To gallstones, we say ‘be gone’

‘Breaking up’ is easy to do with new technology Relief from the distress of gallstones has taken a new twist, thanks to recent technology being employed at Maria Parham Medical Center. There is a high rate of gallstone problems among the citizens of this area and, until recent years, this was handled by invasive surgery that kept patients in the hospital approximately a week and restricted their normal activities up to six weeks. Then laparoscopy came along, which allowed physicians to remove the gallbladder through four small holes in the abdomen. Most gallbladder removals are now done laparoscopically. Stone formation is due to our ever growing problem with high fat diets and obesity. These stones grow bigger over time, said Dr. Mark Dubinski, a gastroenterologist at the hospital, leading to a gall bladder attack. Symptoms of gallbladder distress include abdominal pain (usually right under the rib cage or breast bone) generally occurring off and on especially after eating fatty or spicy

foods, nausea, bloating sensation, vomiting, and, at times, yellow jaundice. Even after removal of the gallbladder, stones may remain in the bile duct leading to the small intestine. The doctor then performs a surgical procedure called an ERCP by which they can inject dye into the common bile duct and using X-ray, can see any stones that are trapped in the duct. Using X-ray and a special balloon, the doctor would weep the duct and attempt to remove the trapped stone(s). About a year ago, the SpyGlass Technology development was completed. Maria Parham acquired the technology from Boston Scientific Cooperation in mid-summer 2009, and Dr. Dubinski began using it to break up the stones and remove them. This is an outpatient surgical procedure that usually lasts less that 60 minutes, and the patient is only in the hospital a couple of hours. “Most patients are able to resume normal activities the following day,” he said.

Because actual visualization of the ducts is more advanced than a radiological view, SpyGlass Technology has enabled physicians to see the stone in real-time on a computer screen after inserting a tiny scope into the ducts. A saline solution is used to wash the stones so they can be seen using electronic frequency (SpyGlass). After these stones are broken up, a balloon is inflated above the fragments and is used to sweep them out of the ducts. If necessary, a stent may be placed to permit drainage of bile and any left over fragments. Occasionally, there are some mild side effects from the procedure, but they have been infrequent, Dr. Dubinski said. SpyGlass has been used only infrequently since its acquisition, “But the beauty of it is we have it here,” the doctor added. He expects its use will increase in the future. Maria Parham’s Lunch & Learns are a big hit Maria Parham Medical Center’s tagline is ‘Healthcare inspired by you.’ — and we take that very seriously. A few years ago, the hospital began offering Lunch & Learn programs to the community. Participants enjoy a complimentary lunch and then listen to a program on a particular health topic presented by a physician or other health care professional. Some topics have been irritable bowel syndrome, breast cancer, aging, and women’s health issues. These occurred quarterly and always received very positive comments. Participants liked getting health and wellness information from professionals presented in language that they could understand, and they also liked being able to pose questions to them afterwards. Often heard from individuals as they left the program: “The hospital needs to do these programs more often. They are so beneficial, and I look forward to them each time.� The hospital took that into consideration and, starting fall 2009, began offering the Lunch & Learn programs monthly. Prior to October of 2009, attendance at these programs was good, and since they have been offered monthly, attendance has nearly doubled! The February event was

titled “Matters of the Heart�, since February is recognized nationally as “American Heart Month.� Speakers were Dr. Willie Sessions, cardiologist; Janice Satterwhite, RN, CDE, coordinator of the Heart Watch Congestive Heart Failure Program; and Erin McIntyre, program director of Cardiopulmonary Rehabilitation at Maria Parham. Attendance at this Lunch & Learn was in excess of 150, and once the seats in the hospital auditorium filled, people were so interested in hearing the program, they chose to stand in the back. Because of the popularity of the Lunch & Learn programs and the amount of benefit they seem to provide, they are held more frequently now. to detect the presence of cancer. About 3,500 endoscopies and colonoscopies are also performed annually by the department. The American Cancer Society provided funds to initially establish a Cancer Resource Center in the hospital, where people can receive pamphlets and books on the disease, view videos and have internet access to cancer-related subjects. The center is located in the basement of the Brodie-Waddill Building next to the oncology unit and is free and open to the public. Carswell has worked to get several grants to assist in the outreach program. A three-year Duke Endowment grant made possible Maria Parham’s participation in several areas — community education and support groups, nutritional consultation for patients and their families and physicians and their staffs, and funding for a part-time research nurse to work with clinical trials at Maria Parham. Maria Parham’s Oncology Center is an affiliate of Duke Cancer, and the oncologists at MPMC are also Duke physicians. Rehabilitation services help patients return to active life People of any age can sustain injuries, develop various diseases or suffer from strokes. The severity of the incident can mildly or severely impair their ability to perform daily activities like walking, talking, and driving. Rehabilitation therapies are often needed to facilitate their return to a more active and normal life. Maria Parham Medical Center offers a variety of inpatient and outpatient Rehabilitation Services to help affected people improve their quality of life. The hospital employs more than twenty Physical Therapists (PTs), Occupational Therapists (OTs) and Speech Therapists (STs), most with a Master’s or Doctorate degrees. They provide skilled therapeutic services to patients both during and after their hospital stay. The therapists can work directly with the patients in their hospital rooms and continue care after they are discharged if the doctor thinks it is appropriate. The hospital has an 11-bed inpatient rehabilitation center for intense and comprehensive therapy services. They also offer home health therapy and provide two outpatient

rehabilitation centers to service the community’s rehabilitative needs. Physical Therapists evaluate and treat patients to improve their mobility, balance, endurance and pain management strategies to allow performance of daily activities. “We help people ‘re-learn’ how to do the things they’ve done all their lives,” said Nick LaRaia, lead physical therapist. Physical Therapists develop a treatment plan to address the patient’s physical needs. The therapy sessions may incorporate therapeutic exercises, balance retraining, manual therapies, and heat, cold, and electrical stimulation treatments. “Over the years, we’ve begun to focus more on exercises as the most important component of rehabilitation,” LaRaia said. LaRaia has worked at MPMC for 13 years and has personal empathy for rehabilitating patients. Seven years ago he was riding his bicycle on a country road and was hit from behind by a car. His left leg was severed, resulting in an above-the-knee amputation. “This was a life-changing event for me,” LaRaia said, “but it was almost that my life had

prepared me for it. I had worked with prosthetics at Maria Parham, had a mechanical background and knew what was possible. I was more able to accept my situation than many people would be.” LaRaia feels the Rehabilitation Department at Maria Parham has grown a great deal as physicians have become more aware of the benefits of therapy. He is Board Certified in Neurological Rehabilitation and has developed a Vestibular Rehabilitation program to help people who have inner ear problems with balance retraining. He receives referrals from throughout the Triangle area and southern Virginia for this specialized training. Occupational Therapists work with patients of all ages. They evaluate and treat those who have lost daily living and working skills due to injury or disease. This, in turn, restores their independence and enhances their quality of life. Digital transition under way at Maria Parham Imagine if you can, 18 million pages of medical records and patient files that take up thousands of square feet of valuable space. Add to that another two million sheets of X-ray film and other patient information, and you have what is stored at Maria Parham Medical Center. If you consider the amount of effort it takes to locate all these important items, you can see why the hospital has looked for a better way to store patient records and X-rays. Several years ago, Maria Parham chose to move to a hospital-wide digital system that includes electronic medical records (EMR) and picture archiving and communication system (PACS). The hospital is well along with that transition. An Information System Task Force was set up to oversee a strategic plan to implement this transition. Randy Williams, the hospital’s information technology director, heads up this effort, along with members of his staff, directors of various departments, and members of the hospital management team and members of the medical staff. Early this year, Maria Parham Medical Center kicked off the medical records segment of the transition. Initial steps include having all components of patients’ paper medical records digitally scanned. This reduces storage space and allows faster and more efficient access to their health information. Patients’ medical records will be available to patient care providers who have access through the hospital’s patient information system. The hospital ensures that records are under heavy security, requiring passwords that only available on a need-to-know basis. All data is also routinely backed up to insure safe record keeping. Electronic medical records have many advantages over the old paper charts. Physicians literally have patients’ health records at their fingertips, both while they are in the hospital and after they leave the facility. For instance, a patient may be told to schedule a follow-up visit with his or her family physician. On this visit, with EMRs, the physician will be able to

instantly pull the patient’s hospital record up on a computer. Obvious benefits are the physician will be better able to answer any questions the patient may have and also be better able to prescribe appropriate follow-up treatment. Williams has been at the hospital six years, and he said they have been moving toward a digital records system since then. “I look at it as a journey toward a paperless environment to use digital wherever we can. It is a huge undertaking,” he said. This initiative to follow the technology trail is one of several that Maria Parham has recently initiated. Phase I of the records change has been to build a repository of the 800-900 patient forms in use. Over the next two to three

years, the use of paper records will be replaced, Williams said. In the hospital, nurses will now document patient care into the computer system. There is no immediate plan to digitize current paper records. In the spring of 2009, the hospital began using PACS, which allows for all radiological images that were previously stored on film to be stored digitally. Also, all current radiological procedures are recorded straight to a digital file, making it easier to get the images to the radiologists to interpret. developmental and acquired speech, language, cognitive and swallowing disorders. Speech Therapists can determine if augmentative communication devices — like computerized voice synthesizers — are indicated and, if so, train the individual on use of the device in daily activities. They also help patients recover lost communication skills, so that they can, once again, convey messages to others. Speech Therapists can also conduct objective test measures to evaluate a patient’s swallow function and set a treatment plan based on their findings. They can even develop exercise programs to strengthen and improve the coordination of swallowing muscles, instruct patients on safe swallowing techniques, and recommend diet modifications to compensate for weak muscles. Maria Parham employs a Speech Therapist who is certified to conduct electrical stimulation to the muscles of the throat for improved swallow function. “It is extremely rewarding to help someone communicate and enjoy a meal for the first time after sustaining an injury to the brain,” commented Keesha Rowan, Lead Speech Therapist. Rubio highlighted other specialty programs

provided by the Rehabilitation Department. Lymphedema treatment is provided by an OT certified to address the unique needs of breast cancer survivors. Aquatic therapy is offered twice weekly at the Henderson YMCA’s pool in group or one-on-one training. The buoyancy of the water takes weight off the spine and helps individuals with conditions including arthritis, joint disease and fibromyalgia to exercise safely and more easily. Women’s Health Rehabilitation is emerging to educate women on exercises and techniques to alleviate pain associated with the pelvis, incontinence, pre- and post-partum and osteoporosis. Maria Parham’s therapists are currently planning a Sports Medicine Symposium later this year. It will be offered to coaches and student athletes to educate them on exercises to prevent injury, treatment options for common athletic injuries, and nutritional information to optimize physical performance.
