‘Cotton Ball’ Sensation Is a Sign of Neuropathy ---
DEAR DR. ROACH: I’m a 63-year-old male diagnosed with chronic myelogenous leukemia, Type 2 diabetes and coronary heart disease. I am 5 feet, 11 inches and weigh 240 pounds. I have two stents and take Sprycel for the CML. My levels are good, and the coronary disease is under control. For a few years I’ve had what feels like “cotton balls” under the skin in the area of the balls of my feet. Now it feels like it’s migrating to the arch. While not too uncomfortable, it is almost impossible to walk barefooted. Some type of foot covering is needed to keep from noticing the feeling. My doctor says it’s the diabetes, and to lose weight or see a podiatrist. Other than losing the weight, do you have any opinion about this? -- N.M. ANSWER: Cotton-ball sensation is one way that many people describe the beginning of neuropathy, a general term for a variety of different conditions that affect the nerves of the body. In a person with cancer (like CML, chronic myelogenous leukemia, a cancer of the bone marrow) and a new symptom, it is always wise to consider whether it could be due to the cancer, the treatment for the cancer or from something else. In your case, usually CML is not associated with neuropathy. However, the medicine you are taking, dasatinib (Sprycel), has been associated with a severe kind of reversible neuropathy. I read the case reports of this condition carefully, and I doubt that is what is going on with you, as the symptoms you have are different from those reported. People with longstanding diabetes frequently develop a particular disease of the nerves, diabetic neuropathy. This almost always begins with sensation changes in the feet. I agree with your doctor that diabetes is the most likely cause. Seeing a podiatrist is absolutely a good idea. He or she can test you for this condition (usually a careful physical exam will make the diagnosis), in addition to looking carefully for any early signs or risk factors for injury or infection. You also should learn how to do a daily inspection of your feet and recognize the signs, so you can get to medical attention promptly should any damage to your feet occur. The podiatrist will make sure you have appropriate footwear. Many people with diabetes benefit from weight loss, but the important thing is to be sure your diabetes is under good control. This is achieved through good diet, regular exercise and medication, if needed, in addition to weight loss in people who are overweight. ***
DEAR DR. ROACH: Due to cancer, my prostate was removed about seven years ago. My PSA reading has increased since 2012 from 0.09 to 0.304. Should I be concerned? I am 78 years old and in relatively good health. -- B.D. ANSWER: PSA readings can go up and down, but a threefold increase could mean that the cancer is coming back, and you should see the specialist taking care of your prostate cancer to discuss getting additional testing. Not having disease for seven years after treatment is a good sign, but the PSA test is pretty specific for cancer recurrence if two separate readings are above 0.2. You may need additional therapy, but only further investigation will show how concerned you need to be. *** Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. ADVICE FOR BLOOD DONORS Unlike other marvels of modern medicine, quality blood is an important health component that cannot be created synthetically. Blood shortages continue to plague medical establishments across the globe. For those who require transfusions, blood from volunteer donors can be the difference between life and death. However, many people are apprehensive about donating blood because of their own unique fears of the unknown or from past poor experiences with the process. With a little knowledge and some preparation, donating blood can be made much easier. Confirming donors’ health: One of the first steps to donating blood is to determine donors’ eligibility. Potential donors undergo a short health screening and are asked various questions to determine their eligibility. The presence of certain illnesses or other disqualifications helps ensure that the blood will be healthy and that the blood drawn will not have a negative effect on the donor. The American Red Cross states that potential donors will undergo a pin prick on the finger to test for iron levels in their blood. This is to ensure that donors will not be made anemic from giving blood. Frequency of donation, weight and age also will be considered before prospective donors are allowed to donate blood. Many organizations urge prospective donors to avoid giving blood if they have had simple dental work done within 24 hours or major dental work in the past month. Dental work in general can dislodge bacteria, which can get into the blood stream and cause systemic infection. Preparing to donate: Potential donors are urged to eat a healthy diet full of iron-rich foods. It is adviseable to drink an extra 16 ounces of water or a nonalcoholic fluid prior to donating. A healthy, low-fat meal prior to donating also is recommended, as the Red Cross says fatty foods can adversely affect blood tests that check for infectious diseases, and the blood may not be used as a result. Needles are no big deal: According to Justin Kreuter, MD, medical director of the Mayo Clinic Blood Donor Center in Rochester, Minnesota, while the needles used to collect blood are slightly larger than those used for immunizations at the doctor’s office, the needles have silicone on them to make the process of donating more comfortable for donors. Donation duration: Donating blood does not take a long time. It may take between 15 and 20 minutes for donation center workers to extract blood. Donating plasma or platelets may require more time. Allocate at least an hour to undergo the medical exam and to rest after the donation. Post-donation: Donors are urged to rest for a few minutes after the donating and to enjoy the snacks and beverages being offered. The Red Cross says people should avoid performing any heavy lifting or vigorous exercise for the rest of the day after donating. It’s also adviseable to drink an extra four glasses of liquids and avoid alcohol during the day following a donation. Donating blood can save lives and reassure donors that they’re doing their part to help others.

STUDY SHOWS GIFT CARDS CAN GIVE A BOOST TO SMALL BUSINESSES (NAPSI)—If you own or run any of America’s nearly 31 million small businesses, recent research reveals gift cards can be a real gift to your bottom line. According to a 2019 Small Business Gift Card Study commissioned by First Data, now Fiserv, gift card programs help small business owners bring in more sales, attract new customers, and build relationships with their customers.

In fact, 74 percent of consumers surveyed said they regularly buy gift cards from small businesses. And for business owners, offering gift cards can: • Grow sales—Nearly 80 percent of consumers said if they have a gift card valued at 88 cents, they will visit the store to redeem it • Attract customers—90 percent of consumers who receive a gift card from a small business they have never visited said they would shop at that business and return in the future • Build loyalty—56 percent of consumers who shop at small businesses join their loyalty or frequent shopper programs. Of consumers that join loyalty programs at small businesses, 73 percent said gift cards are a preferred way for their loyalty to be rewarded. Restaurants, Coffee Shops, and Salons When asked about the ideal gift card to give someone, or receive and use themselves, consumers agreed on their top three: casual dining restaurants; coffee shops; personal services (i.e., hair or nail salon). Other popular businesses from which to give or receive gift cards included supermarket/ grocery stores, entertainment/movie theaters, and quick-serve restaurants. Employee Incentives Gift cards are often a popular choice for employers who use them to say thank you or reward a job well done. For 90 percent of consumers, they’ve not only received a gift card from their employer, but view it as an incentive to reward their hard work. Additional Findings • When purchasing gift cards, 60 percent of consumers prefer plastic cards or paper certificates, but interest in digital gift cards is growing. • 58 percent of small business owners said plastic gift cards cost the business owner less to manage than paper certificates. Manage Your Blood Pressure And Protect Your Kidneys (NAPSI) - Did you know if
you have high blood pressure you are at increased risk for chronic kidney disease? High blood pressure, also called hypertension, is the second leading cause of chronic kidney disease in the United States. About 1 in 5 adults with high blood pressure may have chronic kidney disease, according to the Centers for Disease Control and Prevention. Blood pressure is the force of blood pushing against the walls of your blood vessels. If your blood pressure gets too high, the blood vessels in your body‚ including those in your kidneys‚ may become damaged. This damage makes it harder for the kidneys to filter blood and remove wastes and extra water from the body. Kidney disease can get worse over time, and if not treated it can lead to kidney failure.And while high blood pressure can lead to kidney disease, the reverse is also true: kidney disease can lead to high blood pressure. Many people with kidney disease don’t know they have the disease until their kidneys begin to fail. Research suggests that fewer than 1 in 10 people who have kidney disease are aware they have the disease. This is because kidney disease often doesn’t have any symptoms early on. The good news is that you can help protect your kidneys by managing high blood pressure with healthy lifestyle habits.

Carter answered calls from all over the country from his desk in the Oval Office. Approximately 9 million calls flooded the CBS radio studio during the two-hour broadcast. On March 6, 1899, the Imperial Patent Office in Berlin registers Aspirin, the brand name for acetylsalicylic acid, on behalf of the German pharmaceutical company Friedrich Bayer & Co. In its primitive form, the active ingredient, salicin, was used for centuries in folk medicine. On March 7, 1876, 29-year-old Alexander Graham Bell receives a patent for his revolutionary new invention -- the telephone. Bell’s patent filing beat a similar claim by Elisha Gray by only two hours.

Blood pressure medicines often play a key role in lowering blood pressure. Aim for a healthy weight. If you are overweight or have obesity, reducing your weight may lower high blood pressure. Select healthier food and beverage options. Follow a healthy eating plan that focuses on heart-healthy foods such as fruits, vegetables, whole grains, and lean meats that are low in sodium. Stop smoking. Smoking damages blood vessels, increases your risk for high blood pressure, and worsens problems related to high blood pressure. For help quitting, call 1-800-QUITNOW or go to Smokefree.gov. Get enough sleep. Lack of sleep can have an adverse effect on your blood pressure and metabolism. Aim for 7 to 8 hours of sleep each night. Manage stress and make physical activity part of your routine. Healthy stress-reducing activities and regular physical activity can lower blood pressure. Try to get at least 30 minutes or more of physical activity each day. “You can manage your blood pressure and its complications through healthy lifestyle habits, which include exercise, following a healthy eating plan and taking blood pressure medication as prescribed by your doctor,” says Gary H. Batavia’s Paul & Bridget Ohlson Celebrate 20 Years of Care-a-Van By Amanda Dudek
W
hile many people have deeply held religious beliefs, some of them may feel uncomfortable entering a church. For Paul Ohlson, this was an opportunity to serve. Care-a-Van, located in Batavia, brings support and spiritual encouragement out of buildings and onto the streets. In 2000, Paul Ohlson and his wife, Bridget, launched Care-a-Van with a motor home and some food. “We decided we were going to help people out with groceries,” said Ohlson. “We started parking up on State Street on Saturdays.” “In 2000, no one knew who we were,” Ohlson recalled. “A lot of people came out of curiosity to find out what we were about. Care-a-Van has never missed a Saturday in twenty years,” continued Ohlson. “It doesn’t matter if there are snowstorms or pouring rain. We go out even when the mailman won’t!” Care-a-Van grew quickly after this, adding more vehicles and outreach activities. “In 2003, we incorporated and became our own 501c3,” shared Ohlson. “In 2007, an old transit bus was donated to us, and we began going out and offering morning prayer and coffee.” It was also in 2007 that Ohlson left his career of 27 years to lead Care-a-Van fulltime. “It has been 13 years,” he said. “God has provided for us like we haven’t missed a beat.” This life-changing decision freed Ohlson to dedicate himself to being Committed, Consistent and Faithful to the ministry and community. Now with five vehicles, Care-a-Van reaches out to the Batavia Community in more ways than ever. It currently serves the City of Batavia with Saturday morning groceries and prayer, weekday morning bus coffee and prayer, summertime cookouts every Thursday with short teachings, worship and prayer and cookouts at community events, including Caring for the City Day which offers free dental care from Dr. Russ Marchese. Care-a-Van also cares for Paul Ohlson and his wife, Bridget community members during Ohlson, Care-a-Van founders. the holiday season. “We hold a Halloween night outreach,” explained Ohlson. “For Thanksgiving, we provide holiday food packages for people. We also create Christmas gift bags, each of which contain 28 items, for 350 seniors at eight different senior centers in Batavia. We sing Christmas Carols, tell the Christmas story and in the spring, we make strawberry shortcake for these seniors, as well. For 19 years, Care-a-Van has offered groceries from a different location each Saturday of the month: Blondies Ice Cream the first, Liberty Square the second, the old town firehall across from K-Mart the third, St. Paul’s Lutheran Church on Washington the fourth and if there is a fifth Saturday, Northside Meadows. “People can always call 585-343-0328 to listen to our message which tells where we will be that week,” shared Ohlson. However, for Paul Ohlson, Care-a-Van is about a lot more than food. “We have been developing relationships for 20 years,” he said passionately. “Everyone gets prayed for. God cares, and so do we.” The community is responding. “People thank us; they know who we are,” said Ohlson. “Our vehicles are our buildings, and people are familiar with them. People

are always waiting when we pull up. Some of them aren’t even looking for food. They just want to talk and get prayer. Some ask us to officiate funerals for their loved ones, too. After 20 years, people start to listen to what you are saying.” About 80% of the support for Care-a-Van comes from individuals who have seen the consistency and unconditional nature of the work they do for Batavia’s communities, the other 20% comes from churches and businesses. “We are bringing the Gospel to the streets,” said Ohlson. “We are a 100% street ministry. We are where the people are.” Ohlson also thanks dedicated volunteers. “We have really good volunteers who are so appreciated,” he praised. “They put just as much into this ministry as I do.” Ohlson also shared that volunteers and those looking to complete community service are welcome to work with them, as well. Care-a-Van continues into 2020, thanks to the generosity of those who see its impact. “Our original bus wound up in the scrapyard this past August,” shared Ohlson. “An older S&S limo was anonymously purchased for us. God has always provided.” Still, there is one Care-a-Van vehicle which Ohlson would like to see put to use much more often: the mobile church. “I would love for churches to start using it to take the Gospel to the streets,” Ohlson enthused. “A church preached off the back of it once, and that was cool. Professionals can work on different projects throughout the day, but allot time for each project rather than trying to work on several at once. Schedule each day. Establishing a schedule and making it as accessible as possible is another way to stay organized and increase productivity. Professionals can make use of scheduling apps on their tablets or smartphones, even setting alerts so they do not forget about projects or meetings. Routinely check the schedule throughout the day, marking off tasks as they’re completed. Cut back on time spent in meetings. A 2014 survey from AtTask conducted by Harris Poll reported that American workers at companies with 1,000 employees or more spend just 45 percent of their workdays tending to their primary job duties. Survey participants reported spending 40 percent of their time in meetings, tending to administrative tasks and dealing with interruptions. In lieu of in-person meetings, professionals can discuss projects via mass emails, which can be a much more efficient means to organizing and discussing a project than sitting in a room several times per week. Take breaks throughout the workday. Busy professionals may feel as though they don’t have time for breaks during a typical workday. But such breaks can benefit workers in ways they might be unaware. For example, the Association for Psychological Science notes that research has shown that building breaks into a workday helps professionals stay sharp and productive. And when workers take their breaks matters, too. Researchers at Baylor University discovered that mid-morning breaks were more beneficial than late afternoon breaks. Staying organized and increasing productivity at work go hand in hand. Professionals tasked with juggling multiple responsibilities can employ various strategies to be better organized and get more done in less time.

Put a healthy spin on an old family favorite Meatloaf is a dinnertime staple for many families. A favorite of adults and children alike, meatloaf can be prepared however cooks see fit, making it a versatile dish for family dinners. Parents who want to enjoy meatloaf with their families and do so while promoting healthy diets can try the following recipe for “Turkey, Vegetable & Oat MiniMeatloaves with Marinara Sauce” from Michelle Dudash’s “Clean Eating For Busy Families” (Fair Winds).
Turkey, Vegetable & Oat Mini-Meatloaves with Marinara Sauce Serves 6 Expeller-pressed canola oil spray 1 8-ounce package crimini (baby bella) mushrooms (about 21/2 cups) 1 small yellow onion, cut into eighths 4 cloves garlic, minced 1 tablespoon extra-virgin oil 1 cup dry rolled oats 2 large eggs 1 pound lean ground turkey (or beef or bison) 13/4 cups pasta sauce, divided 3 tablespoons red wine vinegar 1 tablespoon Italian herb seasoning 1/2 teaspoon salt 1/2 teaspoon freshly ground black pepper Preheat oven to 375 F. Coat a 9 x 13-inch baking pan or 6 mini loaf pans with spray. Pulse mushrooms in a food processor until finely chopped and add them to a large bowl. Repeat with onion and garlic. Place a large frying pan over medium heat and add oil. When oil is shimmering, add vegetables and sauté for 7 minutes or until water releases and evaporates completely, lowering the heat as necessary. Set aside. Process the oats until they are of a fine consistency. Whisk the eggs in the bowl used for the vegetables. Add processed oats, turkey, 3/4 cup of pasta sauce, cooked vegetables, vinegar, Italian seasoning, salt, and pepper and stir together with a fork until blended. Scoop the mixture into 6 mounds on the pan or into each mini-loaf pan, about 3/4 cup each. Shape each mound into a 4 x 2-inch loaf. Spread remaining 1 cup of pasta sauce on top of the loaves, distributing evenly. Bake on middle rack for 25 minutes, turning pan midway through cooking, until loaves are firm or a thermometer inserted in the middle reads at least 165 F. Allow loaves to rest for 5 minutes before serving. Recipe Notes: You can finely chop vegetables with a knife rather than using a food processor if you prefer. Whole oats can be added to the meat mixture for a more rustic texture.
