A MESSAGE FROM DR. CHARLES CONLON Oncology Program Highlights I am pleased to present the 2013 Houston Methodist Sugar Land Hospital Oncology Annual Report. This report encompasses Oncology Program activities and statistics for 2013. The Oncology Program at Houston Methodist Sugar Land Hospital was established to provide cancer patients and their families top-quality care close to home. Our multidisciplinary cancer committee consists of board-certified physicians representing surgery, medical oncology, radiation oncology, diagnostic radiology and pathology. Non-physician membership of the committee includes administration, clinicians, social work, cancer registry, quality improvement, dietary, pharmacy, rehabilitation and pastoral care. Representatives from the American Cancer Society and CanCare have an active role on the committee as well. The cancer committee is the policy-making body for the Oncology Program and ensures that an active support system is in place for our patients, families, employees and physicians. The committee reinforces Houston Methodist Sugar Land Hospital’s commitment to education and clinical research as well as a multidisciplinary approach to treatment. The cancer committee also monitors quality and performance improvements through well-developed and proven methodologies that will correlate to improved patient outcomes and more effective programmatic processes. Throughout 2013, the cancer committee focused on a number of strategies for improvement that ranged from programmatic enhancements to patient safety and care initiatives, meeting quarterly to develop and evaluate the annual goals and objectives for the clinical and programmatic activities related to cancer. One important programmatic enhancement in 2013 was the development of a General Cancer Navigation Program. Houston Methodist Sugar Land Hospital has offered a Breast Care Navigation Program since 2012, but we discovered a service void in helping all oncology patients navigate from diagnosis to survivorship. Therefore, under the direction of Frances Goerlich, RRT, general cancer navigator, focus was placed on developing a program for all other cancers. You can read more about general cancer navigation on page 9. Additionally, several patient safety and care initiatives were explored and implemented throughout the year. Outcomes of quality reviews and studies led to process redesign, policy implementation and continuing education of our care team in the topics of molecular diagnostics, chemotherapy administration and national treatment standards. These accomplishments translate to reassurance that the care provided to oncology patients at Houston Methodist Sugar Land Hospital is Leading Medicine. For additional information on advancements in molecular diagnostics, see page 20. This year’s report highlights some of our 2013 accomplishments and reflects the teamwork that takes place every day at Houston Methodist Sugar Land Hospital so that we can provide our patients with the highest quality care and experience in a spiritual environment of caring.

CONFERENCES AND PROGRAMS CANCER CONFERENCES

ONCOLOGY PROGRAM/CANCER REGISTRY

The cancer conferences are treatment-planning conferences available to all physicians and staff. The general/hematology and breast conferences are each held monthly. The conferences are coordinated by Maria Socci, RN and facilitated by Dr. Clive Shkedy, radiation oncologist and medical director of the Houston Methodist Cancer Center and Dr. Charles Conlon, hematologist/oncologist and chair of the cancer committee.

The Oncology Program at Houston Methodist Sugar Land Hospital works diligently on the logistics of the program and also maintains the hospital’s cancer registry. The cancer registry is an important part of the program and functions in accordance with Commission on Cancer guidelines.

The conferences are well attended by physicians and staff alike. Physician attendees are specialists in pathology, radiology, medical oncology, radiation oncology, surgery, and plastic and reconstructive surgery. All cancer conferences offer attending physicians Category 1 CME credits. There were a total of 135 case presentations throughout the year, of which 84% concluded with prospective treatment planning based on national evidence-based guidelines. The remainder of the cases were presented as educational opportunities to the audience in attendance.

Oncology Program

The registry data system, ERS, is designed for the collection, management, analysis and follow-up of information about Houston Methodist Sugar Land Hospital’s cancer patients. All malignant, benign, brain and central nervous system tumors, as well as some borderline malignancies are reviewed for reporting in accordance with the Texas Health and Safety Code. The information is gathered in a universally recognized format which is then reported to the Texas Cancer Registry (TCR). The Texas Cancer Registry is a statewide population-based registry that serves as the foundation for measuring the Texas cancer burden, comprehensive cancer control efforts, health disparities, progress in prevention, diagnosis, treatment and survivorship, and also supports a wide variety of cancer-related research. The TCR is the fourth largest cancer registry in the United States, and currently meets the high quality data standards of the National Program of Central Cancer Registries (NPCR), Centers for Disease Control and Prevention, and is Gold Certified by the North American Association of Central Cancer Registries. The ultimate goal and purpose of the TCR is to collect, maintain and disseminate the highest quality cancer data that will contribute towards cancer prevention and control, improving diagnoses, treatment, survival and quality of life for all cancer patients. The Oncology Program consists of the oncology program coordinator Priya Dabhi and certified tumor registrars Kathy Dunlap and Roshan Engineer. Administratively and programmatically the department is supported by Sharene Arocha, director of health information management; Maria Socci, RN, breast care navigator and manager of the Breast Care Center; and Frances Goerlich, RRT, general cancer navigator and director of the Cancer Center.

ANALYTIC CASES Of the 987 newly abstracted cases, 675 of the cases met the definition of an analytic case1. The top five analytic disease sites for 2012 were Breast, Bronchus & Lung, Prostate, Colon and Corpus Uteri. In fact, these top five disease sites equated to more than half of our total case volume for the year. 1 Definition of an analytic case is initial diagnosis and/or first course of treatment at Houston Methodist Sugar Land Hospital.

OVERVIEW OF THE BREAST CARE CENTER Located on Houston Methodist Sugar Land Hospital’s campus facing Town Center Blvd., the Breast Care Center offers a full range of breast diagnostic services to women and men. This dynamic center is designed to meet the needs of the patient by providing a comprehensive array of services blueprinted to the individual. These services include: •

The center is staffed by a dedicated group of breast radiologists who are board certified in diagnostic radiology and fellowship trained in breast imaging and intervention. The radiologist personally visits with every patient that has a diagnostic study or procedure, sharing findings from the exam in real time. Another feature of the center is the Breast Care Navigator Program. The breast care navigator is a resource expert who helps guide breast cancer patients through the emotional and complex journey from diagnosis to survivorship.

The breast care navigator is a registered nurse with specialty training in oncology and is available to assist patients with appointments, coordination of care, resources and referrals. The Breast Care Center is open for patient care Monday through Saturday. In 2013, the center saw 11,331 patients—a 12% increase from 2012—and recorded a total of 18,330 visits.

OVERVIEW OF THE CANCER CENTER RADIATION ONCOLOGY

GENERAL CANCER NAVIGATION

Located in the cancer center, the Radiation Therapy Department of Houston Methodist Sugar Land Hospital has been providing service to Fort Bend County since 2004.

General cancer navigation services were initiated this year to provide assistance for patients receiving a cancer diagnosis. Frances Goerlich, RRT, who is the director of the cancer center, also acts as the general cancer navigator. The focus of the navigator is to explain treatment options, schedule expedited appointments and coordinate second opinions. It is the goal of the program to also provide emotional support and guidance throughout the patient’s journey to survivorship.

It is our commitment to provide the best standard of care and treatment options to our patients close to home. Our Varian treatment equipment enables us to provide treatment to all disease sites by delivering external beam; intensity modulated radiation therapy (IMRT) as well as high dose irradiation (HDR). The cancer center staff includes a boardcertified radiation oncologist, medical physicist, medical dosimetrist, physician’s assistant, registered nurse and three registered radiation therapists as well as support staff. All staff is dedicated to helping the patient on their cancer journey.

The systemwide lung screening program was initiated in November 2013. The goal of the program is to provide individuals who are at risk for developing lung cancer with an early stage diagnosis and evidence-based treatment plan. This program includes annual low-dose CT scans, consultations with health care providers, access to tobacco cessation programs and referrals to physician specialists, if cancer is found.

We provide patients with comprehensive infusion services in a comfortable, private setting, delivered by well-trained and experienced staff.

Imaging and diagnostic services at Houston Methodist Sugar Land Hospital include a complete spectrum of imaging services, including x-ray, fluoroscopy, MRI, CT, ultrasound and more.

Effective treatment of a condition begins with an accurate diagnosis. Houston Methodist Sugar Land’s state-of-the-art diagnostic radiology procedures include:

Interventional radiology provides diagnosis and treatment using advanced, minimally-invasive radiologic techniques. Examples of interventional procedures include, but are not limited to, MRIguided biopsies, CT-guided biopsies and ultrasoundguided biopsies.

The range of patients served by the department and the variety of technologies it uses is reflected in the specialization of its technologists, nurses, radiologists and cardiologists. As the Department of Imaging and Diagnostic Services has grown and added new technology, it has developed specialized teams dedicated to those capabilities; providing expert patient care. Ongoing communication between the referring physician and the imaging team is a priority — keeping the patient’s needs at the top of the list.

PATHOLOGY AND GENOMIC MEDICINE

SURGERY

Houston Methodist Sugar Land Hospital offers a comprehensive spectrum of pathology services to our patients. With the full resources and technology of the Department of Pathology and Genomic Medicine of Houston Methodist Hospital supporting our efforts, our physicians are able to provide fast and accurate diagnoses and the best medical care available.

Within the 18 operating rooms, physicians and patients have access to some of the most innovative technology and techniques in surgery today, such as:

A dedicated professional, technical and administrative staff supports this vital service. The department faculty is comprised of accomplished pathologists, many of whom are leaders in their respective fields. Their clinical experiences and original research are published in the most respected medical journals. Regular clinicopathologic conferences are held to discuss difficult and interesting cases, further ensuring accurate and timely diagnoses. These conferences are especially important when diagnosing rare conditions or reviewing cases presented through the second opinion consultation service.

• Active electrode monitoring (AEM) instrumentation, an advanced monitoring system that shields patients from electrosurgical burns. Houston Methodist Sugar Land Hospital set the standard by being the first to bring this type of technology to Fort Bend County.

Our world-class facilities are equipped with the latest innovations in laboratory instrumentation and staffed by trained technologists with extensive expertise in medical laboratory science. Our laboratories are registered with the Food and Drug Administration (FDA) and certified by the appropriate accrediting boards and certification programs as required by the Clinical Laboratories Improvement Amendments (CLIA) and the Centers for Medicare and Medicaid Services (CMS), including the College of American Pathologists (CAP), American Society for Histocompatibility and Immunogenetics (ASHI), and American Association of Blood Banks (AABB). Each member of our technical staff is expertly trained in medical laboratory science. Molecular Diagnostics, a section of the Department of Pathology and Genomic Medicine, bridges anatomic and clinical pathology. It uses multiple state-of-the-art technologies to capture the genomic information of infectious diseases, cancer and hereditary disorders. These molecular data are key for making diagnoses and guiding therapy.

• da Vinci® Si Surgical System, a robotic surgical tool that enables surgery with less pain and scarring, diminished blood loss, a shorter hospital stay and reduced recovery period for a quicker return to daily activities.

• StealthStation® neuronavigation allows surgeons to visualize the patient’s anatomy in 3D before and during cranial surgery. It also allows the surgeon to see the exact location of their surgical instrumentation. • Faxitron X-Ray allows for biopsy x-rays to be done in the operating room and sent electronically to radiology, decreasing the time it takes to get the specimen read by a radiologist. •

In 2013, outreach was planned based on top cancer sites at Houston Methodist Sugar Land Hospital with the strongest focus being on breast cancer as our top site. Numerous ads and articles regarding cancer awareness and prevention ran in local magazines and newspapers throughout the year. BREAST Pretty in Pink–Town Square, October 11, 2013 A Houston Methodist Sugar Land Hospital cancer awareness and charity event for the entire family sponsored by numerous physicians and health and wellness vendors. One hundred percent of raised funds stayed within Fort Bend County to assist patients with personal costs associated with fighting cancer. •

An event presented by the Fort Bend Junior Service League and Houston Methodist Sugar Land Hospital. More than 115 vendors set up in the Stafford Convention Center, and a record 7,000 attendees came to shop, network and most importantly raise $223,000 to distribute between 17 Fort Bend County charities. Throughout the market, Houston Methodist Sugar Land Hospital placed posters and signs on the importance of early detection. In addition, video monitors displayed important statistics and facts which were broadcast regularly throughout the weekend prompting attendees to schedule their mammograms at the Houston Methodist Sugar Land Hospital booth. •

• 132 mammograms scheduled Fort Bend Junior Service League Houston Methodist Sugar Land Hospital works closely with League members to help with community awareness of breast cancer. The League members — well recognized women in the community — are the faces of our women’s health campaign and assist Houston Methodist Sugar Land Hospital in spreading the importance of early detection and mammography. Joint events included Pretty in Pink, Sugar Plum Market, check presentations and an annual breast cancer awareness talk at their October member meeting, which drew 150 attendees. Race for the Cure®, October 5, 2013 2013 marked the first time the Houston affiliate of Susan G Komen ranked first among 122 national affiliates in three categories: overall revenue, highest race revenue and highest funds spent on missions. Houston Methodist Sugar Land supplied Breast Care Center information and materials for the Houston Methodist table and was supported by a team of employees running and walking.

Houston Methodist Sugar Land Breast Care Center Open House, May 23, 2013

Throughout 2013, physicians and staff had many opportunities to promote cancer prevention, early detection and general awareness through speaking engagements and health fairs hosted by Houston Methodist Sugar Land Hospital and other community organizations.

An open house was held to showcase our new MBI equipment and explain guidelines for use. Radiologists and technicians were available to give tours and discuss the importance of early detection. Two hundred men and women from the Fort Bend community attended.

Baker Hughes Breast Cancer Awareness Day

A colorectal surgeon, gastroenterologist and oncologist physician panel discussed the importance of early detection, screening guidelines and what to do if cancer is diagnosed. All 84 attendees were given an at home EZ Detect fecal occult blood test. Patients were instructed to contact their physician if they had an abnormal result. Results are immediate.

Understanding that cancer care extends beyond the patient’s physical needs, we offer a number of programs designed to support cancer patients, families and caregivers.

Houston Methodist Sugar Land Hospital serves as an American Cancer Society Resource Center to facilitate dynamic education and support programs. Patients have access to the latest information about cancer prevention, diagnosis and treatment. Patients also can access an array of support services offered by the American Cancer Society, such as transportation to their treatments. SUPPORT GROUPS Our Breast Cancer, Grief Support Groups and Powerful Tools for Caregivers classes provide information, services and support for patients, family members, caregivers and also for those who have experienced the loss of a loved one. Facilitated by the Department of Spiritual Care, these groups are open to individuals of all faiths and beliefs, and offer patients, family members and friends the opportunity to meet others, find support and share experiences. GREAT AMERICAN SMOKE OUT

We are a proud host of “Look Good Feel Better,” a free program offered in partnership with the American Cancer Society that teaches beauty techniques to women actively undergoing cancer treatment. This program aims to help these patients combat the appearance-related side effects of radiation, chemotherapy and surgery, and begin to recover both physically and emotionally. Houston Methodist Sugar Land Cancer Center is affiliated with CanCare, a volunteer outreach program. CanCare volunteers are cancer survivors who devote their time to offering cancer patients emotional support, understanding and insight. In the month of November an information table was set up for employees and visitors in the public cafeteria area of the hospital during lunch hour. Information was provided from the American Cancer Society regarding lung cancer facts and smoking cessation. All in attendance were given the opportunity to receive a questionnaire requesting additional informational to be sent to their home or to a loved one regarding assistance to quit smoking.

REACH TO RECOVERY Reach to Recovery is an American Cancer Society program in which breast cancer survivor volunteers provide emotional support and information to assist with informed decision making for breast cancer patients as well as the patient’s family members. NATIONAL CANCER SURVIVORS DAY CELEBRATION Every June, Houston Methodist Sugar Land Hospital celebrates National Cancer Survivors Day®, which is an annual, worldwide celebration of life that is held in hundreds of communities throughout the United States, Canada and other participating countries. Participants unite in a symbolic event to show the world that life after a cancer diagnosis can be meaningful and productive. MOLECULAR DIAGNOSTICS Houston Methodist Sugar Land Hospital recognizes the advancements in molecular diagnostics and how it steers physicians down a new path of treatment. Genetic research has found that not only does tumor type aid in care determinations but also tumor subtypes as well. These subtypes are determined by gene expression patterns. Further research has revealed that treating some tumors based on genetic expression pattern has proven successful outcomes. Also, research reveals knowing genetic information of the tumors can guide physician’s pharmacologic decisions that will ensure the drug chosen will target the specific cancer cell without affecting normal ones. With these industry movements, Houston Methodist Sugar Land Hospital focused on education and service development in 2013. Using national treatment guidelines and industry best practice as a guiding principle, our cutting edge molecular diagnostic services bring exciting new technology close to home.

“Before molecular diagnostics, clinicians categorized cancer cells according to their pathology --that is, according to their appearance under a microscope. Borrowing from two new disciplines, genomics and proteomics, molecular diagnostics categorizes cancer using new technology. Genomics is the study of all the genes in a cell or organism, while proteomics is the study of all the proteins. Molecular diagnostics determines how these genes and proteins are interacting in a cell. It focuses upon patterns--gene and protein activity patterns--in different types of cancerous or precancerous cells. Molecular diagnostics uncovers these sets of changes and captures this information as expression patterns. Also called “molecular signatures”, these expression patterns are improving the clinicians’ ability to diagnose cancer. Soon all cancers may be diagnosed this way. Clinicians have discovered that molecular diagnostics has many uses beyond the creation of new screening and diagnostic tools. Expression patterns also can provide information for the design of new cancer treatments, monitor the treatment’s effectiveness as it is studied in a clinical trial, and even predict the patient’s response to a new treatment.”1

“With help from molecular biology and genomic sequencing technologies, it is now possible to discover good therapeutic targets within cancer cells and then to design treatments that selectively interfere with them Targeted therapies have already begun to make personalized medicine a reality and will continue to help doctors tailor cancer treatment based on the characteristics of an individual’s cancer. As many new types of targeted therapies become available, patients will need help deciding among them.
