Is There Such a Thing as a Weather Allergy? --DEAR DR. ROACH: Is it possible to be allergic to the cold weather and temperature changes? My son is 23 years old, and when he goes out in the cold, he breaks out in hives. What can he do? -- K.R. ANSWER: This sounds exactly like cold urticaria, which isn’t an allergy, but is similar in some ways. Doctors may test this by placing an ice cube (in a plastic bag with water) on the skin (usually the forearm) for five minutes, then watch the skin as it rewarms. If a hive develops (with raised skin and redness), that confirms the diagnosis. Cold urticaria can be associated with some infections (Lyme disease, hepatitis and HIV, among others) and with celiac disease. People with cold urticaria need to worry about systemic reactions. Avoiding cold (especially swimming in cold water) is critical, but even cold beverages can cause a serious systemic reaction, including swelling of the mouth and throat. Even anaphylaxis, a life-threatening collapse of the circulatory system, can develop. Many people with severe cold urticaria carry an epinephrine auto-injector in case of emergency. Antihistamines, such as loratadine (Claritin) and cetirizine (Zyrtec), are the best pharmacologic treatment. Experts in cold urticaria include allergists and dermatologists.

*** DEAR DR. ROACH: I saw a new physician for the first time two years ago. As he had no EKG machine, he sent me over to the local hospital for an EKG (just for his records, I wasn’t having problems). I had the EKG. The tech said that I was in atrial fib and needed to be admitted, as blood clots could form. I disagreed and went home. The doctor called me and persuaded me to go back to the hospital, which I did. I was preparing to be admitted when a cardiologist came in and said: “She doesn’t need admitting; she is not in atrial fib, it’s just PVCs.” I found out later that the EKG machine spit out the interpretation; no cardiologist had read it. I did complain, of course, and received an apology letter from the hospital. They also stated that they would change this procedure. Do hospitals actually rely on machines to do cardiology interpretations? And then go along with the machine’s interpretation without a cardiologist looking at the EKG? Please tell me it isn’t so. -- M.K. ANSWER: It isn’t so, or at least it shouldn’t be. There are several errors here. The first is that the EKG machine made the wrong interpretation. The machines are pretty good most of the time, but there are times when it is very difficult to tell and the machine is wrong. The second error was that the technician told you rather than getting the results confirmed by a human being who could correctly interpret them. I give the technician credit for trying to act in the patient’s interest, but you should be sure of your facts before telling the patient. It’s the third error – that of the doctor who called you – that upsets me the most, since the doctor was also wrong before calling you. I would be especially angry if the doctor had relied on the machine or the technician and hadn’t checked the EKG personally. It is good you wrote to complain: Hospitals rely on feedback to change the system. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Oliver’s Chocolate Heart Suckers Support Batavia’s Crossroads House By Amanda Dudek

F

or just over 20 years, Crossroads House of Batavia has tenderly nurtured those among us who are particularly vulnerable, who need special compassionate care. Crossroads House, a comfort care home which has served over 500 residents, houses individuals from Genesee County, as well as its contiguous counties, who have been medically determined to be in their last stages of life. This refers to those persons with three months or less to live. Crossroads House treats these incredibly fragile people with honor, respect and dignity. “Families whose loved one comes to us have often been caregivers for a long time, or sometimes their loved one received a sudden diagnosis, and they are in shock thinking What do we do?” explains Crossroads House Director of Development Peg Patterson-Case. “The daily care Crossroads House provides allows the family to be just the family again, the daughter or grandson or spouse can be just the daughter or grandson or spouse again.” Patterson-Case goes on to explain that caregivers often are so overwhelmed or focused Jeff brings a furry visitor to see Joan. on care that they are not able to simply interact and build the memories they want to. When their loved one moves into Crossroads House, they are able to focus on those things again. Supporting residents’ families in this endeavor is Crossroads House end-oflife doula program. Patterson-Case shares ways in which those at Crossroads House have helped residents do the things they have always wanted to do and things they particularly enjoy. “They scrapbooked a cookbook with a mother and daughter, which was something the mother had always wanted to do,” says Patterson-Case. “There was one woman who always loved homemade ice cream, so we held a homemade ice cream social for the woman and her family, and the family was able to come and simply enjoy it with their loved one. Families also have the added peace of mind that, when their loved one is actively dying, a doula sits at his or her bedside 24/7 to ensure that no one dies alone. Crossroads House allows families to enjoy every day they have left with their loved ones.” “Sometimes folks wait too long to call,” warns Patterson-Case. “We only have two beds available because if we had any more, we would be considered a medical facility.” She advises families to submit a referral as soon as possible because even if you are not ready when a bed becomes available, you can always decline and go back on the waiting list. “We get referrals from many different sources such as the discharge planners at hospitals, doctors, people at colleges, churches, anybody really. With permission, you could even call in for a friend,” PattersonCase adds. “Maybe your friend says her mother now needs 24-hour care. Even as the friend, you can make the first call, providing basic information. Within 24 hours, a referral caregiver will contact the person referred, go to see and evaluate them.” Crossroads House then determines if the person referred is a good fit for the level of care they provide. “We do not do IVs or feeding tubes,” Patterson-Case explains. It is important to determine if a comfort care house is the right choice for each person referred. “We focus on pain control,” says Patterson-Case. “Here, care is all about comfort, avoiding pain, nausea, things like that.” Peg PattersonCase goes on to explain that sometimes people haven’t yet understood that they are dying, and helping with that understanding is part of what Crossroads House does. Naturally, end-of-life care requires a lot of resources. “Our services are provided without charge to the residents and families; this doesn’t mean there isn’t a cost,” says Patterson-Case. “Crossroads House does not get state, federal or other government monies.” She goes on to explain that Crossroads House

is entirely funded through private means. Funds come from memorials, donations, fundraising, grants, trust & legacy giving as well as through building the endowment which was established in 2003. Crossroads House staff enjoy attending community gatherings, church services, civic and social events to give short presentations about the mission, recruit volunteers, share upcoming events and items needed on their wish list. PattersonCase explains that it takes 60 volunteers each week to fill the Crossroads House resident care schedule. Volunteers receive 20 hours of active, in-house training and are paired with a seasoned volunteer. She adds that “volunteers are asked to volunteer for one 4-hour shift a week. Some people volunteer two shifts a week; however, even if people do only two shifts a month, they are still considered active volunteers.” Crossroads House also fund raises 10.5 months out of the year, building both community awareness and connection. This includes outside fundraisers run by folks who come up with an idea on their own and hold their own fundraiser. Sometimes, they literally show up with a check; however, if Crossroads House is made aware of an upcoming fundraiser, they try to send someone to be there, show their appreciation and be a part of the event. “Nobody’s too young or too old to help,” says Patterson-Case, citing examples of bowling league contributions, redemption monies and individuals who donated stamps. “We saved over $1,200 in postage last year thanks to stamp donations.” This month, as they have for several years, Oliver’s Candies is partnering with Crossroads House as it holds its annual Oliver’s Chocolate Heart Sucker Sales. Each one costs only $1, with Crossroads House receiving 50 cents of each dollar. “We annually sell 7,000 suckers. They are sold all over,” Patterson-Case adds enthusiastically. “They’re in schools, breakrooms, Doctor’s offices, shop counters and at Crossroads House. Businesses have bought them for employees and customers, they are great gift idea for Valentine’s Day treats for your child’s classmates, a bouquet for the teacher, gift bags... who doesn’t love chocolate?” Oliver’s Chocolate Hearts Sucker Sale takes places every January and runs until Valentine’s Day. If there are any suckers left, Crossroads House sells them at their annual Betty Ellison Memorial Bowling Event, to be held this year on Feb. 15, 16 and 17th. Patterson-Case cautions people not to wait until the last minute for your suckers: “Last year, there were only about two dozen left at bowling and folks were wanting more!” She asks that people consider buying these suckers or supporting Crossroads House in another way so that it can continue to provide a peaceful, beautiful, end-of-life experience to those who come to them.
