Common Pain Reliever Can Have Side Effects --DEAR DR. ROACH: I am a 44-year-old man with chronic headaches and migraines. Should I worry about the long-term effects of taking an NSAID (such as diclofenac) at least once a week? -- G.W. ANSWER: Nonsteroidal anti-inflammatory drugs such as aspirin, ibuprofen and naproxen are among the most commonly taken classes of drugs in the world, with 17 million Americans alone taking them daily. Although these drugs are generally safe, any medication has the potential for side effects. Typically, the higher the dose and the more frequently it is taken, the increased risk of side effects. So, a once-aweek dose would be expected to have a low side effect risk. Adverse effects from this class are among the most frequently seen in clinical medicine. Although they can affect multiple systems, let me briefly go over the side effects of NSAIDs on the GI tract, kidneys and heart. NSAIDs might affect the stomach, and although we think of ulcers, stomach pain without ulcers is more common. Ulcers happen most often in people who take higher doses for long periods of time. A history of ulcers, older age and use of other medications (steroids, warfarin, antiplatelet drugs like clopidogrel, or Plavix) increase the risk of ulcers and bleeding. NSAIDs also might affect the small bowel and colon, causing bleeding and diarrhea, among other symptoms. Again, these are most common in high doses for extended periods. NSAIDs can affect the kidneys, causing both acute and chronic kidney failure, but only in a few percent of people. Periodic blood and urine examination is appropriate for people who are on long-term NSAIDs. The risk of heart disease among people who use NSAIDs regularly is increased, but in people without known heart disease, the risk is small -- about one bad event, such as heart attack, stroke or episode of heart failure, per thousand people taking the medication for a year. I would advise you that the risk of any of these side effects is unlikely in someone taking the drug only once a week, but people who take these drugs every day should know about the potential harms. *** DEAR DR. ROACH: When I was 64, a cyst was found in my bladder. When the cyst was removed, it was found to be cancerous. My urologist wants me to continue to have an annual cystoscopy, which I do. I’m now 76 and in good health. Do you feel the annual cystoscopy is still necessary? -- C.K. ANSWER: Now that more people have had successful treatment of cancer, we are starting to learn about the risks they face in subsequent years. The risk of another cancer is generally higher, but it’s the exact form of cancer that determines how much risk there is, and consequently, how aggressive follow-up must be. In the case of bladder cancer, I have read some studies that the risk is quite small after five years. However, the American Urologic Association has clear guidelines: Because of the increased risk for recurrence, it recommends annual cystoscopy indefinitely. Cystoscopy is a pretty safe procedure, so I think your urologist’s recommendation is reasonable. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Addicted seniors a serious problem: Opioid use among the older population The public is quickly learning that drug abuse goes beyond the illegal substances that are purchased on the street. Abuse of drugs extends to the prescription medications sitting in many medicine cabinets. While teenagers and young adults may be the first to be stereotyped as prescription drug abusers, seniors may have unwittingly become mixed up in one of the most misused prescription classes: opioid pain relievers. The problem of opioid abuse has been a growing issue for years. The National Institute on Drug Abuse estimates 2.1 million people in the United States suffer from substance abuse of opioid pain relievers. Older patients are increasingly and repeatedly prescribed opioids to address chronic pain from arthritis, cancer and other problems that become more apparent as people age. Data from U.S. Medicare recipients found that, in 2011, roughly 15 percent of seniors were prescribed an opioid after being discharged from the hospital. When followed up on three months later, 42 percent were still taking the medication. Fast forward to 2015, and almost one-third of all Medicare patients were prescribed opioid painkillers by their physicians, says AARP. The Canadian Institute for Health Information says adults between the ages of 45 and 64 and seniors age 65 and older had the highest rates of hospitalizations due to opioid poisoning over the past 10 years. AARP also indicates nearly three million Americans age 50 or older have started to take painkillers for reasons beyond what their doctors prescribed. Experts from the Physicians for Responsible Opioid Prescribing warn that dependence on opioids can set in after just a few days. Discomfort and side effects can occur when the pills are stopped. Opioids can decrease pain at first, but many people find they can be less effective over time. As a result, patients need to take greater amounts. Although many people can take opioids in small doses for short periods of time without problems, many find

themselves overcome by a troubling addiction. Some doctors prefer not to use opioids as a first line of treatment for chronic pain. Another possible risk of opioids among seniors is that the medication can cause disorientation that may lead to falls and fractures. The senior care resource A Place for Mom also says that prescription narcotics may increase risk of respiratory arrest. What’s more, an older body may not absorb and filter medicines as effectively as younger bodies can. This means that older adults might become addicted to or have side effects from a prescription drug at a lower dose. Seniors concerned about opioids can discuss other options with their doctors, such as nonopioid medications and alternative therapies for pain management, like massage or acupuncture. If opioids are prescribed, ask for the lowest dose and don’t exceed the time frame for taking the medicine. Only take the pills when absolutely necessary, and never mix opioids with alcohol or other substances.

By Amanda Dudek When one first enters this attractive yet unassuming storefront in Fairport Village Landing, one may be tempted to think, “They must have everything!” Truly, regardless of which tool or part one is searching for, they very well may! Tools, parts, accessories, implements, nuts, bolts, fasteners, hardware and all manner of toolrelated items are arranged along shelves, on floor space, hanging from walls and categorized in drawers, filling every crevice of available space, with additional items being cleaned, sorted and priced in a back room and donations being given throughout the day. Items are sorted together and grouped under Hardware, Painting/Wallpapering, Automotive, Miscellaneous, Plumbing, Electrical, Vintage, Handheld Power Tools, Hand Tools and Lawn & Garden. Several friendly volunteers cheerfully assist customers and process and organize the donations which are to be placed for sale. Still, some may ask why a tool thrift shop, and how does it help the elderly? Tool Thrift Shop opened its doors in 2010 as a means of raising funds for Senior Options For Independence (SOFI). SOFI is a partnership with the Fairport/Perinton Senior Living Council and Fairport Baptist Homes, and it assists seniors living in Fairport and Perinton, in a wide variety of ways. SOFI advises seniors regarding any questions, concerns or needs they may have, and it also advises them regarding the support services which are available to them. SOFI also provides transportation, in-home assessments, consultations, safety checks, support groups, educational programs and social outings. It was SOFI that asked founding volunteer Jon Gehret to direct the development of their idea, a tool thrift shop, to help keep SOFI funded, especially as other funding for seniors was on the decline. From its grand opening in 2010 to June 2018, Tool Thrift Shop has raised $405,000 for area seniors through SOFI. Amazingly, they have done so while maintaining incredibly low prices. Donated items are priced anywhere from 1/3 to Ω of retail cost, with hardware often priced considerably below even that threshold. For instance, in their Lawn & Garden section, yard rakes were priced at $5, and garden tools such as trowels were $1. The shop is completely run by volunteers, and the atmosphere is uplifting, the pace unhurried. Volunteers take the time to greet customers, often by name, and chat with them, helping them search

for items of interest. Volunteers range in ages from high schoolers to young adults to elderly and very elderly seniors, all working in harmony together, and it is this harmony in the shop, Jon Gehret says, which is the only requirement for being a volunteer. Volunteers, except for cashiers and managers, do not have set hours. Some work a few hours a week, while others work 40 hours a week or more as if at a full-time job, based on individual preference and availability. Volunteers are needed and always welcome. Their work in the Tool Thrift Shop not only raises money for SOFI, but it fulfills a people’s need to give to others, as well. Many of those who currently volunteer find fulfillment in contributing to the seniors in their community and working together to do something beautiful. Certainly, donations are welcome, and those who donate are given receipts for tax purposes. Those wishing to donate may drop off items or call for those items to be picked up. It is this variety of donations that makes visiting the shop so exciting. Long-time volunteer, Dee Wickham, observed that it’s, “just like Christmastime every time a box comes in!” Jeanne Gehret, wife of Jon Gehret and shop cofounder, mentioned that many visitors are excited to not infrequently find American-made tools, prized for the quality of their steel. In fact, it is this double prize the Tool Thrift Shop offers patrons: the opportunity to care for seniors in our communities while finding great deals at the same time.
