Local vendors are more likely to source their materials from other local businesses, reducing their carbon footprints along the way. “It really is neighbors helping neighbors,” said Steve Hirsch, volunteer firefighter and chair of the National Volunteer Fire Council. “There is nothing more rewarding than being a volunteer firefighter—we are there for our neighbors when they are having their worst possible day.” The need for volunteer firefighters is stronger than ever. While the number of calls has tripled in the last 30 years, the number of volunteer firefighters has decreased. This trend has become especially noticeable in the last few years. According to NFPA data, there were 682,600 volunteer firefighters in 2017, down from 814,850 in 2015. Despite the decline, volunteers play a critical role in the nation’s emergency services, particularly in small and rural communities. Of the nearly 30,000 fire departments in the U.S., 83 percent are either entirely or mostly volunteer. These volunteers respond to nearly every type of emergency—structure fires, wildfires, medical emergencies, natural disasters, vehicle crashes, hazardous materials spills, search and rescue, active shooter threats, and more.

In fact, the National Fire Protection Association (NFPA) reports that fire departments responded to over 35 million calls in 2016. What people may not realize is that the majority of these firefighters are volunteers, donating their time and energy to be there for those in need.
“We need more people to step up and serve so that we can continue to provide critical life saving services in our local communities,” said Hirsch. “The biggest requirement is the desire to help others. The rest can be taught.” For those who want to help but aren’t able to commit to becoming a firefighter or EMS provider, volunteers are also needed to fill non-operational roles. Community members can join a department’s auxiliary program to provide needed support services such as fire prevention education, disaster planning, fundraising, administration, and much more. Volunteering with a local fire department takes commitment and dedication, but the benefits are many. Making a difference, saving lives, developing new skills, and being part of a close knit team are all reasons people choose to volunteer. “I’m a lawyer by trade, but I don’t think about that every day. I think every day about being a volunteer firefighter. It truly is the most impactful way I can serve my community,” said Hirsch. Thankful for all Caregivers. Being a Caregiver is a very hard, stressful, demanding job whether it be for a family member or in a professional capacity. The physical, emotional and financial consequences for the family caregiver can be very overwhelming. If you know someone who is a Caregiver, consider offering
to sit with their loved one so they can have a couple hours to themselves. Give them the opportunity to go out to lunch or have coffee with a friend, do some unhurried shopping, pamper themselves at the hairdresser or barbershop or take a relaxing bath. Just let them get away and take some time for themselves. If this is not possible, offer to do the grocery shopping, take items to the dry cleaners, help with the house cleaning, shovel the driveway or whatever would be helpful. Caregivers often feel isolated and lonely. Just being there for the caregiver, who is giving all of themselves to the one they are caring for, is so important.

5 Important Medicare Changes Coming For 2020 (NAPS)—If you’re enrolled in a Medicare Advantage plan, or are considering one, you will have more plan choices with a wider range of benefits to help you better manage your day-to-day health needs during this year’s open enrollment period. Medicare’s Annual Enrollment Period, which runs from Oct. 15 through Dec. 7, gives Medicare enrollees a chance to revisit their Medicare insurance coverage, make sure it still fits their needs and make changes for the coming year. Online health insurance broker eHealth outlines five of the most important Medicare changes to be aware of this year: 1. New benefits to help those with chronic illness. Starting in January, 2020, some Medicare Advantage Plans will include benefits for people with chronic illness that will help to support their care. A few examples include nutrition services, transportation to doctor’s appointments and home improvements like shower grips and wheelchair access. More traditional services like dental care, vision care and hearing aids also have been expanded.

2. Medicare Advantage premiums will be lower next year. The average monthly premium for Medicare Advantage plans is expected to be $23 in 2020, a decrease of 14% from an average of $26.87 in 2019, and down 28 percent since 2017. The average Medicare Advantage premium this year is the lowest it’s been since 2007. 3. Part D drug plan premiums will be lower, too. The average Medicare prescription drug plan premium for 2020 is $30, a decline of 13.5% as compared with 2019 costs. Savings on out-of-pocket costs may be available as well. A recent eHealth analysis of people using eHealthMedicare.com to compare Medicare plans found that fewer than one in ten were enrolled in the lowest cost plan for their personal prescription drug regimen. Those who switched to their optimal drug plan stood to save an average of $900 per year. 4. Seeing a doctor without leaving home will get easier. Many seniors will have access to Medicare Advantage telehealth benefits to connect with health care providers online or by phone. For 2020, more than half of all Medicare Advantage plans will offer added telehealth benefits, reaching up to about 1.7 million enrollees. 5. Medigap plan options are changing. Two of the most popular supplemental plans (so-called Medigap plans) used by those enrolled in original Medicare—Plan F and Plan C—will no longer be available to new Medicare members or those born after January 1, 1955. Fortunately, Plan G, another Medigap policy, offers the same level of coverage provided by Plan F—minus the Part B deductible—and will continue to be available after January 2020. The Part B deductible was $185 in 2019. Even if you’re happy with your Medicare coverage, eHealth recommends that you review your current coverage during open enrollment because each year plans can and do make changes in costs, physician networks or drug benefits. Shingles Vaccine Causes Severe Side Effects ---
DEAR DR. ROACH: I received the first dose of Shingrix this week and had pretty severe side effects, though in the range of what can be expected: high fever (101.5), intense shivering, a severe headache and 12 hours of extreme fatigue. The entire reaction began 11 hours after the shot and ended 18 hours after that. I am 61 and in excellent health, taking only 25 mcg of levothyroxine daily. I have a few questions that I hope you can answer. I’ve tried looking at the clinical trial results for Shingrix, but I am not qualified to understand the information as presented. 1. Is it likely that I’ll have a similar (or worse?) reaction to the second shot? The package insert says one can react to the first, second, neither or both. 2. Are both injections identical? 3. When I had chickenpox as a child, I was way sicker than the average kid. Related? 4. If I get shingles, do I have a higher risk for a severe case? -- J.G. ANSWER: Compared with the previous one-time vaccine Zostavax, the new two-dose Shingrix vaccine is much more effective. However, it does have a higher risk of side effects. You have had the most common side effect, though only 10% of people will have symptoms as bad as yours. These symptoms are caused by your body mounting an inflammatory response to the glycoprotein in the vaccine (the new vaccine does not contain a live virus). You clearly have a robust system to fight off infection. To answer your questions in order: You are at higher risk for a similar reaction for the second shot. I would plan your day accordingly, and premedicate with Tylenol (even though it may make the vaccine slightly less effective). The second vaccine is identical to the first. I have read reports that there have been shortages of the vaccine. Chickenpox and shingles can affect you both by the virus attacking you and by an exuberant inflammatory and immune reaction. I would guess that you are at lower risk for viral complications (such as infection of the eye, brain or lungs) but at higher risk for symptoms due to your own system, such as high fever. In many cases, the body’s response to infection can be as damaging as the infection itself. ***
DEAR DR. ROACH: Last year I was treated for gout and was prescribed allopurinol (100 mg) once a day. Approximately five to six months after I started taking it, I became lethargic and was not feeling myself (I am a very young 77 years old) and suddenly lost my sense of taste. I was taken off the medicine and still have little taste sensation other than citrus fruits, apples and spices. I was told that this could last for months, years or forever. Do you have any suggestions for me? I am eating as before, hoping that I will recover my taste. -- N.K. ANSWER: I certainly found that allopurinol can cause loss of taste sensation, but the Food and Drug Administration case reports don’t say how long it can last. Unfortunately, I can’t find any reliable way (or even plausible way) to increase the likelihood of getting your taste sensation back. *** Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. PREP-YOUR-PLACE POINTERS (NAPSI)—Many people may be surprised to learn that fall is actually the most affordable season for home prep and repair projects. There’s less demand, so homeowners enjoy breaks on equipment and services. An easy place to begin is at the window. Cracked and warped windows can often cause heat to escape. “Windows that produce a draft or feel cold on the inside should be replaced,” explains Richard Kasunic, a home repair expert with Window Nation, one of the country’s leading window replacement companies. Modern vinyl windows fit tightly to shield homes from wind, water infiltration and inclement weather overall. With that said, many homeowners have older windows and are often “heating the outside,” adds Kasunic. “On average, homeowners spend 40 percent of their utility expense dollars on heating and cooling,” he says. “But if new windows aren’t in this year’s home budget, you might want to pick up insulated curtains, window and door gap seals, and add draft snakes on windowsills.” He also suggests replacing the weather stripping around doors if you see any light around the edges. Beyond windows and doors, exterior outlets, plumbing and utility access, chimneys and walkways should be sealed for unwanted holes, says Kasunic. And homeowners should have their furnaces and roofs professionally cleaned and inspected. Finally, cover the water heater and change the furnace filter so your furnace doesn’t have to work as hard. O
n the evening of Saturday, November 16th, Victor Wainwright and the Train, a 2019 Grammy nominee, thundered into Geneseo’s Riviera Theater, courtesy of Fanatics Pub/Jim Shelly Promotions. Seats were sold out with standing room only in the beautifully restored theater venue. Opening artist, Owen Eichensehr, with his upbeat, eclectic blend of blues, folk, funk, jam, jazz and rock, was very well received. Next, the Train took the stage, featuring Billy Dean on drums, percussion and vocals, Terrence Grayson on bass and vocals, Pat Harrington on guitar and vocals as well as special guests Mark Earley on the sax and Doug Woolverton on the trumpet. Guests grooved to their scintillating sound as the flowing rhythm of the Train showcased the musicians in turn, each outstanding in his own right. Hailing from Florida, Billy Dean expertly emcees the crowd while driving the band forward in seemingly effortless rhythm. The nine-year Victor Wainwright and the Train veteran, Dean’s dynamic drumming style has topped Billboards and lit up stages across the United States and beyond. Terrence Grayson of Jackson, Mississippi, cut his musical teeth playing songs in church before falling in love with the full, powerful sounds of the bass. Early in his musical career, Grayson fell sway to the groovy, intoxicating blues, eventually incorporating elements of soul, R&B, rock & roll, funk and roots music into his vibe. Guitarist Pat Harrington from our nearby Gasport, New York, is perhaps the newest member of the Train, joining the group in 2016. His exciting guitar leads and improvised riffs promise to add fuel to the Train’s fire as it charges ahead, onward and upward.

That evening, the Train continued to play, preparing the crowd for none other than Victor Wainwright, talented and surprisingly youthful composer, producer, songwriter, vocalist, and awardwinning entertainer and piano player. He has won as slew of prestigious awards, including six Blues Music Awards, such as multiple BMA Pinetop Perkins Piano Player of the Year awards, a BMA Entertainer of the Year Award and a BMA Band of the Year Award. He soared to the Top 10 in the US Billboard Top Blues Album chart in 2013 and in 2016, won the B. B. King Entertainer of the Year Award. This year, Victor Wainwright and the Train became a Grammy nominee for Best Contemporary Blues Album. Upon his introduction, Victor Wainwright paused at the front of the theater to fist-bump a young audience member before jauntily heading down an aisle to sit at his electric piano under the cool blue lights of the stage. Immediately, his fingers began finding melodies as he welcomed the enthusiastic crowd. He praised the

Train, noting how good they sounded and joking that he “hoped they would let [him] sit in!” Wainwright and the crowd vibed and joked like old friends in what he referred to as, “a new location with a familiar crowd,” before launching into “I’ll Start Tomorrow,” an uproarious, boogie-woogie infused hit from his 2018 album, “Victor Wainwright and the Train.” Wainwright’s hands flew up and down the keyboard, skillfully driving the melody as the likewise talented vocalist sang the song’s story with hilarious facial expression and inflection. He riffed and improvised, never missing a beat or a key, even when he tipped his keyboard sideways to play alongside guitarist Harrington. The song concluded to thunderous applause as did each song throughout the night. Wainwright transitioned effortlessly from upbeat, honky-tonk jams to the rhythmic rolls of brooding blues, slow and glorious, and back again. His hands were lightening, striking, rolling and sliding so quickly yet correctly, one is left wondering how so much talent can exist in one person. Wainwright himself is ever humble, offering as one of his original songs a tribute to B. B. King for whom he once opened at the Peabody Theater. Yet, he seems destined to leave his own mark on the blues music scene as he continues to reinvent and redefine its meaning and its possibilities. STEPS TO LEAVE A JOB GRACEFULLY WHY IT’S WISE TO HIRE VETERANS WITH DISABILITIES (NAPSI)—“Today, nearly 4 million Americans have a service-connected disability. At a time when many employers are struggling to fill positions, hiring veterans with disabilities can boost diversity and inclusion efforts,” explained Jeff Hall, national employment director for DAV (Disabled American Veterans). For example, Dave Ellis found it difficult to get a job after receiving an honorable discharge from the Army. “I had a severe back injury, but I also carried mental scars from active duty,” he said. After six months of searching, he was able to find a supportive company that understands and values the traits that come from military training, including job-ready skills, tested leadership abilities and a mission-focused work ethic.
Deciding to leave a job is seldom an easy decision. However, it’s a more common decision than many people may know, as the U.S. Bureau of Labor Statistics indicates the average worker holds 10 different jobs before age 40. People change jobs for a variety of reasons. Some do so because of better opportunities elsewhere, while others simply need a change. Regardless of what’s motivating a job change, leaving a place of employment can elicit various emotions, including some that may contribute to negative thoughts about a current employer. But it’s always best to leave a job gracefully, which can pay dividends down the road.

For others, however, it can be a struggle. In fact, a survey in The Journal of Vocational Rehabilitation found that 57 percent of veterans with disabilities transitioning to the civilian workforce feared hiring discrimination due to their disability. “The reality is that many of America’s businesses, big and small alike, recognize the importance of hiring veterans with disabilities, but often don’t know where to begin,” added Hall. To address this need, DAV has published “The Veteran Advantage: DAV Guide to Hiring and Retaining Veterans with Disabilities.” This free, comprehensive guide is the result of more than four years of study about what the veteran community contributes to the workforce and how hiring veterans, especially disabled ones, can improve a company’s bottom line.

IT’S NEVER TOO LATE TO ACHIEVE A HEALTHY WEIGHT (NAPSI)—People often think of the New Year as a time to set new goals for healthy eating and physical activity. But weight management requires ongoing effort, during the holidays and every day. That’s why the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), part of the National Institutes of Health, is promoting simple tips and tools to help Americans start and maintain healthy habits. “Why wait to start improving your health when you can start today?” NIDDK Director Dr. Griffin P. Rodgers said in a statement. “It’s never too soon or too late to achieve a healthy weight.” Reaching and maintaining a healthy weight through healthy food choices and regular physical activity may help lower the risk of developing health problems associated with obesity, such as heart disease, kidney disease and Type 2 diabetes. About 40 percent of adults and 19 percent of children and teens in the United States have obesity, according to recent data from the Centers for Disease Control and Prevention. Continue or establish healthier eating habits, such as reducing the overall calories you consume and limiting your intake of foods and beverages with added sugars, fats and salt. Bringing healthier versions of your favorite dishes to holiday gatherings, and choosing smaller portions of holiday treats and alcohol, may help with managing weight during this time of year. The NIDDK recommends making physical activity a priority for the whole family. Choosing a set time for physical activity and sticking to it may help you manage stress, improve your mood, and give you the energy to tackle your busy schedule. If it’s too cold to walk or run outdoors, take your workout to your favorite mall or indoor sports field. Even if you’re pressed for time, doing some physical activity is better than skipping your workout and doing nothing. Another tip to consider during the holiday season and every day to improve health and manage weight is to reduce screen time and time spent sitting. Watching TV for more than two hours a day has been linked to overweight and obesity. Try creating a holiday-themed playlist and having a dance party. You can have fun dancing alone or with family or friends. To further encourage adults and youth to be active, the U.S. Department of Health and Human Services also recently released new Physical Activity Guidelines for Americans. The new Guidelines recommend that adults aim for at least 150 minutes of moderate-intensity activity each week, such as brisk walking or dancing. Youth ages six through 17 need one hour each day, and children ages three through six should be active throughout the day. By establishing and sticking to healthy habits, parents, caregivers and youth may achieve better health together during the holiday season and beyond.
