THE OPTIC NERVE The optic nerve is a bundle of more than 1 million nerve fibers connecting the retina to the brain. How does the optic nerve get damaged by open-angle glaucoma? Several studies have shown that eye pressure is a major risk factor for optic nerve damage. In the front of the eye is a space called the anterior chamber. A clear fluid flows continuously in and out of the chamber and nourishes nearby tissues. The fluid leaves the chamber at the open angle where the cornea and iris meet. When the fluid reaches the angle, it flows through a spongy meshwork, like a drain, and leaves the eye.

G

laucoma is a group of diseases that can damage the eye’s optic nerve. It is a leading cause of blindness in the U.S. It usually happens when the fluid pressure inside the eyes slowly rises, damaging the optic nerve. Often there are no symptoms at first. Without treatment, people with glaucoma will slowly lose their side vision. They seem to be looking through a tunnel. Over time, straight-ahead vision may decrease until no vision remains. However, with early detection and treatment, you can often protect your eyes against serious vision loss. A comprehensive eye exam can tell if you have glaucoma. There is no cure, but glaucoma can usually be controlled. Early treatment can help protect your eyes against vision loss. Treatments usually include prescription eye drops and/or surgery.

In open-angle glaucoma, even though the drainage angle is “open,” the fluid passes too slowly through the meshwork drain. Since the fluid builds up, the pressure inside the eye rises to a level that may damage the optic nerve. When the optic nerve is damaged from increased pressure, open-angle glaucoma—and vision loss—may result. That’s why controlling pressure inside the eye is important. Another risk factor for optic nerve damage relates to blood pressure. That’s why it is important to also make sure that your blood pressure is at a proper level for your body by working with your medical doctor. Would I develop glaucoma if I have increased eye pressure? Not every person with increased eye pressure will develop glaucoma. Some people can tolerate higher levels of eye pressure better than others. Also, a certain level of eye pressure may be high for one person but normal for another. This level is different for each person. That’s why a comprehensive dilated eye exam is very important. People at risk should get eye exams at least every two years.


Is it possible to develop glaucoma without an increase in my eye pressure? Yes. Glaucoma can develop without increased eye pressure. This form of glaucoma is called lowtension or normal-tension glaucoma. It is a type of open-angle glaucoma. Who is at risk for open-angle glaucoma? Anyone can develop glaucoma. Some people are at higher risk than others: • African Americans over age 40 • Everyone over age 60, especially Mexican Americans • People with a family history of glaucoma How is glaucoma detected? Glaucoma is detected through a comprehensive dilated eye exam that includes the following: • Visual acuity test. This eye chart test measures how well you see at various distances. • Visual field test. This test measures your peripheral (side vision). It helps your eye care Continued on page 14

Understanding the Affordable Care Act by Rodney Hurt, Rodney Hurt Insurance Services

T

he Patient Protection and Affordable Care Act (ACA) or “Obamacare” is a United States federal statute signed into law on March 23, 2010. This piece of legislation represents the most significant regulatory overhaul of the U.S. healthcare system since the passage of Medicare and Medicaid in 1965 and it has been highly controversial since being signed into law. On June 28, 2012, the United States Supreme Court upheld the constitutionality of the ACA’s individual mandate as an exercise of Congress’s taxing power. Since the ruling, the law and its implementation continue to face challenges in Congress and from certain state governments, conservative advocacy groups, and some small business organizations. The ACA includes numerous provisions that take effect between 2010 and 2020. Policies issued before 2010 are exempted by a grandfather clause from many of the changes to insurance standards, but they are affected by other provisions. Significant reforms, most of which take effect by January 1, 2014 include: • Guaranteed issue prohibits insurers from denying coverage to individuals due to pre existing conditions, and a partial community rating requires insurers to offer the same premium price to all applicants of the same age and geographical location without regard to gender or most pre-existing conditions (excluding tobacco use). • Minimum standards for health insurance policies are established. • An individual mandate requires all individuals not covered by an employer sponsored health plan, Medicaid, Medicare, or other public insurance programs to secure an approved private-insurance policy or pay a penalty. The


law does include subsidies to help people with low incomes comply with the mandate. • Health insurance exchanges will commence operation in every state. Each exchange will serve as an online marketplace where individuals and small businesses can compare policies and buy insurance. Policies purchased through the exchange will commence coverage on January 1, 2014. Also available to all applicants is the opportunity to work with their local health insurance agent to purchase these very same products. In reality, the only people who are required to purchase through an exchange are those who are financially eligible for a government subsidy. All others can purchase policies through the agents who have taken the required study and testing to qualify to sell ACA compliant insurance coverage. And, the premiums are the same whether an agent or broker is involved or not. • Low income individuals and families whose incomes are between 100% and 400% of the federal poverty level will receive federal subsidies on a sliding scale.

• Businesses which employ 50 or more people but do not offer health insurance to their full-time employees will pay a tax penalty if the government has subsidized a full-time employee’s healthcare through tax deductions or other means. This is commonly known as the employer mandate. In July 2013 however, this provision was unilaterally delayed for one year. The delay of this provision only sharpened the rhetoric of those opposed to “Obamacare.” On the face of it all, the ACA carries some provisions that would ideally help many Americans. Those people who have not been able to obtain health insurance from an insurance carrier due to their pre-existing condition can now subscribe with no health questions asked. However, it does make one wonder how to build the pool of funds large enough to pay for all the additional claims. Increased premiums must surely be just down the road. Pets of the Permian Basin A by Joy Harriman

cross the United States a staggering three to four million healthy, adoptable pets are euthanized every year at shelters. Still others wander the streets, sick and hungry, given up by the families that once loved them and spent every waking minute playing with them during the holidays. By spring, their names will be lost. Their home will become a small space surrounded by cement with a steel fence.

These animals are a part of the Christmas syndrome: cute puppies and kittens bought as presents for loved ones on Christmas that end up in shelters by the next year. In the Permian Basin shelters are overflowing with cats and dogs for those and other reasons. While locals do adopt, the growing number of abandoned animals is unsettling. Why do we have an alarming number of animals who are abandoned, unwanted, and euthanized each year? How can we help them find their way to warm, loving homes? Last year, the American Humane Association’s Animal Welfare Research Institute released the results of the first phase of a three-part study to better understand pet ownership and discover the most effective ways to ensure animals find their

forever homes. The study found that there are many barriers to pet ownership including the costs, perceived lack of time to care for an animal, outright dislike of certain pets, and lasting grief over the loss of a previous pet. Despite big public education encouraging people to adopt from shelters, fewer than one quarter of previous dog owners (22%) and one fifth of cat owners (18%) adopted their prior pet from a shelter or rescue organization.

Before adopting, we encourage people to recognize that this is a lifetime commitment.

Other research has found that more people abandon their pets as they are unable to find rentable apartments or homes that accept animals. In the last five years, Texans have seen a 56% increase in the number of people giving their dogs to a shelter due to housing issues. T

he arrival of the New Year heralds a time of reflection, that yearly self-assessment of what we can do to become better people. Not surprisingly, after weeks of excess eating, most individuals put losing weight and getting healthier at the top of their New Year’s resolutions. Keeping that resolution is a little harder. Two resolute Odessa Family Y members share their inspiring journey—detours included—how they achieved successful weight loss. JACLYN GAONA Before/After Weight: 185/145 Dress/Pant Size: 14/6 Jaclyn Gaona has always enjoyed an active lifestyle. The fitness enthusiast regularly hiked, ran, and participated in high energy sports such as racquetball and basketball. For Gaona It seemed effortless to stay fit and healthy.

After the second baby, Gaona was even more determined to keep fitness a part of her routine. The new mom decided to try working out at home with Zumba DVDs. While using the videos, Gaona accidently reinjured the knee. “I told my doctor now that I had two babies, surgery was not an option. I did physical therapy instead. I was very angry. I was stripped of any kind of aerobics.”

Then life dramatically changed after a skiing accident. Gaona was side-lined with several knee surgeries. Not long after, she welcomed the birth of her first child. Seventeen months later, baby number two arrived.

Then Gaona had an epiphany. “I decided to invest in a personal trainer to focus on prevention of injury rather than the reactive approach I was taking. It was the best decision I made.”

“It was no big deal to stay fit and healthy before kids. I could go to the gym mornings and evenings. I had more freedom.”

Porras for over a year and a half. In the process she has succeeded in recapturing what she feared was lost. “I love to work out and hated that I was living in a limited way. I was afraid of reinjuring the knee. She monitors my form and knee alignment, with Judie I got back to being the Jackie I was.” Over a 17 month period, Gaona lost the postbaby weight and gained back her confidence and physical strength. “Judie has been a godsend. She plans my workouts. She sets my weights up. I can’t dread what the workout will be. I just do what she tells me to do. I love it!” Occasionally the thirty-five year old has days when motivation wanes. “Frankly, some days the only reason I show up is because I have an appointment with Judie. I know she is waiting for me. It keeps me accountable.” If you could give advice to someone who is just starting out, what would it be? “Get a trainer! Judie motivates me when I need it. I also feel that because she is a wife and mother she understands my stresses and empathizes with my roadblocks. It’s time consuming, but so is everything that is worth having.” NEYELI DOMINGUEZ Before/After Weight: 298/236 (still in progress!) Dress/Pant Size: 22–24/14 Neyeli Dominguez struggled with extreme fatigue. Always tired, the twenty-seven year old felt she was in a nightmare she couldn’t wake up from. She had no energy for herself or her family. “I felt I had the weight of the world on my shoulders. I was so tired all the time. I knew something was wrong with me.” Dominguez went to the doctor and was diagnosed with fibromyalgia and was prescribed medication that could possibly lead to weight gain. Already overweight, she refused to take it and two months later decided to take action to lose weight. Sticking to a fitness plan past three months had always been a challenge for Dominguez, and this time was no exception. She too trained with Porras. Tears mixed with sweat trickled down her face as she pushed through burpees and push-ups.

Silently she prayed “I can do all things through Christ who strengthens me.” “I don’t think Judie knew I was crying. I wanted to quit. It was hard. I felt like I wasn’t strong enough. I felt like I couldn’t do it.” Then Dominguez reminded herself why she was there. “Every time I would hit a plateau I would quit in the past. I kept going. I was going to break the plateau this time. It’s all mental.”

Th r e e months later, an overjoyed Dominguez found proof of her success. “I have a fitness watch that tracks my activities and heart rate. When I first got the watch it fit on the Neyeli After second hole. At six months it was fitting on the sixth hole.” In nine months Dominguez lost 62 pounds. February will mark her one-year anniversary of adopting a healthy lifestyle. “I don’t think I really had fibromyalgia, I was just overweight. Honestly, I don’t have a goal weight in mind. I just want to be healthy, toned, and fit.” Having awakened from the fog of chronic fatigue, Dominguez reveled in her newfound energy. The change was obvious to her, but it was a comment from her 7-year-old son that stunned the mother. Continued on page 16 an apple a day january 2014 13


Glaucoma medicines need to be taken regularly as directed by your eye care professional. Most people have no problems. However, some medicines can cause headaches or may cause stinging and redness in the eye. Many medicines are available to treat glaucoma so if you have problems with one medicine, tell your eye care professional. Treatment with a different dose or a new medicine may be possible.

A comprehensive eye exam can tell if you have glaucoma. Early treatment can help protect your eyes against vision loss.

Because glaucoma often has no symptoms, you may be tempted to stop taking or may forget to take the medicine. You need to use the drops or pills as long as your physician asks you to. Regular use is very important. Make sure your eye care professional shows you how to put the drops into your eye.

Continued from page 7 professional tell if you have lost peripheral vision, a sign of glaucoma. • Dilated eye exam. In this exam, drops are placed in your eyes to widen, or dilate, the pupils. Your eye care professional uses a special magnifying lens to examine your retina and optic nerve for signs of damage and other eye problems. After the exam, your close-up vision may remain blurred for several hours. GLAUCOMA TREATMENTS Immediate treatment for early-stage, open-angle glaucoma can delay progression of the disease. That’s why early diagnosis is very important. Glaucoma treatments include medicines, laser trabeculoplasty, conventional surgery, or a combination of any of these. While these treatments may save remaining vision, they do not improve sight already lost from glaucoma. Medicines, in the form of eye drops or pills, are the most common early treatment for glaucoma. Taken regularly, these eye drops lower eye pressure. Some medicines cause the eye to make less fluid. Others lower pressure by helping fluid drain from the eye. Before you begin treatment, tell your eye care professional about other medicines and supplements that you are taking. Sometimes the drops can interfere with the way other medicines work. 14 january 2014 an apple a day

Laser trabeculoplasty helps fluid drain out of the eye. In many cases, you will need to keep taking glaucoma medicines after this procedure. Laser trabeculoplasty is performed in your doctor’s office—the laser makes several evenly spaced burns that stretch the drainage holes in the meshwork, allowing the fluid to drain better. Like any surgery, laser surgery can cause side effects, such as inflammation. You may have some drops to take home for any eye soreness or inflammation. You will need to make several follow-up visits to have your eye pressure and eye monitored. If you have glaucoma in both eyes, usually only one eye will be treated at a time. Laser treatments for each eye will be scheduled several days to several weeks apart. Conventional surgery, often done after medicines and laser surgery have failed to control pressure, makes a new opening for the fluid to leave the eye. Called trabeculectomy, this surgery is performed in an operating room. During this a small piece of tissue is removed to create a new channel for the fluid to drain from the eye. For several weeks after the surgery, you will put drops in the eye to fight infection and inflammation. These drops will be different from those you may have been using before surgery. Conventional surgery is performed on one eye at a time. Usually the operations are four to six weeks apart.

Another situation that has been disturbing to Americans has been the fact President Obama said on many occasions that if people like their current plans they could keep them. That is fundamentally untrue as new policies have to be designed to meet the criteria put forth in the new guidelines. He has since backtracked and apologized for misleading the American public.

Also, there are new taxes and penalties which will be levied to support this far-ranging program. Starting January 1, all employers with group health plans will start to pay a tax on premiums of approximately 4% to fund government subsidies and aggregate insurance coverages. And, if an individual chooses not to obtain coverage in 2014 there will be a $95 penalty assessed. That doesn’t sound so bad but that penalty goes to $525 in 2015 and up to a whopping $695 in 2016. There may be quite a bit of unrest when those mandates flow into the timeline. There are financial experts who say it is only a matter of time before “Obamacare” will cause our entire economic system to crater. Of course only time will tell. We must all hope Congress will keep a close eye on the Affordable Care Act as time goes on and will produce legislation that’s meant to curb financial disaster. Steg represents The Midland Humane Coalition, started in 2011, in partnership with PetSmart Charities. The organization goes to Animal Control or other rescue groups and saves the adoptable animals from being euthanized by bringing them to the Pet Adoption Center. They help them find their home. The Coalition assesses and cares for each animal, taking them to veterinarians for a thorough exam, shots, cleaning, microchip implants: essentially, doing their best to assess the health risk of a potential pet. Many animals are quarantined at a foster home if available for further behavior assessment to ensure they’re friendly and adoptable. The Coalition does not discriminate against size, type, breed, sex, or cuteness. All pets are equally accessible to possible adoption. Since 2011, the Coalition has adopted out 2,023 rescued animals. This has cut the euthanasia rate in City of Midland Animal Shelter by 56% in the past two years. In partnership with PetSmart Charities, the Pet Adoption Center is located in the Midland PetSmart store. “PetSmart provides the place and provides the food and cat litter, but we pay for vetting, and supply everything else such as toys, treats, blankets, etc.,” said Steg. “PetSmart has provided us with 21 kennels for dogs and 10 kennels for cats. As soon as one pet is adopted we bring another in.” “The Coalition is a nonprofit organization. We live by donations and fund raisers. We have a great Board of Directors. Our Pet Adoption Center was the 10th adoption center to open in the United

States. Now they’re up and running across the country, working with rescue centers and shelters to cut back on euthanasia. All our pet adoption fees go to the Midland Coalition,” commented Steg. She said, “I think one of the reasons people don’t adopt at a shelter is because it’s such a depressing place to go. The good thing we do is to bring pets out of that dreary environment— we place them in a better environment that potential adoption families are happy to enter.”

I

t’s a new year and time for a renewed vigor for health. So, I want to introduce you to a super food that may be new for some of you. Remember the old commercials advertising Chia Pets in the 80’s? The 80’s are back, and so is chia! Who knew that decades later we would be rushing to the store to buy chia for a different reason? We have been using chia seeds in my cooking classes for some time now, but I’ve noticed chia has recently been gaining popularity in the health, diet, and fitness circles. Now, if you don’t already use chia seeds, you are missing out! Here’s why. Chia seeds are currently being used for their nutritional and medicinal properties—increasing endurance and energy, suppressing the appetite, promoting weight loss, leveling blood sugar, and aiding intestinal regularity. When chia absorbs liquid, it forms a gel due to the soluble fiber. Researchers believe that this same gel-forming phenomenon takes place in the stomach when chia seed is consumed, thus creating a physical barrier between carbohydrates and digestive enzymes and slowing the conversion of carbohydrates into sugar. Slowing the conversion of carbohydrates into sugar helps with endurance and metabolic rates. This translates into good news for athletes, dieters, and those battling diabetes. Chia is the Mayan word for “strength.” It has been said the ancient civilizations used chia as an energy food to help them maintain strength and endurance on long journeys or during battle. Chia seed has hydrophilic properties, and can absorb more than 12 times its weight in water. Which means chia seeds can prolong hydration, helping retain moisture and regulate more efficiently the body’s absorption of nutrients and body fluids, including electrolyte balance. Because of these claims, endurance athletes have started using it to stay hydrated, stabilize blood sugar, and sustain their energy levels—even in hot climates.

Chia seeds are an appetite suppressant. They can bulk up a meal, reducing the amount of food consumed. Chia (seeds, gel, and powder) may also be used in place of eggs, fats, and flour within recipes. Other uses include: a thickener for soups, stews and gravy; toppings on salads, baked goods, and yogurt; a healthy addition to smoothies, oatmeal, or any of your favorite recipes. The flavor-neutral seeds are not only naturally gluten/grain free, but also one tablespoon of chia seeds has more calcium than a glass of milk, more omega-3s than salmon, and more antioxidants than blueberries. Chia is also an abundant source of dietary fiber; vitamins A, B, E, and D; minerals including calcium, phosphorus, potassium, sulphur, iron, iodine, copper, zinc, sodium, magnesium, manganese, niacin, thiamine, silicon; and is 30% protein. Perfect? Almost…if you eat them whole, they do get stuck in your teeth and can cause some severe bloating issues if you eat too much. With all new “super foods” comes the promise of magic weight loss and miracle cures. But while I do love chia, it is still just a food. Nothing can replace your responsibility of taking care of your body through proper diet and exercise. As with all new foods, be sure to discuss the addition of chia with your doctor if you are on any medications as chia can reduce blood pressure, cholesterol, etc. (which is a good thing!). But your medications may need to be adjusted. Note: I want to clarify that you do not want to eat your Chia Pet! Those seeds are not cleaned well and do not go through the FDA’s safety regulations for food consumption. R

eflecting on the past year has made me truly grateful and appreciative of all the people around Odessa and Midland that work diligently to save animals. It makes me realize how inadequate I am when I compare myself to others that spend so much time, effort and money on the idea that every animal deserves to be rescued. Every life saved is worth all the time, effort and money it took. Nothing is better than seeing a lost pet go home with their family, or a rescued dog go home with a new family. Petspotters has dedicated people on the page that work around the clock trying to make matches of lost dogs with the dogs that have been found. There are success stories posted every day by people who appreciate Petspotters and the hard work it took to get their lost pet home. I really would like to take credit for all the happiness, but the truth be told, it wasn’t me. It was the admins on the page that spend their time looking for the lost pets. It’s their labor of love, searching for your lost dog. They cry when you cry and they are happy when your pet is home. They are sincere people that go beyond the call of duty and they make the magic happen.

Every life saved is worth all the time, effort and money it took. Nothing is better than seeing a lost pet go home with their family.

As the New Year rolls in and goals are being set for the future, I realize that I shouldn’t compare myself with the others, and I shouldn’t compare my efforts with someone else’s. While they are a great deal better than I am at what they do, it doesn’t diminish

my efforts. Comparing myself with them will only make me feel bad about myself. People are all different and our skills and talents are different. It’s okay if someone is better at something than I am. I may be better than they are at something else. That’s what makes a great team. Petspotters has a great team. We all have our talents, and knitted together we complement each other. Where one is weak, the other is strong. This strategy has been very effective in many other types of teams and organizations, and for Petspotters it has increased the success stories and rescues by the dozens. As we go forward into the New Year, it’s great to reflect back on the past year and list all of our accomplishments that we have, but it is important we don’t settle there. We recognize our triumphs and our mistakes and see what improvements we can make that will increase our productivity and maximize our potential to rescue more, and reunite more animals with their owners. Our mission is to help people find their lost pets. Along the way we come across many that need a new home, and those aren’t to be neglected because every life we can save will eventually save them all. If we focus on our mission, one day we will be as great as our dog thinks we are.

health & beauty HEALING TIPS FOR DRY, CHAPPED LIPS

W

hile most people think of chapped lips being a wintertime woe, the truth is dry lips can occur in any season. Chapped lips can become very painful, especially if ignored. Not only are flaky lips unattractive, but living with sore lips can be severely aggravating! MAIN CAUSES OF CRACKED LIPS Loss of Lip Moisture: Chapped lips (medically known as cheilitis) can be the result of several things. While this condition is not a serious one, it can be uncomfortable and unsightly and most people would prefer to avoid it. One of the main causes of chapped lips is dehydration. The skin on the lips is very sensitive to the loss of moisture. Lips can become chapped when exposed to dry weather, wind, sun, or any other harsh conditions. Applying Vaseline or lip balm before facing the elements can help prevent chapping. Reapply as often as necessary to combat the harsh effects of the wind and sun and keep lips moist. Licking Your Lips: Another common cause of chapped licks is excessive licking of the lips. It can be difficult to avoid the urge to lick your lips when they feel dry or chapped, but this will only lead to further problems because of the irritation that it will cause. To resist the urge to lick lips, keep them constantly moisturized with a good lip balm. Allergic Reactions: Lips may become chapped because of an allergic reaction or sensitivity to beauty products. Not all products are safe for all people to use, and those who may be more sensitive to certain ingredients should use beauty products cautiously. To avoid this problem, try using natural products like Burt’s Bees or products with fewer extra ingredients, like Vaseline or Aquaphor.

MOISTURIZE LIPS MORE THAN YOUR SKIN One of the best ways to prevent chapped lips is by remembering to consistently moisturize your lips. Sure, you may spend a great deal of time applying lotion and cream to your face and body, but what about your lips? Beauty tips experts agree the skin on the lips is as sensitive as the skin on your eyes. It should be treated with care! FINDING A REMEDY THAT WORKS Finding a treatment that works for your chapped lips can be a bit of a challenge. What works for some people may not work well for others. While there’s not a technical cure for chapped lips, there are many remedies you can try at home. To soothe chapped lips, apply a thick lip balm. Many people choose to use plain Vaseline instead of trying to medicate with other lip treatments. Any ointment with natural ingredients is ideal to help your lips heal with ease. While your lips are healing, be sure not to lick your lips or get excess saliva in mouth and lip area. As your lips are almost healed, you can take a toothbrush and softly brush it across your lips to remove all dead skin. Be careful when doing this, as you don’t want to cause further damage. FINDING GOOD LIP BALMS The best lip balms consist of natural ingredients and do not have excessive or unnatural fragrance, color or flavor. Lip balms with flavor may encourage lip licking, which will only dry out the lips. Those lip balms that contain some wax will help to add a protective layer that can last longer and require less frequent application. Also, lip balms that have SPF protection are considered to be preferable for keeping lips moisturized and limiting sun damage.

Chia Chocolate Pudding by Kim Clinkenbeard, CPT, FNS

1 cup organic unsweetened canned coconut milk (or almond milk) 3 Tbsp. chia seeds (dry) 1/4 cup Dutch cocoa powder (or carob powder for a dark chocolate flavor) 2 Tbsp. stevia (or honey) 1 tsp. organic vanilla extract 1 banana 1 pinch sea salt

Place all the ingredients except the chia seeds into a blender, and blend until completely smooth. Pour the mixture into a bowl, and add the dry chia seeds. Whisk in the chia seeds well, making sure that all the seeds are moistened. Let the mixture stand for 5 minutes and whisk again, breaking up any clusters that form. Cover and refrigerate for 5 hours (overnight is best). Just before serving, top with fresh berries.

Mix ingredients together in a mason jar with a whisk, and let stand 3-4 minutes. Whisk again, and let stand 10 minutes more. Whisk a third time, seal, and store in fridge for up to two weeks.
