Is an Appropriate, Albeit Powerful Medicine for Osteopenia
DEAR DR. ROACH: I’m a 65-year-old, overweight white woman. I recently had a DEXA scan, and based on the results, my general physician prescribed Fosamax for osteopenia.
The first dose created a horrible reaction two days after ingestion, with severe pain in both of my arms, my neck, and my legs. I was in absolute agony for two days, then it cleared up.
With the next dose a week later, I hesitated, but still decided to take it. Two days later, the reaction hit me again, only worse. I could barely walk, and my energy was so low. I spent four days in bed, except to cry when heading to the bathroom. I had not taken my Lasix prescription for those four days, as I could barely make it to the bathroom.
I contacted my doctor, and she discontinued Fosamax. She’s referring me to a rheumatologist. I read that this drug has all my symptoms, plus more, listed under the severe reaction warning. I will not take it again.
Also, a friend told me that this drug is not effective for osteoporosis after more than 20 years of drug tests, and I should not have been prescribed this drug, since I have osteopenia, not osteoporosis. Your thoughts? -- C.W.
ANSWER: Alendronate (Fosamax) is in the class of drugs called bisphosphonates. These are powerful drugs that should not be prescribed lightly. They do have the potential for severe side effects. Severe musculoskeletal pain can occur days or months (sometimes years) after starting the medicine. The Food and Drug Administration has advised prescribers to discuss this possibility with their patients so that the drug can be stopped immediately if it occurs. There are other options available if the person really needs treatment.
Your friend is half right. Powerful drugs like Fosamax are usually not indicated in people who have low bone mass (osteopenia) without osteoporosis. However, a history of a fragility fracture (breaking a bone from minor trauma that isn’t expected to cause a fracture) can make the diagnosis of osteoporosis, even if the bone density isn’t in the usual osteoporotic range. A person can have a high risk of fractures for other reasons, and a clinical tool called the FRAX score helps clinicians identify those who might benefit from treatment.
But your friend is also half wrong. When used appropriately, Fosamax and other bisphosphonate drugs are effective at reducing the risk of fractures. A vertebral body fracture can cause years of pain and be avoided easily, while a hip fracture can be devastating. So, there are very good reasons to prescribe these medicines.
Finally, let me warn readers against complacence in treatment with these drugs. They may be appropriately prescribed for a high-risk person, but most people should not stay on this drug forever. Follow-up DEXA scans should guide therapy, and after three to five years, a conscious decision should be made whether to continue therapy.
Readers who have been on these drugs for more than five years and haven’t had careful follow-ups on their results should ask their doctors whether they still need to be taking them.***

DEAR AUSTIN: What a great idea! You can try out pet sitting and see if you like it, and your dad is there to give support and advice. Here are a few tips:
-- Bring a notebook and pen with you to every pet sitting visit.

-- Keep to the agreed-upon time to visit and/or walk the pet. It's important for the pet to stay on a regular schedule, and it builds trust with your customer.
-- Do not feed the pet any food or treats not provided (or specified) by the owner.
-- Contact the owner if you have additional questions or if an emergency arises.
ese are just the basics. As you build your pet-sitting empire, you will encounter different situations and will learn the best ways to respond. Your parents can help, and so can that trusty notebook! Strings of Inspiration: Vincent Kaverud’s Musical Odyssey

Vincent Kaverud’s journey as a musician is a story of dedication, passion, and an unyielding love for music. Born and raised in Connecticut, Kaverud studied violin at the Hartt School’s Pre-college division for several years before pursuing his undergraduate degree at University of Connecticut (UCONN). Today, Kaverud pursues a master’s degree in viola performance at Rochester’s Eastman School of Music.
Whilemusicishisprimaryfocus,Kaverudalsohasadeeppassion for foreign languages, a connection he attributes to his Italian roots. His mother immigrated from Italy, and with his girlfriend training to become an Italian teacher, Kaverud says he gets ample practice. He’s also an avid runner and a self-proclaimed culinary enthusiast, making time for his hobbies despite his busy schedule.
Kaverud’s entry into the world of music is a story that echoes the gratitude he feels toward the many individuals who have supported his journey. From an early age, the violin captivated him; listening to Bach’s Solo Sonatas and Partitas during long drives to Rhode Island, where his father worked, solidified his desire to dive deeper into music.
Since then, Kaverud’s accomplishments have been notable. He has had the honor of performing in some of the world’s most revered concert halls, including Vienna’s Musikverein, Berlin’s Philharmonie, NY’s Carnegie Hall, and Boston’s Symphony Hall. A particularly proud moment came this past January when he performed at Carnegie’s Weill Recital Hall as the first prize winner of the New York First Ukrainian Music Competition.
Kaverud’s versatility in both the violin and the viola make him a valuable collaborator in chamber music. He has performed with groups such as the Boston Youth Philharmonic Orchestra, Eastman Philharmonia, and chamber ensembles across New York and Connecticut. One of his greatest joys is creating his own chamber music concerts, curating events in Connecticut and now in Rochester.
For Kaverud, chamber music holds a special place in his heart. He likens playing in a string quartet to “a family sitting down to dinner – exciting conversations, love, and even fights!” His “massive fascination with Beethoven’s string quartets” runs particularly deep, as he says the “range of character and color they capture is truly remarkable.” Studying them from both the violin and viola perspectives, he describes it as learning something new about an old friend.
He says, “I am endlessly inspired by music’s ability to connect people on an emotional level. rough this unique form of expression, we are exploring what it means to be human, and to go through the great joys and overwhelming despair of the human experience. Music reminds us that we are all here together, and that everyone has astronomically more in common than they may think they do.”
Looking ahead, Kaverud has his sights set on a Doctorate in string performance, with aspirations to teach at the university level while maintaining an active performance schedule. Teaching is already an integral part of his life, having taught 10-15 students
weekly at his private studio in CT. His experience as a certified Suzuki-method violin instructor has shaped his approach to teaching, a collaborative learning process where both he and his students grow together.
As for the Rochester music scene, Kaverud is deeply impressed by the city’s rich musical culture. However, he hopes to see more free admission chamber music concerts to give all people access to high-quality live classical musicperformances.HedevelopedanewseriesatSt.Joseph’sChurch in Penfield to help do just that. e first concert of the series will take place on September 29th at 2pm, featuring works by Beethoven and Mozart. Admission is free, and Kaverud invites everyone to attend, whether they are seasoned classical music lovers or simply curious newcomers. Subsequent concerts run 10/26 and 12/1.
When offering advice to aspiring musicians, Kaverud is clear: perseverance is key. “People don’t fail, they just give up.” He says he is inspired by a former violin professor Dr. Solomiya Ivakhiv, who came to the US from Soviet-run Ukraine to study at the legendary Curtis Institute of Music, building a life and career in the US through grit and determination.
At Eastman, Kaverud raves about his current Professor Phillip Ying, the violinist of the quartet that inspired Kaverud a decade ago, who encourages his students to “think critically and consciously about the decisions we make in our musical interpretations, helping us become our own best teachers.”
Kaverud says “Music is an ongoing journey that provides us the beautiful honor of endless learning.” e path may be filled with challenges, but perseverance can lead to a life filled with incredible opportunities and profound experiences.
As he continues to grow as both a musician and a teacher, Vincent Kaverud remains committed to sharing the beauty of music with the world. rough his performances and his teaching, he hopes to inspire others to embrace the joy and emotional connection that music brings.
