DEAR DR. ROACH: I need some hope. My 24-year-old granddaughter, recently married, came down with ulcerative colitis. Right now, she is getting two Humira injections a month. She has lost so much weight and her diet is so limited. Do things normally get better? Will she ever be able to become pregnant? It has been about four months. She is presently seeing a GI doctor. Should she be under the care of a specialist? -- L.M.O.S. ANSWER: Ulcerative colitis is one of the inflammatory bowel diseases. The underlying cause is not known, but there is a familial predisposition. Women are more often diagnosed, and the leading time of diagnosis is in the 20s. There is a wide range of severity, with some people doing well with topical treatments (suppositories or enemas), some needing oral medication, and people with more severe disease needing much more powerful agents, such as the adalimumab (Humira) your granddaughter needs. Fortunately, those kinds of drugs have approximately a 60% to 70% success rate. Diet is important for people with UC of any severity; however, as the disease becomes better controlled it is likely that she will have a less restricted diet. A GI doctor is a gastroenterologist, the specialist for inflammatory bowel diseases. There are some gastroenterologists who specialize even further, making IBD their area of particular expertise. Pregnancy is certainly possible in women with IBD. However, they should be in remission before attempting pregnancy, and her medications may need to be adjusted for maximum safety. There are many places to get additional information, but I recommend the Crohn’s and Colitis Foundation website as an excellent starting point. It can be found at www.crohnscolitisfoundation.org.

DEAR DR. ROACH: Last year, I started getting Raynaud’s. I can remember from my youth that my father had it, so it didn’t seem too terrible. It’s not just the fingertips that turn white, but also some of my toes are affected. I asked people who know of it or have it how one gets this. What is the cause? No one knows. So I asked my doctor, and even he said he did not know. How do I get these episodes, and what can I do to prevent them? -- R.S. ANSWER: Raynaud phenomenon is an exaggerated response to cold or stress, causing color changes in the skin of the fingers and toes. There is a long list of causes of Raynaud phenomenon. Often, no cause is ever found (in which case it is called primary Raynaud, which just means we don’t know what’s causing it). The most common known causes are the autoimmune rheumatic diseases, especially scleroderma, lupus and Sjogren’s syndrome. Hypothyroidism is an unusual cause, and some drugs can cause it as well. The current thinking is that primary RP is caused by abnormalities in the alpha receptors in blood vessels (alpha receptors respond to adrenaline and similar molecules). Keeping the whole body -- and especially the hands -- warm is the first step. Sudden temperature changes can trigger the effect. Warming the hands in warm water at the onset of an attack can stop it. Anxiety makes it worse, so a positive attitude can really affect this condition. How to practice social distancing during a pandemic Social distancing protocols encourage people to avoid crowds, such as those at concerts and sporting events. As COVID-19 continued to spread across the world, it quickly became evident that the public would have to take drastic measures to slow the transmission. In addition to practicing generalized sanitation and good hygiene, people in some of the hardest-hit clusters were advised to take additional, more aggressive measures. Quarantines and travel restrictions were implemented, and the term "social distancing" became a buzzword. Social distancing involves people keeping a physical distance from each other during disease outbreaks in order to slow transmission rates. Social distancing also is employed to lessen the impact of the disease on the medical care system, which quickly can become overwhelmed with a high number of cases presenting in a short period of time. In best-case scenarios, social distancing also may enable a few people to avoid infection until a vaccine is available. So how can people socially distance themselves? Here are some of the recommendations from the Centers for Disease Control and Prevention, the World Health Organization and other leading health groups.

A Look Back… MOMENTS IN TIME The History Channel --* On March 22, 1765, in an effort to raise funds to pay off debts, the British government passes the Stamp Act. It levied a direct tax on all materials printed for commercial and legal use in the colonies, from newspapers and pamphlets to playing cards and dice. * On March 21, 1804, the Napoleonic Code is approved in France. The Code strengthened the authority of men over their families, deprived women of any individual rights, and reduced the rights of illegitimate children. All male citizens also were granted equal rights under the law, but colonial slavery was reintroduced. * On March 20, 1852, Harriet Beecher Stowe’s anti-slavery novel, “Uncle Tom’s Cabin,” is published. The novel was so widely read that when President Abraham Lincoln met Stowe, he reportedly said, “So this is the little lady who made this big war.”

· Opt out of group events. Steer clear of events, whether meetings, sports games, conferences, and other gatherings where large amounts of people congregate together. · Stick to non-contact greetings. Avoid hugs, kisses and handshakes. Substitute a smile, a wave or a bumping of elbows, instead. · Work from home. Many companies are now equipped to allow employees to work from home all the time or a portion of the time. Businesses can encourage employees to stay home and utilize the internet to get their work done. · Alter shopping schedules. Try to visit stores in the early morning or late at night when they are less likely to be crowded. · Make changes in worship practices. Celebrants may have to make modifications to the way they worship. The Catholic Diocese of Trenton, NJ, recently advised all diocesan churches to halt the distribution of the most precious blood (wine) from communal chalices; encouraged clergy and eucharistic ministers to sanitize their hands before

The lowest layer of the Earth’s atmosphere is the troposphere, which the U.S Environmental Protection Agency notes extends from the Earth’s surface to roughly six miles in altitude. The troposphere is where nearly all human activities occur. Even climbers who make it to the peak of Mt. Everest are still in the troposphere, as Everest is less than six miles high. But anyone who has ever boarded a commercial flight has likely left the troposphere and entered the stratosphere, where the majority of commercial airplanes fly. The stratosphere begins at six miles above the Earth’s surface and extends to about 31 miles above the surface of the planet. The EPA notes that most atmospheric ozone is concentrated in a layer in the stratosphere that’s roughly nine to 18 miles above the Earth’s surface. The ozone layer in the Earth’s stratosphere absorbs some of the sun’s radiation, including the UV and UVB radiation that has been linked to cancer. The third layer of the Earth’s atmosphere is the mesosphere, which extends from 31 miles above the Earth’s surface to roughly 53 miles above the surface of the planet. The height of the next highest layer, the thermosphere, varies due to changes in solar activity. he Churchville community responded generously to the fundraising efforts of students at Churchville Elementary School (CES) with donations for the American Heart Associationís Kids Heart Challenge totaling over $19,000. The school has been participating in AHA Heart Events since 2001, raising an astounding amount of more than $157,000 for the organization. Students celebrated this year’s success on Feb. 14, 2020 with a school-wide assembly honoring their top student fundraisers: Analise ($517), Lilly ($698) and Carlene ($750). Carlene was also honored with the title of Principal for the Day. The winning fundraising class will enjoy a heart healthy snack party, courtesy of Wegmans. Program coordinator, physical education teacher Bethany Matsko, said, “The entire school participated in the healthy heart challenge during PE classes, with special races and exercises. More than 140 students volunteered to raise donations. Our kids here at CES want to stay healthy and make a difference for others in their community.”

Students also had a special incentive for reaching their goal this year: the promise of classic pie-in-the-face for several teachers, including Matsko, PE teacher Shayne Sudol, kindergarten teacher Amy Francis, Assistant Principal Renee Mulrooney and Principal David Johnson.

As the moment of truth approached, the gym was filled with anticipation and the happy sounds of hundreds of students chanting, “Pie in the face! Pie in the face!” As lucky student representatives shoved the whipped cream pies into each nervous adult face, the audience laughed and applauded: Not a bad reward for all their hard work. More than 3,000 schools nationwide participate in the AHA Kids Heart Challenge program. The events are fun, promote sports and fitness, educate students in the risks of heart disease and stroke, and help lay the groundwork for life-long community activism and volunteering. Churchville-Chili Central School District schools and students have been supporters for many years. The Churchville community takes the annual Hoops for Heart event very seriously. The CES school record for a year stands at $21,350 and the school has consistently placed in the top echelon of the state’s fundraising schools.

Spring is when you feel like whistling even with a shoe full of slush. ~Doug Larson FLU-FIGHTING FACTS (NAPS)—The flu is nothing to sneeze at. According to the Centers for Disease Control and Prevention (CDC), influenza has resulted in between 9.2 million and 35.6 million illnesses annually since 2010. The best way to stay out of such statistics, the CDC says, is for everyone who is eligible and at least 6 months old to get a flu shot as soon as the vaccine becomes available. To help, all CVS Pharmacy and MinuteClinic locations have the vaccine available seven days a week with no appointment needed. Study Shows That may be just as well, since the annual survey, conducted online by Harris Poll on behalf of CVS Pharmacy [1], found that two-thirds of those surveyed get a flu vaccine every year or plan to get one this year, a 5 percent increase from last year. One reason so many are flocking to get the shot is the knowledge of last year’s flu season—which according to the CDC had flu levels that were the highest observed since 2009. Twenty-two percent of those surveyed[2] say that knowledge of last year’s severe flu season makes them more likely to get a flu shot this year. On a similar note, among the 27 percent of parents of children under 18 whose child did not get the flu vaccine last year, 26 percent say they are more likely to get their child vaccinated this year after learning about the notably high flu levels during last year’s flu season. The percentage who get vaccinated at a pharmacy stayed at 22 percent, but the percentage of employed adults who get vaccinated at their workplace fell from 22 percent to 15 percent while those who got vaccinated by a health care professional rose from 44 percent to 48 percent. The survey also found that doctors’ opinions (39 percent), where insurance is accepted (38 percent), and convenience (33 percent) rank among the top three factors in determining where to go for the shot. The flu vaccine is a preventive service under the Affordable Care Act, so it’s fully covered and available at no cost through most insurance plans, including Medicare Part B. Nurse’s Advice “Surprising to many, it can actually take up to two weeks for the flu vaccine to build immunity,” explained Angela Patterson, Chief Nurse Practitioner Officer at MinuteClinic. “Because of this, CVS Pharmacy recommends patients get their flu shot as soon as the vaccine becomes available to ensure patients are the most protected before flu season peaks. Furthermore, influenza strains tend to change each year, so it’s very important to get vaccinated annually to make sure you are protected.” What Else You Can Do Here are four suggestions: 1. Stay home if you’re sick. The survey found that two out of three respondents would still go to work if they had flulike symptoms. In fact, flulike symptoms haven’t stopped some people from going out in public in general: • 28 percent have gone to the supermarket • 12 percent have picked up a child from school or day care • 10 percent have taken public transportation. 2. Wash your hands often with soap and water. If soap and water are not available, use an alcohol-based cleansing gel. 3. Avoid touching your eyes, nose and mouth. Germs spread this way. 4. Clean and disinfect surfaces and objects that may be contaminated with germs. STRAWBERRY-GLAZED FRENCH TOAST WITH SWEETENED SOUR CREAM Serves 4 • • • • • • • • • • 1.

8 1-inch-thick bread slices, cut from a country or peasant loaf 2 cups half-and-half 4 egg yolks 3 tablespoons light brown sugar 1 tablespoon vanilla extract, plus 1/2 teaspoon 1/2 teaspoon cinnamon 3 tablespoons unsalted butter 1/2 cup sour cream 1 tablespoon granulated sugar 1/3 cup strawberry jam or preserves (see note 2) Arrange a rack at center position and preheat the oven to 400 F. Place the bread slices on a baking sheet and bake until dry and very lightly browned, about 8 minutes per side. Watch carefully so that the bread does not burn. Remove the bread from the oven and reduce the oven temperature to 200 F. 2. In a medium bowl, whisk together the half-and-half, egg yolks, brown sugar, 1 tablespoon vanilla, and cinnamon. Pour the mixture into a shallow pan (a 9-by-13-inch glass baking dish works well). Add the

toasted bread slices and soak them 4 minutes per side. Remove to a large plate or platter. 3. Place a large, heavy frying pan over low to medium heat. Add about 2 teaspoons of the butter, or enough to coat the bottom of the pan lightly. When melted, add enough bread slices to fit comfortably in a single layer. Cook slowly until the slices are golden brown and crisp on both sides, about 4 minutes per side. Remove to a baking sheet and place in the warm oven. Repeat, adding more butter to the pan as needed until all the bread slices have been sautéed. 4. In a small bowl, stir together the sour cream, granulated sugar and remaining 1/2 teaspoon vanilla. 5. When ready to serve, spread each toast with a thin coating of strawberry jam and top with a dollop of sweetened sour cream. Note 1: The best bread for this dish is an unsliced loaf of good-quality peasant or country bread, preferably one without an extra-hard crust. One that is rectangular, rather than round, is more convenient, but either will do. Cut off the ends of the loaf, and reserve for another use. Then slice the bread into 1-inch-thick slices. If your loaf is large and the slices seem large, cut them in half. Note 2: You can try other jams, preserves or marmalades. Cherry, raspberry or peach preserves and orange marmalade are other possibilities.

My son is heavily into collecting basketball cards, but we have not been able to find any. The shelves are always bare, and the stock man said that grown men are following him and buying up all the cards for eBay value. Thanks. Thursday the 12th, we had a service call with Spectrum because of difficulties we were experiencing. I was very hesitant because it was quite bothersome not having internet and phone service. I didn’t know what to expect. We were at our wit’s end. Well, I was very pleased with the TQA representative they sent to our home. Ryan was right on spot, courteous, and did indeed find the problems. He rectified them, explained what was going on, and I am very pleased and satisfied. He renewed my faith in Spectrum.
