Should Women Still Take Estrogen? --DEAR DR. ROACH: I have a question concerning estradiol. My gynecologist has me taking 1 milligram of estradiol daily. My internist recommends that I not take it. Which recommendation should I follow? I have never had hot flashes, so that is not a problem. My internist says that I need to gradually stop taking the estradiol. He says that this medicine has more problems than benefits. It is hard to choose what to do. What do you recommend? -- H.L. ANSWER: Back in the 1990s, we often recommended that women take estrogen in order to prevent problems, in addition to treating hot flashes. Two important studies came out showing that estrogen appeared to cause problems as well as prevent them. The definitive answer as to whether estrogen causes more problems than it prevents still isn’t known, and it probably depends on many factors. The term “estrogen” as I am using it refers to a group of related compounds. Estradiol is sometimes referred to as “bioidentical,” since it is the same hormone that a woman’s body makes, as opposed to conjugated equine estrogen (Premarin), which was most commonly used in the 1990s and earlier. The type of estrogen may make a difference in terms of risk. Also, estrogen is seldom given without a progesterone compound in women who have not had their uterus removed, and the type of progesterone appears to make a major difference in risk. Briefly, the evidence suggests that estrogen reduces the risk of colon cancer and of osteoporosis and fractures. However, it increases the risk of blood clots. The largest areas of confusion are in breast cancer and heart disease. For conjugated estrogens with medroxyprogesterone, which has the best information, for 1,000 women in 10 years there were seven more heart attacks, eight more strokes, eight more blood clots in the lungs and eight more invasive breast cancers, but six fewer colon cancers and five fewer hip fractures. Recent data suggest that women who start estrogen immediately at menopause have a lower risk of heart disease than those who start five or 10 years after menopause (in fact, they may have an overall lower risk, compared with women who don’t take estrogen). Women who took estrogen alone did not have an increased risk of breast cancer. Because of the known harms of estrogen, at this time neither I nor any expert body recommends taking estrogen just to prevent problems. Until new studies become available, since you have no hot flashes, I agree with your internist. *** DEAR DR. ROACH: I wanted to tell you of the unbelievable relief I’ve gotten from the small, very painful cracks that used to develop at the ends of my fingers next to the nails, usually in the winter. Since I started taking 1,000 mg of fish oil once a day, I have had only one crack. -- R.L.S. ANSWER: I couldn’t find any studies on this, but anecdotal reports say fish oil may be helpful and is unlikely to harm. It’s reasonable to give it a try. I would recommend a supplement that contains both EPA and DHA. Finding a single shed is a thrill, but finding a matching set, or those of a purely impressive adult buck, is worth more than their weight in gold to many. For the inexperienced, it might seem like looking for a needle in a haystack, but for the avid shed hunter with a trained eye it’s worth the effort. And, if your dog enjoys accompanying you in the woods, you can learn about shed hunting with a dog in the February 2017 issue of the Conservationist, a New York State Department of Conservation publication. Winter deer yards are good places to start. During bouts of extreme cold and snow, deer will congregate in lower elevation areas with good southern exposure and evergreen cover to break the wind. Experienced shed hunters scout
these areas pre-season in hopes of increasing their odds. Shed hunting is a great way to get outside as the weather breaks in late winter/early spring. It can also provide the experienced hunters with clues as to what kinds of bucks will be around in the fall during the next hunting season. Remember, “Let Young Bucks Go and Watch Them Grow” to see bucks with bigger, more impressive antlers -- a treasure worth looking for.

Using reclaimed timber when building or renovating a home benefits the environment in various ways. According to GreenBusinessWatch. org, building and renovating with reclaimed timber helps to preserve forests by greatly reducing the need to cut down trees. In addition, when using reclaimed timber to build or renovate a home, contractors and homeowners tend to use locally sourced reclaimed wood, reducing the need to transport wood from afar to complete the projects. That reduced reliance on transportation reduces fuel consumption and air pollution. Processing reclaimed timber is often less taxing on the environment than processing virgin wood. However, reclaimed timber may provide more than just environmental benefits. Virgin timber typically comes from commercially grown trees that are not always afforded enough time to reach full maturity. Timber that is reclaimed from old buildings and vessels may very well have reached full maturity before it was cut down to use as building material. Reclaimed timber that grew to full maturity is likely stronger than virgin wood taken from commercially grown trees that were not given enough time to mature. Reclaimed timber also may prove more durable than virgin wood because the former has already dried out after years of contracting and expanding, making it less likely to warp and split than virgin wood that has yet to endure such exposure.

Assistive devices help people remain mobile Disabilities affect people from all walks of life. The Centers for Disease Control and Prevention estimates that around 55 million Americans have a disability of some kind. Of these people, 33 million have a disability that makes it difficult for them to carry out some daily activities. Statistics Canada states that, as of 2012, 13.7 percent of the population age 15 years or older reported having a disability that could impact daily life. People with disabilities may need assistive devices to reclaim some measure of their independence. For example, millions of people rely on wheelchairs or walking aids to get around. These are called assistive technology and rehabilitative devices, which include tools, equipment or products that can help people with disabilities get around more easily. These devices may be as small as magnifying glasses for reading to as large as wheelchairs. The National Institutes of Health state that 2.2 million people in the U.S depend on wheelchairs for day-to-day tasks and mobility. More than six million use canes, walkers or crutches to assist with mobility. The following are some of the common types of mobility devices available for purchase. • Canes: Canes are handheld devices that provide a little extra stability or support to weakened limbs. Canes are easily transported and can assist those who need only a little help. • Standard walkers: Also known as medical walkers, standard walkers offer substantial support and must be lifted to move. That requires upper body strength, which can be difficult for some to maintain on long trips. • Rolling walkers: Rolling walkers are similar to standard walkers. But rolling walkers have wheels, either on the front only or on both sets of legs. They are somewhat less stable than standard walkers. Rolling walkers with wheel-locking devices may offer stability when needed. Some rolling walkers are called “rollators.” • Wheelchairs: Standard wheelchairs must be powered by the

person in the chair or by someone pushing it from behind. This can be difficult for people with minimal upper body strength. Mechanical wheelchairs are powered with a lever or button. Some people may opt for motorized scooters over bulkier wheelchairs when possible. Assistive devices can be highly effective, but only when they are used properly and safely. Assistive devices should be measured for each user. Many walkers are height-adjustable. The width of a device also is important. A walker or another device that is too wide can affect mobility or require modifications to be made around the home. Weight is another consideration. The device should be lightweight so it can be maneuvered, but also weigh enough to provide enough stability to keep users upright. Users should proceed slowly and favor their stronger sides when relying on canes or walkers. Devices should not be used to climb atop other items like step stools. Children should not be allowed to play with or ride on assistive devices. If there are safety belts or locks on any assistive technology, they should be put in place before use to prevent further injury.

LEARN HOW TO WRITE A CLASSIC LOVE LETTER Today’s digital world has given rise to fast, stinted communications. According to the Pew Research Center, texting is the most widely used smartphone app, with 97 percent of Americans using it at least once a day. Across the globe, 8.3 trillion text messages are sent each year. That translates to roughly 16 million messages per minute. In addition to text messaging, social media posts are generally concise and short on depth. The written form of communication has changed dramatically in the last decade, as letter-writing and other forms of prose have all but faded away. But come Valentine’s Day, it can be special to dust off that pen and paper to express oneself via the written word in the name of love. Love letters are cherished keepsakes that can be kept for years. Such couples can illustrate a couple’s present-day love for one another and shed insight on their relationship years later. Here are some steps for writing a romantic love letter or note. • Jot down ideas and inspiration. Make a list of attributes and qualities in a spouse or romantic interest that you would like to highlight. Write down phrases and terms that come to mind. These words can serve as the foundation for the letter. • Categorize your thoughts into three areas. Jot down things that you like and love about the person, things that the person may do for you or your family that you appreciate, and various memories from throughout the years. • Write what you know. Do not feel pressured to make this letter more complicated than it has to be. The words will come when you write from the heart. While you can use other letters to spur ideas, do not lose your voice or try to mimic someone else’s work. • Make the letter personal and specific. Small details are important and make love letters more meaningful. Such details illustrate to a significant other that this letter was specifically written with them in mind. • Disregard length. The letter does not have to be a certain length. Whether it is a paragraph or several pages, the letter will be a successs if it says what you want to express.
