Trees Not Tombstones: New Options For End-of-Life (NAPS)—The vast majority of Americans have not completed their end-of-life planning. In fact, over half of those age 45-plus have done no end-of-life planning at all, according to a recent survey conducted by Better Place Forests, the country’s first sustainable alternative to cemeteries for families that choose cremation. Can Chocolate Worsen Irregular Heartbeat? ---
DEAR DR. ROACH: I am 82 years old and still very active. I have had an irregular heartbeat for four years, and sometimes I feel lightheaded, dizzy or tired. I worry about it getting worse. Would you please give a detailed explanation about irregular heartbeat, including how or why it happens? What should I do to prevent it from getting worse? Is it life threatening? Will it become atrial fibrillation? Also, I am a chocolate lover and eat dark chocolate every day. But I was told that cocoa will make my irregular heartbeat worse and I should stop eating dark chocolate. Is that true? -- Q.Y. ANSWER: Everyone has irregular heartbeats from time-to-time. Early beats can come from electrically active areas anywhere in the heart, and an electrocardiogram can tell whether they are from the atria (the top chambers) or the ventricles (the bottom chambers) of the heart. An average person may have 500 or so of these a day -- some people have many more. Most are seldom aware of them. Having patients with this concern over and over during my career, I can reassure you that most cases turn out to be nothing to worry about. However, the fact that you are having symptoms, especially the lightheadedness and fatigue, is a warning sign, and I recommend you get an evaluation. Start with your regular medical provider, after which you may be referred to a cardiologist. The EKG in his or her office is a first step, but you may need a longer evaluation, such as wearing a 24-hour EKG (a Holter monitor) or newer technologies that allow longer readings. Atrial fibrillation is a concern, because most people will need treatment to reduce stroke risk or to return the rhythm to normal. Nevertheless, there are many other kinds of irregular heart rhythms, and you need a diagnosis first. If no cause is found, there are medications to reduce the irregularity, but they are seldom used. My experience is that when a person knows they are not dangerous, they can be lived with more easily. Chocolate does contain theobromide, a substance that can make the heart a little more irritable, but it takes a lot of chocolate to have a significant effect. It’s related to caffeine in coffee. Interestingly, a recent study found that chocolate lovers are less likely to develop atrial fibrillation, so the situation is more complex than just one chemical. ***
DEAR DR. ROACH: I read that drinking warm lemon water with honey has many health benefits, partially because lemons are an alkaline food. Is this correct? -- B.W. ANSWER: I think I saw the same article. Lemon water may have some health benefits, but it is not an alkaline food, and drinking it has almost no impact on the acidity of your blood or urine, because the body’s buffer systems (the chemical processes used to keep the pH carefully regulated) are so effective. Lemons have a pH of about 2, so they are highly acidic. Ascorbic acid (vitamin C) makes the urine more acidic. There is no consistent evidence that the foods we eat have a health benefit due solely to their effect on body pH. Many of the foods recommended for their supposed alkaline effects are indeed healthy, but for other, complex reasons apart from pH.

Vanguard and Mercer recently developed a new framework, “Planning for Health Care Costs in Retirement,” that identifies practical tips for forecasting your health care expenses. Here are four top ideas: 1. Personalize health care costs. Start by understanding how your health history and current health status will influence expenses. Even your geographic location, marital status and age at retirement will impact your forecasts. 2. Plan for long-term care. This is a tough one to assess because half of retirees won’t even incur these costs, but on the other end of the spectrum, 15 percent of retirees will spend more than $250,000. Consider potential long-term care options, such as unpaid care from family and less-expensive available facilities. 3. Create a hedge in your budget for other expenses. Research shows that retirement spending in virtually all categories other than health care tends to decline with age. By forecasting steady spending in other expense areas, you may create a buffer in your budget to deal with rising health care expenses. 4. Forecast costs in annual spending. There are so many variables involved in estimating health care costs in retirement that trying to plan around a total lifetime budget can be overwhelming. Experts recommend that you focus on annual spending plans instead, provided that you understand costs will rise as you age. For seniors who are struggling to find cash in their retirement budgets to offset unexpected health care expenses, it may be a good idea to take stock right now of all your assets. Many seniors are surprised to learn that one potential asset for generating immediate cash is a life insurance policy.

WHAT IS IMMUNOTHERAPY? Immunotherapy is a medical treatment that uses portions of the immune system to fight diseases like cancer. Immune cells and the substances they make travel through the body and protect it from foreign invaders, says the American Cancer Society. Immunotherapy includes a combination of treatments. Some boost the body’s immune system in a very general way. Others help train the immune system to attack cancer cells specifically.
You should review your life insurance policy from time to time and determine whether or not it’s still needed. A life insurance policy is considered your personal property, so you have the right to sell it anytime you like. When a consumer sells a policy—something called a “life settlement” transaction—the policy owner receives a cash payment and the purchaser of the policy assumes all future premium payments, then receives the death benefit upon the death of the insured. Candidates for life settlements are typically aged 70 years or older, with a life insurance policy that has a death benefit of at least $100,000. Though Law Enforcement Appreciation Day has passed, it is not too late to thank the men and women who put a uniform on every day to protect and serve. Saturday afternoon, I was sitting in my living room in Webster. As I looked out the window, I saw a man riding his bike. On People need to seriously start picking up the poop from their dogs! I am so tired of, and disgusted by, dog poop all over the place, including my front lawn. I have a large dog that I walk every day, and I 100% pick up after him. You know who you are; stop it! After reading about a disruptive child in a checkout line especially when the child tried to steal personal property and was messing with the cash register, I have to wonder why this cashier did not immediately call for the manager to come to the front of the store and handle this situation. On Sunday, January 26, Riga Recreation held its annual “WinterFest” in partnership with the Churchville Lions Club, the Churchville-Chili School District and the Monroe County Parks Department at the Churchville Elementary School. The Lions, once again, sold out their “World Famous” Chicken BBQ, and there were carnival games, airbrush tattoos, caricature drawings, and a bounce house in the gymnasium. Weeks before Christmas, I had put up my Christmas lights on my house on a typical village street. At dark, I went outside and stood on the sidewalk, inspecting the joy when I heard footfalls in the snow on the street and saw that a woman was jogging toward me. Simple courtesy called for some kind of acknowledgment, and I smiled and said hello, not thinking that a man standing on a sidewalk at dusk could appear sketchy. Immediately, she got an unpleasant look, muttered something unpleasant and sprinted away down the street. I felt bad instantly for making her uncomfortable. I understand, and her reaction wasn’t wrong, but it’s a shame that a simple smile and hello can cause fear or discomfort. The world got a little colder for me that day.

Canines Employeed to Keep VA Drug-free VA Finger Lakes Healthcare System Implements Canine Program for Narcotics Detection By Amanda Dudek
V
A Finger Lakes Healthcare System Police Department (Bath and Canandaigua VA Medical Centers) have invested in and implemented a canine program for narcotics detection to keep patients and visitors safe. Acting Chief for Canandaigua/Bath/ Rochester VA Police, Kent Strege along with Sergeant Christopher Hamilton and Canine Sampson in Canandaigua and Lieutenant Jeffrey Mastin and Canine George in Bath currently work to protect Veterans from illicit substances at their VA Medical Center campuses, both comprised of many acres. Their efforts have been successful in more ways than one. Canines have been used by police in narcotics detection because not only is it a psychological deterrent, but a dog’s sense of smell can scan a room for illegal narcotics and drug paraphernalia quicker than the police can search a room for these items. “The thought of a Canine Program started back in 2014 when it was a relatively newer program within VA nationwide,” explained Sergeant Christopher Hamilton, Chief Canine Handler for Canine Sampson. “In 2016, we started the program in Canandaigua.” The program was found to be such a “useful and successful tool” that it was implemented at the VA Medical Center in Bath, as well. The two Labrador retriever dogs Sampson (4 years old) at Canandaigua VA and George (18 months old) at Bath VA are narcotics detection certified, receive ongoing monthly training and annual certification and each have their own special vehicle for transportation. Sampson has responded to over 60 narcotics related police calls at VA medical centers State-wide and has had 14 finds for substance or drug paraphernalia. George who is just getting started has had three finds since joining VA. Both dogs will help keep the environments safe at both VA medical centers for in-patient and out-patient Veterans, visitors and employees. “The finds have given us either illicit product or paraphernalia or something along those lines,” said Hamilton. “I think the biggest thing is that it is a great visual deterrent, and that is a huge factor.” The benefit of this is readily realized. “It is an invaluable tool when you talk about the safety of the Veterans on our campuses,” stated Strege. “Several Veterans are recovering from illicit drug use. We want Veterans to come to VA who want the necessary help to beat that substance, and having such substances around could trigger a relapse.” Sergeant Chris Hamilton at Rochester Calkins VA Clinic with Sampson.
However, both Strege and Hamilton agree that the value of the canine program extends beyond the clear benefit of keeping our VA locations safe from substance abuse. “The reaction from Veterans has been very positive,” shared Hamilton. “They always seem to be happy to see me and the dog walking the hallway.” Sergeant Hamilton feels that Sampson, a black lab, is often a comforting presence to the Veterans.

That has been very effective for us, too.” Sergeant Hamilton describes the dogs as “multipurpose” in that they are excellent at finding illicit substances and discouraging their presence while also having a calming effect on those around them, stating, “Sampson is a perfect fit in our department.” The safety of Veterans is the priority of the VA canine program, a cause that the officers hold close to their hearts. “All of the officers in our department are Veterans,” shared Strege and Hamilton. “We chose this career field because we have a passion for this. Our goal is to serve our Veterans, and being one gives us an automatic rapport with them. We are able to calm situations relatively quickly.”

LEARN HOW TO WRITE A CLASSIC LOVE LETTER Today’s digital world has given rise to fast, stinted communications. According to the Pew Research Center, texting is the most widely used smartphone app, with 97 percent of Americans using it at least once a day. Across the globe, 8.3 trillion text messages are sent each year. That translates to roughly 16 million messages per minute. In addition to text messaging, social media posts are generally concise and short on depth. The written form of communication has changed dramatically in the last decade, as letter-writing and other forms of prose have all but faded away. But come Valentine’s Day, it can be special to dust off that pen and paper to express oneself via the written word in the name of love. Love letters are cherished keepsakes that can be kept for years. Such couples can illustrate a couple’s present-day love for one another and shed insight on their relationship years later. Here are some steps for writing a romantic love letter or note. • Jot down ideas and inspiration. Make a list of attributes and qualities in a spouse or romantic interest that you would like to highlight. Write down phrases and terms that come to mind. These words can serve as the foundation for the letter. • Categorize your thoughts into three areas. Jot down things that you like and love about the person, things that the person may do for you or your family that you appreciate, and various memories from throughout the years. • Write what you know. Do not feel pressured to make this letter more complicated than it has to be. The words will come when you write from the heart. While you can use other letters to spur ideas, do not lose your voice or try to mimic someone else’s work. • Make the letter personal and specific. Small details are important and make love letters more meaningful. Such details illustrate to a significant other that this letter was specifically written with them in mind. • Disregard length. The letter does not have to be a certain length. Whether it is a paragraph or several pages, the letter will be a successs if it says what you want to express. Roughly 50 percent of those who relocated did so for career advancement and/or more money, while 8 percent moved to be closer to family or for other relationship reasons. Moving is often characterized as stressful, and 31 percent of survey respondents who moved indicated that finding a home was the most challenging part of relocating for a job. Acclimating to a new community (29 percent), finding services such as utilities and medical care (13 percent) and making friends (12 percent) were also cited as some of the most challenging parts of relocating for a job. As difficult as relocating can be, 64 percent of those who had relocated expressed positive sentiments toward their job relocation, while just 7.5 percent harbored negative feelings about relocating (28 percent were neutral).
