DEAR DR. ROACH: I am a 35-year-old white male who

was a drug addict from the age of 14 until 32. You name it, I took it. About three years ago, I quit drugs and have lived a healthy lifestyle since. I do not drink alcohol, and I smoke one or two cigarettes a day. Three years ago, I was tested for hepatitis and was told the results were negative. About two weeks ago, I was tested and found to have hepatitis C. The results were “HCV RNA Quantitative real time PCR 8010 (high) and log 3.90 (high).” These results were given to me by a nurse without explanation other than to see a specialist. Further physician visits were denied by my insurance. I have no idea what this means, or what I need to do. I have no symptoms or discomfort at this time. What happens if this condition goes untreated? -- Anon. ANSWER: First of all, congratulations on quitting almost everything. That was the best thing you could have done for your health. Now you have to deal with a complication of drug use, particularly injection drug use: hepatitis C. Hepatitis C is a slow-acting virus that affects the liver. The course of hepatitis C is variable. Some people have a very aggressive virus, which without treatment can cause permanent liver damage, ultimately leading to cirrhosis and sometimes cancer of the liver. Others have a much more benign course with no evidence of liver damage, even after many years with the virus. Your viral load, a measure of how much virus is in the blood, is fairly low. However, you absolutely should still go see a specialist. This is usually a hepatologist or infectious disease doctor, who has special expertise in treating hepatitis C. They will do additional blood testing, and probably a noninvasive scan of the liver to look for fibrosis, to determine whether there has been damage and the urgency of treatment. Treatment for hepatitis C is so good now (97% or more effective) and the side effects of the new drugs are so mild that most experts treat someone with even very benign disease. Treatment courses are sometimes shorter for people with mild disease and without scarring of the liver. Your insurance company is absolutely in the wrong to deny a visit with the specialist, and that decision needs to be appealed, preferably by your primary care doctor. In the meantime, continue to avoid alcohol, and do not take more than six regular strength (or four extra strength) Tylenol a day. You also should be sure to have immunizations to hepatitis A and B, if you are not already immune.

HOW COMMUNITIES CAN RECOGNIZE NURSES Celebrate holidays that honor nurses as a community. The American Nurses Association notes that National Nurses Week begins each year on May 6 and ends on May 12. Though National Nurses Week is not a federal holiday, communities can still come together during the week to highlight the work their local nurses do. Township or other local officials can encourage businesses in the community to offer special discounts to nurses during the week, while schools can take part in collective efforts to thank nurses. For example, students from all grades can work on a banner thanking nurses and then showcase the banner by the entryway to campus so passing motorists can see it.

Training Your Cat May Save Its Life DEAR PAW’S CORNER: Is it possible to teach a cat to come when you call? I have a 2-year-old gray cat, Billy, who sometimes gets past my feet and runs out the door. I worry that he will get run over by a car or attacked by a coyote. He will look at me when I call, but that is it. -- Kelly in Fort Myers, Florida DEAR KELLY: You can teach cats to respond to certain commands, and calling them is one of those commands. I know that Billy can learn to do it, because he clearly recognizes your voice. He just doesn’t see the value in returning to you right away. So it’s important to help Billy “see” the value in returning when you call! You do this through training that uses positive reinforcement -- a favorite treat or food. Here’s how: * Come up with a noise that you can make just before Billy gets a treat or dinner. A whistle, a clucking noise, even a little song. * Just before you open his can of food or treat bag, make that noise. Do it at every feeding and treat time. * Set aside five minutes, twice a day, for reinforcement training. * During training, make the noise, wait for Billy to respond, then give him a treat. Back up a couple of feet and repeat. * Gradually increase the distance during each session until Billy comes running when you call. Once Billy associates that specific noise with a reward, he’ll come back every time you call. Make sure to always reward him when he comes, even if it’s just lots of praise. Is a Part B Premium Cutback Coming? By now we know just how much our Social Security checks are going to be for this year, after the deductions for Part B. The average dollar increase is $92 per month for singles. The Part B deduction has gone up $21.60, mostly due to the price of the new Alzheimer’s drug, Aduhelm, for a total of $170 per month. The drug costs $56,000 per patient per year. And we’re all going to pay for it, whether we need it or not. The news now is that the price of that drug has been cut in half, to $28,200 per year, after pressure was brought to bear on the manufacturer when it was revealed that Medicare might not pay for it at all. Additional pressure came from doctors who were suspicious the drug didn’t actually work and who wanted their own tests run on it. Since it must be given via IV in the hospital or doctor’s office, it falls under Part B instead of the Part D drug plan. Hot off the press is a proposal that Medicare will cover the drug -but only for those in randomized controlled clinical trials that will settle once and for all the question about whether the drug actually works. They’ll likely have a decision on that proposal in April. And what of our Part B costs, given the lowered cost of Aduhelm? Health and Human Services Secretary Xavier Becerra ordered Medicare to look again at the premium we’re being charged for 2022. There is the smallest chance it might be lowered, which has never been done before. Last year it was estimated that our Part B premium would be $158.50 this year. If that $170 is cut back to that amount, it would net us an extra $11.50 per month. What shall we spend it on? (c) 2022 King Features Synd., Inc.

S E V RA Hope for Humanity I want to give a shout out to a very special citizen that helped me in a dire time of need! This Friday morning after the dump of good ol’ Rochester snow, my car got stuck at the end of my drive way! I panicked because I had to get my prescription at Wegmans. While I was rocking my car back and forth, a friendly knock was heard on my window! There was a young man dressed in what appeared to be orange fur, cat like ears, and a beard. He jokingly said "Never fear! Snarf Maxx is here!" and he held up a shovel! As I sat in my warm car, he began shoveling the snow beneath my tires! And he wasn't bothered or distressed! He just began singing, "I'm the best!" and kept at it! He looked weird with his fur clothes, but good people are good people! No matter what they look like! Then he walked to the front of my car and told me to go in reverse and I was out! He waved goodbye and said "Enjoy the Wonderland!" This is a lesson to us all! Bless you Snarf Maxx! You are the best! Then, after shopping, our adventure factor really increased when we discovered that our car battery had died. Common Concerns A couple of weeks ago a reader wrote about their frustration that 42% of survey respondents were unconcerned about climate change. Most of us want our lives back and are more concerned with getting Covid behind us. The volcanic eruption in Tonga will effect the climate in one day. Earth's climate has continually changed through the millennia. There is historical evidence of this. Most us are for innovations that will improve everyone's lives. The climate change New Green Deal makes the rich richer. I will believe these politicians are serious when I see wind and solar farms in view of the playgrounds of the wealthy. Currently, only the rural areas they don't see are marred by these eyesores. I love the planet. Let's get some real innovations all engineered in the USA.
We have Control To the person who claims that we cannot control the environment, here's a small list of the human impact on the environment that we can control: Not dumping millions of tons of oil into the ocean every time an oil rig bursts. Smokers actually throwing away their butts every time they are done so that birds do not use them as nesting material. Not flushing medicine down the toilet so that pharmaceuticals do not end up in our water. How about the amount of microplastic that is in the ocean that is currently being eaten by us because we eat things that eat the microplastic? Pretty sure we have control over that. Using more renewable resources than natural, and allowing our wonderful environment to do its natural thing instead of forcing accelerated climate change. Hey look: control! What you wrote makes you sound extremely uneducated. Please call up a science teacher from your local high school and ask them how you can help contribute to making our environment better.

ON LOVE

entlemen: Saint Valentine’s Day is Monday, February 14. You’ve been warned; don’t forget. Ladies, if you rip out this page and stuff it in his shoe, hat, or glove that might be a subtle enough reminder. Holidays are past, and the long, cold slog toward spring awaits us. Time to add a log to the fire, curl up with someone special and love them in honor of our annual mid-winter celebration of Love. But what is love? Well, everyone knows that … right? C.S. Lewis tackled the question in The Four Loves. Read the intro; Lewis thought this would be easy. He started by dividing Love into two types: Gift-Love and Need-Love. Gift Love was pure, selfless; Need Love was selfish, needy, and the lesser of the two. But then, after much reflection, Lewis rethought the topic. Here’s the result.
Affection “Once when I had remarked on the affection quite often found between cat and dog, my friend replied, ‘Yes. But I bet no dog would ever confess it to the other dogs.’” The most humble of Loves, affection is also the most universal. It cares not for status, physical beauty, intellect, wit or wealth. It accepts the other at face value, and it values the other. And it can result in the most unexpected pairings of people who, on the surface, have nothing in common … except, of course their affection for one another.
Friendship “The typical expression of opening Friendship would be something like, ‘What? You too? I thought I was the only one!” We find ourselves in our friends. Like a mirror, we see the obvious similarities, but also subtle differences, and we are safe to explore and celebrate these differences. Friendship multiplies: two friends welcome a third, then a fourth. Each new member brings something new to the growing warmth of gathered friends.
“Our prayers for others flow more easily than those for ourselves. This shows we are made to live by charity.” Charitable Love challenges us to take necessary action on behalf of others, perhaps less fortunate, or less able, without expectation of return, gratitude, praise, or even thanks. It is a selfless gift. Unrecognized. Unacknowledged. When done in secret, without expectation of reward or recognition, it makes the angels belly laugh with joy. And there are so many ways to implement it! Youths with snow shovels ready to shovel an elderly neighbor’s driveway for free, a buck or two to a down-on-their-luck friend, the mischievous couple who secretly pay for the Veteran’s dinner and chuckle all the way home. Charity is profound, and most closely resembles God’s Love for us; that’s exactly why it feels so good. If I were to add one additional sub-category, it would be ‘unrequited,’ and applies to each of the above. Unrequited love is that which is either unreturned, or worse, never acted upon. Charlie Brown never spoke his feelings to the Red Haired Girl. He zigged, and she zagged; they never connected. Inaction, whether by fear or procrastination, results in the same ‘loose-end’ – the never knowing, always wondering, “What could have been?” But it could have been awful; and it also might have been spectacular. Opportunities missed become regrets; burdens carried long and far. Love, in any form, takes courage. It’s ‘cool’ to be disinterested, aloof, and ‘above it all.’ Love challenges us to take action, say words that heal, and perhaps make us vulnerable to rejection. “Better to have loved and lost than never have loved at all.” Tennyson knew, as he wrote his eulogy to a college friend. Love, in all of its forms, causes us to stretch ourselves beyond the familiar, beyond the comfortable, and causes us to grow. Once we have grown, we cannot return to our previous condition. Grow. Take action. Even if it’s scary, do it anyway. Be bold. Dive deep – that’s where the good stuff is. Be a revolutionary; be a rebel. Love well and boldly.

Jeromy J. Bell February 4, 2022 Honeoye: At age 39, Jeromy passed away suddenly on Friday, February 4, 2022 in Rochester, NY. He was born in Canandaigua, NY. Jeromy is survived by his parents, Murray G. Bell Sr. and Christine ( Jerry) Ketchum; his brothers, Murray G. Bell Jr. (Shawna King), Jerry Ketchum Jr., and his sister, Julie (Erik) Carvotta, along with several nephews, nieces, aunts, uncles and cousins. Jeromy is predeceased by his sister, Heather, all of his grandparents and his cousin, Jessica Carroll. Although Jeromy’s life was short, he touched many with his big heart. Jason P. Klemmer January 27, 2022 Avon, NY: At age 43, Jason passed away unexpectedly on January 27, 2022 at Grady Memorial Hospital in Atlanta, GA. He was born in Rochester NY on May 17, 1978 to parents, Paul and Patricia (Halpin) Klemmer. He was predeceased by his mother, Patricia Ann “Patty” Klemmer in 2017 and grandparents, Theodore and Marie Halpin & Allan and Vera Klemmer, and his cousin, Matt Halpin. Survived by his beloved son, Liam Klemmer; father, Paul Klemmer; and sister, Erika Edwards (Kyle); nephews, Cole and Grayson Towery and Henry Edwards. Also survived by many loving aunts, uncles, cousins, and dear friends including Jamie Yates. Jason was a beloved member of the Avon Community with a work ethic and passion for friends and family that brought tremendous joy to all he touched. Jason worked at his family business, Dugan Creek Lumber, where he and his Dad, Sister, Dusty Preston and crew built it into a Western New York timber powerhouse. Jason also had a second business, LivCo Tree company, that he operated with his close friend Matt Sattora. Jason built a brotherhood out of cousins and friends who adored him. His cousins Josh, Andy, Lizzy, Lauren, and Teddy had an inseparable bond that all are grateful for. Jason’s ability to bring people together and celebrate the success of his friends and family brought great joy to all. Jason had a gift in generating great political and though- invoking conversation that left all in admiration of his wit and charm. Jason was passionate about the outdoors, to include family trips to Canada, hunting, fishing, and priceless time with his family at their cabin on the family farm and cottage on Conesus Lake. By far Jason’s greatest joy was spending time with his son, Liam. Family and friends may call on Friday, February 11th, from 3-7pm at the Kevin W. Nancy B. Rider (Briggs) February 1, 2022 Livonia: At age 81, Nancy passed on February 1, 2022 at Strong Memorial Hospital in Rochester, NY. She was born on March 29, 1940 in Warsaw, NY to the late Raymond and Katharyn (Walker) Briggs. In addition to her parents she was predeceased by her first husband, Douglas Welch and her brother, John Briggs. Survived by her husband, Christopher Rider; her daughter, Sarah (Thomas) Lauder of Plantation, FL; and granddaughters, Katharyn and Madilene Lauder. Nancy is also survived by several nieces, nephews, and many friends. Having earned an undergraduate degree at Wilson College and her master’s degree from Alfred University, Nancy’s career spanned 35 years teaching math at Honeoye Central School. Nancy was quick to volunteer her time and resources to those in need, whether giving her time to hospice patients at Teresa House, serving in various capacities at St. Michael’s Church in Geneseo or coming to the aid of family, friends or critters. Wherever she saw the need, she gave generously. Throughout her life, she was an enthusiastic lover of the outdoors and of all of its plants and creatures. Her devotion to animals was well recognized from nurturing ill or orphaned wildlife as a rehabilitator or simply taking in another stray cat. Nancy will be remembered for her quick witted, often whimsical sense of humor, brilliant smile and “gift of gab.” Her zeal for life was infectious and she lived her life in her “uniquely Nancy” way. We are grateful for the many unforgettable ways that her life touched and joined ours. A private graveside service will be held in the future at Temple Hill Cemetery, Geneseo, NY.
