KNOW YOUR GENES Find out how genetic analysis can save your life Provided by: Hirslanden

Dr. Henry Perschak, internal medicine specialist, is a Genetic Promoter at Hirslanden Precise and a recognised genetic analysis consultant. His opinion: if everyone knew just a few of their most important genes, there would be fewer heart attacks and fewer hereditary cancers. I have been on the Genomic Board of the Hirslanden Clinic for quite some time. In our meetings we discuss, along with various specialists, cases that have a genetic (i.e. hereditary) origin. From the outset, we pursued the idea that we wanted to practise medicine with ‘rhyme and reason’ and put together gene panels accordingly. The main question was what adequate prevention medicine can look like today. When Hirslanden Precise was launched, I joined as a genetic promoter and consultant.

So can anyone get a genetic analysis at a check-up? Yes, a genetic analysis is always possible. However, the compulsory health insurance in Switzerland only covers the costs in very specific cases. It is mainly health-conscious self-payers that come for a check-up – basically those who have a healthy diet and go jogging. They want to be informed and want to have their genetic profile drawn up. If their family tree analysis reveals a cluster of deaths due to cancer or heart failure, these people usually have a genetic profile done. In addition, there are questions that are not so important from a clinical standpoint, but are interesting nevertheless, such as: how do I react to which foods, or certain sports stressors?

Do your recommend genetic screening to all your clients? No – not simply across the board. In each case, we decide in a detailed consultation whether genetic screening 12

makes sense. But personally, I am convinced that each of us should know our most important genes. In my own case, I have done a so-called Whole Exome, which is the part of the DNA responsible for protein coding. With these variants or mutations, there are a few top genes that can cause big problems. The main groups are around 170 genes that can lead to cardiovascular disease, and about 100 genes that can cause cancer. This list could be reduced to about 70 genes by analysing only those genes whose results trigger established consequences for action, in terms of prevention or treatment of the individual. If everyone knew only these genes, I think we would have a much healthier future ahead of us.

What have you learned from looking into your genes? The most exciting information for me was that I have inherited a so-called

heterozygous fructose intolerance. Like me, my father occasionally struggled with a tummy ache during the night. Now I understand this. I have adjusted my diet, and eat less fruit that is high in fructose, such as grapes or apples. And I don’t drink fruit juices any more. Now I still eat a berry dessert with double cream, as fat relieves the symptoms. I only eat asparagus and artichokes sparingly. This change probably has nothing to do with my life expectancy, but rather with my quality of life. I feel much better and healthier now.

Could you also identify gene variations that are relevant for life expectancy? Yes! A gene for skin cancer. In the past, I didn’t put on a lot of sunscreen, but just stayed in the shade when it was sunny. Now I always use sunscreen. Even my family pays attention to this. And of course I still go for skin cancer check-ups.

Isn’t it a burden carrying such a cancer gene? Of course, you have to think about it carefully. In an initial consultation, people seeking advice are also informed about this. But I have never had anyone in my consultations who fell into depression after a genetic analysis. Rather, people are clearly motivated to change their lifestyle and adapt their prevention programme, quite in keeping with personalised medicine.

In which areas of prevention is genetic medicine already established? Primarily in four main areas. Firstly, cancer genes. This starts with the malignant BRCA1 and BRCA2 mutations. Many people are familiar with them because of Angelina Jolie, who underwent a double mastectomy as a result. If a woman carries such a disease-causing gene variant, she is not only susceptible to breast cancer. She also has a 70% risk of developing ovarian cancer or other forms of cancer in the course of her life. BRCA2 also triggers breast cancer in men, albeit less frequently. However, they can pass the gene on to their daughters. Furthermore, the gene also triggers prostate cancer. If we were to screen the Swiss population for these two genes alone, we could prevent many cases of cancer. In Pennsylvania, this has already started with the so-called Geisinger Health Network. Apparently, the mortality rate from breast cancer there is now the lowest in the entire USA.

What are the other three main areas? The heart genes and the genes for socalled monogenetic diseases like cystic

fibrosis. The latter only ever occurs when a pathological gene has been inherited from both parents. Finally, a pharmacogenetic profile is helpful in the context of personalised medicine for the choice of effective medicaments in the right dosage and combination of active ingredients, and for the avoidance of side effects.

Meanwhile, genetic tests are available on the Internet for as little as CHF 500. That’s true, but the results are very dubious. I had a man in my practice who, according to such an analysis, had a variant of both BRCA1 and 2. The man was very worried, but didn’t have a clue what to do with the results. We then went through each variant in detail. I explained to him that his variants were not associated with a particularly high risk of cancer. Non-medical people usually have trouble understanding analyses from the Internet.

What is your greatest success story with genetic screening? There are two completely different cases. Case one was a 60-year-old woman who came to my practice with a black skin cancer and whose mother already had ovarian cancer. She wanted to know if there was a connection. In fact, it turned out that the woman had a PMS2 mutation, a variant of the socalled Lynch syndrome, a hereditary tumour syndrome characterised by a significantly higher risk of various cancers. Due to this risk, she decided to have her ovaries and fallopian tubes removed – just as Angelina Jolie did with her breasts. Now the woman is monitored more closely and follows a tailor-made prevention programme.

And what was the second case? It was exactly the opposite. The man had a very conspicuous family tree with clustered cancer cases. The cancers appeared on both his mother’s and father’s side. A genetic analysis showed that fortunately, he did not have a single disease-causing variant of the cancer gene. It was one of my most unremarkable gene analyses ever. Of course, something like this is nice for once. Despite his family history, the man does not have to go in for more frequent check-ups. Naturally, he should have the standard check-up. Because most cancers are not genetically determined, but rather arise randomly. Are you interested in a genetic consultation or do you have further questions? which is huge. It wasn’t an economically sound decision. Then I looked into secondhand cars, but it was very hard to find the type of car I was interested in on this market. After all this research, Carvolution started to look more and more like the best solution. I don’t have to worry about anything, not even depreciation. When I go back to the US in two years, I can simply give the car back and not worry about selling it at a loss.

What car have you chosen and why? This was an easy choice - a Ford Puma hybrid. I had already owned Hybrid cars in the US so I knew that technology and also it had the best value for money really.

Why do you need a car? And what do you do with it? Where do you drive it? I use it for quite normal things, like driving to the office a few times a week. But I guess the real reason we got a car is because we are here for a short time and we want to explore all the beautiful places in Switzerland. That would be almost impossible without a car. We have been to many amazing places already: Engelberg, Montreux, Luzern, Locarno. And also abroad, we’ve been to Toulouse in France and to Freiburg

in Germany. We can’t wait to see more interesting places with our new car.

Are you using the Carvolution app? Yes, I changed my kilometer package through the app. At first, when I chose my subscription, I chose a kilometer package of 1,250 km, but because of COVID I did not drive that much. So I changed to 850 km. I’m sure that I will change it back to 1,250 km in summer, when we will drive around and explore more.

“WE MOVED ONLINE IN 2 WEEKS!” Thanks to the Berlitz Virtual Classroom Provided by: Berlitz TX Group, a Swiss platform and venture network, moved their entire language training online in record time right after the pandemic started. Fabrice Fuchs, Junior Project Manager at TX Group and Leonie Leski, Key Account Manager Berlitz Switzerland, explain how.

Before COVID-19, how did you use to teach foreign languages to your employees? Fabrice Fuchs: For us, the relevant languages are German, English and French. Before the pandemic, we were doing classic face to face courses for all three.

How has that changed? Fabrice Fuchs: Now about 110 employees take Berlitz language courses each semester. For all of them, we switched from face-to-face classes to online training.

What were the challenges to moving online? Leonie Leski: Due to the COVID-19 regulations, we had to move to online very quickly. We used a mixture of Zoom meetings and the Berlitz Virtual Classroom. Within 10 days, everything was set up and ready to use.

How did you make sure that the quality remains at the same high level in the online courses? Leonie Leski: We have consistently trained our instructors on how to use the online tools, regularly mentored

them and talked to them about their experiences. This has been reflected in the overall design of the online courses.

How were the online courses received by your teams? Fabrice Fuchs: They loved them – there’s no need to travel, they can do it from their home office. Also the course structure seems clearer.

What advantages do you see with online courses? Fabrice Fuchs: There are quite a few benefits. With the online courses, we can now include and mix groups from all our different sites, which has the nice side effect of creating virtual contact between colleagues who would otherwise not meet. Online, it is also easier to take into account the desired scheduling of the participants and if it does not work at all, Berlitz also offers the possibility to record the learning session as a video. I also see the switch from paper to digital learning materials as a big plus, because we no longer have to handle huge shipments of materials, we conserve resources and produce less waste.

As a language provider, do you also benefit from e-learning? Leonie Leski: The flexibility in arranging the groups that Mr. Fuchs just mentioned allows us to create groups very effectively based on language level. This enables faster learning progress, which is of course very positive for us as providers,

but also offers additional motivation for the participants. We can also more easily use the ideal instructor for a given course, e.g. a native Zurich trainer for the German course in Lausanne.

How will the program at TX continue? Fabrice Fuchs: We expect to continue to do a lot of courses via Zoom, as it has proven successful for everyone involved. A few groups prefer to go back to faceto-face instruction and meet in person at the site. We will be resuming these courses as well. Without the restrictions of the pandemic, the decision between Zoom and face-to-face instruction can be made quite spontaneously and flexibly anyway.

How do mortgages work in Switzerland?

If you have a valid residence B permit, you may acquire property or an existing building for your own use. If you have a C permit for Switzerland, you may buy more than one plot of land for residential use or real estate of any kind without special authorization.

The bank finances a maximum of 80% of the market value of the property. You have to contribute at least 20% of the purchase price. At least half of that amount must be your own funds and not taken from your employee benefits insurance.

FINANCIAL ADVICE

Mortgage financing is divided into a first mortgage and a second mortgage. The first mortgage can cover up to a maximum of 66% of the purchase price. If you need more financing, then you’ll need to take out a second mortgage. The second mortgage must be repaid in regular instalments over a maximum of 15 years before you reach the age of 65.

What is affordability and how is it calculated? Affordability is quite a simple calculation of your income versus the property costs. As a basic rule to estimate affordability, the cost of the property should not exceed one third of the borrower’s gross income. Let’s see what that means more in detail. In order to guarantee affordability, your total living expenses (mortgage interest, repayments, maintenance, and ancillary costs) should not exceed one-third of your gross income. Many financial institutions use a theoretical rate of 5% to calculate the mortgage interest to ensure that the mortgage would still be affordable if interest rates were to rise. Yearly maintenance and ancillary costs such as utilities, insurance, and minor repairs, are then added to this at a general annual rate of 1% of the market value.
