.3 percent more new college graduates in 2015 than in 2014. The growth of businesses and the rising rate of retiring Baby Boomers has spurred employment prospects. Landing a job post-graduation requires diligence on the part of new grads.

Eco- Friendly Ways To Alter Your Professional Life Forget you have a printer. Now adays, few er and few er people rely on printed documents to get through the w orkday. Advances w ith regard to computer technology has made it just as easy to read documents on a computer screen as it is to do so on a sheet of paper. If you’re among the last to embrace this grow ing trend, think of the environment before you print your next document and opt to read documents on your desktop rather than printing them out. Instead of leaving notes for cow orkers, send them e-mails instead. This saves paper, w hich in turn w ill save your company money, and it also reduces your reliance on ink that can potentially harm the environment. W hen making presentations, do so using slides rather than printing materials and handing them out in a packet. E-mail cow orkers the presentation in advance and print out a single copy for the meeting rather than printing multiple copies. If you and your cow orkers simply must print documents, try to use tw osided printing and copying to reduce your paper usage. And urge your office manager to order supplies made from recycled materials and discard of any potentially harmful products, including ink cartridges, in an eco-friendly w ay.

HOW TO MAKE YOUR OFFICE MORE ECO-FRIENDLY Cut back on printing. Printing documents used to be the most effective way to share them with coworkers and clients. But nowadays printing is one of the least efficient and least eco-friendly ways to share documents. Instead of printing documents to show coworkers, create PDFs and email the PDFs instead. And rather than mailing contracts to prospective clients, email contracts that accept e-signatures, ultimately storing the contracts on your file server rather than in a dusty old filing cabinet.
