Each year the Monroe County 4-H Program accepts applications for two different scholarship funds to support learning a er high school. e 2024 cohort of students have successfully navigated through in-person, virtual, and hybrid studies and are ready to step into post-secondary education in the fall.
WeawardtheNormanV.RyanScholarshiptostudentswhodemonstrate leadership and community service and plan to enter a field of study or employment because of their 4-H activities and experiences. In 2024, Gianni Robles was chosen to receive the distinction of the Ryan Scholarship.
Robles plans to pursue a degree in Landscaping and Entrepreneurship. His 4-H career began when he joined the 4-H team as a Summer of Opportunity intern. Robles shares, “Landscaping and owning my own business has always been a dream of mine. I’m proud to say that 4-H is helping me get closer to that dream. I believe 4-H has helped and shaped me to be a better landscaper but also helped with skills I didn’t know before. e 4-H experience has been one of the best opportunities I have taken so far and I definitely recommend it to other teens that are going into the same course of study as me.” He also shared an appreciation of life skills developed through 4-H participation: “4-H has taught me to have better communication skills in many of the activities they have done,” said Robles.
e Ralph S. Harper Scholarship chooses students planning to pursue careers in agriculture, environment, home economics, or closely related fields. It is a pleasure to award two students the Harper Scholarship as they make plans following high school. 2024 awardees are Madeline Basset and Nora Sansouci.
Madeline Basset participated in 4-H through the Posh Ponies 4-H Club. Basset shared about the impact of community service activities saying, “You can’t be a Posh Pony and not be dedicated to community service. My favorite ones were helping clean the parks in the spring, serving dinners to homeless, and of course the removal of the intrusive water
chestnuts in Braddock Bay.” Basset also spoke of the positive impact of 4-H Public Presentations. She was recognized with a proclamation several years ago and shared, “I was accepted at Syracuse University and the enrollment person told me that my 8-year-old Proclamation awarded from County Legislator, Steve Brew, put me in her top 10 candidates. I was blown away that award on my application stood out.”
Nora Sansouci will attend Monroe Community College in the fall to pursue a degree in Liberal Arts with an interest in education or social work. Nora has been an active member of the Posh Ponies 4-H Club. She participated in many community service activities and stated, “In some of the activities like Trunk or Treat, Valentine’s Day for Youth at Risk home, and feeding the homeless helped me realize I would want a career that helps people.” In addition to participation with her 4-H Club, Sansouci volunteers a few times a week at Heritage Christian Stables and works at Wondercare.
It is a pleasure to watch each of these teen leaders grow and support others through their 4-H experiences. In reviewing applications and talking to these youth, the positive impact of their 4-H experiences is evident. We wish them all the best as they navigate their future education and career goals and look forward to hearing about their successes.

August 3, 2024 is National Disc Golf Day!
The 1920s are the earliest known games, allegedly. A group of children at Bladworth Elementary School in Saskatchewan, Canada are reportedly the first ones to play a version of disc golf. In 1965 George Sappenfield is on break from Fresno State University and working as a recreation counselor when an idea hits him as he plays golf — he wonders if the kids in his playground would like to play golf with Frisbees, and he is right. The First O cial Disc Golf Course was built in 1975. American toy inventor Ed Headrick installs the first o cial course at Oak Grove Park in Pasadena, Los Angeles County, California — the targets are simply permanent poles. Disc Golf gains mainstream attention in 2015. Disc golf appears on ESPN's Top 10 Plays on SportsCenter, taking this sport into mainstream media, too.

High Creatinine and GFR Levels Signify Severe Kidney Damage
DEAR DR. ROACH: Last summer, a city policeman came to my door. My doctor had been unable to contact me with the results of a recent blood test and called the police. The policeman handed me a card with the results written on it. My creatinine level was 4.1, and my glomerular filtration rate (GFR) was 14. He said the numbers indicated that my kidneys were failing and that I should go to the emergency room immediately. Except for swelling in one ankle, I had no symptoms.
During a day and a half in the hospital, I lost 14 pounds of fluid after the insertion of a catheter. Within two weeks, my creatinine level was measured at 2.1. After a few months of wearing the catheter, my prostate was “trimmed back” (my words, not the doctor’s) using a procedure called a “TURP.” I am back to normal. What is a TURP, and how routine is it? Are there any long-term negative effects? How close did I come to kidney failure? -- T.J.R.
ANSWER: Your problem was obstruction of urine flow due to an enlarged prostate. The urethra (the tube that carries urine out of the body) goes right through the middle of the prostate, and an enlarged prostate can block flow partially or completely. This is a very common problem. Sometimes the urine flow stops entirely, but this doesn’t have to happen in order for kidney damage to occur. It’s the increased pressure in the bladder that causes damage. Creatinine is an end product of protein metabolism, and since it is removed by the kidney, it’s a good indicator of kidney function. The higher the creatinine level, the worse the kidney function. The creatinine level (along with age and sex) are used to estimate the GFR, a direct statement of a person’s kidney function.
A (stable) GFR of less than 15 is considered stage 5 kidney failure, which usually means that dialysis will be necessary to preserve life. Note that creatinine itself isn’t the problem; it’s a marker for the fact that the kidney can’t remove all the other waste products, which will then build up and make a person very ill.
When a person with urinary obstruction develops a creatinine level over 6, it’s likely that they will have permanent kidney damage, but I have seen people with higher levels recover and some with lower levels experience permanent kidney function loss. My guess is that you were only a few days away from having permanent damage.
A trans-urethral resection of the prostate (TURP) is a traditional treatment for an enlarged prostate. Although newer treatments are often used in situations where there is plenty of time to spare, a TURP is a routine treatment for the type of obstruction you had. There are possible long-term side effects from a TURP, such as erectile dysfunction, but those are usually noticed immediately after surgery and often get better over time.
Finally, a plea: Make sure that your doctor has your up-to-date contact information, including your emergency contacts. We physicians are very grateful to have the police available for emergencies, but would rather not have to use them. I try to remind my resident physicians to be sure that we have a working contact number any time we order a test, but it’s best if both the patient and physician have it on their lists. Even routine tests can occasionally come back with critical results.
Consult with your dentist. Dental hygienists and dentists are experienced at cleaning teeth that have braces. Typical visits for routine cleanings will not be much different than visits without braces. The staff will take X-rays and conduct a thorough examination of the teeth. Hygienists know how to manipulate cleaning tools around braces to clean effectively. Very often, cavities can be filled and even root canals performed with braces on teeth, so there’s no need to worry. Should braces be in the way, the dentist can work with the orthodontist and have a bracket temporarily removed and replaced to facilitate treatment.

Some people really trying to protect themselves can get very sick. Please be responsible. One can easily spread it to six or seven people, and those six or seven people can spread it to another six or seven people, and so on. Some people can still pass away from Covid if they're sick enough.
