Vitamin D Works Slowly

DEAR DR. ROACH: Is there any reason to start taking vitamin D at age 90? I’m in good health except for osteopenia and high blood pressure. How long does it take to have an effect? -- P.S. ANSWER: In your case, there are potentially two good reasons to start taking vitamin D -- if your blood levels are low. One is that with osteopenia (a condition of low bone mass, a precursor to osteoporosis, which increases risk for bone fractures), replacing low vitamin D can improve bone strength and reverse bone loss. This can keep people from needing other medications for osteoporosis, which are much more likely than vitamin D to have side effects. Older people with vitamin D deficiency also often have some muscle weakness. If you can’t get out of a chair without using your hands and you have a low vitamin D level, replacing vitamin D may help with the weakness. Of course, there are many causes for weakness in a person and there is no guarantee of effectiveness, but vitamin D is very safe in moderate doses, like 1,000 to 2,000 IU daily. Vitamin D also may reduce fall risk, probably by improving strength, especially in combination with exercise. Vitamin D works slowly -- weeks to months for muscle effects, and months to years for bone effects. ***
DEAR DR. ROACH: I was seeing my primary care physician every six months for about five years. We had mutual respect, and I enjoyed seeing him. Then, I got diagnosed with breast cancer and had to see other doctors for a long time. Meanwhile, the spinal stenosis my primary care physician had diagnosed was becoming untenable and ruining my quality of life. Even though I am still seeing cancer doctors, I called my primary care physician to make an appointment to discuss the spinal stenosis situation with him. After a long wait, I finally got someone in my primary care physician’s office and, after another long wait, was informed that he could not see me for six months -- there was not one opening until then. I was flabbergasted and asked to be put on a cancellation list. “We don’t keep those,” the lady responded. I reluctantly took the appointment and decided to wait it out. Meanwhile, I saw one of my cancer doctors today, and after explaining this situation to him, he replied: “Doctors don’t do that. He obviously doesn’t want you as a patient anymore. I would find another primary care physician.” What is your response to this? I am 71 years old, and other than spinal stenosis and breast cancer (which was caught early and treated with surgery and radiation a year ago), I enjoy reasonably good health. -- J.E.Y. ANSWER: I hope there is another side to this story, since I am horrified that a physician would make a longstanding patient with cancer and spinal stenosis wait six months for an appointment. That is not appropriate medical care. While I fully understand the point of view of your cancer doctor -- and would be tempted to find another doctor -- it sounds like saving the relationship you had with your PCP might be worth another try. I think speaking directly with your primary care doctor might clear up any misunderstanding. ***

A 93-year-old liver donor from Texas once saved the life of a 69-year-old teacher; and in 2017, one of every three donors was over 50. 5. Transplants grow with young recipients. Organs transplanted into children can grow and function into their adulthood and beyond. These gifts can last a lifetime. 6. A little goes a long way with livers. When transplanted, a segment of a donor’s liver will grow within a recipient until it achieves the functional capacity of a whole liver. 7. Recent innovations mean more possibilities. Vascularized composite allografts (VCAs) like face and hand transplants involve multiple kinds of tissue, and they’re giving hope to service members wounded in combat, people injured in other traumatic situations, and those born with missing limbs or severe disfigurements. (NAPSI)—Don’t let window replacement become a “pane” in your wallet. There’s a disconnect between saving money on energy with window replacements, according to government agencies. In fact, they found, it may take decades to recoup the cash spent on the installation of new, energy-efficient windows. The Problem The assessed value of adding 20 new energy-efficient windows in California, for example, may range from $10,220 to $18,580 based on a 2019 building assessors manual listing $511 to $929 for each window. Annual energy savings for the replacement of existing windows in a home are from $27 to $111 for dual-pane windows and $126 to $465 for single-pane windows, according to the U.S. Department of Energy. This means it may take decades to financially justify window replacements based on energy savings alone. An Answer Fortunately, there’s an alternative. Window films offer advanced energy savings and other benefits and can be quickly installed at a fraction of the cost of replacement windows, often with a return on investment in under three years. “Window films professionally installed on structurally soundsingle or double-pane windows may reduce energy consumption by as much as 30 percent, while the installation cost may be 91.5 percent less than putting in new windows,” explained Darrell Smith of the nonprofit International Window Film Association. DEAR BRAD: Ah, the joys of working from home: like your cat walking across the keyboard. It's clear that your pets are overjoyed to see you at home all day. They're thinking, "Yes! Dad's here! He didn't leave for hours and hours today! We can play with him all day!" And then they pester you for attention nonstop, right? And when you shut them in the bedroom, they're not happy. "Dad's in there! In the other room! Come let us out so we can play with you!" Like millions of people, your work routine has changed for at least several weeks. That means your pets' routines have changed, too, and you need to establish new ones. First thing in the morning, give Shelley and Mutt lots of attention. Take Mutt out for a walk and play/training session where you reinforce basic commands and let him burn off energy. Indoors, grab a cat toy and play with Shelley for a bit. If you have a scheduled conference call, take Mutt out again for a few minutes or play with him indoors beforehand. This may calm him enough that he can remain in the room. Take frequent five-minute breaks and give attention to your pets each time. Use this remote working as an opportunity to train your pets and spend more time with them. They can provide plenty of stress relief, too -- something we all need right about now.

I do not apologize for my anger; I only apologize for what I said to you. Please be respectful of others on the street.
It’s sad that someone must vent about people leaving the painted rocks in parks. All they want to do is brighten someone’s day. They take the time to do this, which I think is great. Especially now, when hiking is about the only thing that we can do. I can’t wait to find one and pay it forward.
Thanks to all my family and friends who took the time to wish me a Happy 92nd Birthday and for the morning serenade on my yard. I appreciate the thought, and it made my day much more special.
Thank you to the City of Canandaigua for repairing the sidewalks at the lower end of Main Street, near the Native American statue going north to Lindsey’s Nail Salon. This has made walking with wheeled vehicles so much easier as you also removed the roots where the trees were pushing up the sidewalks. Thank you!
To everyone who is able to stay home or is employed at a job where you are able to wash your hands when needed: Have you even thought about the people whose jobs take them out of the house everyday -- into their vehicles or a job site where there is not sink to wash? They rely on the hand sanitizer to effectively prevent illness. Shame on you if you are one of those people who won’t share what you don’t even need because you are home or at a job where you can wash multiple times per day or as needed. Regarding the person who complained that their groceries were not packed properly: I had the same problem, plus the bags were packed too heavy. I solved both problems. I fold my reusable bags and place them in one bag. When I get to the checkout, I place one folded bag on the conveyer and place the groceries I want in that bag behind it. I do the same with the second bag, etc. Produce is with produce, meat with meat, etc., and bags are light. I leave the store a happy person.

To the person who ranted about Painted Rocks littering the parks, when I read it, all I could say is wow! I love to see the painted rocks in the parks, and I do not consider them as litter. Everyone I know smiles when they see them! I agree with you about littering; what you bring into the park should be what you take out with you. But the painted rocks spread joy and happiness, which is what we need more of, especially during this time. Please do not be so grumpy about the painted rocks.

THE SIMILARITIES BETWEEN PASSOVER AND EASTER To the casual observer, Passover and Easter may not to seem to have much in common. Nitrogen is essential to healthy soil, but only at certain levels. When those levels are exceeded, the result can be lawn damage. According to The Spruce Pets, an advisory site that offers practical tips and training advices to pet owners, this is what happens when pets frequently urinate on grass. Urine is naturally high in nitrogen, so when pets urinate on lawns, the grass might turn yellow or brown due to the excess nitrogen content. Nitrogen also is present in lawn fertilizers, further exacerbating the problem for pet owners who fertilize their lawns. In addition to urine damage, dogs can trample frosted grass, contributing to problems that may not become evident until spring, and get into areas like gardens where they wreak additional havoc. READING SUNSCREEN LABELS CAN SAVE YOUR FAMILY’S HEALTH (NAPSI)—Studies show that parents need to exercise caution when choosing the right sunscreen for their kids. The Surprising Problem Many parents might be shocked to find that approximately 70 percent of sunscreens—even those labeled kid-friendly—contain harmful chemicals, the most common of which are oxybenzone, octinoxate, avobenzone and octocrylene. Considered toxic to humans, they’ve been banned in Hawaii and they can kill coral reefs. “Imagine what they can do to children,” says Lisa Palmer, mother of two and co-founder of a reef-friendly mineral sunscreen and skincare line. Compared to chemical lotions, chemical spray-on sunscreens may be even worse. They can be just as toxic, the fumes linger in the air and you can inhale them, and they’re often flammable. Whether you use spray or lotion, within 20 minutes of applying chemical sunscreen, the toxins enter the bloodstream, are detected in urine, have been known to cause cancer and have been shown to potentially result in kidney and liver dysfunction, and nervous system disorders. That said, you can’t just expose your kids to dangerous ultraviolet (UV) rays, either. Overexposure to UV radiation can lead to skin cancer, premature aging of the skin, and inflammations of the eye, points out the World Health Organization, adding a health issue of growing concern is that UV radiation can reduce the effectiveness of the human immune system. How To Save The Skin You’re In “Evidence shows that when kids use sunscreen, we reduce their chances of getting melanoma later in life by 40 percent,” says Palmer. “Using the right sunscreen without any chemicals is the safest thing to do. Many common sunscreen brands claim to be mineral-based, but they still contain avobenzone, a derivative of oxybenzone, which is used as a stabilizer, making the sunscreen just as harmful.” The Environmental Working Group’s website ranks sunscreens so you can see if the one your family uses is safe. TropicSport, made from non-nano zinc oxide and titanium dioxide, key to shielding the skin from the sun’s harmful rays, is at the top. The sunscreen comes in a non-goopy formula that doesn’t leave a ghostly white glow. For best protection from UV rays, use sunscreen every day, regardless of the weather or time of year, apply it 30 minutes before you go out and renew it every two hours and after swimming. Stay Fed, Stay Safe Life has certainly changed recently. Depending on where you live, it might have changed a lot. Stay home if you're a senior. Stay home if you have a medical condition. That's what we've been told. That's all well and good, but what if we really need to shop for food? What if we couldn't stock up last week because the Social Security benefit hadn't made it to our checking account yet? If you're really in a jam for food, call the food bank. Of all the places, that one is most likely to still be open. They're trying to keep people fed, especially seniors, as well as children who don't get free meals because the schools are now closed. Ask if they have a driveup pantry where you can be handed some bags of food through the car window. You might find that food is being distributed at places like the senior center or a school parking lot or via Meals on Wheels, even if you aren't signed up as a participant. If you do dare to enter a store, a number of grocery stores are designating the first hour or two in the morning for seniors only. Not all stores are doing this, so call first. If they don't do it, suggest it. You're more likely to find the things you need during this time as the shelves will (hopefully) be restocked overnight. And with fewer people in the store, you're less likely to run into someone with the virus, but stay away from other people anyway. If you go, be sure to take sanitizing wipes for the cart handles. Remember, if you go out, you're really not safe from the virus. Do it as little as possible.
Do you relish the idea of an afternoon nap? If so, you're in good company. According to the National Sleep Foundation, John F. Kennedy, Winston Churchill, Napoleon, and Albert Einstein each fancied afternoon naps. Youngsters nap, and so do many seniors. But by and large, humans are part of the minority of mammals that do not sleep for short periods of time throughout the day, instead dividing their days between sleep and wakefulness. However, there are some benefits to catching a midday snooze. The Mayo Clinic says napping can be a way to catch up on sleep lost during the night or if a person is feeling sleep-deprived. It also may be a way to relax, increase alertness and improve mood. Naps should take place before 3 p.m. and not exceed 20 minutes. Senior Living during Coronavirus: An Inside Scoop BY AMANDA DUDEK
D
uring this pandemic, many continue to be concerned about one of our most vulnerable populations: our nation’s elderly. With many senior living facilities closed to the public and others even asking staff to relocate to their facilities, the impact these necessary restrictions may have on elderly residents is of growing concern. These senior facilities, like Morgan Estates Assisted Living Community in Geneseo, are doing everything they can to provide extra support to their residents. Victoria Finn is an Activities and Resident Care Assistant at Morgan Estates. “We stopped visitation for the residents’ protection,” she explained. “Now, families come to the window and talk to their loved ones through the phone.” Victoria added that this is particularly hard on residents with conditions like dementia who do not understand why their loved ones are unable to visit. “We have to be very encouraging to them during this time,” she said. “We are all staying positive and being a shoulder to lean on, being there for our residents because this is a really hard time.” That they are and more. Morgan Estates is also trying to keep residents’ schedules as unchanged as possible with creativity and flexibility. For example, Friday night entertainment now consists of movies or live performance videos rather than visiting performers. Residents are still encouraged to go on outdoor walks but are escorted on them one at a time. Resident favorite, Bingo, continues but in the dining room so that everyone has their own table six feet apart, and the cards and materials are thoroughly sanitized. There are new activities, as well. “We are creating a spiritual board for our residents,” shared Victoria. “We asked our residents for their favorite quotes or to share something for which they are thankful. A lot of them said they are thankful they are living here, which was very nice. Some residents shared their favorite Bible verse. We have never done this before, but I think it will be beautiful.” Maintaining family connections, while challenging, remains more important than ever. “We are helping them write cards to their loved ones,” Victoria stated. “Our Activities Director, Danielle Pickett, is hoping to decorate eggs for Easter and leave them on the other side of the window with messages and candy from our residents to their families. We’re doing everything we can to help them reach out to their loved ones.”
Of course, this situation has been challenging for caregivers, as Activities Activiti and nd Resident R id t Care Ca Assistant A istant well. “When we come in, we wash at Morgan Estates, Victoria Finn, our hands thoroughly, have our temperature taken and answer a wearing her face mask. few questions,” shared Victoria. “Everyone wears face masks and gloves constantly, and now, we are not permitted to leave the facility on our breaks [to further limit possible exposure].” Even their after work routines have changed. “When I get home, I go straight in the shower and wash my clothes,” she said emphatically. “The last thing I want is for my family to get it, although that is just an extra precaution.” These extra efforts are not going unnoticed. 92-year-old Morgan Estates resident, Tony Caito, said he feels safer. “I can’t imagine if I were on the outside,” said Tony. “Here, they have taken care of everything to make sure we are as safe as possible.”

They make everything as normal possible and supplement some activities with other activities.” Tony shared an example of the daily activities schedule residents receive. “I didn’t come here to die,” he said. “I came here for a new life, and now, I have people to talk and laugh with.” Tony is also looking forward to a visit from his daughter with whom he also Facetimes. “She will be on the outside,” he clarified. “The window will be closed, and we will be six feet apart.” A retired teacher from Dansville, Tony moved to Morgan Estates just eleven months ago. “I feel this is my home now,” he explained. “I feel safer inside here than if I were outside at my [former] home in Dansville. They come in every day, take our temperature, smile, and we joke back and forth,” he continued. “They are really doing the best they can, and although this is their job, I feel we are like family.” At the youthful age of twenty, Victoria shares a similar sentiment. “In all honesty, it doesn’t feel like work,” she confided. “It feels like a second home here. These residents feel like family, and I would do anything for them. I love it, and I love coming in here.”

When asked what others can do, Victoria immediately exclaimed, “Send cards! A daycare facility sent us cards, and the residents loved them. They had cute, little messages like ‘Stay Strong!’ and ‘Thinking of You!’ These brought big smiles all around.”
