Press into an ungreased 9 inch baking pan. Bake @ 375 for 15-18 minutes until browned. Spread jam over warm crust. In a small mixing bowl, beat cream, cheese, and milk until smooth. Ampullary Cancer Treatment Can Include Chemo Prior to Surgery ---
DEAR DR. ROACH: My husband was diagnosed with ampullary cancer. He has a tumor the size of a marble. We are told this is an aggressive cancer. My concern is that they are giving him chemotherapy before the Whipple surgery. Is this standard practice? -- P.T. ANSWER: The ampulla of Vater is an opening in the duodenum (the first segment of the small intestine), where the common bile duct releases bile and pancreatic enzymes to aid digestion. A cancer of the ampulla can originate from the pancreas, bile duct or duodenum, and these can all be aggressive cancers. The standard approach in people with the possibility to be cured is called a pan creaticoduodenectomy, otherwise known as the Whipple procedure. Although this is a complex surgery that used to have a high mortality rate, the risk of death from the procedure in specialized centers is now about 1%. Most people are familiar with getting chemotherapy after surgery to help remove any cancer cells that remain, a technique called “adjuvant therapy.” However, giving chemotherapy before surgery (“neoadjuvant chemotherapy”) has increasingly been used in people with ampullary cancer. The idea is to shrink the tumor to make it easier for the surgeon to remove completely. Chemotherapy may also kill any cancer cells that have already spread. The decision of when to use chemotherapy depends on the individual’s situation, so I can’t tell you which would be better in your husband’s case, as I lack both the details of his tumor and the expertise. I can confirm that giving chemotherapy prior to surgery is being used in regards to ampullary cancer.

THE DANGERS OF BELLY FAT What’s so bad about belly fat? Seeing belly fat as a merely cosmetic concern overlooks the effects it has on overall health. According to the Mayo Clinic, belly fat is not limited to the extra layer of padding just below the skin known as subcutaneous fat. Belly fat also includes visceral fat, which means it extends to deep inside the abdomen and surrounds internal organs. Johns Hopkins Medicine notes that visceral fat in the belly is very close to the liver, which can turn it into cholesterol. From there, the fat goes through the bloodstream and may collect along the walls of the arteries, potentially contributing to a hardening and narrowing of the arteries known as atherosclerosis. The complications of atherosclerosis depends on which arteries are affected, but they can include an assortment of diseases that affect the arteries and a greater risk for aneurysms and chronic kidney disease. Johns Hopkins Medicine notes that visceral fat is just as harmful for the heart as high blood pressure and smoking. So while efforts to reduce belly fat may initially be embraced to improve appearance, individuals should know that reducing fat in their midsections also can reduce their risk for a host of conditions, some of which are deadly.

Behind the Scenes: The Enchanting World of Shortsville’s Reindeer Farm BY SUZIE WELLS
I
n 2017, Mike and Lisa Schaertl started their reindeer herd with a couple of the animals as pets. Witnessing the community’s interest, they decided to open their farm to the public, establishing the Shortsville Reindeer Farm on Shortsville Rd as a seasonal destination with year-round outreach opportunities. Mike expresses, “The reactions of kids, their parents, and grandparents inspired us to do more to introduce these magical creatures to the public. Many people have returned year after year to see the reindeer during the holiday season.” The current herd consists of six reindeer, including four females referred to as cows, with hopes for future calves. Operating behind the scenes are dedicated volunteers and part-time employees, notably the “Sunday Crew” led by Shawn and Sandy Williams, along with Shawn’s sister Gina and friend Linda. This team plays a crucial role in caring for the reindeer, even taking them on road trips when Mike is unavailable. Additionally, about 15 hired staff members contribute significantly to the smooth operation of weekend events at the farm. Mike emphasizes that their cherished moments stem from observing the joy on children’s faces as they interact with the reindeer. The farm actively engages in events for the disabled, veterans at VA hospitals, and others, finding particular warmth in the reactions. An annual highlight for the Schaertls is the Christmas Eve visit to the Golisano Children’s Hospital, expressing gratitude for the positive impact of the reindeer visits. Maintaining the reindeer’s health poses a significant challenge, according to Schaertl, as the animals require periodic worming, tick spraying, as well as general health monitoring. Despite their toughness, reindeer tend to hide signs of illness, he says, which makes early detection difficult. Mike acknowledges the importance of their veterinarian, Kelly, who offers on-site services. Cases beyond Kelly’s scope go to Cornell University’s veterinary hospital.
Unlike deer Western NYers are used to, both male and female reindeer have antlers. Males shed theirs in November/ December, while females do so in March/April. Furthermore, reindeer and caribou, although similar, are distinct sub-species of cervids (deer family), with caribou being wild and native to North America and reindeer to Europe and Asia. Reindeer are excellent swimmers, with large feet like paddles and dense fur that traps air and helps them float high in the water. They swim faster than the fastest human and they can run up to 45 miles an hour. Mike says people with experience with horses may visit the farm and tell kids not to stand behind the reindeer because they might kick. But reindeer don’t kick behind. They tell visitors that the back and sides are a very safe place to be. It’s around the head that they can be dangerous, because a little shake of the head leads to big antler movements. Several misconceptions surround reindeer. Contrary to popular belief, they are more comparable to cows than deer, being docile, leash-trainable, and allow people to stand right beside them, for instance. They also can’t jump very high like deer can.

Should an owner punish puppy after accident? DEAR PAW’S CORNER: I bought a puppy a few weeks ago who was between 2 to 3 months old. Star is wonderful, and housetraining has been OK. But yesterday, he pooed in the hallway while we were out. My boyfriend says I need to push his nose into the mess and punish him. I don’t think that’s the right way to train a puppy. What do you think? -- Daria in St. Louis, Missouri DEAR DARIA: Your instinct is spot on, and you should trust it more. That’s the wrong way to train a puppy. It’s aggressive, negative and doesn’t teach the puppy anything -- except to be afraid. Young puppies are learning machines, picking up habits quickly -- both good and bad. They need to be constantly supervised to keep them safe and to correct them (not punish them) on the spot so they learn what they can and can’t do in the household. When they’re not supervised, I recommend placing them in a properly sized kennel crate. Think of it as a den for your puppy, a safe spot. Again, not a place to punish him. Furthermore, puppies need to pee and poo more frequently than adult dogs -- about four times daily at Star’s age, but sometimes more. It’s up to you to make sure their elimination schedule is adequate and that you are there to supervise them. Star couldn’t wait for you to get home, so he went in the hallway. That’s on you, not on Star. Clean up the mess (spray with an odor eliminator) and continue your training schedu

Because of your caring and response to our requests, we are able to help those in our community with food insecurities. We continue to be amazed by all the individual donors who quietly fill our donation box, provide gifts and reach out, and our volunteers who serve our guests. The Lakeville Community Pantry is overwhelmed by your generosity. Thank you! My Closest Friend - “Joiny” Yep! My closet friend is not Johnny, Jimmy or Joey. In fact it is not human at all. The name is “Joiny” and I coined it for my iPhone. There is nobody or nothing else that is always so close to me, and let’s me connect or join the world, but my trusty Phone “Joiny”. “Joiny” provides me with information that I didn’t know on a daily basis. I learn so much from this non-human, and it is with me constantly at my age of 82. I ask myself how I survived without “Joiny”? While I did survive, I was never as well informed nor able to connect with so many people in so little time. Thanks, electronic progress.
Litter Bugs Big Time Months ago hundreds of posters for a lost dog were posted throughout [3 local towns] and probably other towns. That pet is either lost or found by now and yet the posters remain. The posters are now unreadable and turning into trash. One polite phone call to the contact was ignored. These poster-makers were asking for help in finding their pet, and yet violating the landscape for thousands of residents doesn’t bother them at all! Please come back and take out the trash! Selfishness is still selfishness. And littering is still against the law. [Local Hospital] Needs New Weekend Policies I drove my wife, age 81 to [local hospital] ER on a Thursday and I was able to park close by in the ER open parking lot. My wife was admitted later that day. On Friday, I drove to the hospital and was glad to see Valet parking close to the main entrance. I am 82 with a bad knee. Saturday I arrived at the same valet place and was told no valet on the weekend, and to park in the large garage building free. I had to walk that day and the next day, Sunday, a long way to the front entrance. I was in pain, with nobody to help as I also carried a heavy bag. I also found out from the front desk that two main restaurants were closed as well as the gift shop. Now I call what [local hospital] does on weekends not good. A grade C-.
